This place is not good ! The nurses have no idea what the hell is going on and the LVNS are just as clueless! I recorded a video of MY grandma pressing the call button and they came exactly 35 mins later ..... she needed pain medicine. They don't know how to take care of people with a catheter or any serious medical problems. I could go on but this place is already embarrassing as it is. I was vulnerable just got out of back surgery and was dumped on a bed left to fend for myself I got there at 2 never got any service or assistance not a walker I had to scream for my medicine I was at the back of the hall the only people that are walking around are not nurses they are cleaning but the thing is it wasn't clean it was filthy I was afraid of the staff some guy coming in calling me sweetheart in professional and uncomfortable they need to be shut down whoever is in there needs to be rescued I hear people crying for help for basic needs and no response just ignored it hurts my heart how people thing the bare minimum is 23 hours care they should be put in jail then the head nurse argued with me and stayed in more than happy to leave but they aren't giving me my antibiotics or pain medicine so they were holding me hostage they didn't give me a walker so I could make it to the bathroom if my back doesn't heal right it's because of this place I pray it gets shut down what a shit show read more

DO NOT PLACE YOUR LOVED ONES AT THIS FACILITY. The hospital placed my grandmother at this facility by default and unfortunately spent her final days here. At first glance it doesn't seem horrible, but it is absolutely beyond horrific ! The people that work here are extremely unprofessional, don't care about the residence and are just worried about getting their pay check. There's only a hand full of good caretakers there and they've even told me themselves they were going to quit and leave due to the unimaginable things that have taken place involving residence. The average waiting time when you press the light for assistance is an hour sometimes even more. When you go out to get help, they're all sitting there at the nurse station chatting and on their phones. They gave my grandmother the wrong residences medication and then turned around and gave her roommate her medication. Staff writes notes in the patients log stating medications are being given on time and regularly and patients have eaten when in fact that's not the case. THEY LIE JUST TO COVER THEMSELVES. It came to the point where we had a notebook to long every second of the day to make sure she was getting basic needs met. Hygiene doesn't exist in this place, the smells of some of these older residence rooms and up and down the hallways are unreal. The way these people talk to their residence are so belittling and inhumane. No one should have to go through that EVER. My grandmas final days in that place were unbelievable ! "Nurses" arguing with my family about what is in our family members best interest in her last days. Literally refusing to give medication due to personal beliefs despite the medication being hospice orders. Literally stating that we're were going to be responsible for killing her if she gave her the medication which once again I'll repeat was hospice ordered to keep her comfortable in the time before and during her transition. These people have no compassion what so ever. They were so rough and careless while handling my poor grandmother. If you have a heart, do not let your family get placed here and if you have family placed here, please go and visit them, make sure they're okay and move them to a different facility. If I could give this place negative stars I would. Do not trust them with your family. They are not going to get the care or respect they deserve. read more

This past weekend I went down to see my grandmother at the home she was staying at.  It's called Pavilion at Ocean Point.  I had never been there before.  It was a sad time because my grams not doing good and my abuelo died last year.  When we pulled up I was like whoa, pleace is right next to a U Haul.  I was like what?  I didn't what to expect when I went in.  I guess it looks like any other home.  I havent been to one before.  They had a swing set out front and there was actually a few people on it.  That was cool.  When I saw grams I couldn't believe how happy she was.  She said the staff was very good to her and that she said she actually made some friends.  The pace seemed nice.  Grams was happy.  So for now, place seems pretty great. read more

