My family member was here for 9 days and we found the staff and nurses to be extremely caring and helpful. There was only one instance where a nurse was a bit rough- and the facility was quick to respond. They are working hard to turn this place around and it is MUCH better than it was- it's known as SOLANO POST ACUTE now. The roof is in process of full replacement; maintenance staff are very responsive; and the nurse to patient ratio is reasonable. The physical therapy room is well-equipped and the physical therapists are excellent; they work hard to help each patient achieve their goals. The rooms are a bit cramped with three people in each one, and I wish all three had access to natural light; one bathroom is shared between two rooms (up to six people). The food goes from really good to decent- but in the 9 days there we didn't find one meal where she couldn't find something to eat. With a little more work (e.g., the back entrance is not ADA-compliant; they need to install a better front entrance system with video and vestibule; some lighting needs to be fixed; parking area needs to be made safer with more lighting and reconfiguration), this facility will be one Vallejo can be proud of for those who need care before going home. read more

There should be "negative stars" to rate this place. It is a sh-thole. At the time my mother was admitted to this place, she told us she saw mice in her room. They neglected my mother - she got extreme bedsores that the staff didn't tell us about. She ended up being extremely dehydrated, malnourished, and with a case of pneumonia. She ended up bleeding, and the doctor sent her to the ER. She ended up in the ICU, where we were told she would just have months to live, because of what happened to her at this horrible place. The head of the ICU was shocked. My mother kept going downhill after that, with multiple infections, and passed away several months later. This place should be closed down. read more

I am currently a patient at Windsor Vallejo Health Care Center. I have been here over three(3) years. My physical therapy, consists of doing one repetition each up and down with each arm, one repetition side to side, with each arm. Then, with my legs, I raise each leg in the air, for ten repetitions, and then from side to side. After that, I bring my knees up as far to my my stomach as possible, for ten repetitions each knee. The purpose of my review is to bring awareness to family and loved ones that have them sent to Windsor nursing home and rehab center in Vallejo, that place is worse than a prison. The negligence of the CNAS and the nurses make the conditions hazardous, and essentially leave the patients to die. My father was recently transferred there, and the condition he was left in was shocking. He was covered in feces for hours at a time, severely dehydrated, and starving due to the lack of help. On 4th try, a very rude person said she'd leave him a message but couldn't guarantee he'd call back. He's unable to dial on his own, but she sounded very abrupt and could've cared less about helping us reach him. Sounds like an awful place. We're an hour away, so feeling helpless. read more

I don't write negative reviews lightly. But after nearly two months of my husband staying at this facility, I feel it's important to share our experience--especially for families considering care here for their loved ones. As someone who works in healthcare, I understand how demanding this field is. I also understand the challenges that come with staff shortages. However, what we experienced went beyond what can be explained by staffing alone. For example, my husband's bed linens were not changed for over two weeks. I eventually brought clean sheets from home and changed them myself. The food served daily was heavily based on starch, with very little in the way of fresh vegetables or balanced nutrition--something vital for recovery. There were also ongoing issues with temperature control. Due to construction, staff were using the patio door in my husband's room to deliver items, but they frequently left it open. On cold days, this made the room freezing. On one occasion, it took four hours for someone to respond to a nurse call to close the door. My husband, still in pain and physically weak, had to get up and close it himself. Sadly, this happened more than once. We also requested a firmer mattress multiple times, as soft beds worsen his condition. Although staff acknowledged and agreed with the request, it was never addressed. I sat on the mattress myself--it was too soft and unsupportive, clearly not suitable for someone with physical needs. One of the most troubling aspects was how passive or non-verbal patients--those who don't advocate for themselves--were often met with indifference or even coldness. It made me worry patients who may not have someone speaking up on their behalf. To be fair, there are kind and competent staff and therapists here. We're grateful to them. But their efforts are often drowned out by what feels like a broader culture of neglect and lack of accountability. If you are considering this facility for someone you care about, I urge you to visit often, ask questions, and advocate persistently. Keep track of what is promised and what actually happens. If something feels off, trust your instincts and speak up. Patients and families do have the right to report concerns to the public health department. If you witness neglect or unsafe conditions, you are not alone--and protections are in place to prevent retaliation. This facility is absolutely abysmal and wouldn't recommend it to my worst enemy. My FIL had to use the restroom (number 2) but his nurse was on break. The rest of the so called "nurses" claimed that they weren't available to help despite just sitting around, apparently unfazed but the call lights and screams of "nurse!" From other rooms. I've been thinking a lot about my dad lately and decided to read some recent reviews. My dad was at this hell hole of a facility March 2023. He was only supposed to be there for a week for some physical therapy so he could go back to his Assisted-living facility. After almost 2 weeks of absolute abuse, neglect and little to no physical therapy, I begged them to release him back to the hospital to which they replied, "he's fine he's stable no indication to transfer". I lived out of state and every time my dad would call me crying and begging me to get him out of there because he was going to die, I would ring their phones off the hook and talk to every administrator I could, but in the end, it did nothing to help my dad. I received a call after the two weeks of my dad's horrific stay telling me his swelling was so out of control that they would need to transfer him back to Sutter ED where he ended up in the ICU. I won't go into a lot of extra details from the hospital, but needless to say he never recovered. He did end up having to go to Fairfield Acute, which I did plenty of research before hand, and for the two weeks he was there he received the much needed compassionate care before he died. I should've sued the pants off of Windsor Vallejo. Having been a 21 year employee of Sutter health Systems I am appalled that they would even send patients to this place which contributes to the rapid deterioration of their patients. If you've had even close to this experience with your loved ones and you're within the timeframe to file a lawsuit, I urge you to do so. read more

