My aunt was here for rehab couple months the rehab department was amazing the nurses are Gray I totally recommend this to anyone

Was highly disappointed with my stay at this facility. Friendly staff but poor management. Wouldn't recommend.

After…read moreover a year of making calls to try to move her to a closer facility for our family visits, Claudia was finally able to make it happen for us. She and I had been in touch weekly for over a year and she never lost her patience with me or got annoyed when I called the way other places did. Instead she was apologetic when there wasn't an available bed. The admissions staff was very caring, detailed and informative from the beginning till end. We stayed in close touch with Crystal through the care conferences, when my grandmother had to be sent to the hospital and back, along with when she had to go on hospice. Crystal is kind and considerate. The staff took care of her through her occupational and physical therapy sessions with Alicia and Melody. She was able to self feed and sit up right in a wheelchair again. They got her moving more than she did at her previous SNF. Her dieticians Zakirah and Yesica provided a great diet for her that she enjoyed until she couldn't anymore. I let them know she loves her sweets and they helped make it happen. Her last bites before she passed were pudding and Ensure fed to her by her nurse supervisor, Vivian, who took extra special care during the last days. They built a connection because Vivian spoke Vietnamese to her so naturally our grandma took a liking and was more responsive. Vivian even took the time to stay a little later so she could feed breakfast to her. Dan (charge nurse maybe?), was very detailed, gentle, and graceful in his responses when I called to ask questions about her status. And when he had to break the news, I felt the empathy in his voice over the phone. Her other RN's and CNA's (I'm sorry I don't remember everyone's names) including Adona, have all been a lovely presence in her final stages. My grandma always appeared very clean and fresh and lovely at every visit. The janitorial team is fantastic at what they do too, keeping the facility clean and spotless. Protective PPE is always organized and right outside the door for easy use. It doesn't smell like a SNF. Her last SNF didn't smell clean. Highly recommend this facility and their staff! Thank you for all you have done to take care of her with care, respect, and dignity till her last breath.

They are under LOTS of lawsuits & they violate lotsssss of labor laws. Do NOT send your loved ones here if you love them, this facility will not extend their time here with us.

I want to thank NuNu, Dillon, Isaac, Delfina, Grace, Eddie, Pam, for all of their assistance and understanding during my stay. The physical therapy department is excellent. They are all caring, understanding in all of the different ailments from all kinds of different patients. I want to thank Jacalyn, Jim, Claudia, Max and Ranon for all of the therapy and helpful ideas to continue my own therapy at home. The facility is very clean and doesn't have any kind of smell to it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. I would recommend this place for injury rehab and for your elderly loved ones.

My mom had dementia, fractured her knee and had to be taken there because I couldn't care for her…read more24/7 and don't have the means to have a nurse at home. -My mom's clothes got "lost" day 1. I dropped them off, name on each piece, including a small blanket. I visit the next day and she's wearing someone else's clothes. Her blanket no where to be found. When I went to pick up her clothes after her passing, they tried to give me a bunch of stuff that wants hers. And I only got back one shirt and 1 pant. Out of 4 tops and 3 pants I had bought her. -The place stinks -Nurses were good while we visited, other than that they didn't help her. -She got mrsa, within a week of her being there. No one noticed it until they had to call 911 to take her to the hospital, almost a week later. -Lost like 15 pounds in those 2 weeks. She wasn't eating and they didn't tried to get her to eat. -I asked all the questions, they "tried" to communicate, asked me different things she liked to eat to get her to eat, or liked to do so she can have some distractions. But nothing went any further than that. -Half of the time I visited I found her soiled. I visited every other day, as I was taking turns with my brother. All of this because we don't have the means for the good places. If your loved one doesn't have a voice, avoid this place. Because they won't take care of them at all. It was the worst experience for all of us. *The people who can speak up for themselves, were ok. This was mid-January 2026

