My wife of 55 years is getting poor treatment there. My attorney looked into it and found federal violations, including an F-tag 686 indicating a patient in immediate jeopardy there. F-tags are deficiency codes linked to 42 CFR Part 483 of Federal law. The last inspection was done in November of last year. Solera of West Houston was fined over 17 thousand dollars after this inspection and, since 2021, has been fined more than 107 thousand dollars for violations. Trust scale is an F rating for this facility. This is not the fault of the nurses and other personnel there. This is due to serious understaffing of the facility, resulting in their not being able to keep up with the work load given to them. Solera of West Houston is owned by Cantex Continuing Care LLC, a for profit private equity firm which puts profit above patients, as well as above employees, and is not much better than a hedge fund, given its results. My wife will be home in another week, but she will remember the treatment she got there for the rest of her life. Again, this is not the fault of the staff there, but of the corporation which owns the facility, and looking to maximize its profits at any cost. read more

They have private rooms for skilled nursing and I heard they have some private rooms available for long term care. They would get my client up and out of bed into his wheelchair. He would wheel himself to the gym sometimes. The dining room was nice. The staff were helpful. Saw a few Nurse Practitioners there. They were friendly. My client had a mini refrigerator in his room from day one. The outdoors of this nursing home is nice. I took my client outside several times. He liked that. read more

Its a total nightmare! First night was a dream come true after that its all hell. Nurses don't care at all, always on their cells watching some kind of movies. When you ask for help at the nurse's station they give you a look like why you are here can't you see that I am talking on my cell or watching a movie. My mothers frig had a roach ( alive in it ). the manager cleaning lady is a total gossiper, talking about her staff to everyone and anyone that listens. These type reviews are tough to write because if you're experiencing it, you likely have a friend or family member temporarily residing in a facility like this. I was surprised I was so impressed with the center. My uncle is currently residing here and though his condition is not good, I feel confident he is getting great care. The staff are all very friendly, the rooms are clean and it smelled nice through the entire interior. It is a one story facility with easy parking. There are several restaurant's conveniently located nearby. They have a couple of common area rooms with pretty decor (of of which has a piano), a small shop with boutique items, and a salon for hair cuts and a shave service for the men. Each room has a mini fridge, a single sink and a microwave. The bathrooms are spacious. All rooms, as far as I could tell, are private rooms. Some rooms even have a private exit to a terrace. The TV channels are limited. I think there's only a total of 10. The facility was quiet. I couldn't think of any reason to give less than 5 stars. Again, it's a tough situation and no one wants to be going through, but if this is where he has to be, I feel it's a great option. He is entering hospice, so it's all about keeping him calm and pain free right now. If anyone is going through this and looking for a place they can rely on to care for their loved one, I would recommend this place. Since he has been sick, we have put him in 2 places. The Concierge was the first one. We did not care for that facility at all. Prayers to anyone reading this, but I also hope it helps you make a decision. Side note: I visited on a weekend. Everyone who knows anything about facilities like this knows they don't have their "A Team" on weekends. Not sure why. The patient still need the same care no what day. If the staff was this great on a weekend, and a weekend is not the "A Team", then I suppose the weekday crew is even that much better. I hate to even make that comparison. PLEASE DON'T SEND A LOVED ONE HERE!!!! Very under staffed and very rude employees. Administration staff are very unprofessional. The patient care is very poor. Every time I went to visit my MIL she was sitting in wet diapers, clothes and bedding. I would have to clean her up and change her sheets myself. The staff expressed their aggravation about my MIL incontinence. She would cry because she was so ashamed. My MIL also suffers from dementia. Don't be fooled by the nice facility. If I could give the place a zero I would. I am in the process of filing. Complaint with the state. I hope everyone who has suffered neglect from this facility also file. Her mattress was disgusting. The plastic protection peeling off and urine stained from other patients that used the bed before her. After being there a month they replaced it after I complained! So unsanitary! They didn't use a mattress protector so my MIL was only separated by a sheet and was sleeping on this. The smell was horrible after being urine soaked! read more

