I'm a 83 year old male who was recovering from hip surgery. I unknowingly went in to this facility, expecting care and support. Forgot care and support. They lost all my meds they misplaced my med charts from were I had my surgery done. The majority other the RNs , can't speak English. my grandmother came here because this is the only facility that accepted her after her stroke. The beginning of the process was nice and the staff were nice. Although almost a week in my grandmother (who cannot speak) was left alone for over 15 mins while in the bathroom. They lack empathy and common sense cause clearly she is not supposed to be alone by herself for that long. We went off on the staff because one person would get her then another person was taking her back. it was a mess and in general this facility needs some major upgrading. UPDATE AS OF 10/21/25 There was a covid outbreak in this facility and was not informed about this until after my visit on the same day it was discovered and oh boy the nurses/staff did not know what to tell me in regards to why i wasn't informed when i was physically there. side note: DO NOT LET THEM TELL YOU THAT VISITING HRS ARE ONLY BETWEEN 10:30AM-8PM. I'm so sick of this place the service and care sucks the family member you have there is just filling the bed at that facility, and they are just collecting a paycheck. you walk through the halls and it's like an asylums theres elderly screaming and yelling for help and the facility smells like a barn. And you call to check on your family member and your never get anyone to help you. your left on the phone for 10 mins plus and then they hang up on you. So you have to call back by that time your mad they are mad and it's ridiculous. you have to actually call like 6 times to get a person on the phone and check the status of your family member. fuking insane I hate this place.!!!! Someone please do a well-fare check on all the old people make sure they are getting the medicine and being changed and feed. And fire all the staff members from the receptionist to the activities agents to the nurses at desks that never pick up the phone. read more

One of the worse facilities in California. Not one person has returned phone calls. Doctors don't exist here. Nurses are confused and poorly trained. They do not have the patient's prescribed medication available which is ILLEGAL. Call lights go unanswered. No one here wants to actually work. They collect a check and go home. The website doesn't work and you won't be able to actually speak with anyone. Any positive reviews are fake as I can promise you this place should be less than one star. Do not leave your loved one here. It is unsafe and they will do nothing to help with their recuperation. I am reporting them to the Ombudsman and California department of Health. read more

These reviews must be fake. This is the worst facility to send your love one too. I've been chasing a social worker there so I can get my mom transferred out of there, however she refuses to return my phone call. They don't have her medication. I found out there are no doctors on site and they can't get prescriptions. They are extremely short staff. Call buttons go unanswered and mom sits in her wet diaper way too long. After 8pm no one answers the phone and there are no nurses that come in to check on them. I have a list of complaints. This place is terrible. I feel bad for the patients. read more

Since my mom became a patient here in October 2023, I have felt the impact of great leadership and leaders who go above and beyond for patient care. It feels that you all treat patients as they are: like people who require specialized attention. The staff have worked with me, not against me on special needs that I have for my mom, even allowing me to wash her hair and tackle her hair and scalp issues. I highly recommend this place. My Mother had fallen and the ER Doctor confirmed she was not injured but should be under 24 hour care. She stayed here for three weeks to get her strength back before coming home. The staff took great care of her during the Covid-19 lockdown and even got her vaccinated. We are happy to recommend this care facility. read more

I have a grandmother staying here and I wish she didn't have to. While some staff members are accommodating, it's disheartening to see many more staff not caring about their jobs. I understand it's difficult to work with older grumpy patients, but ignoring BLARING alarms when an older man groans for help does nothing to make situations better. Elder neglect is more and more real to me whenever I see these old patients being ignored. Please be a bit more mindful and patient with some of the older patients that are under your care. read more

We had to place my grandmother at Vermont when we couldn't care for her anymore at home. It was a very tough decision for the family but she was of sound mind and alert to understand what was going on. She loved it! She said the food was great and the nurses were great and took care of all her needs. Mind you she called a meeting with the staff including the social worker to make sure everyone knew what she expected. The staff loved her and respected her. They worked with our hospice program closely to ensure all her medications were correct and towards the end they kept on top of any change and alerted hospice and us, the family immediately. The staff was very caring and professional in her last days and wonderful to our family while we suffered our loss. I gave them a 4 out of 5 stars because the night staff we felt could have been more attentive. We were very concerned towards the end because the night shift doesn't check or monitor the patients as often. We had to remind the night shift med nurse to continue her medications. We just ended up staying the last two nights with her so we were sure someone was with her. We recommend Vermont to anyone looking. Very clean facility and all staff from front office, to nurses to the house keeping were fabulous and we thank them for taking care of our grandmother. read more

