The nurses in the stroke and cardiovascular unit there were exceptional--compassionate, attentive, and truly wonderful in every sense. Through a coworker, I learned that a family member of hers was already receiving acute physical therapy at Encompass, and she spoke so highly of the professionalism and the facility that I shared it with my siblings. Not sure if manifestation did its magic, but the very next day, Gina, the liaison, reached out to me to confirm our interest in moving forward with the referral. She was warm, thorough, and deeply reassuring, guiding us with clarity and kindness. From the moment we arrived, we were struck by the environment: calm, organized, and thoughtfully designed, with each patient given the dignity of a private room. Gina had advised my mother to bring comfortable clothing and activewear, preparing her for the intensity of acute rehabilitation. The nursing staff met every expectation with grace and care. A heartfelt thank-you to Nurses Lupe, Daisy--whom my mother called her "my friend," a name they shared for each other--JP, Tiffany, May, and all the others whose names I may not have learned, but whose presence we felt deeply. The food and cleaning service crew were wonderful in maintaining a clean environment everyday and nutritious meals! My mom loved the food & it definitely provided her more insights on the importance of eating a variety of foods! I wasn't able to attend most occupational therapy sessions since they were held early in the morning before visiting hours at 8am, but I did have the chance to meet Peter and Cassidy. They were grounded, kind, and made my mother feel safe and at ease during her first shower in a week--a moment she described with profound gratitude, calling it "the best shower of my life." And finally, to our main case manager, Nhã Vũ and the physical therapy team Matt Lê, Kevin Lê, and the therapist with the earring who was described as "Kevin's student," (sorry I wasn't there to catch your name) ,Tâm & Christine- each of you left an enduring mark on her recovery. Even those I did not meet were spoken of repeatedly with admiration and warmth. My mother often recalls how one of you said you would care for her as you would your own mother and you did exactly that. There is something powerful in how she continues to remember these sessions. During home PT, she will sometimes pause mid-exercise, laugh to herself, and recount a story or moment from Encompass. Those memories have become part of her healing. We are especially grateful for the diversity within Encompass, and for the Vietnamese physical therapists who played such an important role in her comfort and motivation. Being able to communicate fully and be understood, I believe, made a meaningful difference in her recovery trajectory. She is not yet fully recovered, but she is undeniably on a strong path forward--built on a foundation of skilled care, encouragement, and human connection. On behalf of Cô Phương Lê and my siblings, we extend our deepest gratitude to the entire Encompass team. The level of care you provided was not only professional, but profoundly human.

When my father had a stroke, he was not placed in a good facility, so we were understandably hesitant. However, this center has exceeded all of our expectations. It is clean, well maintained, and the staff is incredibly patient and kind. I truly could not be happier with the care my mom is receiving. Her nurse, Ayelet, is absolutely amazing. She is caring, respectful, and highly knowledgeable. She treats my mother with genuine compassion, and it shows in everything she does. Our family feels confident and grateful for the decision to have my mom at this rehabilitation center.

Everyone worked hard and I appreciate each & every one of them, especially in social services, Vanessa Almaraz was extremely helpful & responsive to critical questions I had regarding my dad. She was instrumental in helping me navigate the next steps for my dad's transition to a permanent facility which provides care for advanced Alzheimer's patients. I'm very grateful for the entire staff at Tustin Rehab for supporting my dad in his time of need & keeping him safe & comfortable for the duration of his stay. God bless you for the work you do on behalf of those who are suffering & desperately in need of help. You have been a light for patients & their families.