Management did not address concerns, but ignored them. While there are some good employees, majority do not help and often hide out and ignore patients. There is a condescending tone in the place. A few nurses reached out to me post stay and stated All of their clients that had a stay at this place were sickened by it....over 50 plus. All felt it should be shut down immediately. I was in Solera a year ago for two weeks. I had hip replacement 4/12/23 but then fell and fractured my femur 5/22/23. Went to Solera for rehab. the worst experience ever....I still think about it and it still upsets me greatly... kind of like PTSD. If possible avoid the place. read more

PLEASE READ THIS!!!! This is an absolutely terrible place! My mother recently spent two weeks at this facility after a fall for rehab. The staff in general was very rude and spent more time on personal phone calls than they did tending to patients. On multiple occasions hours would pass while she was waiting for assistance. Food was not good and very limited in selections. At one point I reached out to the Executive Director Jesse to voice my concerns. A return phone call was never received. Under no circumstances do NOT take your family members here! They sold me in getting my dad to come to the facility. You can never get anyone on the phone. Everyone is always in a meeting. My father stayed here for short term rehab and whatever was done I could have done myself. He left the same way he came. I was constantly popping in and he was always dirty! Dirt literally cacked under his nails. Who can relax or get better in a state like that? Save your time, money and energy and find somewhere else! The only person that ever showed any compassion was Nurse Kishni. Update My dad has been released and is now in a long term care facility. It has been brought to my attention that he has a huge bed sore on his bottom. Know one from the facility made me aware of this! They released him with no wound care supplies. My dad is a diabetic and is completely bed ridden. Prior to his admittance into Solera none of these issues were going on!!! Families keep your loved ones from this place, by all means! read more

DO NOT SEND FAMILY HERE. If I could give 0 stars, I would. My elderly family member was there for a month. Don't be fooled by the positive reviews like I was - they are from employees, people who only toured the facility or younger people who could care for themselves while there for rehab. I was fooled by the tour and the kindness before my family member joined. Then everything changed. in the month timeframe, there was: - Verbal abuse to my family member REPEATEDLY. In her face, threatening her when no one was around. This person is related to family that is in management, so is never addressed, and she trains the other new people of their "way" to handling patients. That is basically to ignore them and verbally abuse them. - Telling your loved one that they can not push the button for help. They say "they" will check on them, but they don't. - Being abandoned all day and sitting in wet beds because they will not change them. - Multiple people whose families were also in the facility stated the same issues, if not worse, including hiding their family members call buttons out of reach. Call lights not working for DAYS, which are critical to notify workers that a patient needs help. - When staff enters the room, even when you are there, they do not acknowledge you or speak. There is no sense of care for you or your family member or even common courtesy. - They tell you that they will get 1-2 hours of PT and OT a day. That's a joke. 15 minute AT BEST, and some days it is skipped, leaving your loved one on their own if they are bed ridden. - Management is the worst. They don't reply when you let them know of issues. Only on email to cover themselves. If you call when you are there and ask to meet with them, they will just talk to you on phone for a minute (if they even answer). - On the front-end, they do a good dance with giving you email addresses for escalation - it's only to cover themselves. I get that nothing is perfect, and am very understanding of that, but this is true neglect and lack of concern or help for the patient, or fort hem to improve. - The "Director" of Social Services is a joke. Clearly not qualified and will yell at you and demean you if you ask for her help in where to go from here to discharge your patient -- which is her job to guide you and help you. - Charge you for extra days out of pocket, but never notify you of the departure date. When you bring it to their attention, there is "nothing they can do". Even their employees tell you that you have to be careful of racking up expenses because of lack of communication on their part. - The staff settle for saying this is the patient's "new normal" without even trying to assist them to get better. - No discharge plan or referrals. Only referrals within their own owned network to make them money and continue to have you stuck in this bad network. - No offering to bathe or sponge bath over a month! No help brushing teeth or hygiene, which is critical when people can not fend for themselves. If you do send an elderly member here, plan to keep close watch on them and make it known that you are there and present every day as much as possible. That doesn't even help, but it's necessary to keep your family thriving in this place. While we did have a few good staff members, many left quickly in the time we were there as they also saw the lack of care of treatment of patients. I've had the misfortune of staying here 3 times since 2020. They were truly dreadful during the height of Covid. At least the management responded to complaints then. They drastically improved during my 2021 stay. I had to come back this week after surgery and the service has been absolutely dreadful. Granted there are some good nurses and PT people here, but overall it's awful. The weekends are the worst. The staff is totally non responsive. The nurses don't get my medication correct. This is Friday night on Memorial weekend. I needed to use a bedpan. There was no response to my call after 30 minutes. I finally started yelling for help. About 15 minutes later, the aide came in and turned off my light. She yelled at me and said, were you the one yelling? Then she left without giving me the bedpan. Finally someone came back after I rang my light again. I finished and rang the light for cleanup. The aide came in at 9:30, turned off the light and left. After 20 minutes waiting, I hit the call light again. At 10:30 somebody finally came to help me. On another note, the beds are horrible, with brackets and bolts under a thin mattress. I tried to get in touch with the administrator, Jessie. I was totally blown off with no response. I have the feeling this is going to be a very long weekend. read more

