But it has long-term…read morecare for as well, therefore it is still considered a nursing home. 3 Elders would share 1 room with 1 toilet / bathroom here. It smells at this nursing home. Social worker who works here is unlicensed since California Law would require social workers to have a licensed social worker if the facility has 150 beds or more, in which this facility doesn't have. Without a license it's hard to keep the social worker accountable. Social worker here threatened us with laws, yet she herself wasn't licensed and our complaint to the California Department of Public Health for their wrongful actions caused the facility to receive a ticket. The social worker and management here tried to keep our loved one here longer than intended to continue to bill the insurance which was shady. Once Medicare started to not pay as much any more for short term care during the pandemic, then we find it that the social worker kept pressuring us to get our loved one out. The social worker here kept interrogating us if our loved one had 24 / 7 care when the nursing facility here had patients who became infected with Covid and passed and they themselves had elders who had falls within the facility. The owner had both a long term and a short term care facility. Their ultimate goal is to find a way to bill the patient's insurance to the max. or sense of actual care or compassion. Don't keep your loved one here. We had financial elder abuse and emotional elder abuse incidents with them. Luz was an unlicensed social worker and the most abusive and unprofessional social worker I've seen in my life. Let alone she has a thick accent. She acts like she's some sort of law enforcement officer when she wasn't even licensed, and her intent was to keep the elder at the facility longer to keep billing their insurance. She doesn't let the elder get the COVID booster. She acts like you don't have the right to talk to her. The most bit...y, ugly, non-compassionate. and most lazy social worker I've ever seen. Don't be afraid to argue against her. I am pretty sure many have verbally fought back against her. She should go to h..l for being abusive towards family and elders.

I liked that it had recently been renovated and had an Updated PT Department compared to what I had seen. My mom recently stayed at Creekside 9 weeks for rehab and she is a complex case due to severe osteoarthritis. Upon arrival she could barely stand up and walk 50ft. - she required 2 therapist during her PT sessions at first. It wasn't long before she was walking further and progressing in both OT and PT. The therapy team is Great! - Angelique, Anisha, Kiko, and Natalie worked with mom often and made amazing progress with her. Aloha Randy! Watching the entire therapy team in action everyday was impressive as to the range of skills they demonstrate patient to patient. Mom is home now walking around the house and outside. This was the primary goal for her being at Creekside. The CNA's are truly Special! Most of them treated my mom very well and kept her clean and if she needed something they took care of it. She had a couple favorites I would like to mention for well deserved recognition - Sauli - Tirhas - Carolina - Jonjie - Thank You for everything you did for my mom!! I really Appreciate YOU! Big Shout Out to RN's Sheri and Hafsa for getting mom her pain meds On Time! The kitchen Boss - Nakea checked in with mom to see if she wanted the menu item for the day or an alternative for Dinner - Very Nice! Annie is Awesome! She was moms Case Manager - All of the issues I brought to her attention were resolved quickly and professionally. I completely trusted her and valued her advise when needed. Emil and Annie make a Great Team and seem to run the place very well. Emil was moms Care Coordinator - Very Hard Working and easy to work with. We had weekly meetings regarding important details about mom's care. My mom had a private room during her stay at Creekside and I was with her most of the time to keep her company. I felt comfortable leaving her there alone at night. Her only complaints were the room was cold and the food not so good but we brought in outside food often. The responsiveness of the nurses and CNAs insured I was well taken care of since I could not get up to do anything on my own. The Occupational and Physical Therapy teams were top notch - they really know their field and made sure I was well educated on my own care once I was released home. I highly recommend the facility, from the administration to the nutrition and housekeeping teams and everyone in between, you or your loved ones will receive excellent care!

My dad had to stay here for almost 2 weeks for rehab after being transferred here from Kaiser. The place was clean and is in a really good neighborhood and is within walking distance to restaurants and shopping. The CNAs and nurses were very nice. I forgot one of the nurses names, I think it's Helen. She brought my dad a packet of hot chocolate for later and always said hello when she saw him. Another CNA or Nurse named Star was also great with my dad. My dad was well taken care of here. If you ever need to put your loved one in a care home whether it's temporary or permanently, I trust this facility.

Everyone, and I mean EVERYONE that I interacted with was so kind. My mother was treated with dignity and respect by all the staff assigned to her case. I visited my mother every day and witnessed a wonderful team effort-Nurses, CNAs, LVNs, OTs, PTs, case managers, kitchen and custodial staff-everybody that I interacted with were helpful, patient, and did their jobs with a smile. Even staff not assigned to my mother's case would pop into her room and check on her. The Director of Admissions spoke with me several times and each time I felt confident that my mother was in the right place. I cannot state enough how happy I am with the care that my mother received, the expertise with which it was given, and the wonderful attitudes displayed by everyone. The facility itself is quite clean, and there is ample parking provided you visit after 3:30 (when staff start departing). There is a large physical therapy room that is fully staffed and equipped. Residents have the opportunity to participate in daily group activities (which my mother did). Dietary requests are also given consideration. Communication between the staff and me was excellent. I was kept informed more than adequately. When it came to discharge my mom I was so relieved that many after treatment details-prescriptions, follow up appointment, arranging of home health care-were handled for me. My mother is very happy with how she was treated by everyone here and I am so glad that Canyon Springs was our choice.

Despite the natural challenges of recovery, the experience was genuinely positive. The facility is bright, clean, and thoughtfully designed with lovely gardens where both patients and families can relax. The staff were consistently professional and kind, and the management took the time to reach out to us as family members to ensure we were comfortable and heard. Overall, I'd give the experience a solid 4.5 stars. The only small area for improvement would be the food options. My mom is Asian, and we noticed there weren't many Asian-flavored meals available. With more and more Asian patients enrolling, adding a bit more variety in that area could make the experience even better. Care is sub par, increased time to locate staff on the unit when you need help. My family member was placed in a 3 person room and had no opportunity to move to a room with less people in the 4 days we had them there. Couldn't leave fast enough.