Chances are that a hospital may recommend sending you or your loved one to The Pavilion. DO NOT agree to go to this facility! Our goal from the beginning was to get our mother moved home or at least to a better facility as The Pavilion felt rundown and unsafe. We were also encouraged to do this by a staff member because, "They (the Pavillion) will try and keep her here to use up her 100 days of medicare!" Regarding the facility, there are homeless encampments near entrances of the two Pavillion facilities and its parking lot resembles a junkyard with beds and medical equipment piled and strewn about. The exterior and interior of the facility is old and in need of repairs. There were problems with the showers, flooding, difficulty in moving the bed tables and sliding glass doors, and general conditions such as ease of opening a bathroom door. Furthermore, when our mother first arrived, it took them weeks to find the right working TV remote but then the TV just kept buffering. As a bed bound woman this left her without much stimulus and led to further stagnation. Regarding her care, it was very difficult to get answers to our medical questions and it felt like the Doctor was in name only. We never saw him and the nurses said they would leave him messages but we never got a response. They were not permitted to give us his email. The nurses tried to be helpful when we approached them but rarely had answers. It was like they adopted a "less said, the better" culture. The Pavilion was constantly understaffed and when we needed a specialty doctor, there were none on staff (a previous doctor had left.) When they finally were able to hire someone, it took weeks for a medication change to take place after numerous messages had been left with the staff and nurses. In addition, it was not uncommon for the facility to be short of other personnel as well, including CNAs. The residents often complain about not getting care in a timely manner and ignored. The food was bad and no amount of gravy (God knows the kitchen tried) was going to make it better. On several occasions the food was actually unrecognizable and we had to ask what was being served. We ended up bringing in food to supplement mom's diet. The one saving grace, and I mean the one, was some of the CNA'S. A few, not all, made this horrible situation better until our mother was relocated. Two, in particular, Manjelika and Noel, enjoyed our mom and her quirks and quirky needs, adapting practices to accommodate and encourage her and her eventual progress. They took care of our mom with care and compassion. We often would plan our visits around their shifts so we could be there for when they weren't. Other times we planned to go when they were there. This was all incredibly hard on mom and us and we needed some emotional support, as well. A smile often turned into laughter and we were all lifted. We will forever be indebted to Manjelka and Noel. read more

My Mother lasted 1yr 2 months and passed away there. And she is not the first or last I'm sure. This place needs to be shut down and condemned. read more

My 90 year old father spent 20 days rehabing at Point Loma Convalescent Hospital. I was there almost every day during that period and sometimes several times. The staff is caring and knowledgeable including the nurses and doctors. I was able to speak with the nurses as often as I needed. The doctors did call me to provide updates when requested. One big plus is that the doctors are associated with Scripps so they can and do communicate with the doctors from Scripps. This proved to be very advantageous since my father came to PLCH from Scripps Memorial in La Jolla. The rooms, common areas and hallways were kept clean. I did have a couple of issues with administrative matters otherwise I would have given them 5 stars. read more

The administration in this office hands down has to be the most unreliable and unorganized group compared to all healthcare administrators. Reading the comments of how the patients are treated and how this entity is ran I do not know how they are still running. If you try to call and reach anyone other than the receptionist you will be unsuccessful. They don't call you back, are never available and do not conduct business in a professional manner. No one can answer your questions. Person on switchboard has many other duties, so not always at the switchboard. Nothing was done for a patient unable to walk safely that kept climbing out of bed. No lowering of the bed, no crash mat on the floor and no alarm on the bed.When I expressed concerns over swollen feet and ankles, nothing was done. The rooms need a deep cleaning.There are so many other problems with this place. They are so unorganized. Do your research. read more

