I could not hug her enough as we were leaving. This is the only program that my daughter has completed. This is the first time we've had real hope in many years. The program is tough and asks a lot of the participants, as it should. My daughter left the program twice, but finally went back and muscled through the hard work. I thought I would never see my beautiful daughter healthy, and sober again. I know that she did the work, but thank you to everyone at Stonegate. Prayers answered.

This cost her much pain and led her to cry and scream, which the staff dealt with in the most humane and compassionate manner. They spoke softly, encouragingly, and continue to let her know that they were doing the best they could to help her without causing her any pain. She is always clean, dressed and clean clothes, and in a good mood when I come to visit. The staff to patient ratio seems extremely high, and the executive director is very approachable. I would refer my other clients here in this part of DFW without thinking twice. What a wonderful secret.

She was two week in intensive care, a week in a regular room and the released to start her treatment at a rehab. I had no idea which rehab to use, and asked for a recommendation, I was recommend Encompass Health, out of many options. My daughter read the reviews and there were a variety of mixed comments, this is mine! From the moment she was moved from the hospital to Encompass, treatment as a patient was very human and very respectful. Once in the rehab, the treatment was nothing short of perfect! The nurses and techs were all very very nice people who obviously take their profession and responsibilities very serious. The director for Encompass himself came to introduce himself and dropped his card in case we needed to contact him for any question or discrepancy, which we never had! The rooms were very nice, clean and kept that way. The therapist were just great, my wife's recovery has been nothing short of miraculous, God in place and the work done the Encompass team and therapist was the best combination to her recovery. The Doctors were constantly visiting and giving updates of my wife's condition. She was offered daily a menu for next days breakfast, lunch and dinner, and the food was just great! She entered the rehab April 6th and was discharged Sunday April 19th, walking, talking and on her way to being independent as she was before the stroke! It is very difficult to see someone you love and have know all your life, suddenly helpless and wondering what the future hold for that person and the family as well! Bear in mind that in this rehab there and many many patients, some in very delicate conditions, so we where not expecting that in a case of some need they staff could react immediately, they did the best at hand and that is accounted for! Our family's experience with Encompass Health of Mid-Cities was appalling. Based on what we…read morewitnessed, I would strongly urge families with medically vulnerable loved ones to look elsewhere. My father-in-law arrived after major surgery with bilateral C5 palsy, a rare condition that left him unable to use either arm. He could not feed, bathe, toilet, shave, reposition himself, or perform basic daily tasks without assistance. Despite detailed discharge notes from his surgeon, during his first week we repeatedly had to explain his condition to nurses because they appeared unaware of the extent of his disability. Families should not have to educate medical professionals about a serious diagnosis already documented in a patient's records. On his second night, while in significant pain, he asked a nurse to reposition him. According to my father-in-law, the nurse said he had back problems and could not help. A man who physically could not reposition himself was left suffering for hours. That level of callousness toward a completely dependent patient is indefensible. We complained immediately, and Encompass did remove the nurse from his care. Our experience with Case Manager Debra Otness was especially infuriating. During our initial family meeting, discharge home was already being discussed despite the fact that my father-in-law could not use either arm or independently perform basic activities of daily living. We were repeatedly promised advocacy and additional therapy that failed to materialize. Instead of feeling that his case manager was fighting for a vulnerable patient, our family felt we had to do the fighting ourselves. We also have serious concerns about Dr. Muppidi's management of his care. Our experience left us deeply concerned that the surgeon's detailed discharge information and the severity of his bilateral C5 palsy were not adequately understood or incorporated into his treatment and discharge planning. The fact that sending home a man who could not use his arms was considered reasonable was astonishing. I am calling on Mindy Hong, CEO, and Prathusha Maduri, Medical Director, to investigate how this happened. This was not simply poor customer service. It represented, in our experience, a profound failure of preparation, compassion, communication, and patient advocacy. Encompass was entrusted with a man who literally could not care for himself. He deserved dignity, attentive care, and people willing to advocate for his recovery. Our family is disgusted by much of what we witnessed. If your loved one is medically complex, physically dependent, or unable to advocate for themselves, I would think very carefully before choosing Encompass Health of Mid-Cities. Based on our experience, I cannot recommend this facility and would strongly encourage families to explore every reasonable alternative.

