It should have been a new patient exam. Possible a debriment since it had been 5 years and see how the patient responds. I was in the dental managment industry for over 25 years, and this kind of crap is why I left. Not to mention there was no fee/copay disclosure before starting treatment. I will be putting in a formal complaint with Delta Dental. I did call and was told they did everything correct.
