After submitting this information, however, the claimant and service provider will be confronted with a demand from the insurer for considerably more, and much more detailed, information to be submitted by a near-term date after which the claim can be denied. In other words, the sales pitch for this dental benefit is a bait-and-switch: sign up for it under one set of conditions and then, when you attempt to obtain the benefit by satisfying these conditions, be confronted by a demand to satisfy other, undisclosed conditions transparently designed to indefinitely defer if not ultimately deny approval of your claim. I have a bulging disc in C67 and had major attack in June of 2024. the symptoms almost drove me crazy with pain- I couldnt lay down or put on my socks- and these "people" denied me an mri three times in six months from three reputable high end doctors/surgeons. Technically they have hearing aid coverage with their Medicare Advantage Plan. However if you miss one of their cryptic rules for getting a hearing aid they will hide behind those rules and deny reimbursing the cost of a hearing aid. I would give them zero stars but not an option. On a positive note the customer service people have been professional. One month with UnitedHealth Care Advantage plan - one month too many. RUN from this company if you are looking for health insurance . They pushed back on the medication for an existing condition (40+ years severe asthma) saying I had not tried all the other available medications INCLUDING ONE I AM CURRENTLY TAKING! They denied the medication then denied the appeal. The same medication had to be approved with BCBS every couple of years for the past 16 years however their process was smooth and professional. Adding to the challenge UH is saying it's the specialty pharmacy that is denying the prescription. The specialty pharmacy is saying it is UnitedHealth denying the prescription. The Doctor's office said the insurance company denied the request. Ummm I'm betting it's UH. While writing this review I have been on the phone with UH - one hour and 19 minutes only to be told I need to call back in 48 to 72 hours so their system can be updated with the latest interaction with the doctor! I am newly retired and never had to deal with such incompetence when it came to health insurance. WOW - I wish BSBC was not so expensive but I guess you get what you pay for. read more

Horrible billing practices. These guys generate invoices a month prior and bill for employees on the same invoice were they are giving credit for after termination. If your considering UHC for group healthcare I'd strong encourage you to look elsewhere as they can't even bill you correctly. This isn't just a one time thing, they repeatedly screw up and their billing statements are never accurate. These guys are a joke. Invoices are generated on the 15th, mailed on the 30th.. yet due the next day. How is that even possible? 5 years of being a customer and going through audits and no one know's their head from their behind here nor can get anything straight. We received a bill for 10k out of nowhere and they want us to pay it, and then credit us next month for it and cancelled are coverage with no warning. In August, United Health Care, AARP Secure Horizons, mistakenly disenrolled 150,000 from their vision care coverage. It seems that almost everyone has just accepted that they don't have coverage, some have contacted United Healthcare. I, myself, have spent over four hours on the phone with United Healthcare. Very nice people there have been supplied with inaccurate information to share with complaining customers. Of all the Secure Horizon patients enrolled with Healthcare Partners, I and only I have managed to get reenrolled. On October 22 I finally had access to my coverage. I went to order lenses an frames today, only to learn that they dropped me again on October 28. After another hour on the phone, they finally accepted what I was telling them was true. They need to resubmit everything to their vision department, and I might have my coverage again next week. Please check to see if this might be happening to you, your parents, or your grandparents. Call United Healthcare, and insist on talking to a supervisor. If they try to tell you that it's all been fixed. Demand resolution before you end the call. Be prepared to spend an hour on the phone, most of it on hold. Write down names, make notes of what transpires on the call. In September I wrote to United Healthcare, and never received a reply. I assume the next steps should be reporting them to AARP and Medicare. Anyone up for a class action suit? read more

I had an aggressive squamous cell carcinoma. After waiting over a month I never received authorization to see a surgeon. After a month they were sending me to another dermatologist to discuss my "skin problems." I had no other choice but to seek treatment myself. They refused to reimburse me because I didn't see a network doctor. If I waited for authorization I wouldn't be writing this review. I had to pay $2,000 for the surgery. They NEVER reimbursed me. Help, I need to find a decent insurance company that doesn't steal your money! read more

