My uncle is currently staying here and our family couldn't be happier with the service and compassion he receives from everyone here. From the front desk, to administrators, to staff. My veteran dude stayed there in the short-term area for 10 days for a caregiver respite. He required very little care, just a chill safe place to stay, give him his meds, oversee his hygiene, and feed him. The good stuff was really good. The icky stuff definitely needs attention. The place is clean, has no unpleasant aroma, the temperature is appropriate, noise level in the hallways was tolerable, staff I encountered were always pleasant and smiling. Let's start with the good stuff - the office admissions process was effortless. The staff coordinated with the VA to make all the paperwork sail. The at-bed admission process was professional for my dude, most probably because I was with him and he's mostly alert. Bad stuff side note: I observed the at-bed admission for his roommate a couple of days later and would not call it stellar. Example: the CNA and aide kept trying to tell a nonverbal patient with dementia that they needed to do a "skin assessment" while they were rapidly removing all his clothes. He was audibly frightened and resisting. "Skin assessment" means nothing to the non-medical person. When I suggested alternate wording to the charge nurse, my suggestion was not received pleasantly. I got that "don't tell us how to do our job" look. I mean, c'mon, wouldn't it make sense to tell someone, "We need to check your body from head to toe to make sure we pay attention to any weak spots." Back to good: Our particular crew, notably Cori the RN, was attentive and responsive and welcoming. Cori stepped up regularly to intercede with the kitchen staff to attempt suitable nutrition for my dude. Cori also intervened to maintain my dude's med schedule, even though it differed from the facility's preferred dispensing hours. Needs work: I provided the staff and charge nurses with detailed info about all the compromises my dude is dealing with, and they still blew a gasket when they observed readings outside their preferred norms. He and I held firm with facts and wishes. More fixes noted: On day 2 he suffered a fall (a direct consequence of fiddling with his diet and/or meds) and required medics and transport to the ED. I drove him back to the care center once the ED had treated his wounds and assessed his impact areas. Two days later when I visited him, his wounds had not been cared for according to the hospital discharge paperwork that I had hand delivered. For the remainder of his 10-day stay, he just stayed in bed to avoid "causing a fuss." He's *that* guy. Worst of all: The food - naw! Hard pass and no interest in improvement from our observations. Worse than the worst Denny's. Overcooked, hot not hot, cold not cold, and unique patient needs not addressed satisfactorily. Think about this: a patient arrives, is not urgently ill, knows he/she is on a "camp away" to give the caregiver a break and all semblance of home is gone. The least the facility could do is feed the patients better. It was easily day 4 or 5 before my darlin' had a breakfast he could eat. Ending on the positive: the patient-facing nursing staff was magnificent to my dude. He said the bed was comfy and that the setting seemed mostly peaceful. We'll pick a different center for the next respite stay. read more

My significant other has been here for a year - an 83 yo disabled veteran, dealing with dementia & an ostomy. The staff on all levels have been caring, attentive, paying attention to his individual needs & style. I & family visit 4-5 times per week & work collaboratively on his care. The nursing staff are diligent, the medical staff are very good to work with, and the activities are varied & frequent. PT & OT are very good, nutrition team works hard to meet each patients needs. Social service team has full breadth of services & is attentive to ensuring the patient has access to other things. Beyond all that the team CARES & works hard. The lazy staff get weeded out quickly as the standard is high. Our experience has been very very good & we are very pleased with the coordination of care & such, which speaks to culture. Our guy has been at 2-3 other places prior that had varying levels of care & efficiency. By FAR this is the best placement. Highly recommend. read more

Nope!! Level of "care" is not what one expects in the new world. And then they make it difficult to transfer to a better facility by threatening to release the patient "AMA" . My mother was admitted here after surgery and we are trying to get her transferred to an assisted living facility and they are making it as difficult as possible, no doubt so they can continue to collect from the insurance company. This is a prime example of how the elderly are treated in this country which is a national disgrace. She has called me several times in tears regarding the care here. Avoid at all costs! read more

