Oleg Maksimov. This is the worst doctor I've ever seen. His manner is best suited for work with cadavers, absolutely no respect or compassion whatsoever. I was sent to him by the VA due to chronic neck and back pain with x-rays and MRIs to back it up, as well as a couple of sports-related injuries, including a poorly-healed broken elbow and a complete rupture of the gluteus minimus and medius (having surgery on that in a couple of weeks). He only cared about the neck, didn't want to hear about anything else. Zero eye contact, no facial expressions, clipped and monotone. I asked a couple of questions but had to repeat myself because he ignored me. When he was done fiddling with his computer, he did have a plan; come back for some nerve blocking (or whatever it's called) which should work for 4 HOURS. Then keep a pain journal so they know if they're in the right area. So... nothing today? "No." Why not? "We need authorization from the VA. Could take days, could take weeks. Don't know." Can I get something for the pain in the meantime?? (No history of abuse or addiction, all injuries well documented.) "No. Nothing. It was nice to meet you." Then he walked out. The office staff were nice, but you couldn't pay me to go back to that place. From their parking lot, I reached out to the VA to find a different provider - which took about 10 minutes total. They were eager to take my statement and apologized for Oleg's behavior. Oh, and the authorization I needed to wait for? They had it the entire time. I was actually called a dope addict (Registered OMMP patient for 10 years) and a danger to their DEA medical license by one of their providers then sent out the door cold turkey after stringing me along for a month on Vicodin. I made a review earlier about this pill mill clinic! Dr. Oleg Masimo is the head of this Clinic- Gresham. On a previous review, I had misstated his name earlier as Dr Vladimir is incorrect. I had one appointment with this so-called Dr., he didn't even have my records or history in front of him. He was typing on his computer the whole time, wouldn't make eye contact, and spent the entire time, except for the knee-jerk test, on his computer! Yet he told me to get a Personal Trainer! Two-hour drive and then being treated so unprofessionally, to be charged/billed over $500 for a medical evaluation when that was not the purpose of this visit. That was all done on my first visit with the Dr.xx I won't name as main problem/concern was with Dr. Ole M. Yes, they charged me $500+ and he didn't even have my medical records. I told him that wasn't my fault. Then I was gaslighted the entire visit. , I was seen originally by a different Dr., for several months I won't name him, who works at the same clinic as/ for Dr. Oleg M. -Maximo- who is part owner and who gives these change orders. Every follow-up visit was a different plan and negating policies/recommendations that were originally given at my previous appointment. I'd ask, "Why again are the policies here always changing "? This Dr would always say, orders coming from above, then he'd shake his head. No compassion, unprofessional, just another fake company out there that offers a terrible experience for helping with the management of pain issues. Imagine being told twice in one visit, that you need a personal trainer, and he doesn't have my medical records on me and bills for a full exam/evaluation! A bad experience altogether. Honestly, I left there feeling like an abused woman who was harassed throughout appointment, even insulting me. I felt taken advantage of because I' m a female and vulnerablel. They just want to bill your insurance and take that money without providing the quality care they advertise! They couldn't care less if they tried. Had my first visit today, 2/10/2025, and was pleased with both the physician and the staff. Everyone was professional and courteous.

