DO NOT LET YOUR LOVED ONES STAY HERE! The nurses are NEGLIGENT, my Grandma, who unfortunately passed away in this nursing home due to their negligence, was screaming out in pain for a WHOLE HOUR before my mother came to drop off food. When my mother came by she had to yell at the people in the front desk to tend to my Grandma. Why did it have to take a person to yell at them to tend to an elder who is literally screaming out in pain so loud you could hear her as you walk into the building? The nurses came to tend to her gave her some kind of strong pain killer after getting approval from a doctor they were talking to and measured her oxygen and said she was fine and kept insisting she was, my mom told the nurse to call for an ambulance multiple times and the nurse kept telling her they did. Ambulances DID NOT ARRIVE UNTIL 15-20 MINUTES LATER. Garden Grove Hospital is THREE minutes away from the care center, UCI Medical Center is FIVE MINUTES away from this care center. They absolutely did not call the ambulance until it WAS TOO LATE, my Grandma had fainted and probably when she passed away and thats when they measured her oxygen and couldn't get a reading and started panicking, when the paramedics arrived they had to start her heart up again and take her to the hospital where the doctor had told my mother she was long gone and she had not received oxygen for FORTY MINUTES. It is absolutely disgusting and terrible how these nurses do NOT care at all this care center should be investigated. If you have loved ones that you love and care about DO NOT send them here, if anything were to happen to them YOU CAN NOT expect the "nurses" here to do anything to help, they will brush your elders off and ignore them. this is disgusting and disgraceful behavior from so called nurses and they should absolutely be ashamed of themselves. Imagine if my mother never came by, those people would've NEVER tended to my Grandma, imagine if it was your loved one and you come to visit to find out they had already passed in their bed because nobody would check in with them or care for them. read more

I don't know who is writing ALL these lying exaggerating reviews? I'm going to have to 100% agree with Vincent, these nurses/doctors need some compassion and more training when it comes to elderly patients. It also wouldn't hurt to brush up on bedside manners. Chapman Care Center is seriously lacking empathy. There is NEVER parking, because the lazy staff parks in visitor parking and not their own. The dated entire facility has a smelly permanent stench, so bad you can smell it just walking up to the front door. The staff does not greet upon entry because they're too busy on their cell phones. You can literally hear patients screaming in pain and nobody's doing anything to help. I would seriously reconsider sending my loved one to Chapman Care Center. read more

Mom no longer is in the Chapman Care Center, and is home. She stayed in the facility for about two months, and overall had a pleasant experience. When she was admitted, she had a great room mate but that lady eventually got better and moved on. That was bittersweet. Mom did not like the food (she's picky) and often commented that other patients caused the atmosphere to be gloom. Overall it was a satisfactory experience for a family going through an ordeal like this for the first time. read more

My grandmother unfortunately fell and dislocated/fractured her shoulder and was transferred to Chapman Care Center for assistance while she recovered. We were really pleased with their services and wanted to thank the individuals who were very professional, kind, and on top of their responsibilities. Andy Yang, was the coordinator, who helped admit my grandmother to this facility. He answered all our questions and made the admission and discharge process easy and prompt. It's stressful dealing with a family member with medical issues but he was kind, understanding, and dedicated to helping us. Miriam (head nurse) was a great asset to the facility as well. She was professional, kind, and top of her game. My grandmother was taking several different medications and she was patient answering all of our questions and updating us about my her health. Gene (social worker) was very useful with setting my grandmother up with various services such as physical therapy, wheelchair access, and other social services upon her discharge. We are all grateful for the services at Chapman Care Center and would definitely recommend them to our friends and family. read more