This dump deserves ZERO stars....The Point Loma Convalescent Hospital was recently bought or put under new management and they are calling themselves Point Loma Post acute Center. What a joke, I just had to call 911 to remove my father after 54 hours there. THEY HAVE NO AIR CONDITIONING IN THIS HEAT WAVE and the joke who was in charge of his wing just kept making jokes every time I asked when the AC was going to be fixed, so his room was at least 85°F even with the 3 fans on him. They had been promising since Monday when I first went there that it was being repaired immanently....But were just lying because there was never anyone working on it let alone a licenced AC service there the 4 of 5 days last week I was there....When I arrived at 4pm yesterday he was barely responsive and his eyes were rolling back in his head. THEY BETTER NOT EVEN THINK ABOUT BILLING HIS INSURANCE..... We picked this place because it had 4 of 5 stars on Google, a friend's husband was there post surgery and they were happy with his care 10 years ago, and it is only 6 blocks from our condo so I could be there 2X/day + my husband at lunch time. But 4 of the 5X I was there in 54 hours he was receiving unsatisfactory care...They never even gave him the oxygen therapy he was supposed to be getting, let alone physical and cognitive therapy because he still has some ICU Delusions. He can't use his arms or hands yet so can't access the call button, but they couldn't have been checking on him more than every few hours, he was not getting turned/re-positioned every hour, so in two days he was already in extreme pain from sitting in one position, they refused to use the condom catheter the hospital had sent, along with replacement sheaths, and I think they were only changing him 2X/day. If he said he was not hungry or thirsty they apparently did nothing to get fluids into him so he was dehydrated. No one was in there trying to engage him or help him with the TV, which he also could not operate without his arms/hands working.... Most of the care staff were just running around with a glazed look in their eyes from over-work until they realized I was not a staff member and would force a fake smile..... When he arrived by ambulance transport from post acute care at Sharp Memorial he was left on the bed in his dismal hot room and no one was even there to make sure he was positioned and setup properly and it took me 20 minutes to get anyone. I told them he had edema in his hands/arms along with blood-clots, and they needed to be kept raised on pillows, but when I came back with his dog to visit an hour later they were still not raised and no pillows in site. When I came on the second day in the AM the mold-able pillow the hospital had sent to reposition him from one side to the other was thrown in the corner. I told someone he needed to have it under him and be moved every hour, but when I came back around 7pm that night it was still in the corner and his diaper was leaking out about a foot on each side of his under-pad, so he probably had not been changed most of the day....There only seem to be about 3-4 people on the wing he was located trying to take care of probably 40+ people. Rooms that should only have one person have 2 beds crammed into them and the double rooms have 3. There is not even space for a chair for visitors if both beds in a room are occupied. When we got to the ER he had a fever, was dehydrated and had abnormal white blood counts and liver values. And an ulcer after 54 hours in this place. God knows what was happening or not happening when I wasn't there.....He probably would have been dead within 2 days if he did not have anyone advocating for him..... AND HIS ROOM WAS 85+ DEGREES THE WHOLE TIME HE WAS THERE....IF YOU HAVE A RELATIVE HERE PLEASE GET THEM OUT! AND IF YOU WERE CONSIDERING THIS PLACE PLEASE THINK AGAIN...EVEN THE PARAMEDICS WERE APPALLED AND SAID THEY WOULD NEVER HAVE A LOVED ONE THERE......... read more

The nurses weren't always able to answer my questions regarding my grandmother's care. I often had to speak to the doctors to find out what was going on. Also, it was very difficult to get in touch with the doctor(s) since it wasn't just one doctor assigned to my grandmother but a team of doctors. The home lost my grandmother's hearing aid and hasn't done anything about it yet. The first room my grandmother was assigned to had a roommate that didn't get along well with my grandmother and the staff was able to get my grandmother moved to a different room promptly. My grandmother's blood level went critical and she had to be transferred back to the hospital. Once out of the hospital I had her discharged to Kearny Mesa Convalescent Hospital which was much better. There is a trick for parking since the facility doesn't have too much. Park in the Vons shopping mall in the south western part behind all the shops. Behind the shops are lots of parking spaces and there is already a little path between the lot and the home. All the employees must park there. read more

