I have been here several times when I have needed top notch post operative quality services. This facility is super clean. Everyone from nursing, therapy, kitchen, housekeeping, maintenance, and especially activities are the best at what they do. The Brookfield Center in Downey, California is the finest rehab center I have ever been associated with. They are taking care of my brother, who has spent the last 17 months recuperating from 2 botched knee surgeries. The care he has been given at this facility is by far the most kind and extensive I have ever seen. The staff, starting with Mary Chris and Sarah, are spectacular and they strive to help in any way they can facilitating all the paperwork and dealing with any type of Medicare , Medical or Insurance needs. The facility chief Dave is always there to listen to anything that might arise. The staff is by far the most patient people on the planet, and they take care of all kinds of disabilities, amputees, dementia, any type of post surgery rehabilitation. They have an extensive physical therapy unit with multiple people who oversee the rehab process, with all the possible workout equipment you could possibly think of. The nursing staff is the best I have seen in any hospital, with sincere patience in the treatment of their patients and residents. Miranda, one of the head Nurses goes out of her way to provide updates and keep you updated as to ongoing conditions. Their is no other facility that I have been to that has the elite staff, and personal care that the Brookfield Healthcare Center has. If you call 562-869-2567, you are always greeted by friendly front desk people, that basically become your best friends. This is a 5 STAR FACILITY, and you could find absolutely no better place for a loved one to recuperate at. The NICEST, most CLEAN skilled nursing home I've been to. Earlier this year, I had the pleasure of receiving a tour from Charissa and she went above and beyond showing me how beautiful their facility is, especially when it came to reassuring me that my father would get the care he needed along with the few accommodation requests I had. My father discharged back home last month and is in the BEST condition, thanks to the amazing rehab Brookfield provided. Forever grateful for the love and dedication the nursing team, rehab team, administrative staff, and housekeeping have provided. My mom fell twice there in less than a week. The food is horrible. Portions are next to nothing. The "juice" is punch from a gallon and the staff gossip more than caring for patients. HELP IS ALL i heard as iwalked down the halls with staff just texting on phones. I advocated for my mother and it did not sit well with them. read more

My mom sustained a fall and was hospitalized on September 11. I was advised by the hospital that my mom needed rehab I visited 5 different skilled nursing facilities and I selected Brookfield healthcare. Why I selected Brookfield healthcare was for several reasons 1. when I walked in I was greeted with great customer service by the receptionist 2 the facility smelled good and was very clean and 3. I did not hear any patients yelling which made me feel comfortable that they were being taken care of not neglected. 4. I read their reviews and nothing but good things being said about Brookfield. I was a little nervous and so was my mom as she has never been in a skilled nursing facility. My mom was admitted to Brookfield on September 16 it was difficult for me to leave my mom as I knew she was scared. The second day of our stay my mom was crying for me to take her home. I was impulsive and I asked the staff that I need to speak to the social worker because I wanted to take my mom home. Immediately the social worker and the physical therapist manager came to speak with me, and if it wasn't for them my mom would not have received the care that she received at Brookfield. I am grateful to the social worker Mary Cris, and Cristina the physical therapist manager, they made me realize that my mom would be safe in their care and that she would receive the best physical therapy and care. As I look back I am glad my mom stayed at brookfield as she will be going home tomorrow and able to walk with a walker. I want to give thanks to the CNA's Leticia Monse and Wendy. They were exceptional CNA's. They treated my mom with kindness, respect and compassion. A special Thanks to Brenda one of the RNs she went above and beyond her job to assist my mom when she had a difficult moment for that I am forever grateful. All the LVN'Sand RN'S that interacted with my mom's care were exceptional. All physical therapist team were also exceptional. My mom's physical therapist. Carlos was very patient and caring with my mom. the occupational and speech therapist were also amazing. Kiana was my mom's speech therapist, she was very helpful and caring. The menu for breakfast lunch and dinner was also very good. special thanks to housekeeping they kept the facility and my mom's room very clean. special thanks to Alejandra the coordinator of the activities she went above and beyond to have all the patients participate in the activities and my mom loved it.. to all the ladies that worked the reception area they were always so cheerful and welcoming. I may have not mentioned everyone that assisted with my mom's care, but everyone at Brookfield touched my mom's life in a special way and helped her with her rehab and for that I am forever grateful God bless every employee and their loved ones thank you and I highly highly recommend Brookfield. read more

Physical Therapy after spine surgery. Staff is very kind and helpful. Communication within staff is lacking and causing unnecessary stress and physical pain to patient's & family. Food is less than satisfactory. Twice sour fish has been served within a few days apart. Patient's diet restriction have been overlooked. Fecal residue was not properly cleaned from patient's genital area. Lots of staff constantly on rotation. Building could use some updating and general maintenance. read more