Our mother died and we notified United Healthcare to stop drafting the checking account for premiums. They assured me they would stop drafting the checking account. They did not. They took out almost $400 for the next premium. It's now been a nightmare trying to get the funds back in the checking account. The customer service is absolutely horrible. I've even written the executives of the company, without any response. Now, we're having to deal with this as we grieve over the death of our mother. If you have a choice, DO NOT USE UNITED HEALTHCARE. I'm 71 years old and feel like they are abusing the elderly by trying to keep their money. I'm sure some of the elderly just give up the fight. I have had to use this horrible health insurance from my employer for the past 20 years. Each year the quality of care and this insurance companies refusal to accept responsibility for their lack of patient care and the criminial behavior of their staff in violation of the HIPPA regualtions is shocking at the very least. I had a question about a claim and after 32 minutes, 3 people and was on hold for 98% of the time because they had to "pull up multiple systems." Even after that, they couldn't answer my question. The absolute worst coverage I have ever had. Do not choose United Healthcare! The main issue is getting a claim paid is very difficult. They do everything they can to not pay it. Numerous times I have had claims denied for simple charges and was only able to get them paid after going thru an extensive appeals process. In the hopes of avoiding denials we began calling them before any test, lab work or procedure was done to see it they would cover it, only to get vague it depends answers. None of these tests were unusual and all was ordered by doctors in their network, yet they still refuse to pay. The company is for the stockholders and could care less about its customers, last time I checked they were a $1billion plus in annual profit business and this is from making premiums high and payment of claims a challenge at best. Finally beware as when they do pay they are one of the lowest paying insurance companies, so don't be surprised if you doctor, lab etc. sends you a bill for the difference between what United pays and what the provider billed. This is my 1st year with an hmo and I chose UHC against my better judgement. I realized immediately, the good doctors in the greater newport beach area were not taking new patients. The catalog United Healthcare sent me was bogus. Either the doctors hadn't been with Uhc for a long time or they no longer took patients. I went online and even the website was not up to date. I was smelling a rat from the getgo. Here are main reasons you don't choose this company. 1. You call the number on the card and you get a very nice person who will hold you on the phone for an hour while they research for answers and return with I'm sorry, no one knows the answer. What??? 2. You get a number of phone numbers to call for an answer, they are disconnected or the person that answers doesn't know what you are talking about. 3. They don't document your call, so you have to to repeat the same story to everyone. Trust me that gets old after several months of searching for an answer to the same question. 4. They don't use email. Could that have something to do with documenting? 5. If you want to complain, you write a letter to a nondescript po box in cypress. Your reply is from a sweet person who has a minimal grasp of the english language and repeats "you file complaint?", so, you no file complaint? what?? Three weeks later you get a call from a surly lady from minnesota who knows you are gonna be a problem and cares less about resolving it. And then she hangs up on you. 6. They have a huge company that is not on a network. In other words, the left hand doesn't know what the right hand is doing. The right hand is not going to stop until they choke the life out of you. 7. If they choose, you can't stop them from changing you provider. Here's a trick, keep calling and change it back. Oh, you will have to sign for the certified letter they send you, telling you to stop it. Did I mention, they like paperwork. Don't worry, remember, they don't document what they do. 8. You will get a new member card in the mail everytime you call them. Something in their effort to inform you of what you should know already? 9. 1/1/2017 Ruth (89) is living with daughter (67) Dian in Simi Valley. She has had continuous coverage with AARP Medicare Complete serviced by United Health Care for several years. She now lives in Simi Valley and has a UTI serious enough to go to the hospital. While in the hospital, I (Dian} am informed she has no insurance. I know we have been paying from direct withdrawal from Ruth's Checking account. I contacted the Customer Service department and they confirmed she was actively covered. I relayed this to the hospital. The hospital shows she is inactive. During February, I was reassured that she was covered. In March a representative took the time to really look at the situation and told me that the customer side of their system showed Ruth as Active, but the provider side (Doctors and hospitals) show her inactive. This began a long process of many times promising to fix it but not happening. I contacted the billing department over several months, each call lasting over 1 hour, promising to fix it in 10 days or the shortest 48 hours. It never happened. I explained to all of the people that Ruth had AFIB, needed to have constant monitoring of her Coumadin levels and zero referrals are being processed. Since January she had no further medical monitoring. It did not matter how severe I told them it was, they kept promising, but no action. June 1st, Ruth is entered into the hospital with a brain bleed. Her blood pressure was extremely high and her Coumadin levels were too high also. I am sure this was caused by the lack of monitoring due to the insurance company ignoring my complaints for 5 months. The medical group Ruth was assigned to also assisted by appointing a case manager. She was also ignored by United Health Care Provider support. Twice (in June) I was told the problem would be fixed and I would be called every day until completed. It never happened, and the person who said that was unreachable. Ruth was discharged on 6/7/2017 and we still have no referrals because the computer is still not fixed. She has been released and is getting no support or medical attention 9 days later. She is on new medication, not being monitored. She is on a restrictive diet, not getting therapy. How long before she returns to a hospital that has received no payment, she is in a serious medical conditio0n and the UHC is acting like they have no care in the world. Why can they not fix their system issues. My mom is really in jeopardy of having a critical reaction to the lack of medical care. Dian read more

