My Father spent time intermittently after I relocated he & my Mother back to our home area a number of years ago. As a Nurse Practitioner, I am pleased to say that that I was thrilled with the care and attention he received while there. That care enabled him to come back to my home with Mom and stay home with the great care that Wauconda Care coordinated. Though Dad had challenges and had to visit Wauconda Care again, I was confident they'd again provide excellent care until he could return to us. I have been so pleased, I have applied to become apart of their team to give back to others and become a part of the team that allowed extra time too enjoy hanging out with my Dad. I highly recommend this if you have a loved one in need...I recommend this location without reservation!! Remember, as I am a Nurse Practitioner, I am very aware of what quality in healthcare looks like and THIS is is!! I wish there was a zero, so had to give a 1. Left my Mom in her #2 diaper, didn't change her. I came 2 nights in a row and changed her myself, had to demand ice water and change her wound bandage because no one did that either. When I complained to the care director that my Mom was left in excrement daily she told me I could transfer because I complained. ???? Seriously? This is neglect and abuse!!! My father has been here for 5 years under two ownerships and major construction addition. The staff is dedicated, caring and kind. All my family is dead I dont know how I will get out of here I dont need any assistance and am completely independent I drive my own car and go on errands and get groceries And now I am being held against my will !!!!!! read more

My 90 year old mom was there for a broken hip. Like every where they are short staffed. The food was great and employees were mostly nice. Therapy didn't happen on weekends and she was left in bed those days. I had to insist they let her use walker to go to bathroom. They just wanted to get her in wheelchair and quickly get it over with. They did not have time to let her walk so she was mostly in bed. She did recover from the hip very well. read more

This place needs negative stars for a proper review. Ownership/ management have not resourced this place properly and it leads to awful and even harmful care. I found my mother slipping out of her wheelchair nearly on the floor and unable to move (she was recovering from hip surgery). I had no idea how long she'd been that way and when I asked for answers no one seemed to even care that she'd fallen out of her chair. And if good food is a basic part of good care, this place fails miserably there too. I'd love to see the owners eat the food they serve here for 14 days straight. read more

I've got to say, the peace of mind I get knowing my aunt is here has been incredibly freeing. She has dementia and downs syndrome, a tough situation for any caretaker. When her old place told me they could no longer get her to eat, I knew it was time to get her out of there. Panicked, I researched every possible placement I could find. This process is terrifying, you literally have a loved ones life in your hands, and it feels like no place could be good enough to take over. After days of research I decided to stop by here for a tour, and found myself very impressed. The place is busy, in a good way. Nurses, cleaning staff, workers, visitors and residents are very active all day. It passes the smell test, an important factor for anyone contemplating leaving they're loved one someplace new. They have activities, a beautician, religious services, and plenty more amenities. I moved my aunt here a few months ago now, and she's better mentally and physically then when we left the last place. Turns out the last people just didn't try hard enough, she's even gained weight back! It's two to a room in her section, and her roommate swears by the service here. She's been there for 10 years and still loves it, that's saying a lot. The nurse recognizes me, and gives me updates regularly either in person or over the phone. They carefully laid out a care plan to match my wants and needs, and I really feel the love from the staff. It's hard to place your trust in strangers, but I sleep so much better knowing she's here being treated like a person, not a paycheck. On an extra note, today's visit was during happy hour. Everyone was enjoying some wine and having a good time interacting with each other, and I thought to myself "If I'm ever unable to take care of myself, I could see happily living like this." read more

My grandpa stayed here and honestly this place is horrible. They would wake him up at 1 in the morning to take his weight. Why would you want to weight him in the middle of the night that's just horrible and not fair to the residents. You have all day and decide to do it in the middle of the night ?! My dad is in his 60s with a rare infection currently getting care. So far the care has been decent. He gets physical therapy just about twice a day, food three times a day, and someone to help him shower and bathroom. His complaints are the food (could be the medicine affecting his taste) and some nurses who are a little lazy but he loves the nurse practitioner who is in charge. It sounds like she put the nurses back in line and listens to my dad with questions and complaints. Another complaint he had is sometimes it takes forever for him to receive something with the call light on. The new addition is nice and clean. Staff has been nice overall. The dietitian, Ray, is very friendly and comical. read more

