There are no words that can truly express my gratitude and appreciation for the team at Pacific Care Nursing Facility. My grandmother spent the last several months of her life there, and she was given such compassionate, dignified, and loving care. [[HIGHLIGHT]]She formed meaningful connections with the nurses, enjoyed participating in activities, and always looked forward to her meals.[[ENDHIGHLIGHT]] My grandmother, who meant the world to me, was diagnosed with pancreatic cancer at the age of 93. When she was first diagnosed, she needed a level of care that I was unfortunately unable to provide because of difficult family circumstances. On the day she arrived at Pacific Care, I was overwhelmed with anxiety, fear, and stress. I remember snapping at the very first nurse I met. [[HIGHLIGHT]]Instead of making me feel like I had made the wrong decision, that nurse met me with understanding and compassion. She recognized my emotions, listened to my concerns, and immediately reassured me that my grandmother would be well cared for.[[ENDHIGHLIGHT]] Throughout my grandmother's stay, every one of those promises was fulfilled. I want to commend each and every nurse and staff member at Pacific Care, especially those who cared for my grandmother every day. [[HIGHLIGHT]]Their communication was always consistent, they treated everyone with empathy and respect, and they brought genuine joy not only to my grandmother but to the other residents as well.[[ENDHIGHLIGHT]] It was clear that they cared deeply about the people they looked after. They took the time to learn who my grandmother was beyond her diagnosis. Every day they made sure she had the newspaper so she could work on her Sudoku puzzles, catch up on current events, and look through recipes that she loved talking about. [[HIGHLIGHT]]They would bring her tea, chat with her throughout the day, and even share some of their own family recipes with her.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]Every time I visited, the staff would smile and tell me how much they loved my grandmother. They would laugh as they told me she loved filling them in on all the latest gossip and stories while they worked.[[ENDHIGHLIGHT]] Those conversations became such an important part of her day, and it meant so much to know she wasn't just being cared for, she was genuinely known and loved. [[HIGHLIGHT]]My grandmother took great pride in being one of the oldest residents at the facility while taking the least amount of medication. She loved sharing that fact with anyone who would listen, and the staff celebrated those little victories right alongside her.[[ENDHIGHLIGHT]] They made her feel valued, respected, and incredibly special. I was fortunate enough to spend holidays with my grandmother at the facility, and Valentine's Day will always hold a special place in my heart. Watching her enjoy a delicious meal while listening to live music was unforgettable. She sang along to every song and happily swayed back and forth in her wheelchair, dancing with the biggest smile on her face. It is a memory I will cherish forever. I will miss my grandmother's phone calls telling me she had won at bingo. I will miss hearing her excitedly tell me what she had eaten for breakfast, lunch, and dinner. I will miss hearing the staff call out, "Carmen, where are you going?" as she happily wheeled herself up and down the hallways. And my favorite response was always, "I'm going to take your grandmother home with me, we love her." Those little moments perfectly captured the love, humor, and warmth that surrounded her every day. My grandmother was truly loved and treated with kindness during her time at Pacific Care. The care, encouragement, and attention she received gave her the strength to keep fighting her cancer and every other illness that came her way. In my heart, I truly believe that without the incredible team at Pacific Care, her life would have been much shorter. More importantly, I don't believe she would have experienced the same peace, comfort, and dignity that she found there. The day I came to collect my grandmother's belongings was one of the hardest days of my life. [[HIGHLIGHT]]Even then, the staff handled everything with such care and compassion and they neatly packed everything up for us.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]They welcomed me inside, allowed me to thank them, and reminded me that I was family. Knowing that I am expecting my first child, they encouraged me to take care of myself and my baby.[[ENDHIGHLIGHT]] Even in the midst of my grief, they continued to show me the same kindness they had shown my grandmother. To the entire Pacific Care team, thank you. Thank you from the bottom of my heart for giving my grandmother a beautiful final chapter filled with love, laughter, dignity, and genuine compassion. Rehab care is really challenging and scary, but we lucked out with this facility. My 93 year old Grandmother broke her leg and needed two months of rehab care. They were able to accommodate our needs, keep us updated on any changes, provide lots of communication, and provide a clean, safe environment for a really challenging recovery. We visited every day and always saw lots of staff, lots of cleaning and nurses available asap even over the holidays. Grandma even liked the food. Amd they got doctors and specialists to her during her stay which made things easier for us. We toured facilities and checked ratings on gov sites before selecting here and this was the right choice for us. read more

