My husband was sent to this...place...for respite for me. From moment one it was awful. The food wasn't fresh or nutritious, no one would help my husband get out of bed and into his wheelchair because he's a fall risk...that's why he needs help, he was left to sit in his own filth for 24+ hours, they had no urinals for patients who have "bags" and they drained his urine into a garbage can liner, he was almost given the completely wrong medicine (if I wasn't there he wouldn't have realized), he had no access to turn his lights off or on because the light switch is at the door, when we asked for a vomit bag, the caregiver made a disgusted face...you work in a nursing home, how dare you make a face at the idea of a patient vomiting. This was one of the most disgusting excuses for a skilled nursing facility...not skilled at all. I normally don't make complaints but this takes the cake. The food and drinks at this place is utterly discusting. My mother stays here and she's seen cockroaches in her food and drinks. What the hell kind of place is this does the kitchen management hire pest control or are they growing a cockroach farm in this dump!? T I'm paying for her stay so I expect it to be sanitary. Trying to find end-of-life care for a loved one is an unfortunately difficult task, as anyone who has ever been in this position knows. After reviewing TLC's website, my mother and I decided to call to inquire about more information. When wee called TLC on 7/26/17, we were informed that we could stop by at any time for a tour of the facility, which we did so the next day, 7/27/17. Upon arriving, I was disturbed by the 5 elder, wheelchair-bound patients who were sitting unaccompanied in the front room. There was no care provider near them, and no one seemed to be paying attention to them. Not exactly a great first impression, but I brushed that off and set off for the front desk. There were two employees at the front desk, engrossed in some very important conversation, because neither one could be bothered to give us any attention or assistance. After a period of waiting, my mother and I decided to introduce ourselves to the possibly-mute receptionist, Donna. Not only did we find out that she does in fact having working vocal cords, we also experienced her great distaste and displeasure for answering simple questions. She curtly informed us that we would not be able to tour the facilities (even though yesterday on the phone we were informed otherwise), and that we would have to wait for Jason, the admissions counselor to answer any and all of our questions. Thinking she might be having a bad day, we once again brushed off that negative experience and waited for Jason. When he showed up, I expected him to be a warm and friendly individual. After all, his job literally includes talking to heartbroken families and taking a shit ton of money from them, so you think he would have manners and a smile. Jason was also extremely rude and curt and told us that they would be unable to provide end-of-life care services, even though their website says they do. In conclusion, there are plenty of care facilities in the Vegas valley. With so many other options for care, please run away from this one as quickly as possible. This place is horrible, the service is bad, I have my friend in there and the other day her daughter came early in the morning and found her with feces down to her knees it was very dry, they don't take own responsibility, they blame each other, we will ask one nurse for something and she will say ohh that's not me let me let the front know, time goes by and no one even come to the room to ask what do we need, this place is dirty, there is trash can in the room bathroom, the whole facility is dirty. Ohh and if you need to use the restroom you have to ask for a key that is clip to a piece of wood like at the gas stations. read more

I cannot stress this enough. If you care about your loved one do not under any circumstances send them here. They are absolutely incompetent. In the short amount of time my grandma was in this facility she contracted a UTI, became so dehydrated that she was hallucinating, and got COVID-19. Her nurses and her doctors had no ideas what her orders were supposed to be, what meds she was supposed to be taking or stopping, not even when she was supposed to be transported to her new home. She was left alone in bed unable to sit up or eat for hours. When she had COVID they sent her away to a hospital even though she was asymptomatic and the hospital sent her back. Zero accountability, the staff here is a joke. The doctors should have to give up their licenses. I can't say enough about how horrible it is here. This is elder neglect at its finest and it's actually pretty amazing to me that they are still an operational facility. read more

