    My Dad was a short-term stay resident in the rehab facility after a hospital stay. As soon as he…read morearrived there, I was received an email and a phone call to discuss his needs. The email had phone numbers for the front desk, the nurses station, and his social worker. The nurse gave me her cell phone number right away and was texting with me through the first day and evening to be sure everything was set up and to reassure a worried daughter. Everyone I came into contact with at Park Place was kind, helpful and caring. The daily COVID email updates as well as follow up phone calls regarding any cases were very appreciated. All safety protocols were followed and I felt confident that Park Place was doing their best to keep all of the residents safe and healthy. Dad was well taken care of and he was always very happy on the phone and during an outside visit. His therapy went very well and I was informed about his progress weekly. Everyone, including the director, nurse and social worker were very prompt to reply to any email as well as any voicemail. Upon his discharge, I was given detailed discharge information as well as all necessary medications and prescriptions. Park Place also calls the caregiver for a few days after discharge to check on his progress at his new living arrangement. I highly recommend Park Place.

Based on…read morewhat I observed during my loved one's stay, I believe the facility falls far short of the standard of care and professionalism that its name suggests. On the very first day of admission, there was a cloth chair in my loved one's room that appeared to have dried human waste on the armrest. I immediately brought this to the attention of the staff. After investigating, I was assured that the chair would be removed right away. However, three days later, the chair remained in the room. Throughout the stay, the level of care, responsiveness, and bedside manner displayed by many of the nursing and CNA staff was unacceptable. The facility appeared to be understaffed, and there was a noticeable lack of urgency when patients required assistance. On numerous occasions, we used the call button to request help for my loved one. Not once did a nurse or CNA respond to the call button in a timely manner. Each time, a family member had to physically walk to the nurses' station to request assistance before anyone responded. My loved one shared a room with another patient, and I frequently overheard that patient calling out for help. She could often be heard moaning, groaning, and asking for assistance for extended periods. When staff eventually entered the room, I was shocked to hear them scold the patient, telling her that she was "always calling" and that she "didn't want anything." Witnessing this interaction was deeply troubling and demonstrated a lack of compassion and respect for a vulnerable resident. Another concern was the apparent lack of engagement with residents. Many elderly patients spent entire days sitting near the nurses' station with little to no interaction from staff. They appeared isolated, disengaged, and unhappy. The most disturbing issue occurred when I noticed what initially appeared to be clumps of dirt or lint along the wall behind my loved one's bed. Upon closer inspection, I discovered that the area was covered with hundreds, if not thousands, of ants crawling along the wall and throughout the room. I immediately reported the issue to staff. One CNA responded by saying, "Well, I just got here." A nurse then acknowledged that she had already informed the day aide about the ant problem. This indicated that staff were already aware of the infestation, yet no action had been taken to resolve it or relocate the patient. When I expressed my concern and disgust regarding the situation, the nurse rolled her eyes and told me that I should contact maintenance about the ants. I found this response unprofessional and inappropriate, especially given that the issue involved patient health, safety, and sanitation. My final negative interaction occurred during the discharge process. The nurse manager reviewed my loved one's discharge instructions and medications. When I asked questions regarding follow-up medications, I was met with visible irritation and an unwelcoming attitude, as though asking questions about my loved one's care was an inconvenience. I strongly believe the facility should reevaluate its hiring practices, staff training, accountability measures, cleanliness standards, and responsiveness to patient concerns. Patients and families deserve compassionate care, professional communication, and a clean, safe environment. The one positive aspect of our experience was the therapy department. I did observe meaningful improvement in my loved one's therapy progress, and the therapy staff appeared dedicated to their work. They were the bright spot in an otherwise disappointing experience. I was relieved when my loved one was discharged from Ignite Medical Resort Dyer. Based on my experience, I would not choose this facility again. I encourage families with loved ones in nursing homes or rehabilitation facilities to visit frequently and remain actively involved in their care. During my time there I was in need of physical therapy, dialysis and skilled nursing care. They responded quickly to personal and medical needs as they arrose. There are some staff members I would like to thank. First is Antntoinette she is the most caring, dedicated person I've met and I was so grateful to herald I will never be able to thank her enough. then there was Abbey,Ramos and aiden a great group of CNA's. Who also provided great care. Then there was my nurse practioner Lindsay. Besides being very knowledgeable in her job she was always in a great mood and willing to help anyone who needed her help. Then there was jayme. Mike, nina, Marisol and Diana. These amazing physical therapists helped me get strong enough to allow me to go home. I will always be endebted to them. Then to Anita, Tammy, Deanna and Pablo you guys are truly wonderful nurses and I thank you. Karen and Joy are great wound care nurse who mastered a challenging dressing. To Valerie, Carla and Sean your facility is smazing and keep up the great work. All these people I mentioned got me thru the toughest period of Mt life and I recommend this place to anyone in need of care for themselves or a family member

The Pearl is absolutely not a good live in rehabilitation center. No care plan is ever provided. They are woefully understaffed. In my opinion they are more concerned about money than the patients. In my opinion, from what I saw, they take horrible care of patients! Non responsive nurses and staff; tried to give incorrect medication to my relative more than once!! STAY AWAY!!!