ZERO STARS. This place is a joke. Long story short, the medicine I was taking was giving me a false…read morepositive for 2 drugs I've never even done before. I paid over 400 dollars to get a lab test (yeah, because they wouldn't send it out) came back negative. The next week I still popped positive, why?!? Because I was still on the meds that my pcp confirmed gives false positives. They then brought me in the office and kicked me out due to my continued false drug test. That wasn't false, most rehabs send the urine out because it's very common to have false and when they send it out it comes back with the truth. However, greenhouse doesn't do that because they are pieces of sh**. This happen to multiple people, but I was the only one kicked out. I've been sober 10 months, and the staff had the nerve to tell me "you're not even crying tears" Um, sir... that watery stuff coming down my face are TEARS. Dumbass. I was sober and they kicked me out even after I proved through lab tests. They know I was sober too, that's what makes it even more sick. How do you work at a place to help people and kick them out for no reason. Most Everyone who works there is miserable, and so they treat us like crap. Todd especially knew I was sober because the links I went to and proved it. Evil. Also, you'll see a lot of positive reviews about Arturo because he stands over everyone while they write a review. So half the "positive" reviews are just people trying not to have issues with staff or ruffle feathers. The reality is if Arturo wasn't standing there, there would be a lot more negative reviews. This place is awful- they know I was sober and they still kicked me out. SICK mofos. Most of the staff is burned out, and the night charge nurse (male) is just plain evil. & Julie (can't really remember her name) but she has major counter transference and projects her miserable life on her clients. She works the best with male clients because she's a B and clearly dislikes female clients. Also, she brags incessantly about her son in Harvard. & everyone knows she's not sober, even two staff members said she's manic af and clearly on something. Knowledgable staff that make you feel not only welcome but wanted and cared for. They not only have done my rehab but have made suggestions for massage places, dry needling, and encouragement that helps me get through these times. As a runner I get troubled when I have to remain immobile for any period of time. They keep telling me stop running. But it hasn't worked yet! Amazing place!!!

Anything in this facility that is not bolted down has the ability to grow legs and disappear and I do mean anything. They have known thieves that are residence in the facility yet nothing is done about it. The other residents suffer for this. Please note there are few nurses that are gold to the core!! I've given them reviews and accolades to other admin there and kudos but it stops there. Morale here is down for the entire facility bottom to top, admin to clinical, including the residents. This reflects clearly that there is a DIRECTOR problem, and it is evident that this place is not being directed in a very good direction. I have seen nothing but decline since my mother has been there at this facility under current management. It's just nasty. Residents have lost all morale and just rock back-and-forth in their doorways. There's no activities director so the biggest thing they do is play bingo for a soda. If you do not do anything else ever, please understand that this facility has violent offenders in this facility and there have been physical altercations while my mother has been there. We were told that my mother would get all kinds of cognitive therapy and physical therapy, but again, the residents are just there to breathe and the evening shift are all too happy to hurry up and get their medicine and food and make them get in their rooms because they don't want to deal with them. Glasses and socks all lost this week. Glad I am going to move. Brand new socks were never worn, marked them with pink thread and was saving them until I got a pair of shoes. My mother is currently staying here for rehab after having broken her femur. The staff have been pleasant, helpful, and competent. As a nurse, I have unfortunately seen bad snf's and bad hospital experiences. This place is not one of them. Dyllan Gadberry is the ceo of this location and from what I have seen, he runs a well-functioning center. I know good care and I know what lazy care looks like, and all I have seen for my mother at this location under Mr. Gadberry, has been good quality care. I think many of the comments concerning this place are either unfair or simply unfounded. I have only known good things here.

The four higher ratings appear to be from the physical therapy and Rehabilitation side, I do agree that the physical therapy and Rehabilitation at this facility is generally good however the long-term care side is what we are reviewing and this has been our experience . My mother lived at this facility for over two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances--especially on weekends--where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse's station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor's name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership--including the Director of Nursing and administrator--went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.
