One of their residents was discharging w/ home health services (provided by my agency), so I wanted to meet the pt while they were still at the facility and help ease them into the transition back home. From the moment I stepped through their doors, I was greeted by a warm and compassionate staff that truly cared about the well-being of their residents.I was definitely impressed; the facility has a clean and inviting environment, creating a welcoming atmosphere that promotes healing and recovery. The staff members, including the nurses, caregivers, and discharge planner, were consistently attentive, professional, and empathetic. Their dedication to providing personalized care was truly outstanding. One aspect that truly impressed me was the level of expertise and skill demonstrated by the healthcare professionals. From managing medications and administering treatments to assisting with daily activities, they displayed a high level of competency and professionalism. Their commitment to staying up-to-date with the latest medical advancements ensured that our patient received the best possible care. I must also commend the facility's commitment to communication and family involvement. The discharge planner consistently keeps families informed about progress, treatment plans, and any changes in care. They consistently encourage family participation and provide valuable support. I worked directly with their Social Services team consisting of: Kate, Cathy, Bea and Andrew. They were friendly, punctual, knowledgeable and easy to work w/ from start to finish. They exceeded all my expectations and our patient loved them as well. To be honest they're all still in touch to this day. Overall, The Avenues Transitional Care Center exceeded my expectations in every aspect. Their dedication to personalized care, expertise of the staff, engaging activities, and commitment to communication truly set them apart. Their care for very old and very sick people, some left unresponsive after strokes was full of humanity. I spent at the Lawton Center 2 weeks and all my requests were met with such willingness and smile that I wanted to ask for something else just to see this expression of kindness and benevolence. I would easier trust my own mother to them than to myself, because they were really SKILLED. I asked my friend working in the medical field how such good work of personnel could be achieved? She answered that it could be only the result of responsible supervision of the attentive bosses. I met Lawton Center's Administrator Dan Daly and Director of Nursing Natasha Daragan, who WERE responsible for their crew's coordination and hard work. They themselves are watching everything like hawks and it shows. I appreciated the work of physical therapists who helped patients as a team and individually. The case worker Rudy also was very helpful in complicated and nerve-wrecking dealings with insurance. Absolutely fantastic was CNA Sheryl, who was not only superbly professional, but personally very kind. The society which cares so much for sick and elderly is a pinnacle of civilization.     FIRST HAND ADVICE: Lawton is a terrible facility. When I was here for 3 months, a CNA physically…read moreabused me several times, I complained about her a few times, later she was removed from working with me. My clothes were stolen from the closet and they had to reimburse me. The food is terrible and often, patients don't eat it l, lots of wasted food. . They are very short staffed on the weekends and night shifts. Many times, I pushed the button at night time for a CNA Nurse Assistant or Nurse and had to wait up to 30-45 minutes. The Nurses that I had were very good, and only a few CNA's Overall I don't recommend Lawton Skilled Nursing and Rehabilitation. Many people avoid Lawton, because they hear of the bad service that's provided bI don't know how they received good ratings.

Providence is ideally suited for late-state dementia cases. Its caregivers are experienced, caring, and tuned in to the unique challenges that come with severe dementia. I especially like Providence Place's pricing model in that they don't gouge you with added costs for every little thing; their pricing is clear upfront. Fresh meals are provided daily, music and light exercise therapy are woven into the weekly routine, and opportunities given to residents to enjoy the fresh air outside. It's never easy seeing the irreversible march of this horrible disease on loved ones. But knowing my mom and the other residents at Providence are getting great care, is so comforting!

