My 84 year old mom fell at home and broke her femur and wrist. After surgery at OC Global, we decided to have my mom begin her rehab at the Hills. It is always a difficult time when your loved one needs to rely on others to recuperate. Over the course of 9 weeks, the Hills staff, LVNs, RNs, Case workers, PT team, Dietitians, Cooks, Medication Technicians, and Room cleaners took great care of my mom. The people at the Hills focus on helping their patients recover in a graceful manner. The staff helped my mom with showers and feeling her best during trying times. The PT team always encouraged my mom to attend her PT time even when she was in a dark place and did not want to get out of bed. Since my mom was in the short time section (less than 100 days), I can strongly recommend for helping your loved one recover and return back to their normal life. read more

I recommend The Hills Post Acute as a place where the employees care not only about their patients but about the families of their patients. I visited my husband every day while he was there and am grateful to for their efforts to make him better. There were times when their concern for both of us truly touched my heart. It seemed to me that the administration created an atmosphere for the employees that fostered teamwork and this translated into good care for the patients. There were many caregivers who were truly cheering us on, and we felt valued by them. Avoid this place, and keep anyone you care about away. This review is my opinion of The Hills as one of the worst care facilities in the county. The care staff is not the issue. It is the management, the doctor, and the defunct administrative staff, including the manager, case managers, social workers, care coordinators, and department heads. Complain to the Long-Term Care Ombudsman (Google them for county contact information) and Yelp. The Ombudsman is the state run oversight for this type of facility and its services. In my opinion, without the whistleblower nurses who urged me to get second opinions, their advice, and personal communication to me directly, I would never have known any updates. A better option than complaining afterward is not to engage them in the first place. If they have no patients, they won't stay in business, or it will force them to change. Based on my opinion and personal experience, you or your loved one may become a victim of their retaliation. Contact an attorney and protect yourself and your loved one. You and your loved one have rights. Protect them and yourself by contacting Medicare Advocacy and searching for Health Insurance Counseling and Advocacy Program (HICAP) offices in your area. They can help and direct you regarding your insurance and care rights. read more

On one side of this facility you have good staff and on the other side is terrible. Minimal English and it makes is communicate. They twice have wanted to send my Dad home. We filed an appeal and it was granted. Now two days after they are again saying he is good to be discharged. He can't walk on his own, wearing diapers and has open wounds that he didn't have when he was admitted. I'm now going to file another appeal read more

This is absolutely the worst skilled nursing facility I've ever been too! They are so cheap they have no supplies to properly care for your loved ones. If you buy your own supplies the staff will steal them from your loved ones room to use on other patients! This place should be shut down!! The doctor they claim is caring for your loved ones there... doesn't even go there! He sends a nurse who barely speaks English and can answer NO questions about care! The medication is not properly cared for or administered appropriately or safely within the guidelines of the laws! They will take away your loved ones food and give them blended food without notification to you because they do not have the staff to monitor meals or eating! This is horrifying! These vulnerable elderly are eating mush for no reason other than LACK OF STAFFING THERE! There's no Administrative people ever there and they NEVER answer or return calls! There is no accountability because there is NO DIRECTOR OF NURSING!! A requirement by law to have! If you have a bedridden loved one expect bed sores they will NEVER move or reposition the bedridden! Stage 4 bed sores are prevalent there! They go from room to room wearing the same gloves too! Even in and out of the infectious disease rooms! This place is DIRTY! THEY DO NOT CARE THEY LIE LIE LIE and DENY! The ombudsman FORGET IT did you know they are there to help the SNF not you! They are there to pacify you and try to encourage you not to sue! My father was left to urinate in his diaper with no one to help him go to the restroom. When i was visiting to test i pushed the button to call the nurse and no came as well. I went to the nurse's station and the CNA was playing on her phone instead of calling someone. They did not make sure my dad brushed his teeth or shaved. On the bright side they fed him but did not remove the lid of the drink which my dad was unable to do so he couldn't drink out of the cup. They just didn't check on my dad. Horrible care. My father in law recently had a cabg is the medical term. I asked them for certain things for example he got soup he said it was cold I asked them to warm it up and 1min later hot soup. The entire staff is fantastic and caring and are always on top of his care. They did a blood test to find that he was low on potassium and the next day they are giving potassium. He gets pt every day he's getting better every day. He is starting to become more independent. The only downside to this place is the location the area has some questionable people that hang across the street. Other than that I can't say anything bad about this place. Beautiful people that work there, they have a nurse that looks like mc dreamy and even when my father in law drops the button not call the nurse we don't even notice it until they r in the rm asking if we need anything. They have a team of doctors that will be on FaceTime in a minute u think or tell a nurse something is wrong with my dads lucidity. Great experience I have never seen care of this level even when I was in a hospital in Simi or T.O. HIGHLY RECOMMEND THE HILLS POST ACUTE FOR UR LOVED ONE TO MAKE A RECOVERY HERE! After reading a few negative comments I have to say that the facility answered all questions whenever any member of the family called and asked. They even called my husband to let him know that his dads potassium was low and they wanted to add potassium to his current routine of meds. My husband is there daily with his dad and is blown away by their promptness and courtesy. Maybe this place has had a complete overhaul with the employees from what we have seen. He sees the doctor 2x a week at the center his personal doctor will check in on him as well. I needed a place for my Mother to go that was safe and to receive the proper care. With so many choices, I had my work cut out for me. I visited several facilities in the area, but when I found this place I was grateful. The staff was extremely kind and caring to my mothers needs. This place is a joke. Dirty, unorganized. My mom she's been getting the exactly meals for the 2 1/2. weeks that she's been there. They didn't change her drinking water container after 2 weeks and that's because she kept asking. The food is horrible I wouldn't even give that to my dog. I have to take her food all the time because she wouldn't eat there. One time they were going to give her the medication/vitamins twice in a day because the first person didn't recorded, and when you want to talk to someone they never answer the phone or they transfer you to someone that never returns the call. My mom has never seen any doctor there, neither do I when I go visit her. I DONT RECOMMEND THIS PLACE AT ALL! read more