DO NOT under any circumstances allow a loved one to come to this facility. My mother was here for 4 weeks of "rehab" and it nearly killed her. I'm not exaggerating. They didn't allow visitors because of Covid so we couldn't see her. She received minimal attention -- they are severely understaffed. You cannot get anyone to answer the phone. No one talks to each other so we would get different updates from different people (i.e., she was given fluids by IV, no she was not given fluids). We never knew what was real. My mother was given a new drug on a Thursday night. On Friday her speech was slurring and by Saturday she was hallucinating. We called anyone we could to intervene and stop the medication. They wouldn't do it without the dr's OK and the dr was not available on a weekend. They wouldn't give us her phone number -- said they would call her and she would call us. Never happened. They continued giving her the drug for 4 nights. Dr. came in on Monday and said "your mother looks really bad. I recommend hospice." I could go on and on. End of story: we got her out of there and into assisted living at The Wentworth in Las Vegas and she has totally bounced back to her old self. Oh, did I mention she got Covid at TLC? Yes, and the dr. there said we had to pick her up because she can't stay there with Covid. What?! We talked to the director and others who told us that's not true, they have a Covid wing and she would be moved there. HORRIBLE place, HORRIBLE dr. (Dr. Choi). So glad we got her out of there alive. read more

So the facility has been closed down for COVID once again after just adding a calendar to see our loved ones. We got the call Staff members with Covid. This is ridiculous! They have been closed down to visitors for months! My grandmother is on hospice. We haven't been aloud to see her forever! And it's not due to the family visiting it's the staff members. They are so careless. They vaccinated my grandmother and we are all vaccinated. The Staff and others deliver admin etc DONT HAVE TO BE TESTED OR VACCINATED! They keep this vicious cycle of bringing Covid into our loved ones. We are all being punished and family members suffering alone! We should be past this! They have no regard for family communications with their patients. The visiting hours and methods to see a loved one are horrible. It's so difficult and the waiting list so long you can never get in. They even make you sign in on-line. It's even impossible to get a FaceTime. I've been trying to get one and no one calls you back. They leave it all on one or minimum staff so you can't connect with loved ones who can't talk or use a phone. Other facilities have so many options in place even the nurses will do it if their designated person is not available. Information doesn't seem to be shared within the staff. The nurses don't know what is going on with their patients. If there is a respiratory issue they will say you need to speak to that staff member. read more

I actually had a friend stay in the rehab part of the unit and his roommate would escape at night through a window for hours because he knew nobody would check on them. I once had to go provide care for the same friend and while i was oit fetching supplies from the charge nurse (Because there was nothing in the room ) a CNA was in the room with him being unprofessional in demeanor and joking that hes asking too much to get a bed bath, she then proceeded to tell all of us that she hated her job and only was going to give it 4 more months. I was transported here from Henderson Hospital 3 days after hip surgery. At first, I wasn't impressed with staff and my room and lack of attention I was used to getting from HH, I clashed with one health provider, complained and TLC took my comfort & healing seriously , thereby addressing this issue with me personally, did their investigations and informed me she is no longer employed here Working with PT and OT on a daily basis has sped up my healing process by in my ability to walk again. They provided me with a wheelchair that's fitting for my size and ever since, I've become active in every activity offered here at TLC, meeting folks here whom are most kind and hopeful to recover as well, I feel like I'm home here, and must admit I will miss all the staff including Dr.Andre when it comes time for my discharge. Yet I'm completely satisfied in knowing that I will be back in a couple months after my hip replacement surgery. My father was a resident in this nursing home for almost two years. The care he received was inconsistent and disappointing, to say the least. I found that some staff members were professional and caring, but the majority of the CNAs who interact with the patients on a daily basis were unprofessional and severely lacking in bedside manners. I strongly recommend exploring other facilities if you are in need of care for yourself or a loved one. There are places out there with caring, competent staff members that far exceed TLC in quality. Again, i felt like walking into a hostile situation. it was about 9:45am, and i already know from yesterdays visit that visit begins at 10am. while at the counter signing in the receptionist was on the phone and i heard her tell the other person to hold on. She then bluntly said, "VISITING HOURS ARE NOT TILL 10" i turned to look at her in disgust of this facility and said I KNOW. I don't like it here and i don't like how that staff are. This shows that they don't give a damn about who's in here. All they care about is keeping the patient in house for insurance payments. For people who are on medicare, Medicare pays 100% up to 20 days. then they have you apply for medicaid to cover the copay after that. We had to go here because we had no other options when my dad was discharged from the hospital. Much of the staff makes you feel like you are inconveniencing them by visiting. Every time I visited, my dads clothes were missing. His phone was stolen 4 times over 6 weeks. They called one day out of the blue and said they are shutting his area down and I needed to move him as soon as possible. And I live halfway across the country. Then I received such a nasty letter from Shannon in finance today with a check from the VA saying change your address. We will not continue to sends checks. This is the only one they had ever sent. And worst of all, allows their patients to sit in pee for 3 hours if they can't get up themselves to take care of it. Management was a joke too, by the way. Because I have had with the staff here, my sister spoke with the lady upfront. We get attitude for having the kids in the lobby. Then ask for her to wheeled out. My brother was a resident there after suffering a massive stroke in February. I fond the staff to be highly professional and compassionate. Any questions I had or his kids had were answered and any special needs were met. Unfortunately my brother's health steadily declined and hospice was brought in to give pain management and care. The staff at TLC worked well the hospice organization. It is a clean facility with area for residents to go outside for fresh air and sunshine, weather permitting. I loved every thing about this place. The staff was universally professional, kind and patient, with me and especially with more difficult patients. The food was good, service was smooth, and no special request was ever refused. The nursing staff, although constantly busy, was always smiling and friendly. The physical therapy was exceptional. They stayed late to teach my husband how to help me at home, left me strong and confident in my ability to do things like getting into and out of a car, transferring smoothly from wheelchair to other seating. They brought in a variety of entertainment and provided activities for all. The building is attractive, clean and well maintained I felt cared for throughout my stay. read more

