My sister has been on various PIC line administered medications to attempt to kill MRSA. She was admitted by Kaiser in Fontana. Her right leg below the knee and the remaining great toe were removed. She was having hallucinations, incredible confusion, and somewhat combative responses. Over the past 12 months, she has had a PIC line 6 times, obviously not killing the MRSA, hence the amputations. She continued to get worse the longer she was on this latest round. So, Kaiser does a CT that says mile Alzheimer's, short term memory loss, but puts her on a med to treat schizophrenia, never notifying any family members, or did they just tell my psycho sister with short term memory loss. Like we are talking 42 phone calls, counted them, asking where she was how did she get there and why? 42 in 3 hours, but Kaiser ditches her, after only 2 rounds of physical therapy. We were told that she can't live alone again. She gets transferred to the Apple Valler Acute Care Center. They ask me if she had any mental issues. I said OCD and Bi-Polar. They ask about the schizophrenia. Mom & I were shocked & said never. I asked why. She said well the other evening, she saw puppies coming out of the walls. Last night, she was asking them why they couldn't see the blood running down the walls, and BHO & FJB gave her a bath, she said it was nothing sexual, but she knew it really happened. I was in shock, went in to see her and she went entirely ballistic. Like Linda Blair in the Exorcist, head spinning & green stuff coming out. She was vile. We had to leave as she kept trying to stand up. It is important to note, that She has not walked 26 days. The "Care Center" gave her 4 sessions of PT. All she can do is get to a standing position from the bed with a walker. She cannot even pivot, much less do a safe transfer. They have made her use a bed pan because she falls. I went home that night and Googled the antibiotic Vancomycin induced psychosis. It states that it can rarely cause neurotoxic effects including hallucinations, confusion and acute psychosis. Most commonly occurring in patients in kidney failure, where she is at stage 3. I called and asked them not to give her anymore, call the doctor. Mind you, they are contracted with Kaiser and have a Kaiser doctor assigned to them. They tell me they can't do this, the only way is to transfer her to St. Mary's in Apple Valley and they would transfer her to Kaiser in Fontana. Mom gets there and they have checked her into the ER. They take her back and do bloodwork, their diagnosis is that she had mild short term memory loss & get with her primary care doctor, and arranged for transport. I was flying up the Cajon Pass when I get a call from AMR. They were dispatching a transport call from St. Mary's to her home. Kaiser is supposed to provide a wheelchair & walker and take care of safety features. She is a retired 37-year Kaiser employee. She has nothing. The plan was to drop, literally drop her off at her home. I refuse the transfer & doing 85-90 mph, I fly into the ER after enough security checks I thought a body cavity check was needed. I had called Apple Valley Acute "Care Center" and ask if they have a transfer order. She innocently answered, "well, I do have a note not to accept her back at all tonight and tomorrow morning, I don't know why." I wonder why Kaiser and the "Care Center" don't want her, she's acting psychotic. Why the HELL do they not consider the source, when I ask them to please contact a doctor, she is having a rare but ENTIRELY possible psychotic reaction to her antibiotics... St. Mary's said she was safe to be released. Did I mention she has no prosthetic leg. She just had the stitches removed. Apparently, when they were told that we refused to accept her at home due to that not being a safe transfer. We asked for her to be sent to Kaiser. The "Care Center" apparently were forced to take her back. Heck, they sent everything she had with her and had checked her out. They had no intention of taking her back. I called the "Care CENTER" this morning. All of the staff were in a meeting, but they would call me back. I had an appointment I had to go to, so I said ok. After 4 hours... I thought maybe they would have been setting up things with Kaiser, etc. I call, nope, can't even get with anyone in upper management. I have taken the information to contact the state board. I will file this with Kaiser I will put this on Facebook, I will post this everywhere I can. This entire system is failing so badly. read more

My 96 year old mother was there for rehab of a broken hip repaired by hip replacement. She also has macular degeneration and is nearly blind. My sisters did visit her twice a day and noticed she was getting dehydrated. The water was out of her sight and out of reach two days in a row despite us advising that she can't reach it. Also she stopped passing urine and nobody noticed. She ended back at the ER and we took her home from there read more

