My problems with him were…read morethese: He gives your diagnosis with long stories leading to the diagnosis punchline, which can be interesting, but when you've brought a list of problems you're having, you might get to 2-3 of them. I'm not in great shape so I really need to go over everything in the short time that's allotted. I wrecked my shoulder pushing a 125 pound Casper box bed up a staircase. Dr. Siu didn't feel like I needed it checked, and instead told me a story about how he and his friends play competitive tennis every Christmas. 4 minutes into it he got to the punchline: They were all getting old and all had tennis elbow, which is what I now have. "Take 2 Tylenol before bed every night, cause that's all you can do." During this time, he sent me to a therapy place he (and they) swore were in my network. They had me doing all kinds of improvised nonsense without an MRI or a confirmation of what was wrong first, and of course made things worse. Went 3 times, knew it was a bad idea, quit going, then a $632 bill arrived because no matter what Dr. Siu or they said, they weren't in our network and just wanted the business. After a year of horrible pain and not being able to lift my hand higher than my shoulder, my wife scheduled an MRI. I have two 1 inch rotator cuff tears and I've needed surgery since the day I told Dr. Siu. I could almost let that slide, but near the end of 2023, I felt my job was in jeopardy, and I told his office that it'd be hard to get down there in the day for a routine checkup. They countered with, "The doctor says he's not refilling your prescriptions until you see him." I take like 16 pills every day, including heart and diabetes drugs (4 Metformin daily) that ran out of refills Nov-Dec. I told this to my new (as of 1/1) doctor who started by meekly defending the holding of my meds hostage: "Well, a doctor really shouldn't give you meds without at least meeting you." "I saw him multiple times!" "Oh! Well then, never mind..!" I won't even mention how a shirt of mine got ruined (blood) when his young staff hacked off skin tags with a dull Bic razor for an EKG ;-) Anyway, that's my novel. Probably a great doctor, but best to see him if you're already in good health.

When my mother became a resident at a board and care facility in Mission Viejo, he became her…read morephysician. All his visits were done virtually via video calls. On the positive side, he is very responsive to phone calls and did call back promptly whenever I had questions and he did spend time to discuss things. I was honestly surprised by how quickly and consistently he would get back to me. On the negative side, he was grossly overconfident in his video-call diagnosis skills and erred on the side of over-treatment even when stated family and patient goals were to be palliative. My mom had all the tell-tale signs of middle-stage dementia. He gave her a diagnosis of "mild cognitive impairment." My mom could not remember the thread of a conversation for more than 20-30 seconds. Still "mild cognitive impairment." At some point, our family discussed tweaking one of my mom's meds to help her mood. He adjusted the medication dosage up. Shortly thereafter, my mom was not able to physically get out of bed. Dr. Siu said we needn't worry. This was a known side effect of having dosed too high. We would just reduce the dose and there would be no long-term effect. It took nearly two weeks before my mom was able to get out of bed. That means she was close to paralyzed in bed for two weeks in her mid 70s. This led to the loss of almost all her remaining muscle mass. I raised this as a possible major issue. He said it was nothing to worry about. Let's be clear that muscle loss is a MAJOR issue if you expect to be able to move without assistance. The loss of muscle means you are no longer able to balance your body and prevent falling and hurting yourself. So at the end of this episode, my mom could barely walk more than a few feet on her own. He did not seem to weigh the risk of being bed-ridden and total muscle atrophy against whatever benefit a tweak in medication would make. Had we known this was a risk, our decision process would've been much different. As my mom's dementia and physical condition worsened (since she now had ZERO muscle and she was having concerning shudders, shakes, and mini seizures), we asked him about evaluating her for hospice. On the basis of a video call in which she displayed "flat affect", he told us that maybe her "flat affect," lack of balance and shudders and shakes were the result of potential Parkinson's and maybe we should take her to a specialist to see if we could get her on some kind of Parkinson's treatment. My mom did not have flat affect in person. But placed in front of a phone for a video call with someone she did not actually recognize, yes flat affect could be a thing. But when I raised this with him, Dr. Siu would not consider that family and caretaker observations might be more accurate than his video call assessment. He pushed for seeking out a specialist to investigate possible Parkinson's. I pointedly asked, "are these symptoms not ALSO a part of dementia? Does it not make more sense that dementia is the larger issue here?" He downplayed this and said "If it were my mom, I'd take her to get treatment. It could help. Couldn't possibly hurt to try to treat it." When we started with him, we were very clear that my mom's wishes and our family wishes were to err on the side of LESS treatment and less invasive medical care to reduce any possible unnecessary suffering. I then listed the ways in which taking someone with obviously bad dementia out of their comfort zone for a dizzying array of appointments, tests, and overall confusion might NOT be worth it and could be actively harmful. All this would be an enormous drain of time and energy, particularly if all the issues are more likely explained by dementia. This would be subjecting a fragile person with incontinence issues to torturous explorations for the hope of what exactly? I pushed for hospice evaluation. At this point she was already having notable trouble eating (though he could not see that through video eval). He said hospice would be unlikely to admit her because her dementia was not far along, but he would make the referral. He made extra effort to point out that once on hospice, we would close the door on any parkinson's treatment/eval. We got hospice evaluations done. Within a month she was admitted to hospice. She died of dementia within two months. I do not expect doctors to get every answer right. No human can or will get every answer right. But his overconfidence in his own assessment, dismissiveness of caretaker and family observations and preferences, and bias to over-treat at end of life was not at all comforting in an already difficult time.

They either have a poor work ethic or are mentally incompetent and can't do the simplest of tasks. All we needed is for a referral to be faxed, they can't do anything right; literally, every step of the way, there is an issue. Plus, they lie or just disconnect the call. Save yourself the frustration and avoid this office; it's not worth it, there are other options out there. Unfortunately, we as a family will no longer be going to Dr Zamani, and will be switching. I will be telling my friends aswell whom I have referred in the past.