I received a "Discharge from Clinic" due to "disruptive and inappropriate behavior towards staff members and other patients and your provider on 1/16/2025 creating a loss of therapeutic relationship". I am at a loss as to what they mean --- my discussion in the waiting room with an elderly lady, her son and daughter-in-law about my recent experiences with Oregon Health Plan and its Qualified Medicare Beneficiary Program. My interpretation was that this cohort and family were pleased to have the information I was providing to them about the QMB program as Secondary Medical Insurance is supposed to cover any charges for any service or part of service that could normally be covered by Medicare, but were disallowed (for whatever reason) such as co-payments, deductibles. I personally have had OHP QMB Medicaid since before my first visit to Columbia Pain and Spine, around 9 years ago. I have had to make cash payments to CPSM Burnside Surgery for deductibles which should have been charged and paid by my secondary insurance, and which can possibly be retroactively billed to OHP and then reimbursed to me. The gentleman next to me in waiting room verbalized that his Mother had had similar experience of being told QMB pays for nothing, and finding out that QMB IS SUPPOSED TO PAY was information they were happy to receive. I did not offend other patients with any disruptive behavior! This letter of discharge makes it sound as if I went on some angry tirade in your office which I certainly did not! While as an independent business you likely reserve the right to withdraw services to any individual for whatever reason it is unconscionable to fabricate some "loss of therapeutic relationship." I have done nothing to the Drs or PA's of the clinic which should have been considered offensive. I have not spoken to anyone there in a rude or aggressive manner. There was absolutely nothing different on 1/16/2025 except for the discussion with the family next to me in waiting room. Why should that disrupt continuation of medical care??? I reiterate that I have been discharged from medical care after 9 years or so, for no reason. Discussing Insurance Issues? That's a problem for all of us that needs clearing up! Bringing it up with Grace (from Billing) is appropriate and was continuing efforts made me over telephone to Grace and Kimberly during the previous week. Like I said, you don't have to treat me if you don't want to, just don't fabricate some "disruptive and inappropriate behavior" reason putting such a letter in my medical files/reports! Is Dr Maksimov even aware of these allegations and this discharge from care? Who is CPSI and BSC Management Team? WHO and HOW have I disrupted? I was seen in clinic without issue, given my prescriptions as per usual, and followed with a UA screening. The course of Business was not Disrupted in any regards! Dr Masimov has provided me with epidural injections of steroids in the Burnside Clinic using excellent technique and followup for the procedure is second to none. I choose to receive no sedation as the injections are not particularly painful to me. The clinic has always been clean and appropriately staffed. The only issue i have is the wait time between the order and the injection. I would like to see a new care plan addressing steroid injections as emergent care. I've had nothing but great experiences there. I did not want to be medication dependent for pain control and they've helped me find alternatives like spinal nerve stimulator, nerve ablation, and more. I haven't wanted or needed pain meds in years now. Unfortunately, my pain doctor retired, and his partner has been less than helpful when I called about a flare up of pain, instead referring me to my PCP. I'm not happy at being ignored. read more

