I had high hopes this was going to be a good pleasant experience. Unfortunately 3 weeks into service our needs and requests have not been met. Made the decision to make a change as this is not the type of care I choose for my cousin. Don't be afraid to make the change if your gut tells you'd one thing is not right. End of life is hard for everyone, however no follow up for 3 weeks is unacceptable. read more

there was no care quality****. i highly recommend that you do not hire transitions to care for your loved one. in my opinion my husband was overdosed with patches by the nurse and by the time that i was told from the main office to remove them but it was too late. he had a major reaction to the overdose and his throat, tongue and lips swelled beyond belief. medication, food and liquids stopped because he could not swallow. he died 15 days later. i was promised that he would have a nursed assigned to him and that was a lie. one of the nurses that came stated that she had nothing personal to go on as far as his condition because she only saw him a couple of days in the six weeks. they kept sending different nurses. he was swollen and i told a nurse and she said that he was holding water but it was his bladder and they had to catheter him, in doing that they hit his prostate and he lost alot of blood. this is only some of the things wrong. I lost my husband last week, and hospice care was there for me, from the start of his journey right up to the very end. They handled everything and I was left to say my final goodbyes to him in private. They were wonderful in giving me comfort and peace of mind. If I could, I would give zero stars. I have worked with two other hospice companies previously, and they were great. Transitions is horrible, and I fired them after two weeks. All seemed good when I first spoke with them, but quickly went downhill. Without boring you with the details, the bottom line is their communication was horrific, staff was disorganized, record keeping appeared to be non-existent, they didn't follow through, couldn't share any details about my mother's care plan, couldn't tell me when she had been seen - or by whom and what they did, and were unresponsive. I quickly started asking a lot of questions because their communication was so poor. There was a different RN every time - went through about 6-7 in 2 weeks. Finally they assigned one RN to be the case manager, she had a family tragedy, but they never told anyone. I was calling and texting her - no response (understandably), finally had to call the office. Don't believe for a second they will call you back in 5-15 minutes. They escalated my call, after 5 hours of no call back to an 'escalated' situation, I called again and was very upset that no one ever responded. Still took another hour to hear from a supervisor. When I told her they should have immediately notified the families this case manager was working with, to detail an interim plan, she said "oh, that's a good idea'. Really?!? A business like this doesn't have a backup plan in place?? That was just one issue. Transitions nurses let my mother's medications run out, never followed through on getting meds ordered, every nurse had a different list of meds - some of which are virtually deadly to my mother, and she had never been prescribed some of them. They don't share details or family input/requests internally, did not address her personal hygiene as they said they would, missed showers for a week at a time, and every conversation with an employee started from scratch. When I called the social worker to do a hospice to hospice transfer, she asked if the owner/owners could call me to get my feedback. I said yes, and of course - they never called. Kim, the social worker, was very helpful, about the only one there that was. Though I agreed to speak with the owners, it seemed pretty clear in my experience that they didn't care one bit. I have since found Suncrest, who has been phenomenal. They have clearly documented all issues, and everyone I speak to has the same information. They have the same RN seeing my mother twice a week, AND the same aide visits my mother 5 days a week - Mon to Fri. That is wonderful, and they are building relationships with my mother. With Transitions, it was always somebody new, aides only visited maybe once n 2 weeks (they claimed it was documented, but they couldn't find that information) and it was causing confusion with my mother. I am so disappointed - they seemed to be more focused on taking her off medications, seemingly to reduce costs, since they get a flat fee from Medicare. Suncrest is not taking her off ANY medications that have been prescribed by her physician. I am glad others had a better experience, mine has been the worst I have ever seen - and I have worked in healthcare for over 40 years. Steer clear. read more

Our experience with Transitions Hospice was not a positive one. Here is just a short list of just some of our concerning experiences: 1. They were slow to respond to the patient's changing needs and family's requests. There were several instances where we requested comfort items for my mom and waited days to receive them. The worst example was we waited 1.5 weeks for booties. We did complain about this, and the company did get a little better with the turnaround time. The company is out of Huntley, and we were in Rockford, and they have a huge territory which I understand causes the delays. 2. The pain management was not tailored to my mom's needs. They administered morphine for EVERYTHING until the family advocated for more personalized comfort care. 3. Communication was sub-par. This was a two-fold concern. The communication with the family was done thru a folder left in my mom's room. Little was written in it for months. The family was looking for basic information about medication administration, bathing, etc. Then, as my mom began to decline, information like vitals were recorded in the folder. But, it was recorded in medical shorthand and jargon, and my family had to make an educated guess as to what it meant. Which is not only an ineffective way to communicate with a family, but it almost served to alienate us. In the age of technology, this company could certainly update their communication mode and practice with families. Moreover, communication between Transitions and the facility could best be described as "The right hand not knowing what the left hand was doing," with both entities blaming each other. I can't tell you how many times our family caught miscommunication or non-communication between Transitions and the assisted living facility. It was very unsettling, to say the least. 4. For my mom's final hours, a hospice nurse from Chicago was sent, not one of the two hospice nurses that knew my mom and family. He was a complete stranger to us, in the time we needed comfort the most. 5. And finally, not a single representative from Transitions attended my mom's funeral services. Not even the social worker. Several weeks after my mom's passing, we did receive a company card from Transitions. The only positive I can say about the company is the CNAs are wonderful. read more

The hospital recommended several hospices to help my mum on her final journey. They all came to my home and explained how little they could do because they could only do what Medicare would pay for. My mum and I both had special needs, and I was very uncomfortable with the options. I decided to search for options myself and found Transitions. They came to my home and explained they exist because of people like me and my mum, with special needs. It seemed to good to be true, but it wasn't. Transitions does more than over-the-phone triage. Ever time I called with a concern, the provided triage over the phone AND sent a nurse as quickly as possible. I think the longest it took was an hour - in no case was the need urgent. No other hospice would commit to sending a nurse each time. One night, mum was very agitated, and Transitions sent a nurse who sat with her the entire night so I could rest. When I woke, mum was calm and resting. Their focus was on love and assisting with the transition from this life to the next. Their first answer was care and compassion, not drugs, although drugs were used as appropriate. They offered 24/7 sitting service as mum's time came closer at hand. In my case, there was no 24/7 sitting because mum passed so quickly. So instead, as I mentioned, staff sat with her. The pastor was also a comfort for mum, a friendly, soft-spoken man who truly listened to mum's concerns. All-in-all they were the calm and professionalism that gave me and mum comfort in her final days. read more