My husband has been stuck in this hell hole for two weeks! This is by far the worst rehab in the state of Nevada. He has been left several times on his bed pan over 30 minutes. It takes sometimes ip to an hour for a nurse to come in the room . I have had to call several times to ask them to go help my husband who has left side peralisis. I cant even get them to put his leg brace on him daily i cant get hospital records i cant get them to schedule a drs appt i. A timely manor . There are only a couple nice nurses or cna's. My husband was sent there for rehab to learn to walk. He is going down hilll and his condition has worsened by being there. Trying to get him out of this nasty place. Well...some of the RNs and CNAs are fabulous. But several should not be in health care. It often took 50 minutes for the nurses to answer my call for help using the restroom. Don't expect to be cleaned up if you are using a bed pan or given a wipe. Twice I had to call the front desk after waiting almost an hour. One of the CNAs lied to me about something I allegedly said. After complaining about a night CNA I was told I was the 3rd complaint that morning. Beware the therapists. I saw two of them together 2 or 3 times. These two were negative and condescending and they did it with smiles on their faces. I should have spoken to a supervisor the first time. I had to be put on antibiotics while I was there. I found out what was wrong with me from a social worker and an outside doctor. The roommate was loud, disgusting and angry. She complained about absolutely everything-food, medicine, laundry. I don't know how many meals she had changed or sent back This woman needs a private room. No one needs to listen to this. read more

