Whenever I visited, I was treated warmly, and my father was always resting comfortably in a clean room and a pristine bed, with carefully laundered, comfortable clothes. He had dementia, so he was unable to tell us how the staff was treating him (and never having been the type of person to complain) we had to take non-verbal cues. All the cues pointed to a staff that was both patient and caring. Trust was essential to us and something we considered heavily before deciding on a place for our beloved father. During his final days, our family experienced feelings that I cannot properly put into words here. Likewise, I am unable to express how sensitive the staff was in their treatment of my father. I imagine that they deal with loss on a regular basis, but never once did they regard our grief casually. My father was a kind, gentle, man and was known for his singing voice, his whistling, his love of jazz and his love of the Word. The staff made sure he was able to enjoy all these things during his final days. They all knew him by name, and said he was easy to care for and easy to care about. This was evident in their heartfelt farewell. You can not pay someone to care, and [care] is what they did here. Perhaps I have not adequately relayed, but in my heart I will absolutely, always appreciate every last member of this staff for giving of themselves so selflessly, when it counted most.

My wife and I really loved this facility and so did my 96 year old father. The Five Star brand is…read morea good one in philosophy, individualization of care, quality of programs and people. However, the execution of the Five Star brand philosophy and standards are greatly dependent of the executive directors of the various facilities. The former executive director at Legacy Heights was superb. A great leader and manager of people and processes. Shea knew the residents, was regularly out of his office circulating, working with his staff, mingling with families and visitors and otherwise was not only an outstanding manager, but was also an involved leader with and supporter of his directors and working staff members. In fact, Shea was such an outstanding executive director that Five Star saw fit to promote him to a regional director position. The facility was then staffed with good people and vacancies were quickly filled on a regular basis. A nice, personable woman, the incoming executive director had a management style as more of an administrator than that of an executive director. She was distant from her staff, was rarely out of her office and was only peripherally involved in the day-to-day operation. She discontinued the monthly resident meetings after her first two months, staff and her directors were responsible for any incident and ultimately a "cover yourself" attitude became the norm. In our opinion, control was more important than care. The memory care unit was dismal. My dad was put there twice due to medication-induced behaviors. He was returned to assisted living once the effects of the medications ceased. He referred to the memory unit as jail and that many of the residents there were "zombies." Care in the memory unit was spotty as we would regularly find dad un-groomed, bed not straightened, laundry piled up and found sitting in a room in front of a TV. Other than meals, the residents were in that room or moved into a larger community room for activities. We were not alone in questioning the care delivered and assistance being provided. Ultimately dad lost the ability to walk and became wheelchair-bound. Legacy Heights offered him to stay on; but only if he stayed in the memory care unit. We decided it would be best to put dad in a more care-focused and less draconian environment. Another Five Star facility had a bed and he now is there. The attention and care he is receiving is similar to what he originally had at Legacy Heights and the environment is more open and comfortable than he had in the Legacy Heights memory unit. And the biggest plus is that the memory care operation respects and caters to resident peculiarities. Meal times are flexible, if they wake up at 3am and want a cup of coffee, are hungry or need their shirt buttoned there is someone there to accommodate. And the management and staff are demonstrating warm, caring and pleasant. Dad is back into a great environment and is pleased. My wife and I had shopped at least 9 facilities when we were looking at a place for dad four years ago. Be sensitive to the atmosphere of the staff and attitude of the directors at which ever facilities you consider for your loved ones.

