Looked extremely warm and friendly! The facility reminded me of someone's warm and loving home. Two large places to relax and visit with patients. One outside and one inside. The entrance looked welcoming because patients and family were hanging out visiting, Lots of socializing at 6:30pm. For us, we were lookin for a facility for an adult Alzheimer's patient who could walk. No locks on doors to keep them in, or door alarms. This is a smaller facility but very clean and they are very responsive. From management to the CNA's and housekeeping. Everyone is friendly and helpful. My 93 year old mother was only here for a short period. During that time I had the opportunity to see them in action. They were never short or unwilling to fulfill my requests. I was there a lot because, I am a helicopter daughter. But if you cannot be there know your loved one is in good hands. Grandma hasn't been this happy in years!!! She's a 97 year old with a sharper mind than most people half her age. She's particular and can be a bit judgy at times, but she's got it all together. Her poor body just didn't stay as young as her mind, unfortunately. Although, at first, she was unhappy to not be living in her home and being without her yard and her things... she's grown to love Jay and his staff so much over the last couple years living in their care. She learns the names of everyone who comes in her room, and loves every one of them. They have all done nothing but make her feel comfortable... and this woman wants to be spoiled!! Socal has a beautiful patio courtyard for visiting out in the fresh air. Inside the facility they always have it decorated festively for the holidays, EVERY holiday! The lobby is set up so that it is comfortable for visiting as well. Bingo, church, crafts, sing-a-longs, donuts delivered to them personally on Sunday mornings. I've even been there for Thanksgiving celebration and it was delicious, fancy, and so warm and loving! I couldn't ask for a better place for my sweet Grandma! Thru Covid, there have been no visits allowed. This has been hard on all of us (thankfully that 97 year old Grandma of mine is on FaceTime and Facebook constantly!!) But, obviously, they are doing the right thing because according to Grandma, they have hardly been affected by cases in the entire facility! Masks, gloves, sanitizing, and virus checks are a common occurrence. Although we aren't allowed to visit in person, I know she is in wonderful hands in her Socal Post-Accute Care home! Staff was overall nice. It's just I am concerned of my loved one . They lost his wedge we bought for him. When we went to visit him, his bed was wet w/urine, his catheter was leaking. Also, we were not told to bring his own adult diapers. (Or we would have provided him some) They lost some of his clothing. The overall staff was nice. read more

Very unprofessional charge nurse and admissions director. On the first day we were told if we did not like it there to look else. All this because man that work there was wearing a faded tshirt abd did not look like a staff worker in scrubs. I made a remark that churned the wheels of gossip. On the third day I found what appeared to be feces or a brown remnants of an insect. I brought this up, but was afraid to be bullied by the staff, so I let it. Later that day my mom mentioned a bald older man giving her dirty headache pills, ut she stated she was in pain and took them anyway. Again, i rekayed this to the charge nurse, she flew off the handle and behaved rather defensively instead of writing up a report. She said, "I saw it, but thought it was a "Mexican" thing like to put cinnamon in water." This water was found by me at 9am. It was water sitting there nit changed by the nurse who first saw it. In addition said my mother was demented. That was it I took her out. Totally violated my mother's patient rights and dignity read more

I have stayed at this facility for 4 months for generalized weakness. Prior to the stay here, I didn't walk for about 6 months. The physical therapy dept gradually worked with me as a team and never gave up on me. I learned to stand and walk again. The nursing staff was extremely accommodating to me whenever I needed them. If my assigned nurse was busy, there was always someone to answer my call light and acknowledge me. Everyone single one of the nurses became my friend. The administrators were involved with everyday care for all residents at the facility and their staff. My family and I highly recommend this facility for any rehabilitation or nursing services. My mother lived here for two years after her stroke The staff was always very attentive and caring. The place is clean and well taken care of. They provide a lot of social interaction and activities for the patients. When my mother was put on hospice and before she passed many of the sttaff came by to show their support and say goodbye to my mother. Loved this place!! Being a deaf (hard of hearing) patient recovering from a serious work related injury. The nurses took care of me the best they did and made sure I understood what they said to me by writing down everything since I couldn't hear too well. The owner and staff stopped by my room every day to make sure I was ok! When the owner and staff check up on you that speaks volumes on how this place is run. The physical therapy department worked me so hard that they got me up and walking in 3 months!! Even my doctor was shocked to see me walking too soon. I can go on about the friendship I made with the nurses, staff and patients the atmosphere was friendly and family like that I wished to go back and visit but due to COVID-19 I'm unable to set foot inside to say hello. My mom was injured in a fall and after the hospital stay, she was referred to this facility. My dad was skeptical, but after talking to Ofelia, the director, his mind was put at ease. My 94 year old mom fell and had 3 fractures in her pelvis. After 2 weeks at PIH we were told that we needed to move her to a rehab facility. We had previous experience with some from when my grandma was alive and were so stressed and upset. We had a choice of a couple but this one had a bed available so with a lot of prayer, we put her here and WHAT A BLESSING! Everyone is so caring and friendly. it is clean, she loved the food and they have an excellent PT staff. Jay and Lily, the owners, are at the facility every day and greet each patient by name. The fact that the owners are at the facility on a regular basis, working alongside the "hired help" should tell you a lot -- excellent staff, pleasant, clean facility, good food. Went to visit this place to see if i could take my mother in law. Jay was friendly enough and said he had two beds available. When i went to do the paperwork, it turns out they didn't have a bed or he didn't want my mother in law there. He should have been honest about who he wants at his facility. read more

My mom was here for 20 days. Jay and his wife are at the facility everyday to check on things that they are done right. The PT and OTC staff are absolutely fabulous . They helped my mom to walk again. The nurses, LVN and CNA are great too. I have never meet such caring people. They truly are professionals-who are passionate about their jobs. This is my husbands 2nd time at SoCal in less than 6 weeks. Due to complications from a procedure which set him back in Physical Therapy he needed to go back to a nursing home. I made a great choice for my mother when I chose So Cal! I know the lady can be difficult at times & they all have the patience of angels. So Cal was rated highly on medicare.gov website, but so was nearby Southland (Norwalk?). The difference between the two facilities is astounding, with So Cal rising far above Southland by all measures. Simply expressed, we love the feel of So Cal - the kindness of staff, the smell, the look, the quietness that prevails - compared to Southland (which at times drew comparisons to Dante's Inferno). This is my mom's second visit to a nursing home; this time around she broke her ankle. We visit her weekly (for 2 months now) and have been consistently pleased with the facility's service and her progress. My wife calls her nearly every day to check in, and things seem as good as could be expected. Special accolades to Jay, the Administrator, who's seems to always be there. He's a great listener, and seems eager to accommodate. When mom passed an important milestone (being able to put some weight on her ankle again), he helped us formulate a timeframe for her return to her assisted living facility. Our kudos! read more