This place is the worst facility I've ever seen. My father is in constant pain. They are very rude, refused to assist. PLEASE don't let your family members go here. I regret allowing St Mary's picking this place for his recovery no human being should be treated the way they do. Absolutely horrible I would not recommend this place to anyone. I've never experienced unprofessionalism threats or lack of care that this Center has shown on my father and our family the unprofessionalism care facility for human beings. The lack of empathy and the knowledge to look in somebody's eyes to see there's something wrong with him, when you asked if your father's had breakfast and TA has not made any notes if he has eaten his breakfast, there is no excuse for that whatsoever. Upon leaving, they did not take any vitals. When they asked my father why he was leaving or wanted to leave he stated directly to all of them, his nurse, Matt, and all the other gurus that you do not take care of me. I've been pushing my button for water and nobody has come, which was also verified by his roommate He had been laying in bed for five days. His back was so bad. He was an extreme pain when we looked at him when they got him in the wheelchair, which all of the staff was there social workers and Matt, the physical therapist coordinator the indentions in his back were horrendous the back of his head, where his shunt is was bright red from laying on it for five days. I expressed over and over and over that I was concerned for his health that he was declining when I went in on Saturday I asked one of the nurses at the nurses station. When was he going to get out of bed? They said weekends are a kickback days for them and the physical therapist do not come in. That was her exact words. His nurse and the social service coordinator informed me that I was going to have to sign a piece of paper that I was taking my father against their will as I told them I had power of attorney over my father, and you are not taking care of him and he's going to die here. I had to sign the paper. They informed me they would not wheel my dad out to help me get him in the car and then another low class white trash woman I do not know who she was came from the right at me while I was standing at the nurses station and told them to call social services and report elderly abuse against me. I asked her if she was talking about me. She said if the shoe fits then I guess yes you Carlos wheeled my dad out and helped me with him to get him in the car. I want to praise this gentleman for his empathy and professionalism to see a 64-year-old daughter trying to get her 86-year-old dad out of there. My dad's spine was so bad that he was an extreme pain when we tried to get him in the car once we got him in, he passed out I call 911 thank you to the paramedics who got my dad to St. Marys while in the ambulance my dad came to once he was arrived at St. Mary. He was severely dehydrated due to lack of care thank you for the paramedics for being so bluntly honest to me about this Center. Shame on all of you. You got my Dad approved for another 30 days with Medicare he was only to be there 7-8 days without even talking to me. The greed and selfishness, laziness most importantly unprofessionalism within the healthcare oath that you all take, and that you have shown. Again shame on all of you. My 75 yr old brother was moved here from St Mary's Providence at there recommendation , The rooms are so outdated tv's and furniture from early 2000's.cable for tv completely outdated remotes don't work half the time or he could only get one channel to come in. He was there for rehab and only received 2 sessions in 11 days was left laying or sitting in bed maybe an old chair,The room he was put in had no air cond. thank god the first day was the only day because I went and told the staff they had to put a/c back there as it was at least 90degrees in the room The nurses are overworked short staffed and need extra help for these people that are there for care ,my brother discharged himself because he could not stand being in such an outdated facility. This place is horrible. You are in rehab and you cannot walk or get out of bed. Their care was unconscionable. We wore diapers and they left us for hours in our soiled diapers causing diaper rash. They have a button for us to push when we needed help, but they would just ignore it. I think this place should be shut down. read more

The CNA's here are very hit and miss. If they are having a bad day they don't hesitate to take it out on your loved ones. They routine make them sit in their soiled clothes/bed for hours (no exaggeration). My mother developed an infection due to this. CLA's like Joanne and Andrea were rude to my mother and everyone in her room on multiple occasions. My mother's neighboring bed is German (but understands Spanish). Those two specifically complain about work, their home situations and speak Ill of the patients. Not every CNA is bad there; but the ones who are, are terrible. You do not want to put your loved one in there if you have a choice. They definitely need a new director in charge of the entire facility. Joanne and Andrea need to be fired. read more