If you care AT ALL about your loved ones you would and should not allow them to enter the doors to this horrible place. My Father was placed here and we were allowed a 1x in person visit. Upon arrival we quickly figured out that he was not given his prescribed medicine for his Parkinson's disease which he cannot miss any doses of. NOT ONE PERSON could answer ANY questions about him, his care, his medicine. No one knew what he should or shouldn't be taking no one read his chart. They wanted to keep him in there for 10 days in isolation with no visitors. We pulled him out which and were treated like ABSOLUTE CRAP because we wanted to pull him out of this shitty facility. We were absolutely amazed that someone would receive such horrible care. STAY AWAY! read more

NEGATIVE STARS IF POSSIBLE! IF YOU LOVE YOUR FAMILY DON'T BRING THEM HERE, they won't make it out! My grandfather would scream when he would see the nurses claiming they were rude and really aggressive and would hit him (he was mentally sound). They would not feed him and when they would the food was spoiled and he would get sick. If anything you people have a lawsuit coming! The best part of all is KARMA! This place is to help our loved ones and they do absolutely nothing.... medicine was never given on time they never bother to wake the patient up to eat or do they PT... they are all lazy.. It's neither elegant nor high class, but the staff is pretty much on top of what's important, and the place is clean. Food is a lot tastier than other similar places or the local hospitals, so that's a plus too. Front office staff all get A+. read more

I'm a registered respiratory therapist and I work in a convalescent facility. Nobody in my family has been admitted to a nursing home until my grandma. She was admitted 3/8/18. On 3/10/18, she was on hospice. On the 12th, I noticed my grandma's nasal cannula was placed on her backwards so I fixed it. It's the lowest, most basic way to give oxygen. The prongs going into the nostrils are curved and they should be curved downward when they put it on, but they were curved up. And I didn't even think about it, I just put it in her nose the correct way. I went to go see her everyday she was there. So I would check everything when I was there. The day before she passed, 3/19/18, I got there late, like 8:30pm and they had changed her nasal cannula that morning so I checked it and it was backwards again, AND her oxygen concentrator was set to 3 liters per minute when the whole time she was there, it was at 4lpm. So I asked her nurse if the order changed to 3lpm and she checked my grandma's chart and said, "no it's supposed to be 4lpm" so I changed it back to 4. But that night when I saw my grandma, she looked totally different.. like it was really gradual the whole time, but that night I even told my cousin from Alaska like, "I dont think she's gonna make it through the night or tomorrow." Sure enough, I was at work the next morning and got the call that she passed away. I'll be blunt. They f**ked up and pushed her to die faster, and she suffered cuz she wasn't getting enough oxygen the entire day. She needed every little bit of oxygen she could get cuz she had fluid in her lungs so it was helping her a lot. But by the time i got to her and fixed everything, there wasn't a difference cuz she she was already gone, she was deprived of the amount of oxygen she should've been getting for too long at that point. Her breathing changed drastically, her appearance, like I knew that look and I was right. Like why the hell was her concentrator at 3LPM? We use those saaame oxygen concentrators at my work and you can't just accidentally bump into the knob and it'll change an entire liter. You have to actually turn the knob almost two full turns to get it to change that much. A couple days after, I went with my mom to a meeting with the DON and social services lady. We told them about what happened and they kept trying to defend themselves. The DON even said, "The tubes were still in her nose so she wasn't getting zero oxygen." She even tried to challenge me and asked, "you're a respiratory therapist, how long can someone last with no oxygen?" I told her, "the tubes were still in her nose but she wasn't getting the amount that she was supposed to be getting" and all she could say was, "yeah.." Then she told my mom and I that they already had an "in service" that night, the night before my grandma died, about the correct way to place a nasal cannula. An "in service" is basically a meeting where the staff is informed on whatever issues come up or what needs to be corrected. So basically, the nurses there were incompetent at their job. Like the night of the 19th, my mom even found the nurse that changed my grandma's nasal cannula and she told him he put it on wrong, and he was insistinggg that it was the right way so she yelled at him and called him stupid lol that's probably why they had the in service. So after we talked to social services and the DON, like they couldn't say anything but apologize. My grandma was on hospice care so we knew she was gonna die. But omfg.. not like that. Oh, and my grandma's hospice nurse said she'd order an air bed for her, that was on the 10th. It never came. Also, my mom noticed they stopped using chux for my grandma so she asked my grandma's CNA about it and she said they didn't have any more. They were using those disposable, cheap, thin ones. My grandma ended up with a flap of loose skin caught under the pad, probably when she was repositioned, and she had a big purple bruise under her right shoulder blade. My mom mentioned the issue to the DON in that meeting and she said they have plenty. So wow, wtf was really going on there? Never again. read more