My mom has been staying in this Facility for couple of months and the quality of healthcare services she is receiving is quite satisfactory. The most outstanding issue about the Facility is that the nursing team work passionately with a high level of responsibility and are very much attached to their duties. Just want to give a shout-out to the staff of The Hills. Came and visited a friend. First, the receptionist is awesome. Greeted me and asked reason for visit. 5 for safety 1st. I was asked for proof of vaccination or last covid test. Since I have been vaccinated I showed her my Vaccination card, checked my temperature and log in visitor sign-in sheet. My older sister got teamster from Hoag Hospital to The Hills Facility this month of May. She wasn't pleased at all with the service there since she is terminal with open wounds. Need a bit more care and pain med should be given when she needs. She had to wait for at least 7 hours to get her med and to be changed on her first day there. I wanted to say a special thank you to the staff at The Hills Post Acute. My grandfather was there for just over a month and while I don't live locally I spent 3 weeks visiting him. He was admitted with a stroke and is now home with the great help of their amazing rehab department. I was able to participate in the therapy sessions with my grandfather and learned quite a few things myself. While my family was present as well and we all benefited. I wanted to take the time to say thank you so much for getting my grandfather back home safely to us. I am grateful for the care he received. I cannot say enough about this facility! do not bring anyone here its out of control . administrator is a narcissist and only cares about your money. even after multiple falls no sympathy forth coming. like it was nothing makes excuses instead of addressing the situation . they will guarantee multiple conditions and when you get in they tell you they didn't say that . drugs are left out without giving them to the patient , sometimes multiple shifts of drugs sitting there .they don't have enough staff nurses so there using registry nurses ........so continuity is poor almost non existent. head nurse is m.i.a and I was told she didn't have to return my call by the administrator. patient having multiple falls do to administrator refusal to put up half rails multiple times even when its in the regulations you can have a half rail for mobility needs. equipment old and in poor condition multiple bed changes all in poor or in control fault condition. they really don't know they just gamble with your loved ones body . no staff continuity you get different people all the time so if you have special needs your screwed because they cant remember them. when she got there she was fine and two weeks later she's in the hospital for dehydration and starvation and a uti so bad it almost killed her . staff frequently don't have the items to change diapers ie used bed sheets and rough towels to clean patients there are no incontantince products like wipes at all , as described by the previous reviewer who lives there. read more

my mother is a current patient here and I'm appalled at the lack of care. They leave the residents for long periods of time without coming to check on them. I have been with her the entire day and I've asked 3-4 times to have someone come change her. it's been more than an hour since I first asked. I had to request a few times today to speak with the social worker, who finally came to see me today. I expressed a concern about what room they will move my mom to and she has not come back to let me know any update. no one has come in to check her vitals. I am at a point now where I would like to find another skilled nursing home. This place does not seem to have a care for the residents. read more

My Experience - 30min to be provided my Grandpa's room location - Dozens of help lights flashing as the nurses circled to partake in social hour(s). - I was greeted with my grandpa going into diabetic shock. In a panic, I sprinted through the halls desperately looking for help, only to be provided a series of excuses including "I am on break", and "that is XYZ's job", or my favorite was the silent treatment/walk away tactic. Thank God my wife had the wherewithal to feed my grandpa some chocolates she had her pocket , while I looked for a nurse to help stabilize him. - Once I finally was able to plea enough and get a nurse to provide my grandpa assistance, I was bombarded with a series of excuses of why this was my grandfathers fault. Although he is the patient under "care" and he has not had a diabetic shock in over 10 years... I later was to find out he was not provided with lunch. I could go on but I don't have all day... I am not sure a care facility could get any worse. read more

My dad was admitted here from Hoag hospital after heart surgery. The team did a great job providing him with food items in line with his cardiac diet. He had Denise as his physical therapist and she was able to show him great exercises he can do without over exerting his heart. That was our biggest concern. The medical director here was not my dads primary doctor but he made sure to keep his doctor in the loop. My dad discharged from here after 2 weeks and is doing extremely well. We most definitely would recommend the hills to family and friends. We know that the team her was a big important part of my Dads recovery. read more