This place was great until I had the misfortunate of dealing with Josie, discharge planner. She never once helped me plan anything as to where my mom went after rehab. In fact she gave my info to a different company to plan for my mom after discharge because she didn't want to do her job. When I did talk to Josie I was always being talked to like I didn't know what I was talking about and treated as such. Nearly every time we were on the phone we were arguing because I wasn't doing something or another her way. When I asked how I was going to pay for a place for my mom, her sarcastic mouth said "well your mom isn't coming back home why don't you sell the house" very coldly. It took me by shock because 1) I had already told her that my husband and I live with my mom in the house with our toddler. 2) she has given up hope on my mom recovering enough to come home. I also firmly believe that Josie is the reason I'm having to deal with Adult Protective Services for no reason. She made up a false narrative in her head to make me out to be the bad guy. All around Josie should not be working in this place, in this industry, or anywhere where she has to deal with vulnerable people, ie; elderly, pediatrics, medically declined. Communication is lacking, when it occurs it can be condescending, and there is definitely a lack of professionalism. There are no case meetings unless you request it unlike other SNF where they schedule them for you with all the disciplines as you get further into your stay. My father felt he was not looked after and care was subpar. Please think twice - family members need advocacy. This is definitely lipstick on a pig. My father admitted to Solera several months ago and he received nothing short of the best care. we did shop around and we must say all of the surrounding facilities had similar reviews so we visited and chose one. I am so glad we did because the experience that others have posted was nothing like that. He would not let us shave him, cut his hair, bathe him, nothing but the staff there did everything we could not do and when i visited my father, i could not believe the man before my eyes. He was up in his chair for lunch, in the dining area visiting with other patients, no one knows how that made his family feel and to top it off, he wanted to stay there, so he is now a long term patient and has adjusted and doing wonderful. Sorry to say my family was deeply disappointed with how personnel at Solera handled visitors and my father's care. Due to Covid 19 procedures are in place to protect those who are able to come in to visit by appointment only. All visitors had to be Covid tested every 7 days, even if you did not enter the building. We were told two emergency contacts are allowed to visit for 1 and half hours a day between 8:00 to 5:00 M-F. No visitation on weekends. Others were given 90 minutes a week visitation standing outside the window, but with the window closed with the mask on at all times inside or out. Even having an appointment it should not take 20 to 40 minutes to fill out paperwork with each visit. As for personnel, some were very helpful and nice, majority were rude and have no business working around people if they do not know how to communicate nicely. One of many things gone wrong was they never seem to get the menu right as to what to feed my father. Several times we gave specific details as to his diet and what he can and cannot eat. They just could not get it right. On one occasion my sister was on the phone talking to my father and a nurse/maybe attendant came in yelling at my dad. When she realized my sister heard her yelling at him and my sister asked him who that was, she indicated she was not yelling, just talking loud due to wearing a mask. Another day one of my sisters was ask if she wanted to step out to the doorway because the attendant was changing my father's soiled diaper and another person walked by and yelled at my sister for doing so. Then she proceeded to enter the room to yell at the attendant for allowing my sister to stand by the doorway. She could have nicely asked if she could be of assistance and that a procedure was in place for all visitors to remain in the room. There is a way to get the job done without being so rude. First trip to dialysis my sister asked for my dad to be dressed and ready. She was told by the driver he did not have pants on and saw the attendees busy texting on their phones. My dad's shirt was dirty and he definitely was not dressed. They were going to release him like that. Last straw was on my dad's second trip to dialysis, again picked up late and on the return my father waited almost 2 and half hours before he was picked up. Prior to that several calls were made from family and the dialysis facility as to why he had not been picked up. The blame was placed on transportation, but we were shown by the drivers the text as to when they were told to pick him up. All this and more within a 5 day period my father was there. We had seen enough and decided to bring him home. No telling what else might have happened on the days we did not have access to see him. We will see he gets the care from home with people who are trained to do their work. This beautiful facility needs a re-haul. I would give them a zero but it does not give me that option. read more