Communication could be better! Windsor had us pay a share of cost of $2258 in which we paid in July 2025. We found out there was no such share of cost for my Mother in law. Daisy & Ivy (Business office) said they will get us a refund in a 4-6 weeks. That never happened. After many calls and leaving messages they finally called back 2 months later saying it was up to their corporate office to issue the refund. Oct 6, still no chk refund, Daisy said she will email corp office again. 10/23 Left msg, still no reply from Daisy. 10/31 Left msg again and nothing. 11/3 Left msg for Daisy & Ivy no reply again. Left msg for General Mgr, nothing. Finally about 2pm Daisy calls my husband and says the chk has been ready for pick up since last week. Really people! You were given consent that it was okay to talk to me, my husband, or my brother in law but you would say you can't, that it had to be my Step Father in law only. Now you decide to call my husband? ‍ Sad that these business accountants have no sense in customer service! Brain is only in numbers but other than that just plain dumb! Windsor's Vallejo facility is poorly run to say the least. Patients have the right to complain to the California Department of Public Health. Please do not make the mistake of leaving your loved one in this den if you can avoid it. As a patient, I have had to endure: a toilet that is not regularly cleaned or mopped; a toilet that was plugged for over 11 hours, and no toilet paper in the same toilet for 10 hours. I got COVID while I was here and they never told me. They added COVID medication to regimen. When I noticed the new meds and googled the pills and that is how I learned. I have nerve damage to my lower back. They use a weighing apparatus that lifts you from the bed. The first time they weighed me, they were careless and aggravated my injury. The second time they weighed me, they did not apply support to my neck and I was injured again. The weighing crew made errors in weighing me and recording my weight, and simply refused to take responsibility for the mistake. At some point, it was decided that my blood sugar should be checked (3 times per day). One of the nurses had a particularly difficult time performing the finger prick. She needed as many as 3 attempts before she could get a blood sample. I asked if the nurses were recording the attempts as well as the sugar levels. It became an immediate concern for me when l was informed that the staff was not recording their attempts. I have also observed a disturbing practice among the staff of either ignoring or speaking harshly to the patients who do not receive visits from loved ones. This abuse is stressful to witness. One of the reasons Kaiser transferred me here is so that I could receive regular physical and occupational therapy. The director of this facility came to my bedside and told me that Windsor was not being paid for my treatment. My injury resulted in the workplace, and what ever the cause of the mix up had nothing to do with me, nevertheless, the facility discontinued giving me physical and occupational therapy. It has been over a month that I have been here without any therapy. The staff is aware that I have stairs in my home. Negotiating stairs had been part of my therapy. Windsor is ignoring my fall risk. Windsor staff ignored my requests for an additional bedside urinal. I needed a second urinal for the frequent urges to pee. I eventually found one. The CNA complained to me that I was peeing too much. I don't see a need to post pictures of full urinals or overflowing toilets. This facility neglects their patients. My dad has dementia & was admitted here after hip replacement surgery. He has fallen several times & the last one was the worst. The nurse on duty that night told me they put him on a wheelchair in the nurses station so they can keep an eye on him yet they walk away doing who knows what and he ends up having an unwitnessed fall resulting in a broken femur. I spoke with their administrator and apparently they do not have any fall prevention protocols in place even for dementia patients. I was told they just do "something different" every time a patient has a fall. Is it me or are you suppose to PREVENT an injury from happening rather than dealing with it after the fact? read more

