    6/22/26 Good Evening…read moremy name is Bridget Branch. I am a hospital unit clerk at San Joaquin Dialysis Clinic. I would like to say that I appreciated my experience with the authorization dept. I had a dialysis patient who really needed a urgent authorization and I had spoke to two other auth representatives on 6/17/26 who did not see previous auth that was submitted by my coworker on 6/10/26. Jimmy Espinoza was the 3rd representative I spoke to and he looked thoroughly to find faxed auth which was above and beyond due to his caring nature and concern for the patient. We were able to get the authorization that was needed for our patient to be seen at San Joaquin Vascular Surgery. Shout out to Jimmy for a job well done. Thank You Bridget Branch- HUC

    The center is great great people But the dental people put…read moresomething on my tooth with telling me even took it off the record as to not admit to doing it takes months to get a appointment just to wait for the next appointment I been waiting 3-4 months just for them to tell me they have to reschedule and now I add waiting months again with no appointments. Same with eyeglasses waited months for appointment to see Dr another month to get my glasses adjusted and another month to actually get them still waiting for my nexted appointment I called in many times to get a appointment only to say they call back and nothing Some employees call patience by funny names I am sure they were joking around but other patience heard it. I sit at home in pain waiting for my appointment that never get made this is directed to the Modesto site

    My mom was at this facility for about 11 days after having a stroke. I say first couple of days…read morewhen mom Needed higher level of care staff was attentive as cna's followed rn's orders real well. When her care was less critical I would say it was bordering on neglect . A cna said she was in charge of 15 patents and almost half had to be spoon fed including my mother. Hence, myself and Family made sure to be there so she could eat. My observations were patients who didn't have family step in for them might of not ate well. I heard one rn supervisor talk extremely rough to like a dementia patient to be still and stop moving. It appeared as work burn out. There were many times mom was soiled and staff would not come to help. I had to change mom myself and do best I can to get her to the restroom, changed and brought back to bed. They had no sense of urgency in helping despite telling me to not take mom alone. I waited but when it started approaching two hours I had to do it . I also Had to help cna put mom To bed as she had no help as mom was too weak and a two person job. They got mom's diet order wrong as moved up to solids and kept on doing a blender diet for days. We had to Go out and buy food so she can eat as she hated blended diets and I don't blame her. I was told this happened cause dieticians use a different system and it don't data share with the facility but nobody could make a phone call. There was like a comittee but I don't think they were proactive (except supervisor of nursing) as made excuses For some of my concerns . I am not going to say this facility is the worse but definitely not the best. I was told this place was the best in town. I am Just going to say it's like any other nursing facility from what I hear. When I thought mom was well enough to go home I demanded she leave immediately as they wanted to keep mom longer. I overheard admin mentioning mom was a "good" patient as had dual insurance. I told them at my house mom is not neglected and will get better care as I had all her bags packed. I don't recommend this facility for your loved one and it's definitely not the best in town but can't say the worse cause I haven't seen many . It missed my high expectations and them trying to say get better care cause they are small is garbage.

This is not an assisted living facility. The administrator has a set of rules which of course needs to be strictly adhered to for the safety of the residents and physical property. Included in the monthly rent are the following features: 1) Three meals a day and a snack 2) Housekeeping including laundry services, vacuuming, and general housekeeping services 3) Help with limited medication management There are a few other limited services to include the parking of power wheelchairs in the attached garage and use of power to recharge batteries This beautiful 3,000 square foot home is a single family home. There is a roomy kitchen which the twenty-four hour staff uses to prepare delicious, nutritious meals. ailments such as diabetes can be accommodated Two very large dining rooms, three large living rooms two of which have large flat screen TV's both of which have Roku and Wi-Fi Wi-Fi is available to the residents at no additional charge. A beautiful backyard with built-in gas powered bar-b-que, ample seating, a decorative water fountain and a gas powered fire ring for cold nights A bed and a modest amount of space for personal belongings This is a beautiful facility and I recommend it to anyone interested in this kind of living arrangement. Of course there are shower and bathing facilities. Home At Last provides a home like environment that is unparalleled in the industry. I very strongly endorse it.