I sent my Mother to Wauconda Care because her previous stay had been good & we hoped to repeat it. Not so much...Many of the administration don't have a clue how to communicate. Most of the permanent employees were very good. The agency ones - they tried but they might have only been there a day or two and didn't know the patients. My mother left the facility for a doctors visit and was admitted to the hospital for a second surgery on her knee. I informed the facility. I was originally told her room would be saved for her and nothing would be moved unless they needed the bed. The hospital contacted her about her return possibly Thursday or Friday. Friday morning, they decided to pack up her stuff and lock it up. She came back Friday around dinner time. The hospital has assured me they would save a dinner for her and would have her necessary medications. That didn't happen. No dinner, no medications. No excuses. I asked to have her stuff returned and since it was Friday night, nope, they couldn't do it. Housekeeping had locked it up and there was only 1 key. She had a piece of banana bread my brother had brought her and just wanted to rest. She was in pain but couldn't get her pain killer. They had sent all of her meds back to the pharmacy. Don't know who the Einstein was who made that decision but it was the wrong one. When she came back, they put her in a different room. Thy had put a new person in her old room even knowing she was returning. After finally getting her things on Saturday, I noticed some things were missing.. A tiny pair scissors, a new full can of Air Freshener, a vase that contained fresh flowers and a roll (10-15) of Glad scented garbage bags. My mother was having hallucinations and thought she was getting out of bed which she couldn't do. She was found by one of the aids trying to get up. I asked for a bed alarm but they said they aren't allowed to use them. I asked them to move her closer to the nurses' station to make sure she was watched and they wouldn't do that either. I asked for a recliner to make her sitting up time better. The wheel chair and the room chair are not comfortable especially since she has a humped back. I went to Housekeeping who told me I had to talk to Admissions who told me I had to talk to Housekeeping and then was sent to Maintenance. They had to order more chairs. The Admissions lady then said she "overheard" a conversation by PT that she couldn't have a chair. She asked no questions. She was rude and sat at her desk on the phone picking at her arm while I waited to talk to her. What a nice picture! When I talked to PT they said she couldn't have a chair because she couldn't get out of it alone. Now, there was a note on the wall that stated she needed 2 people to get her out of bed or the wheel chair. Why was the recliner a different story? They don't talk to one another. It's the he says, she says. The new part of the facility is beautiful. There is no horrible smells. The food was always cold because she was one of the last to get her meals. No thermal trays. They put a heat retaining cover over the food that was supposed to be hot but it doesn't do a good job. The trays came out at 4 and she would get hers at 4:30-5. I had to push for them to get my Mom up to use the bathroom. Carl in PT was wonderful but he is the only PT I met. I would say if you are going to send someone here short term, it may be ok but for any longer, the parties in the front office (minus Joni) were rude and unhelpful and sometimes nasty. They talk a good talk but they sure can't walk a good walk. I wouldn't recommend Wauconda Care to anyone who has to stay any length of time. Not enough nurses, not enough staff. I used to think this was a good place. It is better than some but definitely needs some administration housekeeping. read more

My husband received PT and OT every day by component therapist. His strength increased and was able to go home after three weeks. The nurse, Jennifer, was diligent , personable and compassionate. I would definitely recommend Wauconda Care. read more

My grandfather just returned from this facility. He described his treatment as "barbaric ". He went in for rehab and never received it. The staff was rude and did not care. When he entered he could speak and was mentally with it and during his time his cognitive ability diminished significantly. Do not leave your loved ones here! Wauconda care more like "we kinda care". ***NOT RECOMMENDED*** Very disappointed when my husband was discharged. Social Workers were very unorganized and uncaring. Didn't arrange for all my husband's equipment needs for home. Very disappointed with nursing aids very slow to reply to call lights. My husband waited for a long time to get help to bathroom. Seemed to be always short of help. Food is poor. Parking spots are impossible to find and lot is dimly lit and full of pot holes. Almost fell in them numerous times. WE NEVER WOULD GO BACK THERE AGAIN... read more