TO AGUSTINA "the owner" you always just say "I'm sorry TO HEAR THAT" to all the families who have been negatively affected by your place. NO YOURE NOT SORRY, YOU DONT EVEN CARE AT ALL!!!! Medical facilities should never be warm we all know this due to bacteria and diseases. In the month of July their AC was off and only turned on when the Doctor showed up to make rounds. They have people who aren't even medical staff taking blood pressure (incorrectly and janitorial staff btw) if I could give this a negative star I would (negative a million stars) my dad had to be rushed to the ER FROM HERE. It has taken me a bit to write this because it HURTS STILL... i specifically notified the nurses about signs and symptoms my dad was having that were COMPLETELY OUT OF THE ORDINARY and they brushed it off they were rude especially Shanita one of the so called nurses who as some staff say has a bad reputation of being rude to all patients even yelling at some. I kept telling them something is wrong and they neglected to let his primary know until it was too late we told them if they didn't call 911 we would... my dad was rushed to Desert Valley Hospital and shortly after died. If they would have notified the doctor sooner at least he could have had a chance to recover from the infection he had and be with us a little longer. Unfortunately it shortened his life. They worsened the situation. I had to take pictures when I first walked into this place. DONT ALLOW YOUR LOVED ONES TO STAY HERE EVER! ITS A HELL HOLE AND IM EXTREMELY SURPRISED IT HASNT BEEN SHUT DOWN! I bet if inspection showed up unexpectedly it would be shut down! I miss my dad I know we cannot stop death from happening but this facility worsened it and accelerated this tragic event! I wonder if anyone had ever file a lawsuit against them. I also wonder how many have died due to negligence at this place. Made me gag each time I went to check in on my dad and I was There every day! ! If I could give this place 0 stars I would! DO NOT SEND YOUR LOVED ONE HERE!! My loved one was transferred her from Saint Mary's to receive rehab due to a broken shoulder and hip. They were not going to give her care. This place is terrible. The atmosphere is disturbing and disgusting! It look like it was from the 1900s and look termite infested! They got her in a bed and plopped her there. The nurses had no idea what they were doing and looked like they could all work at denny's. Keep in mind this place is the home for elderly people and they have 12 COVID CASES, covid throughout patients and staff. The also gave someone is a fall risk patient slip socks! This place is terrible and not safe! It needs to be SHUT DOWN!! Even there own staff informed me that they would not want there loved one there!! That says enough as is! Keep in mind the Covid was not addressed to us until we arrived. They let your loved one sit in a diaper for days at a time and do not help them to the bathroom! Get your loved ones OUT! read more

In the last 2 weeks my friend, Margo Smith, has been there following surgery for a broken hip, they have both been kind, friendly, thoughtful, thorough, gentle, and compassionate. While many in this profession only put in minimum work for a paycheck, these two go "above & beyond" to make the patient as comfortable as possible during an extremely difficult part of their lives. read more

Heed my warning. Do not take your loved one here! If they're here, get them out! As a deputy, I use to go here and loved it. I'd go here for natural deaths and other minor issues. Therefore, I was nothing less than ecstatic about my father in law being transferred here. Biggest mistake of my life. Due to my approval and lack of worry due to my history, I regret not doing extensive research on it. This place took a complete 180 degree turn and it will forever haunt me. It started with my pops going into Victor Valley Hospital for a low blood pressure and a minor infection needed to be treated. While being treated in VV, they said he was in good enough condition to be sent to Apple Valley Acute for rehab. So they transported him. He was under the care of "one" specific nurse (but was likely not registered) Tynesha (or Tanesha, T'Nesha ??) From day one, this lady treated my pops with nothing but attitude. She ignored him, yelled at him and just a general mean person to everyone. He went for a minor infection and left with pneumonia. This is because the air conditioner didn't even work. They had fans, but only when they had enough, to supply air for the clients. They even stated he ate excellently on their reports when he never ate one single meal! The first meal was a sack lunch similar to jail sacks given in penitentiary's. When Dr Siddiki finally showed up, the air conditioner was miraculously working. He's likely the owner of this dump. This place is an armpit and killed my father in law. Don't worry, it's not over MS TINESHA AND SIDDIKI. read more

The quality of care given at this facility is sub standard at best. Prompt and needed care for patients is not at the staffs forefront. I had an elderly relative who had the misfortune of having a stay at this facility. There were a total of four patients in my relatives room. The staff appeared more interested in personal use of their phones and chatting amongst themselves then checking on and attending to their patients. Calls for help were ignored, patients were spoken to curtly and attended to as if it were a bother. I highly recommend you look elsewhere for needed care. read more

