I called for 2 weeks asking for them to fax one to Americas Best. So far, no luck. Seriously go somewhere else and save yourself some grief.

Dr. Perry pointed out a retina ripple and the start of macular degeneration in my left eye which was not detected by another ophthalmologist had seen before being referred to Dr. Perry for a second opinion. I was very happy with the care and grateful for their expertise and the follow-up call from Dr. Perry himself after surgery plus the weekly follow ups.