I went there to visit a patient. AlI I can say is to be very careful when driving into this facility. You can damage your car really easily! There are huge potholes everywhere! This is not the result of one winter. This is definitely years of neglect. My mom was a resident of Wauconda Care for about three weeks to receive rehabilitation from a severe brain injury that she had received. When I was looking for facilities to take my mom to for a "step-down" (meaning she was in a far more advanced rehab facility for much more aggressive rehab, and when she was done with that they required her to go to a lesser care facility for more rehab before she could go home) program, Wauconda Care was one of the places on my list to visit. When you drive up to Wauconda Care, the overall look of the outside is very deceiving. I actually wasn't going to go inside because it looked like a nursing home, and I knew that wasn't going to be right for my mom. But reluctantly, I went inside, and I was surprised by the inside. At the time they were going through a remodel so half of the building was new and modern, and the other half was more nursing home-esque. I took a tour of the facility with one of the directors of the center, and she was nice and offered me a lot of detailed information about the care they could give my mom. They had a nice big community cafeteria and four (from what I remember it was four) different "arms" in the building that all offer specific care to a persons needs. After the tour, I left and decided that this place would be a good fit for my mom's next leg of recovery. They had space available, so I drove her there within a couple days of my initial visit and she became a short term resident. My mom was given a room in the remodeled part of the facility and a bedridden roommate and a shared bathroom with that roommate. She had a nice therapeutic bed and her own tv, which I suppose is common place in a facility like this. Here's where I start to have a problem. My mom had two different doctors while she was there-- but the only one that I met was Dr. Kasturi. This woman seems to be a great doctor, don't get me wrong, but she offered so many false promises to my mom, and it was incredibly disheartening for her and very, VERY frustrating for me to see this happen. I am not going to comment further about this, but my advice is that this doctor actually acts on the promises she makes to her patients instead of giving them false hopes. My next issue is with theft. My mom had minimal personal belongings while she was a resident at Wauconda because I suspected that theft might happen. Well, my mom asked that I bring her some face cream because her skin was dry, so I went and bought her some $20 face cream from Walmart and brought it over to her that same night. My mom used it and set it on the bathroom sink so she could use it in the morning too. Well, when I came to visit my mom in the morning, the face cream was gone-- no where to be found. So I asked my mom about it and she said that she left it on the bathroom sink last night. Well, I approached the cleaning staff and they denied seeing any creams like that. Sure, you didn't. A brand new bottle of "expensive" face cream just disappears, huh? We never found the face cream nor was it returned to my mom. So my advice to you is don't bring anything valuable into this place, because you'll probably lose it due to employee theft. On to some positives-- my mom really enjoyed the food that was made in the kitchen. They always came to my mom with a menu of things that she could pick from for her meals, and they always made her food according to her requests. Also, she could eat in her room if she wanted instead of in the dining room. Also, the therapy that she received was right on target. The therapy staff was always very courteous of my mom, but they wouldn't let her skip therapy because she just didn't want to do it. The exit team (for lack of a better term), helped me find care for my mom when she left the facility. They coordinated a nurse, occupational, physical, and speech therapy to come to my mom's house a few days a week, which was very helpful for me since I live out of state. The overall experience at Wauconda Care was positive, but they lose two stars because of the doctors improper patient care and the theft my mom experienced. Aside from those two things, this facility gave my mom great care. read more

They had to be told to bathe my grandma after being there 7 days. My advice to people checking this place out for a loved one: listen for the beeping. Listen to how long the beeping continues before stopping. Thats how long someone is waiting for help. Could be help because someones oxegen machine stopped working. Could be anything. My aunt picked this place because her friends mother was there and she liked it. Mistake. Maybe her mother was ambulatory or could use the bathroom without assistance. Maybe her mother couldnt talk or tell her she had not been bathed in a week. My gmas feet n legs were swollen i told her her wheelchair should have leg n feet rests on it so they can be elevated. She said they did. Someone took them and never brought them back. I went and asked for them and they did bring them right away. Her toilet was filthy with fecal matter. My gma just had urosepsis she does not need to be using a toilet w fecal matter on it. I told them it needed cleaned and they said as soon as i left they would. Gma said they did. Twice she missed bingo because noone came to take her. She has suffered from depression its very important she gets out of her room. read more

My 95 year old father was in rehabilitative care for about 1.5 months. The physical therapy was excellent and he was well cared for by the staff. I was there pretty much every day. Food is great and he enjoyed that very much. He said himself he was well cared for and had no complaints. Very clean and good staff, very caring!! read more

My mom was in and out of Wauconda Care numerous times over a four year period. The staff are very professional, kind and caring. The OT and PT staff are amazing, every time mom came in from a hospital stay and needed therapy to come back home they worked wonders for her. The food is very good with choices for each meal (including a nice salad bar) and they always have a full schedule of activities each day to keep busy between therapy sessions. There is very little staff turnaround and mom always had the best care. Mom passed away last year , and even now when I visit other patients, they remember her. read more

The only redeeming thing about this facility is the Physical Therapy. Other than that, I would give this place a zero. The food was way over cooked. Had better chow at Basic Training at Benning. The staff took forever to answer the pager. The so -called doctor spent a whole 10 minutes with me, and didn't even look at the surgical wound. I doubt that the staff or the doctor there even read the Doctor's Orders from my Surgeon. Avoid this place at all costs! read more

My mother recently spent a month at Wauconda Care following a hospitalization and we were very, very pleased with her care. The caregivers were very kind and promptly responded when she pressed the call button. The physical, occupational, and speech therapists were great and very helpful whenever I went to them with questions. The facility was very clean and the entire staff was friendly.