Unfortunately, my grandpa fell ill during Covid, so we had to send him to a skilled nursing facility. I've heard horror stories of skilled nursing facilities(my best friend happens to work at one), and he didn't even recommend the one he's at. Have another friend that recommended Chapman, and I'm so so so glad we went with Chapman. All of the nurses there are kind and attentive, and my grandpa, a man of few words, has been nothing but happy there. I've been reading some of the other reviews, and felt like they weren't an accurate description of just how kind and attentive the staff has been. I think with anyone, care of our elderly family members can be a touchy subject, but I'm only writing this review so other people in a similar position can feel assured sending their loved ones to Chapman in the midst of scrambling looking for one that'll take great care of their loved ones. I will say, Chapman doesn't come without it's caveats though, so here are some pros and cons I've noticed of the facility. Cons: - Due to Covid protocols, visitations are limited to scheduled 30 minute appointments. (I don't even find this to be a bad thing, I'm glad they're limiting outside exposure, ESPECIALLY because my grandpa has pre-existing conditions.) - Sometimes, we'll have to call to remind the nurses to clip his finger nails or do small things of the sort(again, I don't think this is even remotely close to a deal breaker, I believe that I should be taking part in his care as well) Pros: - Extremely attentive staff that always make sure my grandpa's well being is being taken care of, and they're never late for his timed medicines or meals(I've been there during his meal times/medicine administration times) - One thing I love about Chapman is that my grandpa is always CLEAN(like I've said earlier, facilities in general are understaffed and I've been hearing horror stories of negligence as far as hygiene goes) - The facility is relatively nice and calming, and the atmosphere is always friendly. 10/10 would recommend Chapman. read more

I highly recommend Chapman Care Center for skilled nursing and rehabilitation services. My mother has been treated with nothing but top notch care & respect since I transferred her there from another nursing facility. The entire staff from admissions, nurses, cna's, nutrition, physical therapists, kitchen, facilities etc. are very professional & quick to respond to every question and/or need that my mother has. My mother's health is poor, but her mind is as sharp as a tack. She is extremely opinionated and won't hesitate to speak up to let me or the staff @ Chapman Care Center know if something or someone is not helpful and kind to her. No complaints from my mom. She is happy and loves the staff. I do too. The vibe here is warm and upbeat-like a family- unlike other nursing homes. I especially want to thank Miriam and her entire team for all that they do. I'm constantly being updated on her progress and I feel confident that my mom is being cared for by individuals who want to see her thrive and recover. THANK YOU!!!! read more

This place is the worst. It is every person's nightmare when sending your loved one to a facility. Visitations: - Can be scheduled as early as 3 days in advance - Visits are 30 min - You might get constantly interrupted by the rude Korean lady that monitors the visits - They only allow so many people a day due to COVID precautions Cons: - The CNAs NEVER cleaned grandpa's dentures. During my visits, grandpa would at some point try to take his mask off and before he'd put it back on --- I can see how filthy they are and how denture adhesive wasn't used. There is denture adhesive in grandpa's top drawer where they put it next to his unused tooth brush and tooth paste. - With visitations being made in advance, there were some days the staff didn't not have him prepared for the visit. I don't care what the excuse is. The appointments were made and I know caregivers and nurses have the access to see which of their patients have visitors that day and what time. No care facility I've ever worked at was this a problem for me when I had to get my assigned patients ready and visitation schedules were always available for me to see. Also with the leg up of knowing these appointments vs. spontaneous visits --- staff gets a chance to clean up the pt like dentures and getting them up in bed, not bringing him to me with a very full diaper. Yes, I smelled it one day. Kept wafting in my face during one visit. - I got a call from his assigned nurse one day that he is not eating and lost a considerate amount of weight in a week. I followed up with a different nurse 3 days later and asked what percentage he's eating his meals and she said that staff have been charting an average of 75-100%. So staff had been incorrectly charting at this point because if he had lost the amount of weight that he had, they would have double checked and weighed him again before calling to let family know of the change. - Almost all staff are rude as hell. This includes grandpa's doctor, Dr. Ha. He is also a doctor at Garden Grove Medical Center. Stay away from him unless you enjoy people trying to make you feel stupid. - So, I didn't have a problem with Activities until the end. Activities is the department who books your visitations. They are inconsistent on when they start taking appointments for visiting. Some staff members come in at 830am, some come in at 9am. It got annoying so I started setting an 830am and 9am alarm on my phone to try and call and sometime between 830-9 appointments would get booked when I was told to call back at 9. Eventually, grandpa had to be sent out because I realized he was sick. Not the staff, I did and because I asked about it then they monitored it. At the ICU, the hospital staff were honestly telling me that they've heard the absolute worse about Chapman Care Center. It honestly sucks that this care center is full of staff that are there for their paycheck and I'm sure judging by this facility, it's not even worth it. They're probably literally doing their time before applying for other jobs. This is just a practice gig for some or jobs their stuck at for others and they're unhappy. I emailed the social worker that instead of picking up grandpa's belongings, we'll go ahead and donate it to the facility so do with it what you will. And she still called me to ask if we wanted his dentures back. Did she forget my email? Not understand it? Did they just find his dentures because they put his stuff in storage while he was hospitalized (which they forgot to bring out fyi. I had already asked the staff twice on two separate times before finally bringing it to the social worker's attention. He was wearing other pt's clothing) I also missed a call from Chapman Care one morning and was given a message that grandpa expired at 630am that morning. First of all you don't leave messages like that. You ask family members to call you back. Second, why use the word 'expired'? It's insensitive, but why am I surprised Overall, when I found out grandpa was going to this facility, I was a little worried, but there weren't enough reviews on Yelp or Google to really get a steady idea. I decided to leave a review to hopefully sway others away from this facility. read more