Be forewarned that if you come to this business stricken by excruciating chronic pain with a history of zero relief by steroidal injections, tens units and various implanted gizmos touted to bring much pain relief- those options reign supreme by these pain management providers. However, should you be looking for pain relief by way of a prescribed narcotic medication Columbia is not going to be of any support. The healthcare's knee-jerk response to prescription narcotic due to the opioid epidemic has left the legitimate patient in a lurch. The opioid issue stems from the drug trade flooding the US. The addiction rates leading to prolonged controlled substance use is less than 1% of the total population. Without consideration being paid to the matter of your quality of life, the ridged prohibition of prescribed narcotic medication by physicians throughout metro Portland is, frankly sadistic. Take Mcnaulty in Sellwood. What a sadistic a-hole this so-called doctor truly is. Catastrophic spinal injuries are legitimate reasons for prescribed narcotic. Most of us do not possess the addictive genetic trait and handle our prescriptions responsibly. Can't tell that to the all-knowing medical practitioners here in Portland. Fentanyl & Methadone addicts are created by irresponsible healthcare snobs. Very unfortunately too many of these people have no idea what living with chronic pain & disability is like and instead rely on books, white papers, inflated statistics, and their hysteria about it all instead of using common sense. Don't go here for a second opinion even if you have MRI, Xray's with concerns. MVA injured multiple points of impact of course because I hit the dash some and in pain still. I communicated very clear with questions of how to maneuver with the barriers of OHP as my secondary. I asked what can I do with the 1/3 hip labrum tear. I asked what can be done about the ACL minor tear. Added concern issue of the minor petal foot sprain/break residual issue. I wanted to know what options for the muscles that continue to "feel on fire" as bee stings. My lip hit the steering wheel and of course whiplashed. I WAS TOLD TO GET A PERSONAL TRAINER! What a waste of time! Absolutely "felt" unheard, unseen, belittled, and unproductive waste of time. Was offered 1 treatment after I'd get femur MRI. Umm why about the rest. I must conclude that there wasn't enough substantial injuries or money issues to proceed with this clinic. I also waited a year to see them after front office demanded I get a diagnoses first. PCP won't allow MRI-Xrays until after the "wait" game of "ice & heat"! Then the massive amounts of referrals each needing another visit for each area of body impacted. thanks for zero help imo. I don't know where to start. This is the worst place to go for your pain care. I was referred to this clinic for my hip pain by my doctor. I tried to make an appointment and I could not get hold of anyone by phone. I left messages and no one returned my message for over a month. I was in terrible pain, and I was desperate to see someone. Finally, I drove up to the clinic and talked to someone to make an appointment. I was given appointment that was 2 months away while I was in pain. I was given ejection on my second appointment that did not work. This was a total disappointment. A month later, they billed me for a no-show appointment that I was not told that I have. Avoid this place and their terrible customer service. read more