My family had a very disappointing experience with Legacy Heights while my grandmother was a…read moreresident. From the beginning, many staff members made even the simplest requests feel like an inconvenience. It often seemed as though providing basic care was treated as a burden rather than part of their responsibility. Communication was consistently poor, and we always had to follow up multiple times to get answers or assistance. Unfortunately, the communication issues appeared to stem from the top. Our interactions with the Executive Director and the leadership team were often just as unresponsive as our interactions with other staff. It became clear that the lack of communication, accountability, and urgency started with leadership and trickled down throughout the facility. While it's easy to blame staffing changes or new ownership, our concerns existed before those transitions and continued throughout my grandmother's stay. One of our biggest concerns was medication management. There were multiple occasions when my grandmother was given another resident's medication, which is completely unacceptable. There were also several times when the facility ran out of her prescribed medications without notifying our family. Had we been informed, we could have easily brought her medications from home to ensure she didn't go without them. What hurt the most was the lack of compassion. My grandmother was hospitalized multiple times, was eventually placed on hospice, and ultimately passed away. At no point did anyone from Legacy Heights check on her or our family. We had to repeatedly ask for move-out instructions instead of receiving proactive communication during an already difficult time. I believe families deserve to know what our experience was like. Entrusting a loved one to an assisted living facility requires tremendous faith, and unfortunately, Legacy Heights did not provide the level of care, communication, accountability, or compassion that my grandmother deserved. To be fair, the facility itself is clean. If your loved one is largely independent and requires no assistance, it may be a suitable option. However, based on our experience, I would not recommend Legacy Heights to anyone, especially for someone who needs any care, medication management, or responsive communication with their family.

I really liked a lot about the facility and all the employees we met were super nice and helpful. The main dining room, bar, and lounge areas looked like a country club and indeed we found out that they have a happy hour that is apparently very popular. I liked the fact that the independent living apartments had patios & balconies. However, there were a couple of negatives. The main one was that the facility seemed a little dark & dreary inside despite the fact that it was a beautiful sunny day. I think it is because the facility was built a long time ago (The Carriage Club has been there for over 20 years) and they have added on over time. It's a shame, but we ruled out this facility because of the darkness. They really should do some remodeling to give the space a more contemporary bright & open feel.

The management cannot keep up with basic services such as housecleaning and do not offer a rebate for services omitted but already paid for. The food has become so mediocre to bad that many residents go out more often than eat in, eve though meals are provided 25 times a month. And the place is decrepit. If it did not cost so much to move, most residents would be gone. Worst of all, the resident manager, Melissa, hides away in her distant office so that she does not have to interact with residents.

Once released, he required a stay in a rehab facility where the wound could be treated until it healed. He was admitted to The Pavilion and, from the beginning, there were problems. I was forced to be heavily involved, finding it necessary to follow up with the nurse's station daily to ensure his bandage was changed and he was showered on his scheduled days. On the night of October 29th, my father called and said he couldn't feel his legs. After three calls to the facility someone finally called me back to say they checked on him and he was fine. The next morning I received a call from my father and his speech was slurred. My wife arrived at the facility at noon to check on him. She arrived to find him lying in his bed, unclothed, feverish and agitated. The foot with the wound appeared purple. He looked up and called out for water, in a scratchy voice. The temperature in the room was set on heat, it was 85 degrees in the room and the window was open. She pushed the help button, but no one came. She searched hallway carts until she found a cup and water pitcher. My wife found a nurse who looked at my father and said they would run some tests, which could take three days for results, but could start him on antibiotics. They then stated they didn't have the antibiotics and they would need to be ordered. At that point we were not comfortable leaving him in the facility and requested that she call an ambulance. While we were waiting for the ambulance, my father asked to go to the bathroom. We pushed the button, but no one arrived, and after 15 minutes, he soiled himself. My wife walked the halls, found the nurse and told her he needed to be changed. Her response was that "they'll change him when he gets to the hospital". After a brief contentious exchange, the nurse and an aide came in to change him. He was taken to Novant Presbyterian where they determined he had sepsis and it had spread throughout his body. After discussions with the doctors, and family, he was placed on hospice and passed away four hours later from septic shock. In our experience, the apathy and unpleasant demeanor of the Pavilion staff, in general, was horrific. Without our active intervention, it is our opinion my father would have passed away unclothed, soiled, dehydrated and sadly alone, as not one person stopped to check on him over the several hours we were there. I filed a complaint with the North Carolina Department of Health and Human Services and they have opened an investigation.
