This is a okay facility with many hardworking and caring staff members but there are a few areas that I believe could really change. One concern is staffing during the morning shift. It feels like there are not enough cnas scheduled and those who are working are often responsible for 8 to 10 patients each. That is a heavy workload and it is clear they are doing their best but it makes it difficult for them to give each patient the attention they deserve. Increasing the number of cnas on the morning shift would make a big difference in patient care. I also think the facility would benefit from having more male staff on the morning shift and the men need to be put to the females to help with physically demanding tasks as some patients require more strength and support during care. Just like the women cnas get men, Same should be with the male cnas don't know why it's sexist. I do want to highlight some of the amazing staff who truly stand out. Juana is incredible she seems to know just about everything and is extremely good at her job. The cnas Sury Monette Rachelle Cindy Ashley and Mary Louise are also outstanding. They consistently go above and beyond showing compassion and dedication every day. In my opinion they deserve raises for the level of care and effort they put in especially compared to others I have seen who do not put in the same level of work. My father was originally in Station 2 and later moved to Station 1. He definitely prefers the care in Station 1. Unfortunately our experience with Station 2 was not as positive the cnas were often hard to find changes were frequently delayed until the last minute and the overall attitude felt less attentive and sometimes rude. I have to give Berkley skilled nursing center a huge applause once again. My mother had a stroke and lost her speech. She was very frightened but when she left Valley Presbyterian hospital for a stay at Berkley - it was all a positive experience. I am writing again because after a month she had to be moved to another facility for further care in studio city that was just awful. She regressed on her progress and although she was having treatment for mental disabilities- it was by far the worse treatment of a human being. She was drugged beyond comprehension and instead of the careful attention necessary to walk her to the restroom - instead she was left to soup herself with an adult diaper when she was quite capable only requiring assistance. I called Trevor the director at Berkley and told him what was occurring and if he could help us transfer back - he was beyond helpful and we got her back to Berkley where they treat each patient with the best care and attention. I can't tell you how stressed my brother and I were until we got her back to Berkley. Trevor is a true hero! Even my mother's priest who visited her weekly was appalled by the condition he found her at the studio city rehabilitation center. He also said she was doing so well when she was at Berkley. We are all so grateful for Trevor helping us move her back there! Thank you!! read more

A couple months ago, my family began the laborious process of placing my dad in a rehab facility after 2 unexpected surgeries. After several tours, we decided on Berkeley. From dad's late night arrival to his discharge to a Board & Care, Berkeley's staff guided me through the process. They accommodated our requests in a timely manner that helped my dad feel comfortable & safe. We visited on a daily basis for the 3 weeks while he was there. Physical Therapy encouraged dad to get out of bed, walk, & stretch to regain strength which gave him a more positive attitude. Ashley, Medical Records, kept me informed as she diligently made follow-up appointments & ordered necessary tests. My family is especially grateful to Christina, Social Services, who patiently answered my questions until I understood. She gave me a list of Board & Care options in which we found a great match. I feel that she cared about my dad's well-being. Discharge Day went smoothly as all departments communicated with each other, & Christina communicated with me the plan. It's hard to watch a loved one go from independent to dependent in a few weeks. Placing someone in a facility comes with mixed emotions especially after reading various reviews. It's important to take time to research & ask questions. Looking back, Berkeley was a good choice for my father. read more

My family and I have had an excellent experience with this facility, and I wanted to take a moment to share our gratitude. From the beginning, the care has been truly outstanding. The nursing staff is attentive, responsive, and takes concerns seriously. Whenever my wife has had a medical issue, including symptoms that needed evaluation, they have acted promptly and responsibly. The CNAs are consistently kind, patient, and supportive. They go out of their way to make residents feel comfortable and cared for, which has meant so much to our family. The facility also provides excellent physical therapy services while maintaining a high level of medical oversight. It's rare to find both combined so well in one place. I would especially like to recognize Trevor, the administrator. He has always been supportive, kind, and incredibly responsive. Any time I have reached out with a concern or question, he has taken it seriously and acted right away. His leadership and genuine care make a meaningful difference for both residents and families. Most importantly, I feel confident that my wife is safe here. Given her medical and mobility needs, that peace of mind is invaluable. As you have entered onto and advance period of your life, certain decisions are of the upmost importance. Following a sad accident of my wife, and the medical procedures that followed, I was faced with, not only the decision of selecting an Rehab Center but, pressured by the hospital to discharge my wife on a very short notice! I was immediately forced into, not only researching for a suitable facility but, traversing the San Fernando Valley and visiting several facilities for a suitable location for my wife. As a person of Faith, I felt someone higher guided me to the Berkley Acute and Rebab Center on Sepulveda in Van Nuys , Ca. May I invite anyone finding themselves in a similar situation, to visit the Berkley Rehab Center. I'm sure you will find (as I did) that the Center is impeccably ample, clean and well-staffed. I was impressed with the accommodations offered by Director, Trevor, and Social Worker, Frencessca. A special mention for Certified Nurse Assistant Rosa for not only performing her duties but, showing personal care for my wife. The facility provides good care, and my mother truly loved being here and appreciated the staff's efforts. However, from her perspective, she would feel more comfortable and better supported if male cnas were also assigned to assist female patients, especially those who require heavier lifting. She shared that in other facilities she's been in, both male and female cnas cared for all patients, which made things run much more smoothly, and she believes other residents would benefit from that here as well. A lot don't mind a male nurse , She also mentioned that she even spoke to a nurse about this preference, but felt that her concern was not acknowledged. She was more comfortable having a strong male cna assist her with lifting and transfers, rather than relying on two or more female staff members. At this facility, she often had to wait a long time for her shower or ended up being one of the last to be helped because she had to wait for a male cna to become available or finish their duties to assist her female cnas. Because of this ongoing issue we decided to put her back to Burbank Rehab. Thank you, Trevor, for helping my mother your support did not go unnoticed. read more

