Horrible place. I was there for 2 months. The facility smelled like urine and feces. There was NO sanitizer available at the entry or any other location in the facility. When I asked why, employees indicated the products they used had been recalled. OK,THEN GO PURCHASE SOMETHING ELSE. We bought our own for my room and several employees used it. Even a couple doctors. Side note, they did ask permission, and of course, we said yes. I was left in soiled diapers on several occasions. I was unable to walk so I was completely dependent on the staff as many patients are. Most of the CNA's did not care about the patient. They took their time responding to calls for help. Their goal seemed to be to get the lights turned off but not to address the reason for the call light. Some would talk on their phone while in my room, the conversations were often inappropriate. On several occasions, we saw roaches. Gross, gross, gross. The nurses were better. The CNA's were horrible. High praise to the PT/OT staff! The only good thing about this facility. I blame upper management for not firing the unqualified, uncaring staff. Also, they are the ones who did NOT supply the place with hand sanitizer. This facility needs to be out of business. The care they give is abhorrent. My loved one was in a soiled brief every time I visited. The medication management is pitiful-medicine late or lost, or simply not given. The Director, CNA, nursing staff turnover is unbelievable. They don't help your loved ones eat if they need it. I've walked in and--the 1st floor nursing station at midnight and all three nurses were asleep right heads on the desk or computer monitor --on the desk. When not asleep-- watching TV on their phones. I arrived one day and a patient in a wheelchair was in the middle of the parking lot and could have been hit because it was difficult to see over the front or back of a vehicle. None of the staff even realized she was missing. It is my understanding that the patients welfare was to be checked every two hours. I sat several nights six hours and no nurse, CNA ever came in the room. Imagine my loved one was soiled for six hours. The nurses are extremely lazy with the exception of first shift. They will not even change a brief. They don't follow the doctor's orders. If you have a concern-they will not even provide you the doctor's name. I only saw a couple nurses in a period of two years ever assist a CNA. There's no permanent weekend crew. So, the facility developed a strategy to send the needy patients to emergency room if they were shorthanded or even cover an entire floor on the weekend. They employed this tactic and would inform the family after the fact. Please for the love of Jesus don't put your loved one in this facility unless you plan to take care of them there at the facility yourself. During the time my loved one was there I witnessed three different Directors. At least five social workers departed. Too many CNA's to mention left. The turnover was atrocious at all positions. Again, I wouldn't recommend putting any human in this facility. read more

They keep the residents too long to use up the insurance money. My mom got a respiratory infection there and she died in the hospital. Some staff may care but administrators do not. Night staff is horrible. They know people don't have choices because of insurance. read more

This is not a skilled nursing facility it it more of an assisted living facility. If you dont have the expertise to speak to and handle a person thats suffered trauma or has dementia type symptoms than your not "skilled". If you just want to lounge and scroll your phones until meals are served then thats assisted living. Theres a need for that but its not skilled nursing. The people were very nice but told me several times they just were not equipped to handle my husband who wanders. I didnt make the assessment they did and then proceed to just keep calling me day and night so the whole stay was traumatizing for me and him. I sorely regret it read more

There's only one person that really cares out of everybody that works there with my friend! When I try to call no one ever answers the phone. I will ask the staff before I leave to do this or that that my friend needs and they never show up or they never go back to take care of it. They are short staffed. But every time I'm there, the staff are talking and having fun laughing. I have been there several times talked to the nurses and staff about my friends needs, but they are never met and some of them that are on the staff are very rude and mean to him. read more