Understaffed at this facility , don't care about patients needs . A family member is in there laid in a dirty diaper for 3 hours , physical therapy is non existent since she's been there only received therapy 2 times in a week , foods always late . When you ask for help they agree with you then they do not follow through ! HIGHLY RECOMMEND NOT GOING TO THIS FACILITY IT NEEDS A LOT OF IMPROVEMENT ! SENT PATIENT HOME WITH A NEW ILLNESS ON SUNDAY ! Said she's ok to go home but wanted to order a wheelchair for her and a medical bed ? I waited a while before reviewing because I wanted my mother to take legal action against these inhumane people who call themselves "caregivers" - but she didn't do that, for whatever reason, so here it is: This is a dreadful place. They "forgot" to give my mother her medication on a number of occasions. There was a mix-up with one of her meds and because of this she lost an entire month of a prescription, which really messed her up. Communication between facility and doctor is poor, poor, POOR. They "lost" all of her clothing when they took it away to wash. She had to purchase a ton of new stuff after leaving. When she tested positive for Covid (she had no symptoms and wasn't sick) they isolated her in a room, and basically neglected her. She shared a bathroom with a dialysis patient, who's equipment was everywhere and made it difficult for her to even use the restroom. Hello, ADA!? She cannot have gluten, they told her her meals were gluten free and they were not! They lied and gaslighted and STARVED her the whole time she was here. I was alarmed when she got out and I saw how much weight she'd lost, how weak she was, and the haunted look in her eyes. Absolutely unconscionable! We were unable to visit and check up on in person her room, how they were treating her, etc, because they would not let my minor child in. I called them at the time to ask about the missed medication and they straight-up LIED to me. Eventually she was fed up with the neglect and they would not allow her to leave due to the positive Covid test. She had to force the issue and eventually got herself out - but they really attempted imprison her there against her will, saying she could not leave the room due to the positive test. I'm grateful she found the strength to get out and that she had a friend nearby to pick her up! She didn't get any kind of decent recovery (in fact, she went downhill) in there but once she left AMA, she made a swift recovery. She was able to take her medication without having it withheld and doled out by uncaring, unreliable liars, and eat safe and nutritious meals. They are huge liars, they are irresponsible and inhumane, this place is unsafe and unhealthy. Beware! read more

It is a pity Yelp does not allow zero-star ratings... this facility is filthy, staff lack patience, empathy, and ability to effectively communicate care. Post hip fracture, my grandma was recommended for a Skilled Nursing Facility for recovery. Pacific Care Nursing Center was selected due to proximity to home and in-house hemodialysis capability. Initial care seemed promising as she was working with physical therapy daily. The staff was aloof but caring enough, the facility itself was poorly lit and questionably cleaned. When a COVID outbreak occurred in the facility resulting in no visitors, a steep decline was noted. With few updates that were received, there was concern for grandma's lack of appetite, movement, and overall will to live. After 20 days and being cleared of COVID for two, she was suddenly discharged from the facility with no explanation and the expectation she would be picked up immediately. This was suboptimal as there was no plan for her if/when she returns home--and there was no ownership of who is responsible for coordinating. As a family we rushed bedside to evaluate the situation and to attempt to bridge gaps in communication. When we first laid eyes on her, grandma and her roommate were lying in a dirty dark room in the middle of the day--Two elderly women already prone to delirium not set up for success. The room was mislabeled for the residents in the room and there were no resident identifiers (i.e., patient wrist bands). During our three hours stay, no member of the staff checked on the residents. Food was left outside of the room without a word/request to potentially assist with the set up of trays. While we set up grandma, the remaining trays on the cart were left and eventually picked up by another staff member with no regard to whether anyone ate. After hours of phone calls between multiple agencies affiliated with the facility (i.e., home health, dialysis, physicians, case management) it was agreed upon that she was appropriate for home, or at least better off at home. Again, no staff was present though their assistance was requested. Staff contradicted each other sending us to multiple doors with no ramps to assist grandma from the wheelchair to the car. Unfortunately, this was a mere few hours to highlight the multitude of insufficiencies of this facility. DO NOT send your loved ones here. read more

A family member was admitted here following a devastating injury. The family member resided at Pacific Care Nursing Center for a few months until the family member was finally strong enough to come home. The staff at Pacific Care Nursing Center were attentive, caring, and professional. Of course, no facility is perfect and no care compares to one from close friends and family. However, it is important for me to state that I did not worry about my family member's well-being during their stay at Pacific Care Nursing Center. The reviews are mixed and choosing the right facility for your own family members or friends requires extensive research. Speaking from personal experience, I can confidently say that the progress my family member has made was due to the wonderful quality care provided to them from the staff at Pacific Care Nursing Center, especially the staff from the Subacute section, the respiratory therapists, and the physical therapists! My dad was transported there after being in the hospital (Kaiser) for some rehab. We were nervous at first because he had had such a horrible experience at a SNF (Skilled Nursing Facility) in Torrance. But, to our delight this place was amazing! The staff is friendly, accommodating and skilled. We were met with a team of staff members who helped us with paperwork and explanations about what their plan of action was for my dad's specific needs. They also were very helpful with questions and wanted to know if we had any concerns. We also had a meeting with a Registered Dietician who was thorough with every dietary need as well as allergies, likes and dislikes. Each patient had their own dietary card with all their detailed information on it that was present on their trays at each meal. The food was "very good" according to my dad. The Physical Therapy team is amazing! They really know what they are doing and are very helpful and encouraging. My dad left them after only 3 weeks and was so much better physically because of their awesome assistance. I did not meet the Occupational Therapy team, but my dad was also very happy with them. The Activities Directors are great and very involved with the patients. They had some fun activities and events happening. They came by daily to drop off newsletters, newspapers and to interact with the patients. They even have a pair of parakeets for the patients to enjoy. This staff knows every patient and family members (even names). They strive to do their very best to make the experience there a good one for everyone involved. It's a thankless job, but you would never know by their demeaners. A large percentage of the nurses there are simply amazing and good at their jobs. I've seen them interact with the patients they are caring for and was very impressed! Only a few of them there (mostly on the night shift) might want to consider a different vocation - not working with people or wanting to do their jobs. But like I said, a huge majority of them are wonderful! Parking is not so easy at any time of the day. It's a very small lot and is often packed with cars; double parked. But, they have a very nice/sweet valet gentleman there who helps you out and makes it work. After he was discharged, we received several calls from them to see how he is doing or if we needed any assistance with anything. This is just one more thing they do to assure good service to patients and their families. My dad really misses it there - especially the staff. Other than a couple small details (and they really are small and petty on my part), we were extremely happy with his time there and would definitely recommend it to others who need rehab for their loved ones! read more