    I would like to highlight some the exceptional caregivers that are there, if I can remember all…read moretheir names. Most recent, Jasmine J. went out of her way today, since I had already called before, to make sure I was able to speak to my mom on a facility roam phone. She even helped her turn it for facetime. The whole staff is good about communicating all details -- medication changes, asking permission for bed changes, any change in her weight, diet, etc. Ivy, the Dir. of Nursing, made sure my mom got her cork board in her room to pin up her photos and greeting cards. My mom was delighted! Joanna, Soc.Svcs., attended to all details of her 93rd birthday, items I sent to mom beforehand and helping serve the Susie Cakes I sent for friends and staff. Everyone was wonderful to her that day. Balloons, Birthday Sash, helped with the wrist corsage and helped run it down when it went missing. Everyone there, the whole staff, in the Dining Room and CNAs, LVNs, etc., made her feel very special. Assistant Nursing Director Krysta was very helpful in getting mom's xrays and for helping set up the dr. appointment. When I have input and concerns about mom's medications, etc., I find the nurses on the phone to take the time to have real conversation with me. There are a couple of special nurses (LVNs?) Mark, Nico that are very good to mom. A CNA Nella (?). People treat her with care. Being so far away, I tend to check in a lot, and everyone is helpful in getting me connected. Jasmine J at the front desk fields all calls and is helpful in running people down to connect me. That is a difficult job. Mom's laundry (marked with hr name) goes missing often enough, and Joanna has been helpful to find it. Marlene of Soc.Svcs, Maria in the Business Office, and, again, Jasmine J. manning the front desk are some people I can think. The Dir of Phys. Therapy Noondita listens carefully to family and follows through to see that mom's PT is appropriate for her abilities. There is also Eric the PT who usually takes my mom and I find them observant and honest about her condition. I just love this department. I will edit this post if I think of someone else. The Dr. Farhouki for the facility is sadly always in absentia, both at Adv.Rehab and Foothill Reg.

The whole entire staff are friendly and loving and most of all they care about their clients. I have been discharged with so much education and relapse prevention skills. And another wonderful aspect is that they have a wonderful alumni group of people that get together every last Monday of every month to celebrate all the people that have gone through their program, and they offer super fun activities, so we all can stay connected and thrive in our recovery. We are eternally grateful for her and Cornerstone and their Alternative sentencing program. She was there for us in our scariest moments and stood by our family through it all! She was truly a guardian angel and has helped thousands of people and their families. Dr. Herring continues to stay in contact with us and shows us nothing but genuine love and support! We LOVE her and are so grateful for her support through all the court proceedings and her heart and compassion. She has become part of our family!     LTDCH provided incredibly compassionate and professional end of life care for our mother. Our…read moreelderly parents had health issues within weeks of one another, necessitating a move from living independently in their own home to needing 24-hour care. LTDCH was able to offer a shared room for the two of them allowing them to be together again after being separated for months in hospitals and rehabilitation centers. Unfortunately, our mother required hospice care as she transferred from rehab to LTDCH. Cristina was able to recommend an extraordinary hospice provider. Between hospice, Cristina, and her professional and caring team, our mother's every need was anticipated and met, as she peacefully transitioned. As our father was also living at LTDCH we thought it would be easiest on him to have lunch following the funeral at LTDCH. Cristina had tables, chairs, table clothes, plates, napkins, silverware, fruit and dessert set up when we returned with our father. Just one example of the above and beyond care that we are grateful that our parents have received at LTDCH.

    LTDCH has been the complete answer to our needs for a safe, warm and home-based environment for our…read moreparents who have lived independently in their own home until three months ago when our father had a stroke. As he transitioned to rehab and we learned that he would require 24-hour care, our search for the right environment began. We needed an environment that would keep him safe, maintain his dignity, and provide for his medical, physical, and mental needs. We knew that we wanted a small, home-based environment after experiencing weeks in the hospital followed by a stint in an impersonal, institutional, rehabilitation center. As we prepared to transfer him from the rehabilitation center, Cristina proactively assessed his needs, arranged for a hospital bed, coordinated home health, medication delivery, and medical supplies. Within his first week she had restored what had been lost during all the previous care, including personal hygiene such as a professional haircut, twice daily teeth brushing, as well as more regular bathing, etc. When physical therapy was slow to schedule she again was able to intervene and expedite with a therapist our father really likes and feels motivated by.  All of these services have come to him at LTDCH, coordinated by Cristina. He appreciates the house and lovely outdoor area in the backyard. Everything at LTDCH, including his soft food meals are approached thoughtfully and served in a way that maximizes his feeling of independence as he recovers from his stroke. Cristina and her staff care for his needs, while encouraging increased independence and maximizing his dignity.
