We are forever grateful for their love, compassion and dignity showed to our mother and our family. You were truly God-sent. Your gentle care, kindness and patience brought us comfort when our hearts were heavy. You did not just provide medial support; you ministered to me and my family with grace, encouragement and peace. We felt covered, supported and never alone. Thank you for walking us through this journey and honoring my mother with such respect. Special thanks to Hospice Liaison, Kerri K.; Nurse Brandy W.; Nurses; Social Worker Danielle S.; Chaplain Beth, Staff Coordinator Rhonda; CNA Supervisor, CNA Fanning and all the Certified Nursing Assistant that made sure my mom was taken care of. They provided transportation to his son's resident in Contra Costa County. When they arrived, the equipment needed had already been delivered. They assigned nurses to come daily, including a Chaplin and 24/7 hotline. After services were no longer required, they sent cards with their condolences as well as literature on their "The Grief Cafe" bereavement support. I highly recommend Suncrest Hospice for home care.

My brother and I very nervously approached Beth Quintong and Classic Care Home of Walnut Creek. Beth and her staff were extremely welcoming and kind hearted from the moment we met them. Sadly my Mom had a stroke soon after entering the home and passed away. But Dad, albeit exhausted after years of taking care of Mom, was brought into Classic Care and treated with exceptional kindness. He lasted three more years and just passed away this week. From the moment of entering Classic Care Home Dad was treated like "the Admiral"(yes he was ex-Navy) and was showered with kindness. We were lucky to find this home for Dad and couldn't recommend it with any more appreciation. Beth, JoJo, Gwen and the entire staff are angels.

Classic Care Home of Walnut Creek Having visited dozens of assisted living and residential care facilities in the Diablo Valley area, the prospect of finding a residence home which provided an optimal environment for our Mom's progressing 24/7 care needs and particularity of preferences, seemed unlikely at best. Our family was very fortunate to receive a referral which led us to the doorstep of Classic Care Home of Walnut Creek. Beth and Ed Quintong, their family members and their family of exceptional caregivers; Maria, Lynn, JoJo, Peter and Chris, were able to provide our Mom with extraordinary care and companionship when she most required it. Affording her a true sense of home, safety and belonging in her final years. Beth and her care-giving staff make a point of personally connecting with their residents. This allows them the insight to identify and provide for the individual's needs beyond rote care scenarios. It translates to more opportunities for residents to experience joy and contentment, despite the daily challenges with which they contend. Encouragement and personal social engagement are the cornerstones of Classic Care's approach to providing positive and caring support for their residents. Most importantly, they listen and address any concerns as they arise. That applies to both the residents and the family members. Beth is actively involved in all aspects of the home's daily management and oversees the care plan for each resident. This includes dietary, Rx, hygiene and other necessary assistance. Beth collaborates directly with the resident and primary family representative to best optimize the resident's sense of dignity and quality of life during their stay. Her personal involvement, focus, openness and rapport with medical providers and family members provide an informed and clear communication exchange. Her extensive experience is pivotal in finding solutions and recognizing behavioral progressions. Her partnership, honest feedback, compassion and humor are invaluable. Our Mom was a resident of Classic Care Home for just over 4 years. Beth always extended her sense of home and family into the lives of her residents and their families. That was an unexpected benefit which brought our Mom much peace during those years and at the end of her life in 2022. Beth, Ed, their family and the extraordinary individuals who provide this "classic" care to the residents, demonstrate an uncommon benevolence and personal concern for those in their charge. For all of these reasons and many others, we are deeply grateful that our Mom had the opportunity to be the recipient of such thoughtful and compassionate care. They are performing great works with grace. Our profound appreciation for their dedication and sublime efforts cannot be adequately conveyed. S. Jones and Family

I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Thank you from the bottom of my heart and the hearts of my family.

Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

Not only is the building new and well-maintained, with private rooms and bathrooms for each patient, but the quality of care truly stands out. The Director of Nursing makes a point to round on patients regularly, remaining visible and available for questions, concerns, and feedback from both patients and staff. That leadership presence carries over to the frontline nurses, who consistently treat patients with empathy and compassion--qualities that can be lacking in many other SNFs. The food is also impressive. It may not be a five-star restaurant, but even as a family member, I found the meals enjoyable and something I wouldn't turn away. Like any facility, there are opportunities for improvement. Therapy services at times seemed to be provided at a minimal level, likely reflecting staffing and workload challenges--an issue I recognize is not unique to this SNF. That said, I'm grateful my family member was able to maximize her rehabilitation. Her extended stay following an acute stay helped build her endurance, and she received excellent nursing care before being discharged home. As a family, we felt more prepared and better equipped to care for her. Now, she's slowly but surely thriving again. Thank you, Shadelands team, for being part of our journey!