My dad was a patient at this facility, and it was not a good experience. The building itself looks great, but that's about it. They are short of staff, which leaves patients unattended. The social worker, was adamant about my dad going to a private home, instead of trying to focus on home health care, which we received from the new rehabilitation center. Every time I visit, he was full of urine or poop. After being transferred to another rehab facility, they discovered massive bed sores on my dad, which I've attached a picture below. The major complaint, is they do not have any time of schduling to give updates on your love one. If they are unable to answer the phone, you will not talk to your love one or get any updates on their well being. You will literally have to harrased the service desk people, and they will continue to say, we will have someone call you back. read more

The main pros I can give after a 2 week stay is the food was great & my room & the common areas were very clean. The cons are another matter. The worst I experienced was my 1st night there. When I arrived I was unable to get out of bed on my own & needed help so I could use a walker to use the bathroom. The night nurse or aide that came after I used the call button refused to help me stand & told me to go in my diaper & she would change it. When she came back after I pushed the call button when I was done, she said she was busy & would come back later. When I had to go again (I need to mention I was still retaining lbs. of fluid from extensive back surgery & was finally losing it quickly), when she came back again, she told me to go in the diaper again & she'd come back & change me. She never returned & when I had to go a 3rd time & buzzed her, she said to go in the diaper again & she'd be right back to change me. She never returned & by the time I had to go a 4th time, the 4 laxatives the hospital gave me before being transferred there earlier in the day was ready to kick in. By then, my diaper & all the sheets were completely soaked & I was getting angry & decided there was no way I was going in my diaper 4 times w/ the addition of the results of the laxatives. By sheer determination, I struggled to stand up w/ my walker & made it to the bathroom. I made sure I reported the incident to numerous day nurses & aides since what happened was completely unacceptable & dehumanizing. The other disturbing ing con I'll mention is the lack of getting my pain meds. The dr. sent a prescription into their pharmacy for me to get 2 pain pills every 6 hrs. & over the first 5 days, I got a total of 4-5 pills & that was after begging & begging through out the day to have my pills & they would only bring 1. The usual response I got was I'll check your chart, but they'd never come back. It wasn't until my 2nd week there that Christine, who I believe was a P.A., changed my chart to say I was to get 2 pills, so I finally got the correct dose & on time. read more

I went to Solera for rehab after having a stroke. The private room I was in consisted of broken furniture and the call button did not consistently work. No steps were taken to repair the call button during my stay. The staff complained insistently about the facility being under staffed. I was expected to do rehab on my own in my room. Out of the three weeks I was in the facility I saw someone from rehab maybe 5 times. There was poor communication between the rehab and facility staff and I was put in an awkward situation multiple times (he said she said). Meal times were inconsistent and there were no alternatives if you didn't like what was provided. When contacting the staff regarding one evening meal, that I hadn't received, I was told they didn't have one for me and I was expected to eat snacks that a friend had provided. I'm sorry I didn't take my family up on going home directly from the hospital and there taking care of me. read more

