He's very attentive n shows ya what you're taking n why. He doesn't miss a stroke n I take lots of meds n he made SURE I had them all n the nebulizer stuff. I'll miss him. Thank you for a GOOD stay.     Woodcrest Acute Rehabilitation Facility is easily accessed off the 91 Freeway by taking the Adams…read more(Auto Center Dr) exit and head North on Adams about 1/4 mile to Magnolia Ave. make a right to the first available u-turn location, make a u-turn now heading West on Magnolia Ave. travel about .1 mile to the facility on the right There is parking on Magnolia and also in the back of he facility which also has several handi-cap stalls. My wife was in need of a rehabilitation facility and we were sent by our insurance (Kaiser) to Woodcrest. I called over to the facility to obtain the information on my wife's whereabouts and talked to Leigh Ann the Director of Marketing who one of the sweetest directors I have ever met. She explained everything to me about my wife's check in and made sure all my questions were answered. So for the visit, I have visited family many a times in rehabilitation facilities in my lifetime so being aware of how they can be, I was prepping myself. On the outside the facility looks average with a little wear. When I first entered the lobby in the rear I was immediately hit with a very, very pleasant odor which shocked me as I had prepped for lets say a different odor! The lobby was very inviting and pleasant to the eye with a balloon tower, plants, nice seating area and lighting. There was a reception desk which I discovered later is occupied by a staff person however, due to my arrival being later in the evening around 6pm there was no one there so I sat thinking someone would come out. I witnessed several other folks entering and signing in and going on in so I figured out it was self registration, a nice help would be to have a sign at the desk stating to sign in. As I made my way to my wife's room the hallway was very clean and uncluttered. Again the odor was very pleasant and the wooden floors were that like a basketball court, with my sneakers screeching as I walked, as I later met Jose who is very proud of his work and polishes those floors to an immaculate shine! One way I can determine if a business is true to cleanliness is the customer bathrooms! When I entered the coded door restroom, WOW I thought I was at a high end hotel! It was immaculate, very well decorated and super clean. So on to the rooms. There was 3 beds in my wife's room with privacy curtains around all 3, Her's was the first bed. Everything and I mean everything, was clean, she had a nice TV that is on an arm to maneuver around as needed for comfort. She had a nice 2 drawer nightstand and a double wide closet. There were folding chairs for visitors to sit on, room for her wheelchair and a rolling tray table. I first met Beatrice the nurses assistant who was a super sweet person and she asked if we needed anything and to let her know if we did. Now I was expecting this somewhat for the first time but as time went on I soon discovered that this positive and pleasant attitude is the norm for all their staff! I have interfaced with 3 of my wife's nurses Diamond who is truly a gem, Mary who is the sweetest ever and awesome Daniel! They have always supplied me any and all information I am in need of like her medical treatments, medications and health statuses. They have relayed all of my medical requests to her onsite doctor for determination. My wife will tell you the same story every one is very positive and pleasant, "ALL THE TIME!" All the staff's response time is very quick which truly amazes me! The food is great however it is a set menu for all. I have sampled the food several times myself, even as my wife is on a Renal diet. She will sometimes tell you otherwise because she really wants to come home however she can't until she is totally rehabilitated. I am allowed to bring in food and snacks for her that are meeting her diet needs. Sometimes the meals come in cold due to all the delivers they have to make but here again they will reheat her food up for her every time she needs it, if she asks. There are a lot of very cool activities throughout the month which she/we have participated in, so it keeps the patients busy if they so choose and if they need assistance to get into a wheelchair and be taken to the activity room, here again the help is immediate and willing. There is an outside patio area for visiting and a spot for a patient to take in some warm sun rays. For taking your loved one to a doctors appointment or just out to visit or out to eat, the check out/in process is very easy with previous communications to the nurse or case manager. Now I know you might be a little reluctant to believe all the above, it does sound too good to be true. However after 4 weeks there we have not experienced any deviations to what I have written. So if you want proof drop by and see for yourself, you will not be disappointed and your loved one will thank you for such a nice place to rehabilitate at.