This morning Monday 1.3.22 at 9:45am. This clinic f---ked up my start time. They had it set for 10:15am. I have to be transported to my appointments by Trimet Lift buses. My ride back to work from the clinic was scheduled for 10:30am. The appointment was supposed to take 30 minutes (9:45 - 10:15) would have left me enough time with a cushion. They did not even take mee back to the exam room until 10:05am, at that time it was to late to complete the appointment. If I did not show up to the appointment I would have had to pay for the meeting. That being the case I think they should have to pay for my work time loss and my travel and the frustration of the whole unprofessional way they treated the whole situation. I do not recommend this clinic. If they f---ked up the appointment time what else are they going to f--k up, my back by making it worse. I have started a complaint with Adventist and with the Oregon medical board. If you do not want to waste your time and risk further pain do not use these doctors or the clinic. I have been on pain management for over 10 years my clinic just shut down so I need to find a new place to go. This guy dismissed me the minute I walked in. They would not let my husband in with me who is supposed to be with me at all times. I have fibromyalgia , MS, Lipadema, osteoporosis in my lower back and neck. Also Panino. This establishment is hands down the worst medical facility I've ever been to. The doctor I saw previously was a know-it-all jerk who did an unnecessary spinal injection on me. I almost lost my job due to Dr. Garmin's lack of courtesy. Most recently I saw Dr. Maksimov. I was really hoping that it would be a positive experience, but it was far from it. Firstly, Dr. Maksimov gives off old world vibes. What I mean is that I felt like I got sub-par care because I am a woman with a chronic pain disorder. He barely spoke to me, poked at my foot for about 30 seconds, and sent me on my way with a prescription I can't even get filled at my pharmacy. They've been trying to contact him for about a week to get a different prescription, but it's no surprise that Dr. Maksimov hasn't gotten back to them. She received good service from the doctors but we have had a terrible experience with billing. We have call multiple times with no return calls, the sent us to collections after saying everything was billed through insurance and we did not have a balance. Several weeks later, ultimately accomplished procedure. Doctor was wonderful (as usual), staff I dealt with during this time were wonderful, and pleased with follow-up call day after procedure. My initial issue was I learned, with one individual there, and not the Institute. Am returning for more help from them and now can recommend them highly - in spite of the one staffer. Until yesterday's scheduled procedure (1/31/18) that is. Was informed after fasting and getting there at 8:15am by their anesthesiologist, that she was refusing to perform the procedure because "it was too difficult waking me up the last time." This was either my 4th or 5th procedure by them - there was nothing new going on with me - I still needed diazepam because of PTSD - and was denied service. I tried submitting verbal complaint so whoever's in charge would be made aware of how one staffer affected their business, but my calls have not been responded to. As my first review revealed the difficulty I had in getting my calls returned, so it continues. That particular staffer is either always out 'running errands' or just takes her time returning phone calls perioid. Left three messages over several days while awaiting a call-back. Pain management is a difficult balancing act, you have to discern between the seekers and the people that truly need help, on top of that you have to decide what is the best way to help them. It's a difficult field because you can't measure pain, you have to go by feelings, we're all human and make mistakes. They helped me when nobody else would. Yes, they could have done better, but I believe they did the best with what they had. They did more for me than many other doctors did. Remember, nobody can take away your pain except the good Lord himself, best you can hope for, is a couple of point drop... One thing I gotta tell you guys, is Dr. Maksimov is amazing at scheduling, If you come on time you will never have to wait for more than five minutes, he knows how to read a clock, I really like that about him. There is nothing worse than sitting in the waiting room in pain. Yes, if you're late, you may get rescheduled, but that's an awesome price to pay, it's totally worth it. These people work hard at accomodating my needs to the best of their ability. I will never go anywhere else nor would I reccomend anywhere else if someone were to ask me. Unfortunately I believed them... there was no numbing of any kind, injections into my damaged tailbone was the worst pain ever! I rarely cry and I was balling. Then I had to sit on it while driving myself home and crying. Not only was it traumatic but it didn't even help! When I returned and told them I'm still in excruciating pain and the injections didn't help they were very patronizing, condescending and refused to give me any medications, saying all I need is physical therapy! Fyi i did PT for 6 months with zero improvement. TO MAKE THINGS EVEN WORSE the injections caused psoriasis that my dermatologist is still treating to this day 10 years later! I didn't even know that was a possible side effect! PLEASE DO NOT GO TO THIS CLINIC! YOU WILL REGRET IT! IT WAS A VERY TRAUMATIC EXPERIENCE WITH LONG LASTING SIDE EFFECTS! We were referred to this clinic after my son was released from the hospital with a very rare and extremely painful complication of diabetes. The hospital had him on a regimen of pain control meds until it would resolve within 4 months. Jordan Garman, board "eligible " DO saw us. He was rude and condescending. He, by his own admission had never treated this condition. He would not refill a prescription prescribed by the hospitalist on my son's discharge. He gave him a medication with did not work. As a result my son was taken by ambulance and readmitted to the hospital where he was treated with his medication that worked. Upon release with a few days prescription we returned to Garman who refused to refill this but rather added another med which sometimes works for pain. He totally blew off the hospitalist saying he didn't agree. He also challenged my son's renal doctor about the dosage of the non kidney friendly drug he was prescribing. I called Garman's office telling them Garman s pain regimen was not working. Their answer was "take him back to the ER if you are having a medical crisis" but his medical assistant would text him and get back.... still waiting for that to happen. This man is arrogant, rude and condescending. He should not be practicing this field of medicine. We are filing a formal complaint with the State Board and will be looking for a doctor who cares to listen and share advice with the other professionals who have been involved in my son's care. And shame on my son's primary care physician for referring us here! read more