Staff are NOT well trained! During Open Enrollment, my husband spent day after day researching insurance companies/plans. In addition to plan coverage and Rx, he wanted to make SURE that all of his specialists were in network with his PCP. Again, spending an immense amount of time on the phone with representatives, he received confirmation - and double checked the confirmation that indeed, all of his health care providers are in network with his PCP NOT TRUE!!! NONE OF HIS PROVIDERS ARE IN NETWORK WITH HIS PCP - CONTRARY TO WHAT HE WAS TOLD BY THE REPRESENTATIVE! Now he is being sent all over the place and told that he has to start going to new specialists, receiving denials for "out of network" requests. To make it worse, he has a mobility disability and some of the criteria to choose his long-term providers is based on disability accessibility (parking, office location in relationship to the parking area, etc.) However, United Health Care is not concerned about access to services. UNITED HEALTHCARE GAVE WRONG INFORMATION! IT'S JUST THE BEGINNING OF THE YEAR AND HE CANNOT CHANGE INSURANCES AND UNITED HEALTHCARE WILL NOT DO ANYTHING ABOUT IT - JUST "TOUGH LUCK, BUDDY! The worst experience I have ever had dealing with an insurance company, every time I call I get a different answer. I asked one agent if they could fax some information to a doctor and they said "we do not fax" I asked "why not is it a legal issue or something?" she said "nope, we just don't fax" it went on and on like this with almost every issue I have had to contact them about. They are impossible to deal with, the sad thing is that some of the reps are trying to help you when you reach them but they are limited. I would not recommend this company to my enemy. If you are a decision maker in HR for any company, please do not screw your employees by supporting United Healthcare. They literally deny all my claims and I have to constantly fight with them on the phone. The amount of stress that this company has caused me has been so great that I no longer use the insurance, I just pay cash for everything. My neurologist charges me $100 an hour to make phone calls on my behalf due to the extreme amount of time wasted waiting on hold. They denied me of (2) MRI's which were ordered from my doctor because they said I wasn't in pain long enough. Also, my copay through the insurance was $662 per MRI, but if I just paid the MRI company cash directly it was on $400 per MRI. They said they have such a hard time dealing with United Healthcare that they are forced to charge more for any service when it goes through United Healthcare. What is the point of insurance when they charge more to use it? Please, go with a different insurance company. Seriously, my experience was so bad that I will be doing everything in my power to influence people not to go with United. I have my new mission in life. read more

My husband and I had a big wedding in July but do to the court system delays with his divorce and the misinformation from them about getting our marriage certificate we couldn't get married by law until Sept, which is what we needed to give my husbands work. All kinds of hurdles to jump though for the military benefits to kick in put us only 5 days late to being able to chose the plan that has no deductible or out of pocket. They automatically start you off with the higher out of pocket plan and put a deadline that you have to make. So at 10 weeks pregnant I was told that "you have insurance!" as if all the plans mean the same thing to my wallet! Because we missed that deadline by 5 days (not our fault or laziness) the first month if I see a doctor we pay $150 deductible and 15% of the fee once that ded. was met). We were told there's a form to request that the 2nd plan start sooner, so we filled it out, faxed it, and UHC manager was real friendly saying he'd push it through faster. Turns out we were denied; I asked to speak to another manager for details and she knew nothing of customer service, did not want to hear that the reps were each giving different info and didn't want to clear up why we were denied for such a time sensitive issue, just said, "you have insurance and have to follow rules like everyone else" with complete lack of empathy. I've been so stressed out from this process and making handful of calls, still not knowing if the info is correct. They don't care. We can't afford paying for all the costs right now (which is essentially the cost to us being on a plan for one month that has a deductible). We're stuck with this crap of health insurance because it is free to us via the military. One of the UHC reps said it was the military who chooses these things; I say bullshit, because if we have the option to be on a no cost plan all together paid for by the US military at all, then why would they be the ones to deny a leniency due to 5 days that easily could have been given, if we didn't have to make appointments with military's DEERS enrollment that delays things?! It was a UHC employee and manager that gave me false hope of some form that might make the plan effective earlier, other reps that said different things too that turned out to be inconsequential, and a UHC manager that didn't seem to want to hear let alone understand that some situations aren't so black and white, and because they're all behind a telephone, they can all spew bullshit. read more

United Healthcare will do everything to collect high premiums but deny or in our case ignore medical claims. 4 months after United Healthcare incorrectly entering the date of service of a $2300 medical claim and speaking with their claims reps 6 times including them verifying the info with the doctor and mailing all requested docs, the claim itself disappears. Keep in mind that UH even had a claim rep assigned to the issue, who did call a few times to make sure we were kept informed. To absolutely no avail. I call this morning to follow-up (4 months after UH made an incorrect and entry and 5-8 months after the dates of service) and they basically had ZERO information about anything. I work for Cal State University and I am researching on UH Alliance HMO California due to low $57.04 per month but after reading these reviews I am very concerned. I currently have Blue Cross Select HMO California and next year my family premium will be $197.60 up from $82. However, I do like my doctor which is supposed to be on UH as well base on her medical group listed on UH website, but I need to confirm. she is not taking new patients so I don't know if I change plan, will she accept us still? read more

