This place has a Rodent problem. The night staff , primarily the African man has a problem. He lies about anything. He is rude and unprofessional. I hope to remove my mom soon to a better nursing home for this place is run like a prison. Beware if you ask a question they treat you really bad and talk badly behind your back.Please research other nursing homes.I for one think something is is not right with this place. my rehab therapy stay (11/08-11/19/2018) (i was in the 300 section rm 302 in a very clean mini suite) from day one i received excellent care ! i was there with a total knee replacement & i needed a lot of care with the pain level i was experiencing ! the food was good & i had great choices ! all of the employees who cared for me was amazing (from my therapists, nurses, management, cna's, housekeeping, activity, front desk, dietary, office personnel, doctor & any employees who care for me during my stay i would like to thank you all ! your kindness in making me feel so welcomed while caring for me will never be forgotten ! i was a first time patient ! yes, i do recommend (the villa at evergreen park for your rehab care treatment) thanks again ! Villa at Evergreen Park had a rocky start with me, and I can blame it primarily on Jackson Park Hospital's social worker lying about it being a long-term care facility. (Even the Blue Book says it's short term.) This set off a train of confusion, frustrated phone calls, rush to find a (legitimate) long-term care facility, and unanswered questions for the family member who was sent there. The social worker (Terry) at Villa should get 5 out of 5 stars because she was helpful, patient, professional, friendly, and just an all-around good person. There was also a tech there (forgot her name but she was brown-skinned and a Gemini, cannot remember how we got on the topic of astrology) who was phenomenal: funny and friendly and the family member grinned as soon as he saw her. He thought that tech was great. Now comes all the things that irritated me to no end: 1. The family member, who was a fall risk, was left in the hallway unattended when I got there. I went to his room to find him and the bed was empty and made. I finally found a nurse at the front who walked with me to find him, and that's when I noticed a familiar face in the hallways in a wheelchair. He was cold. He was tired. He wanted a blanket. And he didn't know where he was. He was also ALONE. Now if someone is a fall risk and must be by a nursing station to avoid an accident, what's the logic in taking him out of bed to then put him in a hallway by himself? 2. Paul, the family member's nurse, was nowhere to be found for hours (I was there for three, and he didn't appear for about 2.5 of those hours) even though I'd specifically asked to speak with the nurse. 3. Paul was hiding behind one of the nursing stations eavesdropping on a conversation I had with a C.S. rep named Ann (friendly, upbeat, I like her) about my family member being left unattended in the hallway. I was baffled that when I turned around, he LITERALLY ran behind the nursing station in hopes I didn't see him. (I already knew who he was because he'd been described as a brown-skinned guy with locks.) 4. The nursing manager Ms. Love made entirely too many excuses for Paul disappearing. When family member said he was forced to be in hallway in wheelchair until midnight, she blamed it on dementia. 5. Ms. Love again rationalized her statements by saying since Villa doesn't have medication to make fall-risk patients be still in bed so that that was the reason nurses had to keep him in the hallway. Why her first line of defense was medicating the patient took me right back to my biggest issue with Jackson Park Hospital. The motto seems to be drug the patient first, try to verbally talk with the patient as a last resort. No matter what I told her, she had an excuse for it by blaming my family member with dementia. Meanwhile he's sitting there perfectly alert and talking to me clearly and calmly every time I went to see him (for HOURS and several days). The only reason she stopped trying to blame the family member was because I asked her how was she going to blame dementia for something I'd seen with MY OWN eyes. She couldn't figure out a way to somehow blame dementia on me too, so then she suddenly wanted to take my complaint. I asked her to go away. I would never trust her to pass my complaint along. 6. I encountered another nursing floor manager and customer service rep. who were trying to get to the bottom of why he was left alone at midnight and midday when I found him. No one knew. The nursing floor manager openly stated that on weekends the workers can get a bit lackadaisical in their work. News flash: You're the manager and YOU set the tone. 7. The head nursing manager in charge, Odett Delapaz, never called me back nor made any move to contact me when she physically knew I was in the hospital after the Paul and Ms. Love incident. Then again, I'm not surprised that Odett never contacted me. She still had some random woman named Marie Manuel on her voicemail. If she can't even take the time to correct her voicemail, I'm barely convinced she bothers to listen to any of her messages. 8. The TA (don't know her name, heavyset light-skinned woman with entirely too much attitude) up and left my family member on the toilet because it was time for her to go. She literally put him on the toilet and bounced seconds later, telling me someone else would come to get him. The next day she did ask me did "someone come in a reasonable time," but my issue is that if she had to ask me that, then clearly she wasn't sure. I refused to say more than a couple words to her. She'd already pissed me off because she got mad that she had to help the family member to the restroom and it was a false call. That's what you get paid to do. To get mad at someone for not using the restroom is insanely idiotic, and I don't think she has any business in the health industry. Find a job where you can work solo and have zero communication with sick people. 9. Family member left with a bed sore. read more

