I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Thank you from the bottom of my heart and the hearts of my family.

Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

I know hospice care doesn't always last…read morevery long, but it was so nice for my mom to really get to know and become friends with her caregivers for 8 months. She appreciated and enjoyed all of them. My mom's nurse was Pam and she treated my mom with such tenderness, patience and compassion and my mom felt it. Rachel filled in for Pam a couple times and mom said she was delightful. Melissa, our social worker, offered support to my mom and all of us with such a caring heart. Julie was the nursing coordinator and was always ready to answer my questions or order meds or supplies. Sheila the nurse practitioner, and Maria the Chaplin, as well as the on-call nurses that made themselves available after hours to be there when my mom needed someone including Raj and Erica, all made us feel like our mom was their priority I hope I'm not forgetting anyone but this group of Sonata employees were like a family to us and my mom and my siblings and I grew very close to them. I've only had a few days of hospice experience from many years ago when my late husband passed away. We did not get Hospice, soon enough and only had it for four days. I had no idea how much Hospice could really do to help a patient and their family feel so loved and supported. As a retired nurse, I know I asked a lot of questions with my many phone calls to Julie, but always felt supported and heard. My mom also enjoyed everyone from Sonata that came to see her and when they'd leave she'd say, I really like her or him. It was so comforting for us to see our mom feeling so well cared for. If my mom needed anything from a medication to home care supplies, it was an easy phone call away. We love and miss our mom, but we feel so good about our decision to have Sonata Hospice be there for her last several months. In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. I, too, was a caregiver for a loved one with two, severe brain injuries. As he aged, dementia, secondary to the injuries, set in. So no easy case to manage. To state it politely... I had been caring for him for over two decades but it was time for him to be placed where his needs could be better addressed as he was about to be wheelchair-bound. A year living in a conventional, assisted living facility was a disaster (the place eventually went out of business), which was similar to the experience my mother had about a decade earlier in another facility. I worked with David Cohen of Exceptional Senior Placements (ESP) to find Moraga Retreat on Woodford Drive, in Moraga. We had finally found a *real home* for my loved one! It was a struggle for my loved one as he wanted his independence and wanted to go back to his home. Both things were not possible because of the damage to his brain and his increasing fragility. This is what Moraga Retreat owner Ana Blaj and her dedicated staff were tasked with overcoming, in addition to dealing with all of the medical and physical challenges he presented. Ana and her staff patiently and painstakingly softened his resistance, worked diligently to enhance his comfort in every way he asked and needed, and addressed his food preferences. It was heroic! No exaggeration. My loved one liked living there, knew he was cared for as well as possible and bonded with Ana, Donna, Wayne and the other caregivers like family. It was mutual affection, more than I ever thought possible. Oh sure, I was in the middle of things too much thinking I knew better about his care. Ana is an expert, certified care manager and she is locked down on the details and managing the staff and the home. It turned out no one knew better what to do for my loved one than *they did.* Period. After so much time devoted to my loved one's care, *I* could finally stand down and let people I trusted take care of him. What a relief for both of us. Moraga Retreat gave both of us our lives back. There are no words to thank them for five years of unwavering service as much as they deserve. I strongly suggest if you need help caring for your loved one, make an appointment to visit Ana and the staff at Moraga Retreat. I don't know what I would have done if I'd never met her. Truly.

However, this could not be further from the current reality. Recently my mother's care costs were randomly and dramatically increased without proper communications or justification. This egregious occurrence was met with management silence and non-communication. Upon the advice of my legal counsel, I immediately moved my mother out of this facility. This situation has placed an immense mental, physical, and financial burden on my family. In addition, my earlier Yelp review was truncated and a portion was posted on their website. Unacceptable.

The intake nurse called me on Friday and left a voice message that she would like to see my mom the following morning. She also texted me at the same time. I immediately called my mom to confirm and called her back in less them 10 minutes. No answer so I left a voice mail and a text to confirm Saturday. No response even though i returned the message immediately. The following evening she text again asking to come on Monday. I left a voice message and asked her to call me to schedule to avoid another miscommunication. No reply ever. My mom never got the home health she was ordered. No follow up whatsoever by the nurse or the company. It's a good thing I am a proactive daughter and followed up right away with her primary care physician. A more vulnerable patient without an advocate would be left high and dry.

Once treatment was not an option he was referred to home hospice and they quickly enrolled him and began providing comfort care. They made sure he had all the equipment he needed to be comfortable. This included medication, O2, bed etc... Unfortunately my dad passed away within 2 weeks of home hospice but I believe he died comfortably and with no pain. They were always responsive when I called and when my dad passed, they helped me give him his last bath. In these tough times we think that we are the only ones that need attention, I get it, I've been there but they did a marvelous job and I'm pleased with the support we received. Thank you all!!!

The entire staff is friendly and they showed they really cared about me getting well. The Nurses and CNA's were fantastic and did everything to make me feel more comfortable in a place that is hard to feel comfortable. They really did well. As far as my Physical Therapy went, it was absolutely the best I've every been to. Unfortunately I had a severe gram mal seizure and that affects the way I can walk sometimes. Some day's I couldn't even walk at all, but with the two scheduled therapy sessions I had each day I progressed to standing up with the walker and was able to safely go use the restroom when I needed too. I had to ask the nurses at first so they could keep an eye out for a fall, but that never happened. Eventually I was able to stand and walk sturdy enough I no longer had to ask during the end of my stay. The Physical Therapy Staff and department deserve all 5 stars is how I rate them. I'm a bit of a grouch so when I say 5 stars I mean it and it's for real. The Dr's were friendly and of course knowledgeable too. The front host and administrator Victoria was always on top on things as Vicroria is the perfect person to greet visitors when entering. She makes a great first impression. Abe the G.M. was extremely nice and approachable. Most G.M's don't hardly even say hello to you or acknowledge your presence , but Abe took time out to speak with me and always had a positive smile on his face.. Great guy. Letecia the activities director was great too. Wonderful interaction with her patients and always tried to make a dull day at the nursing home/ short term rehab facility positive and a little more fun.