I usually don't write review on Yelp. I know how hard it is to find a good, reliable nursing home for your loved one. We are so thankful we found this facility for my dad. The staffs are professional, friendly and helpful. Plus, they provide good meals (including Korean food which is very important to my dad). I was be able to see my dad having meals through face time/ video calls and was relieved that he was in good hands. I highly recommend this place and contact Mr. Andy Yang who helped my dad to be admitted to the Chapman Care Center. My mother was admitted here for rehab about 3mos ago. The truth was I thought we lost her. To watch her recover and be back to her normal shelf is pretty amazing. This facility is professional, caring and the staff is absolutely amazing. I would like to personally thank Gene Eister for her amazingly hard work. We admitted our grandpa to Chapman Care due to health issues that we couldn't care for at home. In short, he has lung fluid build up that needed to be drained on a regular (sometimes daily) basis, and so we needed to find a facility that would be able to provide a high level of care. The first nursing facility (that I will not name) we found for him was honestly terrible. He would complain that the nurses never were around and wouldn't provide simple things to him like water. At that time, his health seemed to be declining even more and looking back I'm sure the nursing facility had a large role in that. However, only after a couple of months of receiving care at Chapman Care, his health improved by a lot. He has trouble eating and so we drop off boxes of Ensure for him every now and then. The nurses provide it to him at his request and let us know when the stock is getting low so we can replenish it. I'm surprised to see reviews saying the staff is rude because I've experienced the complete opposite. The staff is super friendly and very easy to reach. Ive never had an issue with getting somebody on the phone or at least getting a call back on a timely manner. In fact they call us all the time to provide updates on my grandpa status, which the other nursing facility would never do. Like some of the other reviews have mentioned, the visitation rules are pretty strict and only limited to 30 minutes. I'm not sure if this is a temporary restriction due to COVID, but I personally don't think it's that big of a deal. It does require some planning in you want to visit your loved one though. Overall, I would say that Chapman is a great facility. The coordinator makes the whole process so easy and as simple as possible. I would definitely recommend this facility to others, especially if you're looking for a place with Korean speaking staff. read more

Highly recommend Chapman Care center. My dad was here for rehab for nearly three months after 2 weeks of ICU/emergency care. Because he needed IV care and rehab in preparation for surgeries, his doctor recommended Chapman. The employees and his nurse updated my family weekly on his health and liaised between us and the hospital to coordinate his follow up visits. They made sure he had transportation to and from the hospital. He enjoyed his meals and was able to facetime us regularly using their equipment. Toward the end of his stay looked healthier than he was even before his accident. My family and I are grateful to everyone at Chapman for taking care of my dad and for helping us coordinate care with his doctors. read more