My mother lived at PLCH for four years until she passed away this weekend. She had visitors every single day during that time, so the family got a good look at how the place was run. We have nothing but good things to say about how the staff treats even their most difficult patients. We had some difficulty with billing and reimbursement in the beginning but they were able to solve those problems. The staff dealing with my mom and all other patients in general always behaved in a professional, patient and caring way we will never forget. They face challenges daily caring for people who desperately need them and they do it cheerfully, professionally and with great compassion. This has taken me over 2 years to write, my father was moved here after having a stroke. The first time I went to visit him he was covered in his own urine and his call light was on. He told me he called hours ago for help being cleaned up and no one responded( I later tested this with him and found it was no exaggeration). His skin was red and irritated from the urine,not to mention he was compleatly nude. I informed the nurse and they took their time to come to his room and seemed bothered that he needed to be cleaned and dressed. (This was not the only time, I would call daily to tell them I'd be picking him up and please have him cleaned, weather I was comming or not just so they would properly bathe him)The next day I bought him cloths,I came back 2 days later and they were ALL GONE! they told me they were sent to laundry and probably dispersed to other patients. So I buy him more and put his name on everything, and tell the staff i will be doing his laundry, a week later AGAIN all of his cloths are gone and thay have him in a womans shirt and say his cloths were taken to laundry and lost. They proceeded to blame me for not logging his possessions with the front desk. So I AGAIN buy him cloths , put his name on them and log them with the front desk, and told all nurses I'd be doing his laundry and put up a sign above his laundry basket saying" do not remove laundry done by family". And a week later you guessed it, all gone. This was problem was never solved, and went on , but this is not even the worst of it. My father passed away wile living on this poor excuse of a convalescent home, and I was notified of MY FARTHERS DEATH VIA VOICEMAIL! I recived a voice mail saying " I just want to let you know your father passed away about 10 min ago," I was devastated, my dad was my best friend and they treated that man like dirt! Please PLEASE DO NOT TRUST YOUR LOVED ONES TO THIS PLACE! They are heartless, uncaring, Unprofessional ,rude, thieves. It breaks my heart knowing my father had to live out the end of his life this way. PLEASE DO NOT TRUST THEM WITH YOUR LOVED ONES! Wish I could give negative stars (black holes?). Here's my story: My ex-wife died and we had tens of thousands of dollars in medical supplies, medicines, and medical equipment. (200+ Herparin syringes, probably a few thousand alone). I called PL Convalescent and talked to someone (didn't get a name), who said they would be happy to get a donation, and would write a donation letter for my taxes ("Dear Bob, Thanks for the junk, worth whatever..."). I dropped the stuff by and was told that Edna or Arlene (Business director and manager) would call me. After a couple weeks I called them: 3 times for Edna, 4 times for Arlene. Nobody every called me back, in another two weeks. I finally got some second-tier flunky who said, "No, we don't do that". SKUM! I despise liars; I dislike people who won't call you back. Several of these reviews say the administration stinks. I could not agree more. Go somewhere else with you elder loved ones. That's my two cents. read more

I genuinely wanted to leave a positive review but these circumstances do not allow it. Received a phone call that opened with, "your (relative) can't stay here any longer... He's leaving tomorrow." Before I could do anything, he was transferred with only the clothes on his back. Gone were his hearing aids, dentures, and all his belongings. Point Loma's response was, "well, we have the paperwork that says we sent everything in a box after he left. Yes, he left without anything but we sent it later in a box. Oh yes, he has lots of clothes." He arrived at his new home without even a change of underwear. The photo of him and his brother, gone too. All I can say about this company is I have never felt better to people here really really care about their patients they treat you with love respect and honor the food is great here the staff is great here Physical Therapy will get you back on your feet before you know it because Physical Therapy doesn't give on give up on you and they don't let you give up on yourself they will do physical therapy with you as much as they can to get you back on your feet and if you were going to be a permanent resident here staff is excellent the nurses really take care of their patients CNAs take care of their patients and again the food is very good and if you don't want what's on the menu they have an alternate menu that you can order different food from so you don't have to go hungry because you don't like what they're serving and it is a great place it's very big and everything and I would recommend it to anybody who needs a facility like this and also believe it or not I am trying to get a job here because that's how much I liked it that they made me better and everything is I would like to work for this company so just to let you know that this is a great place to come to my name is Kendra read more

This place use to be very good. My father was here 3 times with 3 different surgeries. I don't know what happened but it's not the same. You call nurses they don't come, you ask for things you don't get it, no communication at all. They will tell you they will do it but they never get back to you. My mom had to stay here for 4 weeks and it was terrible. Patient are not tended to properly. It's sad. My mom did not feel safe.