    Providence Place is a loving place with excellent care…read more We are so glad we were able to move my husband to Providence Place! He has been there a year. Previously, he spent 2 years at Ivy Park at Cathedral Hill. They are very different. Providence Place is a small facility that focuses on excellent care, among other important things. Ivy Park, by contrast, is corporate, and spends money on marketing and open houses. Providence Place (PP) spends money on the residents -- on excellent and loving care, tasty food, and staff who are loyal and remain with PP for years. Staff turnover at PP is very low! At Ivy Park, however, staff turnover was frequent! We never knew who would be my husband's caregiver that day. Consistency of care is important in noticing subtle, but important, changes in residents. This can affect their condition. My husband's main caregivers at PP, Pia and Saul, are outstanding - loving, respectful, caring, patient, and kind. Catherine, the manager, is on top of everything, knows each resident and is very responsive. Often, she sends photos of my husband doing something that day. Twice a week a guitarist comes and plays live music, and an activity director comes 5 times a week to do activities with the residents, such as "bowling," balloon tossing, and coloring. In addition, the residents are bathed every day! This is amazing to me, especially because my husband is wheelchair-bound, but as Catherine says, "The residents feel better this way." It is true. Also, their teeth are brushed! This was not done at Ivy Park and there they were bathed only 2-3 times a week. My husband is a vegetarian and that is no problem at PP. The food is high-quality, tasty and varied. At Ivy Park, it was so difficult for him to receive adequate nutrition; the staff there took away the main course (non-veg) and gave him only the vegetables - no protein! My husband declined. He also became very dehydrated. At PP, however, he is stable and doing well. We are so glad that he is at PP where we feel like we don't have to oversee things. We trust the staff to do their jobs. It is a big relief!

My sister was in an assisted nursing care facility for 6 months, and the thought of her continuing to reside in a similar environment was heartbreaking. While there, the numerous and almost monthly visits to the ER due to UTIs, inadequate doctor and care oversite, lack of consistent and timely drug and therapy services was truly unacceptable for her health. My sister is now living with my family in our home, and CrossRoads has allowed us to find an alternative path to her care along with using other support teams. From CrossRoads, we received scheduled physical, occupational and nursing services. Their exceptional services have allowed my sister to remain in a home environment and us to build a secure loving environment for her long-term care. I have used other home care services, and would consistently request for CrossRoads services. Her medical needs are addressed by a caring, effective, reliable, proactive and incredible staff at CrossRoads. I could not have asked for a better team to help with this unprecedented emotional and physical challenge my family has undertaken. Our physical therapist Jason, engages, motivates and physically prepares my sister for mobility and flexibility and encourages her to maintain and maximize her existing mobility. My sister enjoys their conversations. He is responsive, consistent and timely. We could not have asked for a better partner in providing the physical and emotional support my sister and my family require. He is simply the best and anyone would be lucky to have him on their team. I worked with Shanel, nurse and case manager, for both my mother's and sister's home care. She is incredibly responsive and experienced in addressing the medical needs that arise. She is proactive in recommending solutions and next steps, all of which provides assurance that as medical needs arise, they will be addressed immediately. Shanel de-escalates the stress and uncertainty surrounding medical situations. Shanel provides the confidence that home care is a viable and very workable situation in partnering with my sister's medical team. It's very obvious that Shanel enjoys her work and patients, and truly focuses on the customers' needs. When my sister arrived at our home, Sabrina, Occupational Therapist, immediately identified tools, equipment and recommendations that provided additional comfort to my sister and to our family and support caregivers in mitigating our physical demands. She was incredibly efficient and facilitated accommodating our needs immediately with her experience and resources. I feel our sister received the best care from Sabrina and CrossRoads in tailoring our home as effectively as possible in maximizing my sister's care. Equally important as the effective direct skills of the CrossRoads staff, is the intangible support they provide my sister and to us. We don't feel alone in caregiving, many times a heartbreaking process. We feel there is an incredible team available to support the care of my sister. My sister is engaged and enjoys interacting with the entire staff providing for good moments.