If you have an option, I would recommend not letting your loved one stay at the Villa. My father was placed in The Villa for wound care in October by his team of surgeons since they work closely with the wound care teams at UIC and Christ. He was not able to walk or stand on his own, so he needed assistance getting into bed, out of bed, into his wheelchair, etc. The wound care team was fantastic, and rehab was decent, but not very trying or effective, but the staff there is horrible. My father's room was very dimly lit with burnt out light bulbs, and the bulbs were not replaced at any point during the several weeks he stayed there. What's more unsettling is that he was in the Wound Care wing, where I would think light would be necessary to clean and dress massive wounds. Some of the unacceptable situations he had to suffer through during his stay included: being denied a bedpan by a CNA who sat on the couch in his room asking him repeatedly if he "was sure" he needed a bedpan; being helped to the toilet by a CNA, pressing the call light to let her know he was finished, and her walking away and leaving him sit there for 20 minutes when she happened to return and questioned why he was still sitting there. She'd been one of his CNAs for many days and still wasn't aware that he couldn't stand unassisted; having them place his dinner tray on his table in his pitch black room and not turning the lights on, when he was unable to get out of bed and turn the lights on himself; taking 3 days to replace the batteries in his TV remote, so he had nothing to do buy lay in bed and stare at the ceiling for three days though he repeatedly asked about the TV. When I finally asked, they replaced them. His room was at the end of the hall, farthest from the nurses station, and he was a fall risk. His call light accidentally got wrapped around his bed and pulled from the wall. When I went to tell the nurse at the nurses station the call light was pulled out and broken, she said she'd call maintenance. A very helpful, nice gentleman that works there told me if maintenance didn't come to his room to fix the light in 15 minutes, to remind the nurse to call them again. After I waited 25 minutes (and needed to leave for work), I again approached the nurses station and told her the gentleman told me that if they hadn't come in 15 minutes to remind her. She snapped at me that she had called the Admin Assistant. I asked her what my dad should do in an emergency, then, until his call light was fixed. She said he could call the Receptionist (at the front of the facility). I said--so the receptionist is going to bring him a bed pan or help him up if he falls? She then realized my dad's situation and replied that she guessed he could call the nurses station if he needed help. The residents there are not fed enough food. I had to bring my dad dinner and snacks and Ensures everyday to supplement because he was losing weight in The Villa. Dinner, for instance would be 1 taco. That may be enough for the very elderly or ill, but not for an active person doing rehabilitation. His meals were always cold because he was at the end of the hall, as well. My dad did point out that there were 1 or 2 good nurses that worked there, but overall, they were neglectful, took a long time to answer the call lights and did not have any idea about his condition of not being mobile without assistance. I was worried about him every single day he was in there. He left there for surgery, and we opted for another facility to rehab in when he was discharged from the hospital. read more

I really want to give zero stars. The Villa looks like a spa hotel when you initially walk in but looks can be deceiving. My mom was there for rehab after a fall and subsequent surgery for a broken hip and shoulder. The care she received was terrible. The CNAs had to be reminded to wear gloves and wash their hands. They would dress her an leave her in the chair for hours without checking. She would press the button at night and they would not come for hours. The food is horrible. I had to tell the nurse something was wrong with her to get them to run test. The results were so bad they sent her by ambulance to the hospital. By that time she was to weak to fight. She died a week later. If your loved one cant fight for themselves don't put them at the Villa. read more

0 stars if possible. Upon reading how wonderful this place is; executive chefs, awesome food, etc. My brother never had that service. I have picture of the bugs all over his food; he was unable to feed himself and no staff ever, ever feed him; wound care was never administered; I, by recommendation of his dr. feed him; provided wound care; (as was taught to me by the dr.'s staff) have picture of urine next to his bed, that was dried up all over the floor and our feet were sticking to the floor as we walked in. He was bedridden and the phone was well without reach, as was the urinal. He decided to go into the bathroom by himself (since no one ever answered his call for help} He fell and lay there begging for help for hours. I have pictures of the bugs; urine on the floor; phone out of reach; urinal out of reach. This place really, really needs to be shut down! Oh, and by the way, my brother is dead!! Filthy rooms, flatware under the bed (have pictures); used gloves on chairs; and most light bulbs were burned out. Took forever for them to replace light bulbs. We brought light bulbs from home. read more