My mother went to this nursing facility and she's dead now. She died on 07/08/2024. I am very angry because she did not die related to her condition. My mother suffered a massive stroke left hemisphere, with a curated artery. She died of a blood born infection. She had a bedsore, and a bad uti. My mother could not speak, so I believe that she was not a priority. After all, she wouldn't be able to tell anyone of their gross negligence. I call the on quite a few occasions. I was left on hold for a long period of time. I have screenshot of the hold times. It was not my decision for my mom to be there. Those who chose, did not failed to do their research. I wish I could give negative stars to this place. The outside is very attractive, but the inside of the rooms is not. My friend's room constantly had flies. Most times when people visited, he was laying in the bed in only a diaper and without even a sheet over him. They also did not follow the treatment plan from the hospital. There were so many things they didn't do correctly that negatively affected my friend and certainly hastened his death. The day before he died, the family had to demand that they call an ambulance. I will never forgive this facility and the staff! My mother was in this facility after suffering from an aneurysm. Due to the COVID pandemic my family was unable to visit my mom. On 4/28/20, I called the facility to check on my mom, the nurse said she was too busy and unable to talk! I called again on Wednesday 4/29, the staff said the nurse was busy and she would return my call. I never received a telephone call! Unfortunately on 4/30/20 my mother passed away! The staff at this facility is uncaring, unsympathetic, rude, insensitive, and inconsiderate! I will never ever forgive this staff, including the nurse, the person answering the telephone, the administrative staff and the guy that did not want to open the door for us to see my dead mother in her room! I will never forgive, my heart is broken in a thousand pieces. I do not recommend this facility to anyone. Avoid this facility for short-term or long-term care. My elderly mother was here for 2 weeks after she broke her leg. We will NEVER return. The smell is overwhelming. Bathrooms are shared. Adulty dirty diapers were not disposed of properly. The food was not edible at all. The day-time doctor didn't listen to any of the problems my Mom had while at that facility and never ordered the tests or medications she had assured us she would. The rehab people were great, but not worth a stay at The Colonnades. If you are not there all day every day, no one is making sure your loved one is showered, properly fed or cared for on a daily basis. read more

My great grandmother is currently here. And the food they serve their patients is so nasty. We have to come everyday day to make sure my great grandmother eat something that is tolerable. She mightiest well get hired there. There is NO hot water. They patients have to take a bath or shower with regular room temperature or cold water. They never have any milk either. The nurses also told my great grandmother to stop hitting the nurse button. I thought that is what the button was for when the patient have an issue. Some employees are actually nice. My mom was a resident for 3 weeks. They maintained care, there were a few good CNA s and nurse. However, there were aggressive, abusive and rude CNAs and nurses that should have their license revoked. I was talked to like I was stupid when asking a question about my mom's care. I was yelled at and told to put her on hospice because after yanking on her arm and telling her awful rude things she didnt respond. I was told that if she got cpr they would break her ribs, so I needed a DNR. This was all in the hallway loud and angry enough that I knew that nurse was a threat to my mom. I live in Pearland, so I was at the facility at night, at the break of dawn, after work to protect her. In the end, they tried to give her antibiotics without testing her bloodwork and did not want to remove a catheter that was placed 8 weeks earlier. At night, this place is dangerous. There isn't a director of nursing or administrator on duty, just agency nurses sent through an app. The fire department responded to smoke in the hallway 3 times. I saw workers smoke weed before entering the facility. Most of the night nurses just sit on their phones until 530 am and then they walk around patching things up so the morning crew wont notice. Awful, scary place. What is sad, disappointing and even worse is that this is not unusual, it is in fact the norm for skilled nursing facilities, long term facilities, long term acute care hospitals and even hospitals in Texas. read more

DON'T COME HERE! The bedding was terrible, sheets weren't washed. The room overall wasn't very clean and didn't feel clean. The room was in bad condition and everything seem really old. The staff wasn't very welcoming, however there were a couple that was nice and helpful. The people at the front desk wasn't very friendly. The restroom in the rooms are shared. There also aren't locks for the bathrooms, which is outrageous when you are sharing the bathroom with someone else. We were also informed about "bath days" which is another problem. They said they have bath days which are 3 times a week because there aren't enough nurses, and they have to help 12 patients for each section. We walked around and most of the rooms were empty. So either they are lazy and don't want to give the patients a bath everyday or they are low on staff. So if they want a bath everyday a love one or family member has to give them a bath. There also isn't wifi which isn't a big deal but they should have it for contact emergencies. Please don't send your love ones here! Also the building might be pretty on the outside, but in the inside is a nightmare! read more