I had the choice to continue living a life consumed by drugs and alcohol which was hurting those closest to me, wasting every second of my life away, and ultimately wanting to end my life, or seek help. Thank God the latter was the choice I made. While a resident there, my expectations were far from what was offered, and it changed my life in a BIG WAY. The staff at Genesis was not only first class, but most importantly personable and relatable. Many of the staff there are still an important part of my life today. Upon leaving inpatient, I transitioned to sober living, which again was safe, enjoyable, and much needed. I can say without a shadow of a doubt Genesis is much more than your average rehabilitation facility. From the executive director all the way down, the staff genuinely cares about saving lives and doing God's work. This team is on the firing lines daily, and they know how to reach those that need it. It is difficult to rate skilled nursing facilities because people go there out of need not want…read more However, when I walked in, I noted it smelled clean and the rooms also were clean and the staff were very friendly. I am also realistic, that things can happen in an environment in which there are many people to be cared for with too few staff. My loved one was evaluated by OT and PT, physician and LVN none of which knew he had very poor eyesight until after I divulged that information, which makes a big difference in a skilled assessments. They also were not aware of his baseline before his hospitalization, which was fully independent. This Made me question chart review, and how much information was passed on to the facility from Kaiser hospital. Unfortunately, he fell that night after being provided medication that made him confused even after we had discussed it. If I had a loved one, that was there, I would be questioning what type of monitoring is taking place after medication's are administered, have they reviewed the medical history and discuss the goals, intermediate and long term to be sure everyone is in a realistic and shared place. This fall could have been avoided and is not looked on as positive for any facility but reality is it can happen. What they do to prevent these events is important. Medication, monitoring and be present for your loved one.

We came everyday, multiple times a day and she NEVER had cold water. Furthermore she wasn't eating and they kept giving her the full dose of insulin even after we requested otherwise MANY times, leading to her having many hypoglycemia episodes. We even requested her having a sitter at meals to ensure she eats, but of course we didn't get that. The cherry on top is us bringing my grandma home AMA & her having a pressure injury on her heel... how this was missed & caused is unacceptable. I had to ask them to turn her more often because she was laying in the same position every time I visited her. But she still has one. Absolutely terrible, regret ever bringing her here

However, in this season of Thanksgiving, I find it's the perfect time to share my family's experience.  With the help and support of Apex, my husband once again has control of his life and his health, my children have their father, and I have my best friend. They literally saved his life from the grasp of his alcohol addiction. When the best Kaiser could offer was a mental health facility for my husband to detox {try to imagine a strip search, outside privileges offered only 2X a day for 30 minutes- and that's in a cloud of cigarette smoke-and people pacing the hallways screaming obscenities at all hours of the night} he was begging to come home after just 6 hours in. That is absolutely NO way for anyone truly seeking help to get it.  I realized pretty quickly that because our Healthcare system is so broken, we would have to find an alternative for my husband to actually begin to heal. Enter Aily Nguyen and the Apex team.  Honestly, Aily is an earth angel and was the balm that my sad, helpless, confused soul needed to get me through one of the most difficult trials of my life.  Her team literally rescued him from the "hospital" where he was and brought him to the beautiful sanctuary that is Apex. There he would find a private home with a pool, a chef that came daily with fresh gourmet meals, yoga, meditation and multiple therapy sessions each day.  Here, in this safe haven, is where my husband could recover, work, heal and ultimately be reborn. 11/27 marks 4 months since our journey began.  I'm happy to report that my husband remains committed to his sobriety, after 5 weeks of inpatient care and 10 weeks of IOP (intensive outpatient program). He has forgiven himself, and has been gifted the knowledge, the tools and the self awareness to stay the course.  Apex was instrumental in getting him to this point, and our family is eternally grateful for the entire organization.  If you are looking for help with your addiction, Apex will rise above and beyond all expectations.  If you want to be treated like a whole human being that deserves special care and attention throughout your recovery, look no further. If you'd like to have a place that still calls and texts just to check in months after you leave.... try Apex. Thank you, Apex.