My mom was discharged from Windsor Vallejo Care Center recently. Our experience was great. The workers were understanding, good customer service and had a lot of patience with my mom and her diagnosis. The therapy department had a lot of patience with my mom as well. Despite the old reviews we took a chance and had a great experience. They have new owners and hired a lot of new staff. The facility was clean and they had a lot of activities for my mom to do while she was there to keeps her busy. The experience was a little rough due to my mom first time at a facility but overall we had a good experience. read more

A friend of the family recently stayed at Windsor Care Center and I have heard nothing but wonderful things. The staff are extremely friendly and compassionate. Good food, friendly roommates, and awesome activities while she was there. This stay was for a brief rehabilitation of a broken ankle, her physical therapist was the highlight of her stay. She had nothing but amazing things to say about her. She was supposed to be there for 30 days and left in 14 days because she met all of her goals. So my grandmother who is 86 years old was sent to Windsor care facility for rehab before she gets back home due to my grandmother's falling. My grandmother has a hard time getting around, so she's pretty much bedbound. She was receiving a bed bath on the night of October 10 by two nurses one was talking to her and the other one was washing her, my grandmother said that the one who was talking to her was making she my grandmother didn't look at the other girl, while the other girl got her very soapy and removed all her rings valued at 10k, during the hand massage and rinsing,(yes we are kicking ourselves because we should have removed the rings from her before arriving but at the time we were just concerned about her safety.) my grandmother called my grandfather when she woke up the next morning and noticed that they were gone, my grandfather called me and alerted the management staff, I called the police who haven't seemed to be much help being that the officer who took the report when on vacation the next day. The manager of the facility said that he conducted an interview with all of the staff and nobody is admitting to it so there's nothing that he can do.this doesn't seem fair or right.it seems very unjust. I have worked at elderly facilities before and I have seen and heard of people getting fired for less. My grandmother can identify these two girls and nobody in the facility has asked her who they were when my grandfather and I told the director that we were going to get the media involved and that we were not going to stop and this was going to turn into a lawsuit he picked does not to because he did not want a black eye for Windsor care. The director has not been upfront with the police. In fact, he is avoiding the police who have tried to make several attempts to speak with him over the phone and in person. while I've been visiting my grandmother, I over heard 2 other women make claims to their family's that their jewelry was also stolen so now I'm wondering if this place just steals jewelry from the elderly and has no accountability and is allowing their employees to take advantage of these people who are there to get better. I'm very pussed off and shocked that someone who works with the elderly would steal from them. It seems like our world truly is going to hell. Please don't send your loved ones to Windsor care. Windsor is an awful facility, this place is very uncaring. Here my mom received terrible care call lights aren't answered, residents aren't taken care of if they need assistance. My mom was there for rehabilitation, but received additional injuries do to neglect while there under a week, she was allowed to get out of bed alone , fell fractured her shoulder , now she's worst off than when it began. I stayed in this facility for five horrible nights. They did not change the bandage on my knee replacement for five days and it got infected and I had to have another surgery. My roommates were horrific, leaving their TVs on all night and wanting to keep getting up and going out to smoke. it was extremely difficult to get a nurse to come and take care of me. The individual efforts of employees and staff made staying at Windsor more of a home field and relaxing for my recovery... My first experience at Windsor was a year and a half ago and a lot has improved and become more successful in the way they treat their patients and how's their patients.. despite the fact of individual employees that are still troublesome overall Windsors a place I choose to come back to.. This place is not safe for your loved ones, I can't believe Kaiser or any other hospital would send anyone there for care. I looked at the reviews ands they weren't good but didn't have any other choice because they were the only place that had a bed available at that time to transport my mom there unless I wanted to go 50 miles away. This place should be shut down, every morning when I showed up my mom was stressed and crying, she was saturated wet all the way from her diapers and leaking through all her bedding and clothes, they don't check on them and when the emergency call button is pushed nobody comes, sometime over a hour. I watch the workers in the hall with several of the light on in the rooms and it seems like they ignore it, if you let them know they say they'll be there in a few minutes and it may be another 1/2 hour to 1hr again. They always blame their other coworkers also. It even gets worse, they don't manage they're medicine right, I went over all the meds with them and still caught them giving wrong meds, also not following directions on what other supplements they need to give her that they said they had and would give to her. Some days she didn't even get fed, found out that they would bring meals late while therapy comes to pick her up and when she comes back from therapy they would already throw her food out uneaten. She was suppose to get 45 minutes to 1 hour a day of both Physical Therapy and the same for Occupational Therapy but was only getting about a hour combined, We actually time led it and questioned it and found out they're logging a lot more hours then what they actually give and then blame they're computer or another person. The other problem they haves is that they don't control other patience and let them wonder around without guidance, they have patience with dementia and Alzheimer's wondering around in the halls and walking into other patience's rooms, one of them came in my moms room twice and when he noticed me he left but my mom says when I'm not there or anyone else that this patient comes in and talks strange things to her, this is also late at night. I talked to the staff and they apologized and says it happens and they try to watch the patients from doing this but they always don't monitor them because they get to busy sometimes. They're definitely under staffed and not managed very well. I'm sure all the facilities are understaffed but this place really needs to do something about it. After about a week into her rehab there I ended up pulling her out early, she was so stressed and crying all the time and nobody around to attend her. They had excuses for everything and none of them would take responsibility, they would just blame it on someone else. We literally seen fights I. The hall and staff members mad at each other. Also there were 2 other woman having a fist fight in the parking lot that my brother had to call 911 on. I can go on and on on more things but hopefully everyone gets the picture that reads this review. read more