My dear friend was admitted with only a fractured ankle; otherwise she was in excellent health and good spirits! After six weeks of the "care" there, I had to have her sent back to the hospital ER with a bad case of pneumonia, severe dehydration and a UTI. Before her return to the hospital, I specifically asked for her to be seen by the attending physician; that never happened either. For the most part, her meals were uneatable and the staff did not even care that she was not eating. She was filled with meds to keep her quiet and sleeping. Other than one nurse, it was totally unacceptable care. Not once did I see anyone cleaning her room; the floors, tray, etc. Do not send anyone there whom you care for. Currently, the owners are spending lots of money on the grounds at the main entrance...they might consider using that money for better personel, a cleaner facility and food acceptable for human consumption. read more

I was in a hospital for 4 days due to a gall bladder infection and missing several dialysis treatments and then released to PLCH (Point Loma Convalescent Hospital). Although I am a "picky" eater I found the 3 meals a day were pretty good. I thought the nursing aides were very attentive and treated me with respect. Some of the patients were quite loud (yelling for attention instead of using the call button) but other than that my 2 month stay there was not a terrible experience. Most of the therapists there were very nice and pleasant to talk to. I hope I don't have a need to go to a convalescent again, but I f the need arises I would choose this facility. read more

My mom has been here a year and half and considers this place her home. The people that take care of her and do it with loving care. Every one works hard to take care of everything from the facility itself down to her own personal wheels chair. The rehabilitation department is the best anyone could find in this county. I only wish the food was a little better but they are constantly trying. Anyone who chooses to come here whether for short time or long term makes a wise decision. read more

I was forced to send my grandmother to Point Loma Convalescent center. The nurses said it was the place was the best in the area. Once I arrived, I could"t get past the scent of urine and feces. When I asked to transfer her to a facility closer to where I lived, they wouldn't have it. She eventually ended at Balboa Nursing. We had the best care and rehab I could ever ask for. This is the only Rehab/ convalescent center I would ever put a loved one in. read more

Do not take your loved one to this facility. I have never seen the amount of incompetence from a health care staff than the staff at this facility. Even after numerous complaints and speaking to their directors of nursing many times, corrections are not made and more and more things continue to go wrong. Since our loved one has been in this facility they are on their fifth director of nursing. We even watched, as one director walked out and quit on the spot because of all the mistakes and complaints. She was there for less than 2 weeks. Our loved one is here because we cannot offer the constant care she needs on the daily basis with work, kids, and life happening in general. However, we have to be there every single day just to make sure the medications are administered and administered correctly, the right doses are given as often they are not, the food trays are indeed diabetic trays, we have to make sure vitals are taken and logged, make sure she bathes or we do it ourselves, and have her bedding changed. Simple hygiene is not provided consistently. Call buttons are never answered. Beds and wheelchairs do not have locking mechanisms that work, thus our loved one has fallen several times as the bed or wheelchair slip out when she tries to stand or transfer. Even orders from the her doctors are ignored when we take her to her appointments. Simple things like providing a BP and sugar log are not provided, even when specifically asked for by her physicians. When providing a list of medications, every single time, this list will be incomplete. If you ever try making a phone call to a specific nursing station, do not expect to reach your desired station as calls normally go unanswered after the receptionist transfers you. Do not take your loved one here if you want them cared for properly. read more