If I could I would give it a zero rating. I've worked here...and I don't have good experiences with any of the staff...but I've met awesome coworkers at this facility and they would always be on top helping me when I needed a hand, cause this is literally hard work but as a team we can do it well and save our backs and keep the patients safe most importantly, and I would help them out as will, but sadly I've never got the chance to work with the girls I got along well with I was mostly on station 2....everyone in that station would tell you that they aren't available to help you out at the moment it was always that excuse and it was getting hard I couldn't stand it. And the nurse specifically Joanne, she would be on top of you every single second and it's even when you were either doing a shower or doing patient care, she would act as if you weren't doing nothing and you were just slacking of which is not fair cause we CNAS are doing much more harder work our backs and feet's pain us and on top of that dealing with her attitude every single shift I had her as my nurse was a terrible nightmare unbearable...she would press on the patients call bottoms to have a reason to get mad at you to say that it's been on for hours. Also the head nurse are the worst they would come up with random weird fights just to get you in trouble. And they would be the "innocent angels" I'm sorry for this horrible message but I wasn't happy at my job I was easily upset cause of the coworkers. Kitchen staff were awesome, house keeping staff were awesome as will. Patients were awesome but I get why some were moody cause of the nurses and staffing. Some CNAs are rough with the patients as will not all are nice people as they might appear in front of family members. read more

The people working at Auburn Oaks seemed to have good attitudes and I take that as a sign that the facility is well managed. My 97 year old Mother had to spend two weeks there while she healed from broken ribs. She seemed comfortable whenever I went to visit her and the PT staff also seemed caring and professional. This place was great when it came to the staff and the care but this review is for their front desk. They are unbelievably rude every time you check in, you call or you ask for help. They must be really stressed out or overworked because they are always irritated toward guests. It's really a problem and there have been times I've avoided calling down there just because I was so tired of being belittled by the front desk. I finally got my dad a cell phone and the case workers direct line just so I didn't have to engage with anyone on the main line. Management should really look into this and maybe give them a training on patient service or communication etiquette. Ruins the whole experience you get from everyone else up there. read more

This place is not great. I came in after a surgery hoping to get some therapy on my hands before going home. Bad choice on my account, should have listened to my surgeon who advised me just to go home I would do better on my own. Now, they are doing nothing for me I'm on no pain meds not needing any assistance and they won't release me. This place is dirty, they have four people using one bathroom commode. They need to release me so I could get a home medical care for wound dressing change and op therapy. They are holding me here just so they get paid by Medicare. This is like holding someone against there wishes. read more

Where do I even begin? Is it with the fact that they continued to serve my mom pork even though she repeatedly told them she was a Muslim and pork was not religiously acceptable. We had meetings and agreements to serve her workable alternatives which nobody executed on. I took it upon myself to bring in food some my mom could eat because she was not able to eat the food they had on their menu but even that was too difficult to manage. They won't put food in the refrigerator and retrieve and microwave to serve her. I should have insisted on leaving the facility after the skilled nursing period. They did nothing to get her back to her baseline. She can't stand not to talk of walking. All these she could do before she ended up in the hospital. It became obvious they wanted to keep her as a long term resident for the annuity that provides. That would be fine if they provided minimally acceptable care. I saw all the red flags with the stinking attitude of the people at the reception and the therapy staff but I told myself that they were busy and just having a bad day and I should let it go. I wish I have followed my intuition earlier. But I am glad we moved mom out of there nobody should be treated the way she was. I mean nobody. read more

My husband had knee replacement surgery and needed additional time for rehab. The facility is clean, the food good, most of the staff is great. My big complaint, as a spouse, is their covid regulations. I had to have an appointment to see my husband, and if you don't get one, you may not get to see your loved one...or maybe 4 hours later if that time suits them. There is a tedious admission process, washing hands outside, temperature inside, computer input as to why you are visiting, show vaccination card, put on N59 mask, plastic shield and gauze gown. I can see this kind of treatment for a casual visitor, but to a spouse or child caregiver of a parent coming every day this is far beyond the realm of common sense. Jeri Leu read more