Grandpa was there for a month and left last week. He went to the hospital because he had a massive stroke. At first he was depressed because of the lack of mobility he was experiencing but therapy and the staff encouraged him daily. When he wasn't working out he went to the activates department and they always kept him busy. My grandpa is doing so good now. He is walking again. We are so thankful for the staff. I visited my mother almost daily-within thee first week I found her in a wheelchair dripping urine! She had been left that way for 3-4 hrs after PT! -other patients complained being left for hrs in similar situations-which I found myself advocating for. Staffing can be so short especially holiday weekends! This facility is a joke. The staff is not in tune with their surroundings, CNAs are not qualified, and the atmosphere is like a mental ward. My mother was there for 10days and she fell out of her bed once and suffered from two diabetic, life threatening, episodes on two occasions. Update: Unfortunately, my father passed away within 2 weeks of leaving this facility! It literally put him ON his deathbed. Kaiser uses this facility for acute care recovery! This is a horrible facility, but we had no choice! My dad fell from his bed one night, and his bed neighbor informed us that it took the staff 90 minutes to help him! We were also told by director that if we did not like it there we could leave! I could go on and on about level of care! Many CTA's work hard, but it is severely understaffed. Hours spent handing out meals instead of patient care. Icing on the cake, an employee said they would not send their dog there! Other employees encouraged us to keep complaining and advocating for our family member. Hopefully Kaiser will stop their contract. If you are sent there, stay vigil. read more

He said they didn't give him a wash up nor bath before he left --reception staff need trainings in reception etiquette --extremely hard to get in touch w the nurse just to find out how pops was doing for the day --rooms where NOT sanitized per Covid guidelines in btwn pt visits or not even cleaned at all On a scale of 0-10 on sending your loved one here with 0 being no to 10 being send, I'd give them a negative 1000!!! I wish I could've left 5 stars again, but I can't. My mom was there for about 3 weeks. She couldn't walk due to UTI and Gout (per Kaiser) But when I asked them if she still has gout because that would affect somebody with dementia to walk, they couldn't didn't know! My sister was on FaceTime with my mom and she could not work the TV - One of the CNA's came in and in a stern voice said "I told you how to turn the TV on". My sister was on FaceTime and heard that and put them in their place. You don't yell or lose your patience with someone who has Alzheimer's or dementia. You don't just leave them in a room to fend for themselves. It's so sad that most of these rehab facilities have one or two star ratings because they are run poorly. The admin, Elizabeth was great, but as far as care, I found it difficult to get answers and understand what was going on with my mom. Kaiser is at fault as well. They are impossible to get answers from. My mom is in a boarding home now and doing much better. If you do have a loved one here, make sure you visit, call them and get answers. read more