    My grandmother was admitted to Windsor Cypress Gardens after a fall at home. The goal was for her…read moreto receive physical therapy to regain strength in her legs and become more independent. Unfortunately, our family's experience was heartbreaking, and I would never recommend this facility to someone I love. From the beginning, it felt like physical therapy was not the primary focus. We also became concerned that she was not consistently being assisted with basic hygiene, toileting, or changing when needed. While she was staying there, she developed a urinary tract infection (UTI). Because of her age, the infection caused severe confusion and hallucinations. Her condition became so serious that she had to be transferred to the hospital. After spending several days in the hospital and receiving appropriate treatment, her hallucinations stopped, and she improved significantly. When she returned to the facility to continue physical therapy, things unfortunately got worse. She developed bed sores, and she also had a foot infection that continued to worsen instead of improving. Our family was deeply concerned about the care she was receiving and whether her treatment plan was being properly managed. One of the biggest issues we experienced was the lack of available staff. My father and I visited multiple times, and on more than one occasion it was nearly impossible to find a nurse. One day, my father walked through the facility twice looking for someone to help, and the only employees he was able to find were members of the custodial staff. They were kind and helpful, but they should not have been the only people we could locate when our grandmother needed medical assistance. The experience that will stay with me forever happened during one of our morning visits shortly after breakfast. We were told that my grandmother had "just woken up," but when we entered her room, she was completely soaked in her own urine and other bodily fluids. She desperately needed to be cleaned and changed. My father and I did everything we could to help her ourselves, but we needed clean briefs and other supplies from the staff. We pressed the call button and waited. We continued waiting. It took approximately 45 minutes before we were finally able to get someone's attention. During that entire time, my grandmother remained in that condition while we tried our best to care for her ourselves. No family should have to watch their loved one go through that. Every patient deserves to be treated with dignity, respect, and timely care. I want to acknowledge that I'm sure there are individual nurses and staff members at this facility who genuinely care and work hard. However, based on our experience, the overall quality of care, responsiveness, and staffing left us feeling that our grandmother was not receiving the attention she needed. We ultimately made the decision to remove her from this facility because we no longer felt she was safe there. I would not trust Windsor Cypress Gardens with the care of my grandparents again, and I hope this review encourages the facility to make meaningful improvements so that no other family has to experience what ours did.

The staff is attentive, compassionate, and always willing to help with any need, big or small. The physical therapy team has been incredible. Receiving therapy twice a day has helped him grow stronger each day, and the progress he has made has exceeded our expectations. We are seeing improvements physically, emotionally, and mentally. Most importantly, we are seeing his spirit return. He is smiling more, showing determination, and demonstrating the heart of a true survivor. The encouragement and support from the entire staff have played a significant role in his recovery journey. Thank you to everyone for your dedication, kindness, and genuine care. We appreciate all that you do and are so thankful to see him thriving and moving forward every day.

Helpful well trained quality nurses ( Carolina one of the best I've ever had ) . Almost instant service after pressing the call light. Almost no requests denied. Almost like staying in a resort. But BEWARE of the next shift. The night shift ( 3pm till 1 am? ) horrible like a nightmare. Make sure you get everything you need from first shift ( food,water, meds, changed, etc ) because myself and many others could receive zero help of any kind from this shift. On many occasions I called for water or blankets and was completely ignored the entire shift. I even got stuck not being able to get off of a very low toilet for 4 hours with no one coming to help me even though I pulled the cord over and over. One late night frustrated and super thirsty, I got in my wheelchair and rolled around to find all the nurses out in the patio area all hitting their vapes and thc pens hard. Even the charge nurse at night would not wear uniform came in my room in camouflage clothing I couldn't even be sure he worked here. There was also a very young untrained nurse who let my catheter bag overfill and burst leaking urine all over the floor she put a towel on the floor demanding " This no problem, day towel soak up all de fluid ". Needless to say she refused to get anyone in charge to fix this I actually had to call 911 before anything professionally got done to rectify this situation. So just make ABSOLUTELY sure you get all of your needs from 1st morning shift, try to get through the night by yourself the best you can and you should be fine. I'm giving a 10 for morning shift, but a 1 for night shift thus 3 star review. Thanks for reading.

My call button didn't work. They said to call front desk if I needed to get up When I called the mailbox wasn't set up yet so NO assistance at all. I asked for a pillow for my head and when they brought one it was dirty. It had a tire track across it. When I left they refused to discharge me with my pain meds. They kept them! It was a horrible experience.

And, I don't mean covered over with some other fragrance, I mean fresh! They keep this place spotless! My first contact was with Charlie. He handled the writing of the contract. Charlie was so calm handling our business. He was like talking to a good friend. Next, we met Michelle, their head nurse. She was like talking to a sister. Michelle did the initial assessment for my mother's level of care and she was spot on. Michelle was so genuinely caring. We met the director, Megan, she was like an Auntie. The caregivers like Rosie and Taye, absolute angels to my mother. Rosie in particular cared for my mother so well. She was the only person that my mother let bath her! The Med Tech Rita, VERY experienced and super friendly and caring and so many others treated us like just part of their big family, I really mean that! I knew, this was the place for my 94 year old mother to enjoy the final years of her life. Every employee that serves here has a heart for the residents. They behave like they want to be here. The receptionists at the front desk and the servers in the dinning area, everyone is a part of this close nit family of caregivers. If you are looking for a place where you can truly trust that your loved one will be cared for as you would in your home, Citrus Place has the caring people that will do just that!