We've had excellent care from Caroline Noel, Physical Therapist, and Serge LaFuente, RN. Both visited regularly and were super sensitive to my husband's needs despite the fact that he is a "difficult" patient. The nurse in particular has devoted extra time to us, and seems determined to see evidence of healing. Our physical therapist, Ms Noel, seemed to have a special understanding of my husband's situation that helped reduce his pain whenever she visited. I also appreciated Kyle Yenko 's friendliness and care for our situation.

    I've had chronic lower back pain and discomforts for many years. When I stand, my lower back…read morestiffens and it was preventing me from standing completely straight. I was always leaning forward a little. When I lay down on my back, I automatically curl my knee up and stretch it to the other side of my body for relief. Standing for 15 minutes begin to feel like all the upper body weight was crushing my lower back. I don't want to live the rest of my life like this. I went to see Dr. Anthony Chau at City Spine Center. He took x-rays and evaluated me fully. The x-rays showed two compressed discs. He recommended me to set aside M-W-F for two months to treat each disc separately by using a spinal decompression machine to safely stretch my spine at a specific angle to allow nutrients and fluids into the disc to relieve some of the compression. My first treatment was instant gratification. The stretching gave me relief and it felt great! Dr. Chau follows up the treatment with neck and full spine adjustments every week which compliments the decompression from the machine. I felt better and better after each visit with these two combined treatments. When I was done with my second disc treatment, I felt 85% better. I was very happy with the outcome. That means 85% less pain. Although the outcome wasn't 100%, it was a huge improvement. Then around one month later, I noticed my back was completely freed! No more discomfort whatsoever. All straightened. Like new. I think it took a few weeks for my spine to recalibrate from all the regular treatments. Now the treatment had stopped, the spine took some time to settle. This is just my theory. How else can I explain from having chronic pain for a long time to no more pain and discomfort after my treatments. It has been nearly three years since I had my treatments as of this writing and my back is still like new. If you have a back problem, see a chiropractor right away. This is their specialty. Although you don't know the outcome, but it's worth trying. Like any medicine and treatments, they don't always work for everybody, but it's worth trying. Otherwise you remain in pain and you will never know unless you try. The question is which chiropractor do you see? For that, recommendations are always the best. Since I have a long term fix, I am very happy to recommend Dr. Anthony Chau. Good luck to you on your way to heal.

The views up here are amazing and you can see the ocean and beach in many places. Truly a beautiful sight. I was able to park in one of the parking structures and wow no fee. Totally surprised about that. They even had valet parking and you can't tip them because this is Federal and they would get in trouble. If I knew that before i would of used that too. Hhahah. This is a federal hospital and there are no weapons allowed in the hospital. However I didn't see any check in or metal detectors. It was wide open so I guess its on the honor way. I did see several Police vehicles near the Emergency room area so very safe. As I said it was difficult finding which building to go to. But everyone here are so nice. We were standing in front of a map and a passerby helped us out to where to go. I assume he worked there since he had an ID on. That was the start of such great customer service. Everyone here are so nice and helpful, friendly. The building I though were going to be old and run down but It looks so clean and probably remodeled. My brother was here for an operation. The staff were so friendly and took really great care of him. I was so impressed. This hospital is really great. The operation was a complete success. Thanks to all the doctors who helped and the nursing staff. Amazing

I don't think there's anywhere in the San Francisco veterans Hospital or doctors offices that I wouldn't recommend because everyone there treats each other like family and they treat people coming in like family. I've never have any anticipation or reservations about walking through their doors and any procedures. These are my VAngels.

I was turned off by that immediately. When we contacted Amanda I immediately felt comfortable and reassured that she was the best fit for us. There was no pressure or immediate schedule or routine because she really didn't know what he needed until she met him. She has a way with our son that not only makes him comfortable but makes his learning and therapy fun. Thanks to her he has come a long way in a short amount of time. She really cares about him and his progress and provides play activities that will best enhance his therapy sessions. Lj always looks forward to seeing her and playing with her, she really takes the time to get to know your child and helps establish a set goal. This is why we believe he is doing so well, she is really great and we highly recommend her!!!