My grandmother was moved from the hospital to TLC. I hate this place. I live in CA and every time I've gone to visit, my grandmother is in fecal manner. Her cross necklace was taken and her clothes are always missing or on someone else. They say they are suppose to check on her every 30 mins but they have no staff and just recently found my grandmother on the floor in a different wing of the facility. I need help if anyone knows of a different place with loving and caring workers. This place doesn't even care if your loved one has been bathed or their die per changed. I spent 6 weeks at TLC. I was transported from Henderson hospital. I had walking pneumonia, my heart stopped twice, they gave me CPR. The person who pounded on my chest must have been 300 lbs. (it did not matter, he saved my life.) Anyway, when I got to TLC I could not move even a finger. The pneumonia was so deadly bad I could not explain the chest pain. Obviously, I should have stayed in the hospital not go to a rehab. They wanted me to go to OT and PT the first day. I was not able to move any part of my body, but the staff at TLC did not believe me and all they kept saying was "Your not in any pain". They picked me up with a sheet and dropped me down to get me in a better position. I never screamed so loud so that the CNA put her hand over my mouth until I couldn't breathe anymore. She kept saying I was making people think she was hurting me. I reported it to the head nurse and made sure she couldn't come in my room again. The experiences I endured was like being in a horror movie. I would not EVER RECOMMEND TLC TO ANYONE. The place should be closed down. One of the young CNA that didn't believe me finally looked at my file and she apologized for hurting me. When my Doctor came in one night and after he left I asked the NURSE what his name was and she told me there was no man in my room and I was imagining it and she said my doctor was an associate CNA. Physical therapy and occupational therapy team are awesome. Roy, Tanya and Valerie were hands on awesome. They worked with you, pushed you and made sure you were comfortable with the exercises. This facility is shit. And I mean complete shit. Worst facility I've ever worked at. Actually, worst place I've ever worked at in my life. Management is shit. The whole focus of this building is who's hosting the next potluck/party in the gym/nurses office. The only time this place is working is when states in the building. Let's not mention the on going problem with the kitchen. Roaches/rats, cold food, inedible food. And don't think a complaint is going to do any good. You either eat it, or go hungry and then hope tomorrows food will be better. But it never is. Oh and to future and current employees, you better use that PTO before you quit, because you can't cash that out. And if you do request time off, you are either told to switch days with someone else or they give you a real hard time. And that's only if they even approve your request to take time off. Also, if you've been there for years, you'll be able to cash it out, but only 75% of your pay wage. But, if you're within managements tight circle, it's a whole other story. I've never worked at a place where I've felt so confused as to what it is I'm doing on a day to day basis. Rules here are so vague and grey. No set rules unless state is in the building. Also, there are no raises unless you been there for many years. No matter how compliant you are, how great your attendance is, how dedicated you are, if you're a team player, flexible and don't complain about anything. You're most likely not getting a raise. I worked there 3 in a half years and only called off twice, and got a 66 cent raise at my 3 year mark. Therapists have to bring in their own computers to do charting cause the owner is too damn cheap to add or upgrade anything in that building. Wifi is slow, most of the charting pads on the walls in the halls don't work, microwaves are ancient, gym/kitchen/employee hall smells like sewer run off, beds are ancient and half the time don't work. Oh and they for sure won't increase the pay for the dedicated and hard working CNAs/nurses/transportation/schedulers/maintenance staff etc., but they sure are willing to pay a whole lot more for agency nurses and CNAs who do absolutely nothing, and have no respect for the patients. If you hate your family member/significant other, then send them here. This place will literally suck the life right out of you whether you're a patient or an employee. Before your loved one gets admitted to the facility, make sure you say your good byes, cause it'll prob be the last. The rehab department is above and beyond awesome and goes out of their way to help out in anyways that they can. The DOR is a back stabber, two face, disrespectful, a gossip, salty, and a bitch. Worst boss I've ever had. Ernie was the best OT supervisor I could ever ask for!! This place needs to be shut down. There's nothing great about this place. You get no benefits except for cheap insurance and your check. Absolutely no incentives. If you're desperate for a job, hardly have skills, and hate working, then trust me, they'll hire you. If you're a fantastic employee, they'll hire you too, but don't think you'll get recognition, appreciation, and respect. You're just another employee covering up space that's needed to be filled. This place is the worst. My 91yo mom was sent there 5 days ago because due to the Covid-Viruse it was the only place that would accept her insurance and had a room available. I have tried to call and speak to the someone at the nurse station to find out how she's doing, and have yet to get through. First of all when you call the main phone # it rings about 30 times or more before anyone answers, almost every time I call. Then the operator gave me the nurse stations direct line. I called that number and get a recording saying that it's a non-working number, called back the main number and she transfers me, same thing the phone doesn't even ring. Called the main number again and she transfers me to another nurse station that is near the one I'm trying to call. At least they answer and she tells me she'll get in touch with them and have someone call me. OK about an Hour later they call me back. Well that was OK finally. I can't go there in person because of the virus no visitors allowed in. I called again today to find out how she's doing. Called the nurse station number again, let it ring for 6 minutes and 38seconds and no one answers. Called the main number again, she transfers me and the same thing, it rings and rings and no one answers. Called main number again, explained that no one answered. She gets snippy and says "well their busy" and transfers me immediately again without saying anything more. again it rings and rings for 5 minutes and no one answers. I don't know what to do. this place is terrible. I can't call moms room direct because she is 80% deaf and can't hear on the phone. I WOULD NOT RECCOMENT THIS PLACE. read more