I am currently a patient at Pacific Care, and have been here for about 2 years. [[HIGHLIGHT]]The facility is clean and staff is wonderful.[[ENDHIGHLIGHT]] The problem is that the staff is underpaid for the amount of work they are expected to do daily. Management cuts down on staff to the bare minimum at times. That means that the ones who stay behind, have to work harder. It really makes me upset when Management would send a nurse home early because they try to cut corners. That means I have to adjust to someone new. How are these nurses, who work very hard, expected to support their families! Management is more concerned with showers, than patient care. [[HIGHLIGHT]]The staff do their best and don't complain.[[ENDHIGHLIGHT]] The rehab here is a joke. They were supposed to work with me, to get me up and walking, but that never happened. I've had a few setbacks, which resulted in being dropped from physical therapy. They would have the CNA come and exercise me briefly in bed, but 10 minutes 4 times a week, doesn't cut it! That's the one big disappointment that I have here. Other than that, it's a very nice facility, but only do to the wonderful staff! My message to Management, pay attention to how hard your staff works, get out if your office and see for yourself. In this time, I have been able to see firsthand the professionalism of the nurses, nurse assistants, physical therapists, administrative officers, social services, activity department, and in general the entire staff. My wife has been treated with dignity, friendliness, courtesy, and above all, with respect. My brother Victor suffered two strokes almost 6 years ago. The outcome wasn't good at all but it was short of a miracle he survived. His tending physician, Dr. Wan recommended my brother be admitted to Pacific Care nursing center to begin his recovery. I believe that both Dr. Wan and the staff at Pacific Care are the true reasons that my brother is alive and well today. Pacific Care team of nurses such as Carlos and Regina and all the team have given my family and I, peace of mind knowing my brother is loved and cared for while my brother has been there. Everyone there, everyday making this time that someone's loved one is away from their family, and making that patient not feel so alone, because the staff at Pacific Care truly goes the extra mile. This place is the worst for your family member, I was here for a week and watched everything for 6 plus hours a day. Your so called love one will be living in a dirty place with staff that don't care, why there independent contractors not staff there to take care of mom or dad with PT OT Speech Therapy all just new people every day. Staff are walking from Red Zone Active Covid all Day. Saw it and had Roaches in Bathroom and huge bug with fangs. Mom didn't get lunch one day Mon lunch at 150 pm and wed 245 pm and they said is communication. BS. Food Sucks Staff Sucks No Adm Guys Don't come here. My mom got covid here and then sen't to another place knowing they didn't do a covid test at discharge it's the law. Total Joke Don't send any family here. read more

I want to give 10 stars to this facility grandpa has been in multiple nursing care facilities. He was affected by Covid He is an 84yr old man. The prior nursing facilities did nothing for him but just let him lay in bed no physical therapy. We are very grateful he was transferred here, Dr Teresita Britney and Levi are very caring. Grandpas health has gave a big turn his health is progressing due to the physical rehab and weaning off machines and the wonderful care of this Team! Unless your primary doctor or doctors are affiliated with the facility so that the patient is seen on a regular basis, I wouldn't dare allow a loved one, family member or friend be placed here. My family member said it was such an unpleasant experience here. I'm a caregiver myself, and the lack of compassion I see here is disheartening because a patient can only see one member of there family per day for 45 minutes. This review is for the sweetest nurse I've met over the past 5 years my uncle has been there. I've seen nurses come & go and I've seen the faculty struggle with barley any nurses. My uncle had a concern and asked for a nurse and I had the pleasure of meeting Amani J. She came in with a smile that peeked through her mask but screamed here I am and how can I assist. Although I even thought my uncles request was RIDICULOUS, she still assisted and never allowed the smile to leave her face. Amani is an Angel that was sent to help & we are so thrilled that she's here. I've never felt more comfortable leaving my uncle behind until now, we were on the verge of moving him. I came to visit him and ask him how he was doing while in the process of looking for better facilities. Amani instilled trust and comfort in my family. She takes care of my uncle as if he was her own family. She's gentle, informative, and even makes him laugh. This is the genuine companionship & assistance we were looking for.