I was admitted to Point Loma Convalescent Hospital after a big surgery.I spent 3 LONG days and nights here and although it was over 6 motnhs ago, I can still feel the terror from my stay there.The staff did not wear name tags,took over 12 hours after being admitted for them to get my pain meds,was ignored when I rang call bell...usually had to call front desk to get someone in my room.I was left on comode for over 30 minutes and was not even allowed to wash hands.They refused to change my sheets.I was not always brought meals.They did work on bathroom and the workers had masks on and I was breathing in fumes!I was hosed down in a smelly shower room ,like an animal.The nurses could not always tell me which meds they were giving me.My visitors were wanting to take me home with them as they could not beleive the shape of this so called nursing facility.There is no security either, a friend of mine came to visit me at midnight as I could not sleep and she said the side and front door were not locked and she walked right in!When I was to be transported to my surgeon's office for a follow up, the ambulance came 5 min prior to my appt and the staff at home said it was paramedics fault but the paramedics told me they were never called til 10 min prior!!!!I was told maybe they could have a shrink come in to give me "relaxation pills"...ha, I said no, I just need good care.I left there with a sever UTI and when I left they handed me TONS of my meds and that was wrong....not one staff member was kind or caring or proffessional.I reported them to Board of Health and they found 2 serious citings.I just worry about the long term fiolks stuck there with no voice.Please do not send your enemy there...WORST place. read more

As a RN of 22 years and counting I was abhorred at the quality of care in this facility, Nurse Rachet herself works here. I had fallen, hit my head and due to lack of quality of care from a system unnamed here was not taken proper care of, wound up seizing at home, back to the ED and turfed to this horrible SNF (Skilled Nursing Facility). Let's look at that. First, Nurse refers to Registered or at least Licensed nurses. At this place, the CNA's tell you they are nurses which is illegal and misleading if not dangerous. The few actual nurses that do work there are actually very FEW and hard to ever see. They didn't know anything about my plan of care, why I was there or where the doc was at and kept telling me they had put out calls but I was there for a week and never once saw an MD....which is a violation of insurance and licensing requirements. How is this place still open? I was brain injured, doped out of my head on anti-seizure meds of some kind and still trying to pull myself out of the fog. There was no swallow study and I nearly choked more than once on inappropriate food, not to mention that the trays were not created for the patient's personal needs. I came home at 72 lbs. No one observed hand washing or other standard precautions especially with the monster flu on the loose, and they accessed my central line IV with no gloves on or alcohol wiping. I was due IV fluids that most days I did not receive. With that green flu I was left in my own stool and now have skin breakdown. I'm contacting licensing and will do all I can to follow up in a meaningful way. The guy at the bottom with the therapy dog might have a conflict of interest and any other positive remarks here are either the shenanigans of buddies or some other distrustful behavior. I would give negative stars or zero stars but the option requires at least one. To start with: I'm recently retired/disabled but I worked in the medical field, in various positions, for 22yrs prior to becoming disabled. Unfortunately, I'm terminally ill and have spent time in several nursing facilities in the San Diego area in the past several months. Most of my care has revolved around physical therapy, getting ready for major upcoming surgeries. Although no facility is perfect, the care I have received here is pretty close as far as I'm concerned. I've read some reviews where people have said the inside is a dump and the rooms aren't clean? Sour Grapes in my honest opinion. My room and every room around my room is cleaned daily. And, I mean cleaned. Fresh linens, mopped floors, emptied trash cans and scrubbed toilets. Even if they skipped a day in between, which they haven't, my room would still be cleaner than if I was as home and I am a self proclaimed "Clean Freak". And yes, the food could be better. But, Hospital food is Hospital food. If it were good, no one would ever leave lol! On to the staff. I couldn't ask for any better care than I receive. From housekeeping/laundry to CNA's RN's LVN's. Med Nurses, Social Workers, Activities, Physical Therapy, Occupational Therapy and the Business Office/Front Office Staff. They are kind, caring, knowledgeable and hardworking. I really don't think people realize how much work is actually involved in caring for people in a Skilled Nursing Facility. We, the patients/residents are basically in the hospital sans the Doctors. And the care I receive here is Hospital quality. Believe me, I know. Besides my vast Medical training over the years (I've been a Certified Healthcare Provider since 1987) I have, unfortunately, been hospitalized more times than I care to remember over the past five years. I've been here for almost four weeks now and I would certainly recommend this facility and I was actually referred to this facility by a friend, Thanks John!