When he eventually appeared he had no idea what I was talking about when I told him that we had spoken yesterday. I am very seriously looking for a facility for my father ... I would not even want my worst enemy to stay here. We saw many residents just sitting and staring or wandering around. One star goes out to the physical therapy staff and the other star goes to the occupational therapy staff. They were the only good thing about TLC even though due to insurance they only have a limited time with patients. I was a patient here for about two months and I'm a witness to how shortstaffed they are. There is not nearly enough nurses for them to be able to do their jobs right! There is not nearly enough CNA's to be able to do their jobs right either. I was left waiting multiple times for more than 45 minutes just to get help up to the bathroom amongst other things. Fortunately I had friends and family helping me through this. Unfortunately most of the other patients in the facility did not. The amount of neglect is ridiculous. There is not enough staff to be able to walk patients around there for delaying the healing process. and the food is no better in the healing process some days it was barely edible...Nothing good comes from being in a wheelchair or your bed all day. I strongly suggest if you are looking for your family or friends to look at more than just one place before deciding to put them in there. As for the activities staff they were all so awesome making a horrible place a little bit better with a smile on their face and my Maganda that brightened my day every day while she cleaned my room making it fresh and helping me start each day with a smile. read more

My Uncle has been a patient here for about 2 years now. When I first started visiting him in the beginning I would have rated the facility a 3 star. The food has always been good and you could tell the nurses cared, however, the leadership was a little dismissive at times. Now, almost two years later they have a new leadership team in place and you can see and feel the difference. I had an opportunity to sit in on my Uncle's care plan meeting and they heard my concerns and followed up. When I raised a specific concern in the meeting the Social Worker grabbed the Head Nurse and one of the administrators to hear my concern directly. I am truly happy to see the positive change here. Plus my uncle loves the food and the activities - he even loves his regular housekeeper who cleans his room everyday and chats with him. read more

After being admitted to this facility my elderly friend received a few days treatment and then was constantly badgered to sign into a hospice facility called Solari. She was told by TLC that she could stay there up to 100 days. Then suddenly, after less than 2 weeks she was told she had to leave...period, no notice. Once again the Solari rep showed up only this time she stated that she could only stay 2 days in Solari. Moved her the following day. TLC on site social worker was usually no where to be found and never returned her calls or the many pages the receptionist made for her stating "I know she's here, I don't know why she's not responding". Run, don't walk from this facility. Picture yourself here and you would pray someone would rescue you. My Dad is a fairly new patient, less than 2 weeks and we are trying to move him already to somewhere else. The PT person took him to the lunch room to each his lunch and assured him he would be "right back". This turned into an hour and a half and he was stuck in a lunch room with NO ONE in site. Finally after finding the PT department to have someone come and take him back to his room, they had a sign on their door " Be back at 1:00 pm". It was 1:15 and when we knocked they were happily dining on pizza with not a care in the world about the patient they had forgotten. This really spells accountability and caring right? Today, they left him in a sitting in a wheel chair for 4 hours . He was scheduled for PT at 11am and once again, no one in site. I have no idea why they are in business. If this was a private company the doors would be shuttered. This is funded by Medicare and they have no reason to work hard because there are NO consequences. We like our Doctor, she is compassionate, smart and caring. However, we only see her perhaps twice a week for a very short amount of time. We understand she cares for many patients. First impression lasts. Don't they know that? Can yelp allow us to give a 0 star? Jasmine the front desk is not in anyway interested to help us. My mother had to be transferred from the hospital to a health care facility and we want to check first where will we transferring her so we drove 25 minutes but to be treated with not an inch of consideration or care. Even the head nurse can't help us show at least how their room condition looks like? They need to change their name! TLC? Tender Loving Care? It doesn't fit at all! What a joke! read more

I am giving this place zero stars. My grandmother was sent to this hell hole, to build and regain strength. After being at TLC for almost 10 days, my grandma had been vomiting large amount of green vomit for days, Her blood pressure was finally checked and it was so low, they rushed her to Er. My grandma is now laying in ICU, with the worst Pneumonia one can have. All of this is caused by lack of staff members doing what they are paid to do. ( Care for the patients) When my grandma arrived at the ER, photos were taken of her bed sores, and amounts of green vomit. My grandma was severely dehydrated, mal nourished , and know where near where the recovery she should have been. According to several staff members at the hospital my grandma is currently at, say that TLC is well known for all of their neglect. I will go to my grave fighting to have TLC's doors closed and pray that all staff members get what they deserve when they need care someday. I am aware that when we get older, we will pass.