My Dad was transferred to this facility from Memorial Herman Sugarland. He had Covid and needed physical therapy to get his strength back. We could not be with him nor could we be outside his window although the salesman promised me that he would let us once the administrator went home. That never happened because I could never talk to him or anyone else for that matter. My Dad got there on Saturday early morning and was dead by Monday morning, I got a call on Monday ....your Dad has died ...do you have a funeral home that can pick him up??? No reason, no condolences, ...just get someone to come get him. I asked what happened and I was told he was alive and she went to go get his medicine and when she came back he was dead. I wish I could rate this facility a 0 to prevent others from bringing in their loved ones here. Nurses are supposed to CARE for the most vulnerable populations. My mother passed a few days after leaving this facility. I have already reported this "rehabilitation center" to HHS for outstanding neglect and malpractice. She would call them for help to use the restroom or give her medication at a certain time, and they wouldn't answer the calls. I had to call the center over 5 times for them to finally answer the phone to administer the medication to my mother. Her food was horrific and too salty. She was on a sodium restricted diet, and the center was aware. They didn't even adhere to any of the discharge plans from the hospital. They did not have her medication, I had to drop it off every day. Her IV filled with blood twice, and the nurses were ANNOYED that they had to change it for her. They took hours to tend to her IV. She was there for a BLOOD INFECTION. She was traumatized by the lack of care. Please do not send your loved ones to this facility. Rest in peace, my beautiful mother. read more

Can I rate less than one star? Very pretty but don't let the looks fool you. The rehab center was great, the people were caring and helpful. The furniture in the room was very uncomfortable. I was there for over six weeks and I had only one good meal. I.e. Bad food. The daytime nursing was very good but that did not make up for the horrible nursing care at night. The midnight shift nursing aides should be fired. I kept a ledger of all the things they did or did not do that I considered wrong. This rating is truly a zero!! My mother was brought to this facility tonight 11/19/20 around 8pm. No one came to her room to unpack her things or check on her. She was ringing the button for someone to come help her use the restroom for over an hour while on the phone with my dad. When someone finally arrived, they gave her a bed pan to use over assisting her to the restroom. Fed up, my dad went to retrieve her and bring her home. The staff refused to allow him to take her stating he had not been tested and could not come into the facility. He had to call the police and show them the room she was in which was 3ft from the exit on the side of the building where he also parked the truck. They refused to just walk her 3ft to the exit. The police had to go in with her stroller, and wheel her to the exit. 2 officers had the walk through the front door and back to her room (without COVID tests) while the staff did nothing. They never came down the hall, didn't escort the officers who went into the room, grabbed her cellphone and duffle bag which were still unpacked and hand her to us out the door. My advice is to not leave your loved ones in this place. Horrible treatment of clients; they don't answer the telephone; and rude. Stay away from this place. read more

We lost our grandmother this morning, from the beginning we suspected negligence. The first night of her stay, she had diarrhea in her bed, she claimed she was left that way all throughout the night until NOON THE NEXT DAY. One night her air conditioning was left way too cold, they left her with a thin bed sheet and the lights on, she called for help and she claimed she could hear the nurses outside laughing as she did so. Yesterday, the night before she passed, my uncle visited her. She had been calling out for help for the past 30 minutes, call light on and everything. After he went to look for someone, a nurse did not come until 20 minutes later. The same day, earlier on, my mom said her catheter bag was full COMPLETELY to the point where urine was in the tube. I went to school for CNA and I was taught that something like that leads to a variety of problems such as UTI and Kidney infection. I was also taught that someone with a FALL RISK should be checked on at least every 30 minutes. This facility needs to have its license revoked and its employees thrown in jail for negligence. This is a facility that hires people only looking for money and not looking to provide true holistic care. After reading all the comments on this page, it is clear to see that this isn't the first time this facility has failed to provide adequate care, it's isn't the first time this facilities negligence has lead to a death. Nice facility with a chapel and all, but don't let the looks fool you. My grandmother came here after surgery and was not taken care of like she should have been. Bandages were suppose to be changed everyday, they weren't here. She would press the call button for help to the restroom and they wouldn't come for 10 minutes sometimes later. All of this may have been because of the low nurse to patient ratio, but it's no excuse for patients to have to go use the restrooms on themselves or develop infections from old bandages. read more