I am very-very disappointed with UnitedHealthCare. Most of their employees have no clue about coverages or how to assist members. Their system is not linked or no way communicates with Medical groups. If they do, then I am sorry to say there is an issue - MAJOR issue with both. One is not talking to another... I signed up with one group and they have me in different group. Long story. I will need few days to type every data, however, I will try to provide somewhat short version. I live in Glendale, CA. I selected Glendale as my group. My PCP is in Burbank. She is in the network. I requested an authorization for my wife to see OBGYN Dr. We called UnitedHealthCare and found out that Dr. in Glendale is in my network and can see him. Then I asked my PCP to send an authorization. PCP submits an authorization to Regal Medical Group. They first APPROVED the request, however, right away they Denied the authorization. We submitted second request, they denied second. We submitted multiple requests, they DELETED all authorizations because they are duplicate. I called UnitedHealthCare and told them what is going on and they said, oh yes, the Dr. I selected with UnitedHealthCare customer service rep. is not in the network. I was asked to change the group. Then I changed the group. Before I changed the group. I asked I need to have my wife to deliver my baby in Glendale Adventist Medical Center. UnitedHealthCare and myself went through the online to select appropriate Dr. We did. We changed the group to make sure the Dr. is in that group. He was in both groups - by the way. So, comes next month. We submitted new request. Regal Medical Group denied the authorization. We submitted again, and again, and again. All the time they denied because it is duplicate and deleted from the system. I asked UnitedHealthCare to follow up and find out why are they denying and deleting all the authorization my PCP is submitting. The word was that they denied because the request is duplicate and they see the same name and same Dr. they denied. They admitted that they NEVER read the group that has been changed and this is a new request. Then Regal asked us to ask my PCP to send a new one. We did - and they denied again. Very annoying. We complained UnitedHealthCare and submitted an appeal to the appeals department. After 2 months of back-end-forth, today we got a call that appeals department is also rejecting our authorization. Funny thing is that they like to authorize a Dr. that is outside of network. Yet they don't want to approve someone who is in the network. The Dr. that they authorized is not a bad Dr. she is great. No problems with her. She just does not work the days that is convenient to us and she does not deliver in Glendale Adventist Medical Center. Clearly there is something broken in this picture. Pure negligence from both Regal Medical Group and UnitedHealthCare. One is telling one thing, the other is saying different. One sends me to another and the other sends me back to the other. I am a ping-pong ball that needs to stop. I am not done here, not all data are said - I just want to take break writing. But will continue if I don't get results fast and I will start publishing names times and recordings of all of our conversations. read more

Unfortunately, my health insurance carrier is United HealthCare. It seems they do everything in their power to deny coverage or pay little to nothing on claims. I have been trying to get authorization for a Cochlear Nucleus 7 Sound Processor for almost two months now. It's the external component of the Cochlear implant, and one is rendered totally deaf without it. It's not considered a hearing aid, but a prosthetic medical device. United HealthCare denied it. I appealed, saying it was URGENT, since it is medically necessary so that I can hear. United HealthCare just now downgraded my Appeal from URGENT to NORMAL, meaning it will take 30 days for them to make a decision, pushing it into 2019, when my deductible will reset. I met my high deductible in 2018. So now I will have to pay lots more out of pocket. They are a pain to deal with, and I would not recommend United HealthCare to anyone. There's a reason they lost their contract with Medicare in 2018. I would guess fraud. RUN as fast and far as you can from United HealthCare. read more

Briefly put: Providers (Urgent care and Pharmacies) WILL NOT be able to locate you in their computer systems, thus you will have to pay upfront... No matter if you bring your UnitedHealth ID card with you. This plan DOES NOT exist in providers systems. Also, good luck if you ever get a refund from UnitedHealth. I am still waiting for my check in the mail... They will not make a direct deposit to your bank account. Yet they do accept direct via internet deposits to their account. read more

United Health Care would only pay for 4 biopsies during my mastectomy. Guess what. Biopsy #5 had the cancer in it. If my surgeon had let United Healthcare dictate the surgery that cancer would have been left behind and could have spread to my brain, liver, bones, or lungs. They still refuse to pay for the sample of tissue in it that had cancer. I thought women having mastectomies were protected by laws. I also got really sick from the chemotherapy and ended up in the ER. I had to be admitted to the hospital and the ER called United to have the admission pre approved. United approved the admission but now months later they will not pay the bill. My plan also has 100% coverage for flu shots. Guess what- they did not pay for flu shots for 2 years until I went over all my bills after my cancer treatment and did my own audit. No wonder they have so much money. They will do anything not to pay! They hope to deny, deny, deny until you just can't take it anymore. They have caused me so much stress during my breast cancer treatment. read more