My mother stayed here for two weeks specifically for physical therapy. The physical therapists at this facility are wonderful and have really made a huge difference in my mother's health. The head of nursing was very helpful and informative. I did meet some very nice staff members, however some seemed less than enthusiastic about performing the duties that they are being paid to do. The woman who handles care and discharge was not at all helpful, rude, and dismissive, not to mention condescending. It is a shame that family members who are concerned about their loved ones and already at a breaking point have to deal with a person like this. My mom stayed here for her final five weeks of life. When she first arrived via hospital discharge, I was ready to only give this place a 1 star. She happened to get there during a shift change and was left on a bed in her hospital gown with no assessment or details for several hours. But as her condition progressed from needing rehab to skilled nursing, I found the staff to be extremely caring and attentive to my mom's needs. They coordinated hospice care for my mom's final week, then after she passed, I arrived to pick up my mom's belongings, and all her belongings were already neatly packaged for me. The food was not particularly palatable but then again, I haven't heard of any SNF with great food. To summarize, the attentiveness in the rehab area was average, but the help my mom got in skilled nursing was outstanding. read more

My sister first came to Auburn Oaks for rehabilitation after a bad fall. The physical therapists were outstanding and she soon graduated out to a group home. After a few short months, she had to return to Auburn Oaks for rehabilitation from a hospital stay due to a significant illness. When it was determined that she would require skilled nursing care for the long term, she transitioned to resident status. It was such a relief to know that my sister would be well cared for and I wouldn't have to worry about her any more. She was happy to be there; she felt safe and people really appreciated her positive attitude. After about a year, however, she fell very ill and passed away there at the home. During the last week of her life, while she was on comfort care, I was allowed to be at my sister's side for as much time as I wanted, despite the visitation limits due to Covid-19. I will be eternally grateful for that opportunity. I want to thank the nurses who tended to my sister's needs during that time: Angela, Brooke, David, Jesus, Kevin, Melissa, Norma, and Suzanne (I hope I'm not leaving anyone out!). They really loved my sister. We were fortunate that our loved one was able to live out the last chapter of her life at Auburn Oaks Care Center. read more

What you have here it's Foothill Oaks Care Center that has been renamed Auburn Oaks Care Center. This is the same disgusting criminally negligent Convalescent Center that it's always been. I don't where are these supposedly wonderful reviews came from but I'm highly suspicious because I was just there to see my father and I'll be leaving shortly even though it's 10: 45 p. M. To get him out of there. As for the posted link that's supposedly shows a good record I'm calling bullshit on that one too. At the end of this review please see the link from the California State Department of Health which shows numerous violations and complaints. When my father was brought in today the bed was not ready, the first oxygen machine they gave him did not work, the room itself was hotter than hell , the toilet backed-up with waste and he had not been given his pain medication since he got there early this afternoon. They claim it is medication will be delivered sometime between midnight and 2 a. M. Now tell me what's wrong with that picture. Additionally my grandmother died there from complications of pancreatitis and we the family we're not even aware that she had pancreatitis until after she died. Pancreatitis if you've never had it Defines the word pain and how long did she suffer. If You care about your family member and they are there now get them out and if you're considering a Care Facility do not, I say again do not bring them there. My 89 year old mother came here after she fell and broke some ribs at the assisted living facility she was living at. Some of the other reviews I've seen for Auburn Oaks doesn't sound anything like the experience we had. My mother was fortunate enough to be in a private room. She actually enjoyed the physical therapy--that's saying a lot as she has always hated any kind of exercise--the therapist was very helpful and patient with her. I was especially impressed with the nursing staff--the NP found the solution to a physical condition Mom has for years that no doctor was able to help us with. Mom was able to get around easily and move back to her assisted living facility in just a couple weeks. I would bring Mom, or any other family member to Auburn Oaks Care Center if they needed the services they provide. read more