This is the hardest thing i have ever in my 19 years of life had to write. If you love your loved ones this is not the place to take them. It's nice on the outside but like the shinning on the inside. The "Certified Nurses" are unsanitary, rude, unreliable, and egocentric. And keep in mind we've only stayed three days here, and i have a whole bunch more to say. From the moment we stepped into this place it was a complete shit show my father and i were looking and asking what room my grandmother will be staying in, it took someone who "over looked" the facility to help us find her room... we've waited thirty minutes... she gave us the wrong room. We've also waited until two in the morning for my grandmother to get her medicine, however she was prescribed to take it at nine o'clock. The nurses don't wear gloves, use their long nails to pick up the medicine, have their long hair falling into the medicine, didn't even see the on call doctor once no evaluation nothing, the nurses and doctors go by vitals taking from six o'clock in the morning which means if your loved one was a dialysis patient like mine was they will prescribe them a sodium pill (which we all know that a dialysis patient CAN'T TAKE SODIUM!!!!) and when you ask the nurse about it they even say "the dialysis doesn't mean anything... don't rely on it"... Hmm really now, when we asked POLITELY to come help change my grandmother or take her off of the breathing machine for her breathing treatment they don't show up, we were left doing it ourselves. It's so bad here we didn't let her sleep here alone. All my grandmother wanted was the physical therapy if we would've known they would've treated her like this we would've never took her here. Now on her last day at this shit hole was just uncanny. I was watching my grandmother and the nurse had left her breathing treatment on. I go and wash the dirty dishes in the sink and i had an unsettling feeling, i turn around to see if everything was alright, and my grandmother is unconscious gasping for air, i run out of room 701 which was right in front of the nurses station and call for help. They run in slapping my grandmother in the face so she can gain consciousness, then they put her on the oxygen tank. I run back to the nurses station to ask if i can use their phone so i can call my family because my phone was dead, they let me but just an fyi she still didn't regain consciousness. After the phone call with my family i told the lady at the front desk that i was going to call 911. She grabs the phone from me and says "no, you can't" she takes me into the cafe right next to the nurses station and is blocking me from going back in. I end up maneuvering around the nurse and go back in the room, by now my grandmother has regained consciousness, but she still couldn't breathe on her own. My family and was livid about the whole situation and after arguing for five minutes they finally "called" 911... an hour passed by and the "ambulance" comes. Now let me tell you something, the reason why the real ambulance didn't come is because they didn't call 911 they called their emergency transport and the other reason why they didn't call 911 or even let us try to call is because every time 911 is called to any nursing home it's documented. Once their stupid emergency transport came they asked the nurses what happened not even me who witnessed the whole thing, and one nurse who wasn't even there at the time said in front of me "nothing happened a young girl over reacted, but the family wants her to be taken to the hospital". I WOULD NEVER EVER over react when it comes to my families safety. I KNOW MY GRANDMA MORE THAN ANYONE WHO WORKS IN THIS GOD AWFUL FACILITY! IF THERE IS SOMETHING WRONG WITH HER I WOULD KNOW. She ended up getting admitted to the hospital but passed away a week later. During her time in the hospital i made a promise to her that i would write this for her. And for anyone in the situation that my family and i was in and is looking for a Senior facility for rehabilitation purposes, don't do what we did and trust the hospitals recommendations, DO YOUR OWN RESEARCH. They don't actually go to the place and evaluate it they just print out a list of facilities that accept your insurance. And don't be fooled by the facilities rec routers coming in and evaluate your loved one, The reason they do this is to make "sure" that your loved one is fitted for the care that they provide (meaning if your loved one needs moderate or intermediate care) and most of these facilities like solera are all for profit so like i said do your research. As i said before this was the hardest thing i had ever had to write, my grandma passed away late February and it' now June. My grandma was not only my family she was my best friend. And the way she was treated no one should ever be treated this way. Hopefully my message here will save others from stress and heartache that no one should ever go through. read more