My uncle was there during Christmas and I found him laying in his own urine. I'm not sure how long he had been like that. I asked for him to be cleaned up and in about 20 minutes someone came and it was done in less than 10 minutes. I later asked a nurse to give him something for excruciating pain. 30 minutes later, she had not come so I had to ask her again. There were no paper or cloth towels in the bathroom so he could not dry wash his hands after using the facility. His supplied wheelchair (which had a missing foot rest) fell apart when trying to move him. This home appeared to have adequate resources. It appeared relatively clean and free of extreme negative odors. There was one woman who was very helpful and the nurse eventually did get the medication to my uncle. If I had the authority, I would move my uncle elsewhere. I am compelled to write this review with hopes that this facility will improve because it is in a favorable Pearland area location and the residents were very nice. They should put forth extra effort during the holidays since that is when residents will have the most visitors. read more

My grandmother just passed away from Alzheimer's and was previously at The Colonnades. It was a terrible experience and I would give less that 1 star if I could. My grandmother had multiple issues at this facility. She began to lose teeth, and the nurse staff had no clue as to when or how she lost them. Then my grandmother had a stroke, and it went unnoticed by ANY of the staff until my aunt went on her daily visit. When she asked the nursing staff how long she had been like that, the only reasoning they could provide was that "the night staff blamed the day staff, and the day staff blamed the night staff". So they took her in to get a CAT scan and since my grandmother was showing pain in one of her legs, they decided to do an X-Ray as well, only to find out that her leg was broken! Once again the negligent staff had no idea how this happened. My grandmother's hospice nurse told my family that she had worked at the Colonnades for 3 months, and had to leave because of how awful it was. My grandmother passed about a month or so after she left this home. We filed a complaint with the state and they performed an investigation and identified multiple deficiencies with this facility. It's hard enough for us to lose someone we love, but to have doubts on whether her life expectancy could have been longer if they had just paid closer attention, is something that is daunting and not something anyone should have to go through. I wouldn't recommend putting a loved on in this center. This is my Review Of said Nursing home. I went to visit my mother on Sunday February 16, 2014. I was stopped by a nursing home employee who worked on the hallway where my Mother lived. She told me that my mother was no longer there. At first I thought she meant they moved her to a different room. Then I was getting upset because I thought she had been sent to the hospital without myself or any of my siblings being notified. The employee informed me that my mother had passed away! I am so hurt and angry! My brother (who had arrived by this time) asked a supervisor why we hadn't been notified of our mother being sent to the hospital or even of her passing? His excuse was they did not have the correct contact information. This was not true at all since just one month before (in mid-January) my family had a meeting with the nursing home administrator and my mother's doctor. During this meeting my phone number and the phone numbers of all of my brothers had been shared and verified. If nursing home staff would have looked through my mother's chart they would have found my number (and the numbers of all of her other children)! When we asked nursing home staff where our mother's body had been taken and where we could find her belongings, we were told they could not tell us. They said her chart and belongings were locked up and we would have to wait until the next morning to even find out what hospital they sent my mother to! What we found on our own, without the help of the nursing home staff, was that instead of sending out my mother to a hospital just a 15 minutes drive from the nursing home, they sent her to a hospital that was nearly a 40 minute drive. It turned out that our mother had been admitted to a hospital two weeks prior to discovering she had passed away! Two weeks and NO word from the nursing home about her need to be hospitalized! She passed away five hours after she was transferred from the nursing home to the hospital. I will never get closure from this. I will never get to tell my mother goodbye. My mother died alone. My mother had family and children that loved and adored her. We need answers and justice." read more