I just spent twelve days here after a total hip replacement. Once I was put in a private room, moved mainly because my roommate liked her tv on 24/7, I was very pleased. The staff was courteous and professional, everyone from the housekeeping team through the RN's. The CNAs seemed to be the people doing the most visible work and we're always sweet and quick to come running when the call button was pushed. I rarely had to wait longer than fifteen minutes. Considering how many people they care for and the potential for possible emergency situations I thought fifteen minutes was fast. The PT crew is very professional and very thorough. I had both physical therapy and occupational therapy twice a day. I took full advantage of it, never refusing, because I knew the more I had the quicker I would get better and get home. The facility is up to date and clean. My room was very nice, more like a good hotel except for the hospital bed. I had a pleasant view of trees and my own bath. There was even a chair for guests that folded out into a twin cot. The food was pretty good for institutional and they bend over backwards to give you what you want with extra snacks. In fact I had the best ever chili bean soup and cornbread and asked for the recipe. I am home now but wanted anyone unsure of whether this skilled nursing place was a good choice to know that it's an excellent choice. read more

My Mother had broken her hip and was transferred to Auburn Oaks from the hospital. Our family had never experienced this type of event happening in the family before, so we were very concerned about her care and who was taking care of her. Not only did Auburn Oaks take care of her physical needs expertly, they also treated her like she was their family. Everyone from the top like Nina, to the R.N.'s, like Chris, to the O.T.'s, P.T.'s to the C.N.A.'s like Elisabeth. They not only met the needs of my mother physically and emotionally, but treated us with care and was always on top of any concerns we may have had. Excellent facility. AWESOME!!! My grandpa had Alzheimer's and was here. They did not care for him. Didnt feed him. They always seemed understaffed. My grandma got regular daily calls to come take care of him. I would strongly advise you conduct a lot of research before going with any facility, but in my case, our loved one was sent by Kaiser so our family could not research them in advance. Beware they will make you feel confident in their capabilities, and will give you a hard sell on how great they are and kindness will be oozing out of their pores. It stops once the admitting process is done. It goes down hill from there from their administrative staff down to their nursing staff, they have a habit of being dishonest, and will not be upfront. Do not expect an improvement from anyone you send for assistance. I would not let them over see my cat and I don't own a cat. I have seen some poorly run businesses; however, this is a medical facility and the promises made vs. the product delivered are off. The physical therapist did not work with my grandpa because, when he asked my grandpa if he wanted to do his physical therapy, he said "no." Gee, I wonder if that works if he asked if he wanted to eat more vegetables. They don't know anything about the Omnibus act, and make sure you do not consent to any medication over the phone and if you ask them to cease any services, follow it up in writing. Not just this facility but all of them because it's really difficult to trace back who did what and which doctor ordered, authorized, administered anything so do not fall for the welcoming smiles, offer for water or coffee, that's how it starts and it goes down hill from there. Always get the name of the receptionist, take screen caps of call logs, because they will say they called you and that you did not call them and when you provide the evidence the staff get agitated. If you have someone seeking memory care, please read all you can about this facility, they will label your loved one as a "screamer" if they are hard of hearing, they said my grandfather was blind, and when the nurse from a transferring facility came out they realized the facility was wrong. Fortunately, they make themselves look bad so if you are moving them or transferring them and there are problems, although they will try to make it seem like you were in the wrong, the care coordinator, receiving facility and transport will most likely have had problems with them. It's bad when people try to make you seem "difficult" when you are advocating for a weak elder. I am glad that they released my grandfather, it was definitely a blessing in disguise, I actually wish it happened sooner. read more