This place is the worst care center I've ever stepped in. To start off careful with your belongings, they will disappear in a finger snap. Do not being your elders here! The nurses are rude, if you call them to ask about your loved one you get transferred 15 times. This place seems over staffed but hardly attends to their patients. The place looks and feels depressing from the moment you walk in. I won't stop reviewing until this place gets their nurses in order. They need to stop covering the fact that this place is unorganized. I wish there was a negative star for this but for people to be aware I will put 1 star so everyone can see my review. read more

This place is horrible if you have a choice absolutely do not choose this location for your love ones. I had a family member in this location for one month and their health declined dramatically they were severely dehydrated and their condition worsened while they were there. My family member was taken to the hospital due to blue/purple hands and feet and trouble breathing. The office & nursing staff did not care about their patients. After my love one was in the hospital for three days I received a call from Vermont care informing me that they would be moving my family member due to renovation of the room they were staying in. I asked the staff member who called me did they realize my family member has been in the hospital for three days and not at the facility. Again if you have a choice do not choose this nursing home for your loved ones. I'm so glad we found another nursing home for my loved one. read more

If I can give this hell hole a negative star I would. Let me start off by saying please trust this review because if you truly love your loved one I'm begging you not to send them here. The staff will make promises and break every one. My Grandma was sent here after a surgery and the CNA was so lazy she did a half ass job cleaning her that when we got there to check on her she was a mess. My sister and I kept asking if we can get her changed and they didn't even come so my sister and I literally had to change her sheets and bedding ourselves with her still in the bed since she couldn't get out the bed. Then they got upset because we were so disgusted on how dirty her sheets were we placed them outside the door. We were told in a rude way to never do that again. I told them how dare them tell us what to do when they can't even comprehend what to do themselves. They placed a button next to her and said for her to push for help. We were like well she can't push it because she can't move her fingers. Their response was ohhhh but we can check on her when we have time. I said so she can be in pain or want water and she would have to wait until you have the time since she can't push her button? Their response was well I can't make any promises. Then she was supposed to get packed from her surgery and she did but the last time they did such a horrible job she got infected. Then within. 2 days she caught pneumonia and when my mom asked the staff why is there an oxygen tank next to her they said ohhhh that's not hers that's her neighbors so my mom was like is there something we should know? They said no but they didn't want to tell us it was my grandma's oxygen tank. Her status right now is she's in ICU with an infection and pneumonia and it doesn't look good for her. Please STAY AWAY from this place they just try to fill the beds to keep the numbers up. read more

I feel compelled to write this review post discharge, of our mother who was a resident at Vermont from June 2019 - October 2019. I read the reviews and most were older ones. Perhaps this review will balance some of the negative ones. There is always care that should never have to happen. We transferred our mom to a different facility that is closer to family, her church friends and one that is more culturally suitable for her. In retrospect, mom had a good experience at Vermont. Compliments to her CNAs, Juliana, Mayra, Teresa and med tech, Sandra. They were awesome and mom still speaks kindly of them. Stephanie, at the front office was always helpful and cheerful. Sure, there were "growing pains" at the beginning of mom's stay. Input was always solicited and attempts made to correct any issue brought to nursing or other departments. The facility is really clean and rarely smells bad. Staff always said hello when walking through the facility. The large lobby area and garden with the fountain are pleasant and serene areas to meet visitors. Wishing Vermont all the best during the pandemic as healthcare is facing one of the biggest challenges of all time. Keep improving and thank you. Family of H.E. read more

Think twice about putting your love ones in here. First off I have had my mother in here for 2 years. At first things were good, but not now. There is a note on her clothes closet that says Family will do Laundry but I guess some of the CNA's and nurses do not read English. My first clue should have been $300.00 glasses they misplaced during her first week (never to be found). Her clothes come up missing all the time, I buy her new clothes and then they disappear. No one seems to know where they go to. Her slippers and socks also go missing, Always the same answer we will go to the laundry room to see if they are there but guess what no they are not there. Even take her soap that I supply. I do believe there is a thief among them. The care and communication among staff is not there any more. They have a 3 star rating but should be 1. I could go on and on about this place. So the moral of the story keep far far away from this place!!!! read more