Possibly one of the worst medical experiences of our lives! I care for my elderly mother (81 years) and other than the initial visit almost a year ago, she has been treated like a 17 year old junkie. She has been on a maintenance dose of opiates for several years and it was a trial everytime we went to get a refill. We were always treated by PA Barb Troxel with mistrust and other than 1 or 2 staff members, the whole experience was always awful. Today she had a false positive test for a common street drug and we were cut off. Pretty awfyl thing to do to someone of that age. If you can go somewhere else, do it. read more

The Dr. I saw at Columbia Pain and Spine was very good. However, the office staff...particularly the billing staff made my life miserable. I only get paid once a month ,so depending on when my appt. fell during the month my co-pay had to be held for anywhere from a few days to a couple weeks. It was, however, pd. every single month before my next visit, so at no time was there ever any balance. I spoke to the office manager upon first going there as a patient. She assured me they would work with me, understood my special circumstances, told me not to stress about it at all. Yet each and every week, these same billing people treated me like it was the first time they'd ever seen me and put me through a rigamarole every month.It got to where I hated going to my appt. I spoke to the Doc about it, however he chose to stay out of it. I eventually wound up being discharged from the practice because I "couldn't get along with his staff"! True Story. read more

Inconsitant Pain Contract, the Contract says one thing and the Medical Assistanst say another. They don't review each patients specific situation. Dr Maksimov is pleasant and thorough. But after a Dental Procedure, I needed additional Meds and I was treated like a lying Drug seeker. After following my pain contract I was still denied 6 extra pills due to Severe Dental Pain. They could careless that I was in so much pain, and they didn't care that I would have to go without my pain Meds for several days. I've been in pain management for 10 years and I have never had a problem until now. So, if you don't mind being treated like a Criminal or being talked to like a Drug Seeker by the Medical Assistants, this Clinic is for you. Also, the Medical Assistants never answer their phone so you have to leave a message and wait for them to call back. read more

They have severe scheduling issues. My first appointment was in May and Dr. Grattan wanted to try an ablation procedure to help my back pain. They couldn't get me on the schedule for 6 weeks. Finally had it done and it didn't help at all. Another long wait for a follow-up appointment. When I told the doctor that the ablation didn't help, he told me to get another MRI and let him know when I did so he could keep an eye out for the report. Had it done the next day and called to let him know. The person on the phone said, "No, you have to have an appointment. The first available was waaaay far out. I told her that it wasn't okay and I needed to get in much sooner. She magically found an opening earlier, but it was still far out. At that appointment the doctor said, "Let's try cortisone shots." Again, 6 weeks before they could get me in. Finally had the shots and, again, no relief. I had previously made an appointment for a follow-up the week after the shots. And, guess what? They cancelled that appointment! Here it is almost 3 weeks since that appointment was supposed to happen and they just called to reschedule it. Dr. Grattan was nice enough, but he didn't solve my problem. I started there in May and finished in November. It took 6 months to get through their process, all the while in pain, and no relief. The only plus I could give this place is the staff in the surgery center is excellent. They were gentle, caring and professional. read more

As someone with chronic pain time is critical and them wasting 3 weeks of mine put me over edge because I had to start from scratch. read more

Horrible people with no concern for patients rights or feelings. the nicest person there was the recptionist! I told them I was terribly depressed and experiencing anxiety that left me non- functional because of the medications they were using and then told me to stop taking. medication that was causing very distresful GI issues (which they insisted were not a side effect). No apology for my experience or distress and no phone call to correct the problem. I would like to have been treated like a person needing help rather than just a problem they didn't want to deal with. read more

I have been going to CPSI for a number of years. Dr. Maksimov is an excellent physician and most professional. He is helpful to explain why I am experiencing tremendous pain and even provides wonderful diagrams of the areas causing the inflammation and pain. I appreciate that he immediately gives suggestions of what steps can be taken to lessen the pain, too. Therefore, I have increased my exercises and improved my eating habits as well as continuing to receiving procedures by Dr. Maksimov at the lovely new office on NW Burnside Road in Gresham. His procedure schedulers are most efficient and helpful. They set up times as soon as possible for treatments and follow-up appointments. It is wonderful to have a physician and staff who are so kind and courteous. The personnel are aware that their patients are experiencing intense pain and are kind, informative and professional always. I am thankful to be a patient with Dr. Maksimov at Columbia Pain and Spine Institute. read more

Dr. Oleg Maksimov is the primary doctor in this clinic that specializes in treating acute and chronic pain and many of the modes of pain diagnosis and treatment. While he is a fine and knowledgeable doctor, his clinic has some personnel issues that make it an uncomfortable and at times difficult place to be a patient at. It is unfortunate that as an owner Dr. Maksimov has a few bad apples that make the patient experience less tolerable, particularly when you are in pain. A few specific examples are waiting times for appointments in the lobby and a few cold and callous employees, one being a mid 50's moody and abrasive receptionist. When you meet her, you will know what I mean. It is just a sign that if it's acceptable for the face of the clinic (reception) to be cold, distant, abrasive, and negatively biased, it likely can be found elsewhere in the clinic, like with whoever hired her. Possibly the office manager...don't know. Fortunately there are two receptionists so it is possible to choose to do business with the nice and professional one, I think named Christy. (late 30's- early 40's). While i haven't left this practice as a patient, if things don't improve soon, I will find treatment elsewhere where I won't have to deal with difficult people as part of resolving and treating my physical pain.