We chose Auburn Oaks for the care of my 92 year old grandfather after Hip replacement surgery. Their staff went above and beyond to explain the recovery process to us upon arrival so that we could manage our expectations. Their Rehab/therapy team were positive & thorough with the way they treated him and their communication style. He felt truly listened to, respected and cared for. During his stay there, our extended family had interaction with almost every department in the building and were impressed with their teamwork in an effort to help him, communicate with us and be our liaison with other healthcare partners. The facility even had transportation for him to get to his Dr's appointments. Karen, the driver, was so friendly and even moved an appointment so that it wouldn't be "too much" for him in one day. He was so appreciative of her attention to detail and empathic demeanor. Should any of our family members need Skilled nursing again in the future, we would definitely choose Auburn Oaks. read more

This account was created for two purposes, share an experience, and at the request of their social service worker. Not familiar with the protocol so I will do the best to share my experience so others may find this beneficial and to please the request from Auburn Oaks. I want to say Auburn Oaks is a great facility for the elderly. Granted my father was there for one week, but in that time I feel I experienced enough to provide an authentic review. The building itself is quite nice, the reception area is great the woman who does admissions is kind, and there are some really nice employees. But, as with any business, there are going to be things that we do not like, It's impossible to please all clients; however, this is a very delicate issue. Much like having a child, you want to protect vulnerable people who are experiencing pain, transitions, financial woes, family communication because when you are at the end, the last thing you want is to be in a facility that sees you as a bed and not a person. In the beginning, I would have bet money this facility was going to be top-notch. As each day progressed little things would happen that would trigger a hmm... The experience my family had was awful. It is a blessing that there are enough facilities so if one is not happy they will "hopefully" not have too much angst getting their parent, or loved on somewhere else. I will say, document everything. Read everything and always have a witness. Fortunately, the issues I raised can be substantiated. I would highly recommend bypassing Auburn Oaks because even when you are no longer a client, they will still cause headaches. Simple questions, personal privacy, false statements, and accusations were abundant and not just for me, the next facility also had issues and the care coordinator and I discovered inconsistencies. The reason I created an account was their social worker accused me of writing a review. Well, now that I know the way to obtain their attention is Yelp, I suppose this is the preferred method of contact. This experience for our family was beyond detrimental, and we have not concluded the issues but pay close attention to things, know your rights, protect yourself legally and financially as with any business. Auburn Oaks has a good business plan, they simply need to stay consistent, listen to feedback and not take every question personally. read more

Finding a place for my husband within 36 hours was very difficult. I looked online for helpful reviews. Most importantly I found that there is a government website that lists all the complaints, whether substantiated or not, for skilled nursing facilities. Visit that website before making any decisions! Every place had at least some substantiated claims. Auburn Oaks had the fewest. When we arrived, we were greeted outside by one of the nurses. We have found that most of the staff are absolutely wonderful. The night time staff is not that great, poor at best. The food is acceptable and sometimes good. The administration is wonderful and very willing to listen and are available if needed. What makes this experience positive is the exceptional quality of the physical therapy my husband is receiving. He has several hours of therapy every day to help him improve his strength. I see him getting stronger everyday and able to move and function much better than before. There are some issues however, like mentioned earlier, the night time staff is not as attentive at the daytime staff. While usually the place smells fine, but thereQ sometimes is a lingering potty smell near some rooms. I am not sure if that is the patient or the plumbing. I recommend this place but remember, it is a skilled nursing facility, there are patients in wheelchairs in the halls and patients making loud noises sometimes. I have had many family members rehab at various facilities throughout our area and Auburn Oaks is by far the best. The director is what makes the difference. He really cares about his residents/patients and appears to want to do the best for them. He made a point to introduce himself to myself and my mom and remembered who I was days later when he saw me in the lobby. The PT staff worked well with my mom and she made great gains. The food was decent. Actually, some of the meals were really tasty. They not only score the meat making it easier for the patient but they also offer meals to the family members at no charge. The facility itself has been updated and looks clean - kind of on the fancy side compared to other rehab centers. They offer creative activiites for the patients. The staff for the most part seem content and that shows in their care for their patients. I am sure there are issues here as with any rehab facility where people are overworked and underpaid BUT much better than rehabs in Roseville and Citrus Heights. It was worth the drive for us knowing that my mom had decent care! read more