My husband developed awful bedsores so deep, they reached his bone. The stuff told me about it only when they had to transfer him to the hospital. When upper medical staff was asked how it could have happened, Socrates, Ken, Jenna kept avoiding responsibility by saying it's not in their department. Ultimately, you all are responsible for care in your facility. You took advantage of my husband, distinguished Navy Captain, who couldn't talk and couldn't move, to inflict such an unnecessary pain on him. For $700 a day, he couldn't even get his toe nails trimmed, because podiatrist comes so rarely to this facility. This facility is for profit, obviously they want to save on doctors' visits. If they don't have enough medical staff to take care of disabled patients, they should hire more people. It looks like the owners, Adina Cohen, Yitzchok Mandelbaum and other members of Mandelbaum family put profit ahead of patients' care. This family owns several more nursing homes. Please research before you put your loved ones to their facilities, if you don't want them to be neglected. read more

I want to give this nursing home a big thumbs up. My mom was discharged to this nursing home from the hospital. My mom really liked the food, the friendliness of the staff, to the point that my mom did not want to leave because she wants to eat one more day of food there at the nursing home. The staff was very helpful to my mom and her needs. She felt loved and cared for. As her daughter, knowing that this nursing home actually care about the wellbeing of their patients really put me at ease knowing that my mom is being taken care of. I cant even drive past this place without getting mad and wanting to go inside n yell at somone.. My dad was admitted there a few years back he had a urinary tract infection yet the staff there stoppes hia antibiotics after 3 days!! They. Discharged him against my begging n pleading to have him stay be couldnt walk at all but they said he could!! He fell as soon as he got home went back yo the er and thats when we found out his uti was worse!!. My dad died a few weeks later from sepsis.. Maby this place is great for knee surgery or if you have insurance besides. Medical.. Or if your not white!! But ill never recommend this place ever! And i will and have told everyone i know to stay clear from thia place.. My dad passed with other complications as well but the sepsis sped things up and put him in pain n discomfort he didnt need to be in.. If treated correctly!! And by doctors that lnow what there doing! I Could have had another 6 to 8 months with my dad... Bottom line go somewhere else.. Buena park nursing home was great, i should have taken him back There! Wow. Simply wow. This center went beyond my complete expectations. My mother came here after surgery and she had the best time ever. Faith Kim, the marketer for this center, was the one that helped me out. She was the one that helped her out the entire time and she was so polite and awesome!!!! I had no idea there's people as kind and professional as her. I recommend you contact Faith Kim as she is the best around to help you all for any of your needs. Every nurse or assistant I dealt with in this place on December 20, 2017 was extremely rude, not helpful, unprofessional, uncaring group of health care providers I have ever dealt with In my entire life. I couldn't find ONE nurse to answer my question without attitude and they kept giving me different answers. My father couldn't breathe on his own, I walk in to visit him and he's dirty, slouched over mouth dry blood...are you serious ??? Loud and speaking another language to each other while I'm standing in front of my father. Very rude and unprofessional. Told them to call 911 , nurse said ok, 20 minutes later I'm like where are they, another nurse says no we didn't call and said my father didn't need 911 but we'll call and ambulance after paperwork is ready ???? What ? Called his doctor and he immediately said call 911, minutes later all nurses came in to clean him up and get vitals? Interesting. As medics are there they mention oh you guys called but cancelled the 911 yesterday?! What ?!?!? He has been in icu now coughing up blood and was on a breathing ventilator. This place is terrible, how are these people still open. I would give this place ZERO STARS and I do not recommend anyone to come here or send their loved ones here!!!!! And please speak English. I recently spent a week at Chapman Care Center to recover from knee surgery. The care went beyond the care I received even in the acute hospital. Call lights are answered within minutes. The staff is all kind and caring and responsive. It was clean and smelled normal, not stinky and not perfumed, either. The physical and occupational therapy I received was the best, I learned everything I needed to be able to go home and care for myself. I was very impressed with my care. read more