My father was a patient at this facility far longer than I care to admit... the reason we stayed as long as we did, was due to the fact that he had a private room and bathroom. In the end it almost cost him his life prematurely ! DO NOT BE FOOLED BY THE NICE BUILDING! THE STAFF AND ADMINISTRATION ARE HORRID, HEARTLESS AND RUDE!!!! There is a reason why this place has empty rooms/ halls and none of the other skilled nursing homes in the area have any availability at all! Check their website and see how many sitings/ they have that they basically ignore. I have spent endless time trying to be positive with input the help improve the situation, but to no avail. Most of the people I spoke to or wrote at Cantex the parent company,were gone within 6 months. Thank goodness I overruled the nurses' call and had my father taken to the hospital ...where they recorded negligent on my dads records and said he had a second stroke and even a child should have known to call 911! It was not the first time we witness negligence and the result of death due to staffing ignorance and lack of concern. The Hospital also recorded in their notes that the blood in my father's brain was from a fall over two weeks old! No record on his charts or calls were made to notify us -it is a state law! This is with two of us taking turns to oversee my Dad's daily needs...and he still ended up in ER ... The administator at the time we went through a dozen, refused my calls , but her minions gave us a huge discount on our remaining bill, due to the fact I mentioned that the hospital and doctors noted negligence...we packed up and never looked back! BUT MY CONSCIENCE WILL NOT LET ME SLEEP AT NIGHT NOT SHARING WHAT THEY WILL ALLOW...THERE WAS SO MUCH WORSE GOING ON!!! YOU HAVE BEEN WARNED! read more

Unethical management, poor care. Due to COVID, I couldn't get in and my dad's cognitive deficiencies kept me from getting cohesive care reports from him via phone. It took days to get nursing staff to return my calls. When they did, information was lacking and incomplete. The management was even worse. The staff ignored my father's severe cognitive deficiencies and allowed an 88 year old brain damaged patient sign admission and financial papers when he was admitted four days after hip surgery. This was in spite of my known role as his Medical and Durable Power of Attorney. In addition to cognitive damage, he didn't have his hearing aid or his eyeglasses when the papers were signed. There is no way he knew what he was signing and they refuse to give me a copy. He ended up going back to the hospital with pneumonia and an infection in his hip 13 days later. He passed away shortly thereafter. They are horrible. read more

My father was a respite patient there from January 18th-22nd, and the services or lack of services he received was truly horrible and an absolute nightmare. The logo reads committed to excellence, compassionate and delivers the best care that is effective, WHICH IS A FAR STRETCH FROM THE TRUTH! This is one of the worst facilities that my family has ever experienced. The appearance on the outside is beautiful, but when the doors open it is absolute horror! We have all heard bad stories about nursing homes, but I can truly say Cantex/The Colonnades at Reflection Bay is one of the most disorganized, unhelpful, unsuitable and useless facilities. On the morning of 18th at 7:15am my father left his home doing fine, eating, drinking, alert and responsive as of today January 23rd, he is no longer eating, drinking, alert or responsive. On Sunday, January 21st at 1:08pm, my sister received a call from the nurse on duty, and she said that our father was not eating or drinking and his vitals was very low. So we rushed up to the hospital to check on him, only to realize that the staff did not feed or give him anything to drink for 4 DAYS! My father requires oxygen and I am sure it was documented on his paperwork, but yet again, he did not have oxygen for 4 DAYS! I truly cannot understand why a company that prides themselves on being a caring and committed company, would allow such a thing like this to happen to an elderly patient. Words cannot express how angry and hurt I am, and what pain your staff has caused my father and our family. Please note that I know it is very difficult to take care of the elderly and give them the attention that they need, but there is no excuse, not to do your job! My father is 79 years old, cannot feed himself, cannot go to the bathroom by himself and sometimes does not know what the day is! But the Colonnades staff failed at doing their jobs and that is to take CARE of the patient, which is my father! I understand he was not the only patient, but he is still my father and the reason he was sent to the Colonnades is for proper care and skilled nursing, and we did not get any of that, my father came home barely breathing. read more