Thank you so much for providing such great care to my friends Dad. He came in with a depressed mindset as he hadn't walked in weeks. He was supported and motivated to work with rehab 5-6x a week and left stronger than ever! We're so grateful for the drive you put in him and the MD inspiring him to stay longer because he really needed the help. It was sad every time we went in his urine bag was on the floor that was dirty he end up going back to the hospital with septic. My aunt is here for recovery and she asked for ice chips and crackers. We waited about 40 mins and she started using her button to call a nurse. The nurses never showed. Thank god it's not an emergency because the person would have died waiting for assistance. read more

I wish I would of read the reviews before my mother was transferred here! My mother suffered a stroke and was sent her for rehabilitation. She arrived Thursday, March 3, 2022 at 2pm and was already getting neglected by 6:30pm. She was trying to call for the nurses for something to help her pain only to find out she didn't even have a call button nor did they have any medications for her. They then proceed to give my mother a bell that was just as useless as her "call button". Then no more than an hour and a half later I get a call from my mother crying and telling me she fell out of bed and was left on the floor for 20-30min while her and her roommate called for help and her roommate even pushed her call button that was working! All the while she can hear the "nursing staff" laughing and talking!!! My mother finally crawled to the hallway where the staff came and just looked at her. Then proceeded to say "what were you doing that you fell!?" Um... hello dummies... she's a stroke victim with no strength or feelings in her legs maybe that's why she fell out!! Another staff member refused to let her call 911 and refused to call for her. I had to have someone call and go off on them before they finally agreed to call 911 but not one minute before they made my mom sign an AMA in order to cover there own asses!!! Best believe I called the board of directors of this shit hole and filed a complaint with the state. I really wish I got all the POS staff members names! This is unacceptable and everyone we told has agreed! I hope something is done to this place because this is unacceptable and ridiculous!!!! read more

This place is the worst poor nursing quality of care is worse they don't care about their client. They give medication for Dementia and my mom did not have dementia she had to use the bathroom due to the medication they were given to her and had to sit in in for over an hou. I work in the medical field for 20 plus years and have never ever seen a medical facility treat their clients so very poorly . place was clean but care was terrible. My mother deteriorated there and when I transferred her out she has two bed sores and all the tone in her legs were gone. She wasn't being turned nor rehabbed there. They wouldn't allow family to visit properly and punished the patient for not being able to sit in a wheelchair they were not able to physically see their family. WhT that has to do with COVID I don't know. They transferred my mom out and pretty much dumped her without giving report, orders or instruction. Never send your family here. Something shady is going on at that facility and they are using COVID as an excuse for not doing their job. not making your loved ones accessible while there so they don't have to be held accountable for their actions. I don't understand how this place got 1 star. After seeing my husband there for 2 weeks, the nurses, LVN and staff were awesome!! They are human like us and how you want to be treated is how you treat them. Let me tell you the nurses there are doing a GREAT job despite how much they are paid. They care about the patient over than money. THEY ARE SEVERELY UNDERPAID, I am ashamed that they don't get what the minimum wage is!!! If I had owned this place, it would be the best place to go in Apple Valley!! I would change things up. Pay the nurses more, update the non working beds!!! This gives the nurses more work to elevate on whichever body part it needs because they don't work. And the dresser furniture! They are sturdy but marred and missing knobs. i was there about 20 years ago for my MIL and they need to update it high time. The food, meh....but that is first world problem, right? I just wish they would ALLOW ALL DAY VISITING HOURS, this is what patients need, a visit from the family and helping the patient, and of course giving the nurses a break! They were very strict with the COVID protocols too. This is a very happy place! I want to nominate the nurse ESPECIALLLY Telma Recinos who saw my husband at his whim. She deserves a BIG raise! And as well as Daizy. Not to mention these people: Analine, Carolyn, Jocelyn, Susan who is a spunky as a case manager, Mary Washingtion, Sharon, (pronounced as Sher-ON), Lalona and physical therapist, Walter!!! There are so many more people there that we don't know the names of, please forgive us if you know who you are. We love this place!!! The owners just need to update the place. Think twice before allowing a loved one to go to this facility. They ignore patients when they ring for help and use the excuse they are short handed. Normal wait time for help is over an hour. Their normal practice is drop and go with everything they do. The lack of care and concern is beyond shocking. read more