We were pretty apprehensive about placing my 79 year old sister in a place that, from the outside, doesn't show its true value. After all, it's dead center in the middle of Van Nuys. However, my sis has been there for over 6 months now, recovering from a severe heart attack and stroke. And, although quite lucid, she does suffer from a serious anxiety disorder which causes extra adjustments by the staff. So, we have found her care to be fairly understanding and pretty tolerant of her issues. Aside from this, Berkley Post-Acute excels in cleanliness, Covid hygiene, physical therapy (its therapists are superb!,) culinary preparation (the food is tasty and nutritious,) and it responds rapidly to a patients needs. Specific needs are handled most effectively through the Administrator himself, who, through texting, responds and delivers within minutes to a patient's or a family's inquires. Unfortunately, it did take us a while to believe that his promises would be so consistently delivered. The Business Office should also be commended for its flexibility and of the knowledge base it has of the myriad medical costs involved in any patient's billings. Larry C. read more

My mom has been at Berkley Post-Acute for a little over 3 months. I must say she is really happy with the care there. I am too. Before we placed her in the facility, we were in the ER at least once a week and sometimes admitted. She has heart and kidney disease, is nauseous often, has a strict diet to follow, electrolytes to keep balanced, BP and weight need to be taken daily, easily gets swollen ankles and legs due to heart not pumping like it should--so many things, all of which were difficult for me to maintain in a consistent way, which caused her to get sick often. Since being at Berkley Post-Acute, she has been doing so much better. It's like night and day. If she does get swollen ankles or legs, it's resolved fairly quickly. She isn't moody and crying and in pain anywhere. They are making sure that every day is a good day for her as much as possible. I never thought I'd like a nursing home for my mom, but this one is great! Awesome, nice staff and caring nurses that are well-skilled and very diligent and caring CNA's. The activities and special events are fun and she's made lots of friends and loves English and Spanish bingo! read more

Berkley is an amazing place for anyone that has a need for physical therapy. The staff is amazing/outstanding and attend to all my needs. I've noticed that they treat everyone like gold. I used to be afraid of these places because you hear of all the horror stories. However, Berkley has exceeded all of my expectations. I also have a senior mother who I would place here because I know she will get treated right. The staff are constantly motivating me and other patients to move, get out of bed, and to walk in the hallways to maintain their functionality. These are the things that make Berkley outstanding which makes them go above and beyond. I've noticed that management is considerate and comforting. They are on top of the ball and show support to the nurses and other staff which directly reflects to the care that they provide. I have to give credit where credit is due. Me knowing business, this has everything to die with who runs the facility. The structure of how everything is is run is immaculate. The director really knows what he is doing. My father has had a couple of stays in skilled nursing facilities following hospitalizations over the last few years, so I have some frame of reference for my review. Berkley is bright, clean and serves good food made with fresh ingredients. What sets them apart though is the quality of care they provide. They have a high ratio of RN and LVN nurses who are knowledgeable and caring. They were always happy to speak with me anytime I had a question or concern. I can't say enough about their CNA staff! They have a difficult job but make it seem easy. They took the time to get to know my dad personally and were extremely caring, patient and kind with him. Knowing he was in their good hands gave me peace of mind. The social services team went above and beyond helping me navigate insurance issues and later the transition to assisted living. Tanya was extremely supportive at every step of the way. She even followed up after discharge to see how it was going for my father and again offered me further assistance if needed! read more

As you can see from the many five star reviews regarding this facility it definitely is wonderful. When I brought my sister to this facility right off,I noticed it was very clean. I met Helen, who Kimberly Wwas my sister's CNA I was impressed with her kindness, patience, and willingness to be of service. My sister was unable to walk and get out of bed when she arrived here. Due to the time she has spent in rehab she is riding the bicycle, walking with assistance , playing bingo, and participating with other patients. Tanya and Luze from Social Services were right there when we came in the door. They helped us make the best decisions for my sister's care. Trevor , the head Administrator listened to my concerns. He is assisting me in finding where my sister will live permanently. I bring all of this information up because of all the other facility I have dealt with they are nothing like this facility. This facility is extremely efficient They have more than a adequate nursing staff. Right now they have more than a few people that are here for health issues that make them bed bound so with them & those that need some help wearing diapers they change people frequently because they are great at doing rounds. That is a must if you've needed to do it on your family member including a baby. The food is better than ok. All you need to do is ask for coffee or a sandwich & they can go & bring it back if the kitchen is still open. Snacks you can get them too. They have activities, with bingo you can win a dollar & more than a few others too. They have therapy & occupational therapy as well. Honestly you wouldn't need to worry about it here they have good place to stay. You can visit & see that this is a happy place, staff are very nice & always helpful. I have been to another one & this is by far better than the other one by how much is done & how they run it here. read more

I unfortunately fell off a ladder and suffered from a broken back. I had to go through surgery and had to decide where to go for rehabilitation. My wife and I looked at reviews, and we decided that I will go to Berkley Post Acute. Boy will I tell you that was the best decision I could have made. I tried to look for the bad about this place, but I could not find it. Everyone that came into the room, everyone in the hallway, rehabilitation and more all had a smile on their face. You can tell that this starts from top leadership and waterfalls down. The rehabilitation was top notch. They were really logical about my care. They would ask questions, and they came up with the best approaches on how to move forward. In three weeks, I went from a bad state to being able to go back home.