When we arrived, we signed in and waited for someone to come up to the front desk . Moments passed until we were directed to go to another room (though later when we came out there was a security officer sitting there). From there they directed us to our loved ones room, where he tells us that will not let him leave. His primary doctor told him to go in for 3 days because he had been dizzy, it has been 9. He hasn't even seen a doctor since he's been there! He told me there's another guy there and he hasn't seen a doctor in weeks. They also feed them heart disease, cancer causing food as well. I would not recommend for anyone to send a loved one to this establishment, period. There is no apparent management on-site and those supposedly in charge are indifferent, uncaring, unavailable, and inept. My best friend who transferred there to be closer to home was so badly neglected that he had to be brought home, arriving with stage four pressure sores. He is now in a local critical care unit and is suffering from infections, severe weight loss, and terrible pain. He was discharged without any of the medications he'd been taking and no way to get them filled. The food served is of poor quality and preparation and did not follow his personal physician's instructions. Inmates in prison are better fed. We were often forced to bring him meals because the food he was given was so unpalatable. I cannot understand how this "care" center continues to remain open. Vallejo Care Center's name is misleading. It is nothing more than a money-making human warehouse and should not be allowed to be in this business. I am reminded of Dante's words: "Abandon hope all ye who enter here". Soon after my friend was brought there I read over the previous reviews and felt a good choice had been made. But now, after our first hand experiences I am, frankly, quite puzzled: I do not know what care facility all of those glowing comments are speaking of. There exists a major disconnect here. Perhaps the earlier reviews were written during a different management's tenure during a different time. If they're authentic then I'm glad for the writers that their experience was so vastly different from ours. But that is a very difficult pill to swallow and their wonderful words are of little substance or meaning. I give this business NO stars. read more

When my mother-in-law had a stroke she went there for rehabilitation she was put on a G-tube and the G-tube was hanging on the floor when I arrived to visit I asked them to change the set up right away and that should not have happened they also left her in BM while the aid was watching TV in with another patient they are not very clean and I would not send my dog there they should shut down and have their license revoked because the quality of care sucks and now they have a Outbreak of COVID-19 far too many problems for this facility to remain open the state should look into this for licensing revoking. read more

Visited my father in law, my wife and I noticed it smelled like urine in the entry way, we were greeted by the front desk as she walked back to her post I noticed that the names of all the patients were visible, clearly a violation of privacy. My father in law experienced the worst in.care, the first sign of neglect was the blatant request to put him in diapers so.that it would help staff. Like really, try to hide the "understaffed " mentality, or it's just plain laziness. They miss dosage like no other, crucial to rudimentary patient care, like they just put instructions on a label, by a provider and you just have to follow directions, but they failed to follow this on anti rejection medication of a transplant medication, that is crucial to maintain time in dosage so that the transplant works. The food looked like dog food, microwaved and thawed out sides made remember the Army and how they managed quantity over quality . But they gave the wrong food to person which are labeled by name, take a second look at the patients name and the food tray and move on. Again can't follow directions. Here is the kicker, my father in law is ambulatory and needs moderate amount of attention, they left him for multiple hours facing a wall. That's inhumane. When we complain they handed us a "against medical advice" not only an insult to injury but a blatant disregard to patient care and a clear moved to move liability from them. They also allow visitors to sign in, one guy signed in and entered all the patients rooms to say "God bless you" when questioned he said, I am a pastor and i do this for all the patients. I asked for his name and he literally jogged out the facility. Wtf . Upon doing my own research, I've learned that every since their staff has changed it's gone down hill. Even when I was there it seemed like there wasn't enough workers to help the residents, and the ones that are there didn't even seem to care(not to mention the unprofessionalism of being on their cell phones. All in all I am completely disappointed in this place for the complete 360 degree change, and my family will be moving my grandmother ASAP!! read more