I was very impressed with the nursing care and services that my friend's mother received at Brookfield Healthcare. The facility was recommended to him and he was very pleased with the meticulous Nursing care and services she received, from the Social Services Department, the Business Office, Therapy, Dietary, as well as keeping her engaged through the wonderful Activity Department. I visited her several times and the staff would routinely turn and change her position at scheduled intervals and the nursing care and services she received were truly impeccable. DO NOT TAKE YOUR LOVED ONES TO THIS PLACE. They have no business taking care of elderly people. The staff are oblivious to the needs of the patients! They don't know how to administer medications correctly or even follow Dr's notes! They have a big banner outside of the front that claims they are a 5"5STAR" establishment. That is far from true. I would definitely not take my loved ones there again! They shouldn't be allowed to operate. I mean, how do you throw someone's dentures away? And then try to blame the patient? If you care about your loved one's health and well being, read ALL of the reviews. I can honestly say that nI have seen most of the problems the bad reviews say. read more

I was so impressed with the professionalism and care that the whole staff displayed. The atmosphere there was always cheerful and I was relieved to see the residents look happy and the staff glad to be there to help. My aunt is staying there and was very fearful at first but now I am relieved to see her happy birthday I have decided to come to Brookfield. She does have full care around the clock and the staff is very attentive. She is very picky with her food but she actually enjoys the meals she is given there. She is given three meals a day and I am thrilled to know they take into account her health needs when making her food. The kitchen staff there i a May thank enough because they make sure to provide a nutritious meal that she will actually enjoy. I stayed at Brookfield for almost 3 months. At first i didnt know what to expect, since this was the first time being in a Healthcare Center. I never realized just how much work is involved in what these Caregivers have to do to help us patients. They went above and beyond to assist me. The food was Good. It was hard for me to swallow food in the beginning so i had to have soft foods, then little by little i went to regular food. Always had a decent Breakfast, Lunch and Dinner. They have excellent Therapists . When i came in i couldnt even stand, when i was released i was able to walk in a Walker. I would recommend this facility. I was discharged from Brookfield Healthcare Thursday and I must say that this place treated me well. I've been in a few facilities over the years, and this one is tops on my list. The nursing assistants who do the real work were so kind and helpful. Always with a smile! Thank you Darwin, Abby, Wendy, Carla, and a few I can't remember their names. My meds were given on time and thank you to the nursing staff. Thank you Lu Lu for helping me to heal so quickly. The place is super clean. Never have i seen house keeping do such a thorough job. The kitchen staff was more than happy to make me an alternative to meals served. They do a wonderful job making well balanced tasty meals. Activities made sure to offer the residents a variety of things to break up the day to day routines. Laundry services were quick, and never lost any of my clothing. Much thanks for that! Maintenance was quick to address concerns. The rooms had televisions that were wired for headphones that made for a quiet peaceful stay. Not to mention a roommate who was compatible. Brookfield is a smaller facility which makes it easier to give personal attention to the residents. The social services director was there to help with my concerns in a very friendly and professional manner. Thank you Mari Cris for helping me with concerns regarding online courses. I have been a nurse since 2006, with majority of my nursing career working at Many Skilled nursing facilities. I worked part time at Brookfield and absolutely loved working there. Hard days were never hard because of the culture and the teamwork. Being that I was part time I was nervous that I would always have to re-orient myself, but my fellow nurses at Brookfield supported me and did not ignore my questions. Not only were the nurses helpful, but Management is very approachable. The DON responded each time I had a question even if it was after working hours. First impressions last a long time. Brookfield exceeded all mine and my family's expectations. If you ever want to know how a facility truly operates show up on a weekend because that's when most facilities run on a skeleton staff. Although Brookfield did run on a different staff on the weekends they surely did not disappoint with my grandmother's care. They were on it! This facility truly cares for the well being of their patients. It is honestly hard to find people that genuinely care. My grandma was unable to walk or do many things on her own. This being the first time in her life this has ever happened I know it was hard for my grandma to come to a realization that she no longer could care for herself. My grandma was deeply depressed, unhappy and easily disturbed. Brookfield did an amazing job helping my grandma lift her spirits. They slowly took baby steps to get her to involve herself with activities, eating at the dinning room table and talking with her neighbors. In spite of my grandma being nervous (as well as the family) and confused about leaving Brookfield she was in good spirits. Thank you to all the staff at Brookfield who managed my grandmas care.