    I had the worst experience of my life and still have nightmares. My sister was put there for…read morehospice. Their staff left my sister who they didn't tell anyone including hospice providers that my sister was unconscious for a whole day. They left her in soiled pants and sheets for days. One day I came in with her choking because they had her flat on her back while I begged crying for help. She should've been sat up right. The head nurse had no compassion. We asked for our hospice provider to have someone stay wit her while we couldn't be there and she told them noone after 8pm. We were scared she would die and be dead for hours before anyone even noticed. They didn't clean as her floor had sticky stuff on it the whole 3 days we were there. She ended up dying after 4 days after being unconscious alone. I was with her 14 hours most days until that day. She died at 5am. Who knows the true time. 2 nurses that tried but were overwhelmed was the 2 Sherrys. Don't send your loved ones there unless you want them to die a horrible death and for your loved ones to have nightmares every single day because of the experience. I am in agony still today. There was even more that happened so please please do not allow anyone you care about go there.

That…read moreunfortunately became a reality in 2023, after I was hit head-on by a wrong-way driver with dementia on I-39. I just wanted to take the time to recognize the staff at Thrive of Lisle, as they took great care of me. Shoutouts to CNA's Alan & Harrison; nurses Shirley, Mark and Gina. They are the best! Kellie from activities, Jerome and Sue are always roving the halls and engaging with residents. The positivity and enthusiasm of the staff is truly contagious, and made this long recovery journey easier for me. Therapists Susan, Ed, Keyenna and Tiffany have been helping me work towards my goal to be able to walk again and use my left arm. I love Thrive of Lisle, and the staff here. 2023 was easily the darkest time of my life. I nearly lost my life, and was not sure if I'd ever walk again, or be able to return to the career that I love. The staff at Thrive treated me like family. I can't tell you how much happiness and hope that CNA Alan, Nurse Shirley & Kellie brought into my life during this time. They are truly special people. I still come back to thrive and visit at least 4 times a year. It's rewarding for me to visit the current patients during activities time, as well as spreading positivity and encouragement. I also love seeing my extended family (the Thrive staff). My advice & words of wisdom for incoming patients and their families: * Advocate for yourself as the patient; if you're a relative; please advocate for your loved one. * Treat the staff with love & respect (I promise it'll come back to you ten-fold, and will be a much better experience for you & the staff this way.) * You might be shy at first, but take full advantage of the activities that Kellie puts together! From craft-making to group games; it's a fun time for everyone! (Even the patient's visitors are encouraged to participate). * It doesn't matter if you're bedridden, the staff will make sure to get you out to socialize, shower, or PT. You just need to communicate your needs to them early enough in the day. * Family: PLEASE don't underestimate the power of your visits. Frequent visits will greatly help your loved one as they strive to recover. I had at least 2 family members taking turns visiting me throughout each day. *Keep in mind, you ARE allowed to sit in on your loved one's physical therapy sessions. - Just ask the therapist as a courtesy* * Stay positive & Keep on thriving!     I was sent to Thrive for wound care. So I did not require the assistance some people did. I was…read moreable to walk and dress myself. I did receive physical and occupational therapy. The therapy was to target both upper and lower body and I felt it was a great benefit for me. Therapist took time to answer my questions on how or why the different excersises were a benefit. I had Murium and Traci. Nurses and CNAs were all pleasant. When I was not in my room they would look for me for required medications or lab tests. They also seem to work as a team. No task is beneath anyone. The activities director Kelli has such a nice personality that even someone like me that typically does not join in was participating. Because I did not have anyone visiting me they made a point of making me not feel alone. Many of the staff were good but I had a special thanks to Noah Teddi Alfonso Sue Kelli Drew Odette and my wound care team. I was able to heal nicely by the time I was discharged thanks to their attention and care. Only issue is with the evening shift. I think they forget at times that it is bedtime for us even if it's just a work-shift for them. They would be quite loud and slam doors. Rooms are 2 people so if someone needs to be tended to in middle of the night or is hard of hearing that's a problem too.
