This place does not have a resident dining room, placed my mom with 2 other ladies in one room where her walker can't fit to get to the bathroom, and when asked to show the scheduled activities for the day pointed to a tv screen with weather. This place is great for what it is, staff is awesome, but taking off a star because you will lose some of your loved ones clothing in the laundry, regardless of whether or not you said you'll do your own laundry! Both times she was there she lost nice nightgowns, robes, a dress & leggings. Once I asked for reimbursement & it took two months but I did get paid, the second time I didn't bother. If she goes back in the future, I will only take items that I don't care if they get lost that are old. read more

I was sent here by Riverside community Hospital after a recent stroke. They told me it was the highest rated rehabilitation center in Riverside County. The place actually has beautiful grounds as it's situated on a hill having 180° views of the surrounding area, including Mount Rubidoux. The grounds are quiet and peaceful the inside of the building not so much at 4 o'clock in the morning, and staff starts to arrive. They are loud and sometimes obnoxious. Yelling at each other from down the hall. I think they used to call these places rest homes, but I rarely got real rest. If the staff weren't talking loudly than other patients were screaming "help me help me." in some cases they were simply dementia. Patients acting out in other cases. People really didn't need help and we're intended to in a timely manner. It was hard to tell. I have to say that most of the nurses and workers seem to be lovely, caring people however there were major issues, not least of which was the time it took for the nurses to respond when using the the bedside call button which I tried to use sparingly because I'm fully aware of how busy they can get. I saw others calling the nurses for minor issues like asking for a fresh pillow to replace a urinal, etc. once I needed help cause I had to go to the restroom urgently, and after they didn't respond for 30 minutes while I tried to hold everything in, I managed to get in the wheelchair, but I didn't make it. The restroom was 10 feet away and I ended up soiling myself about halfway there so I asked my roommates to use their call buttons no nurses showed up for another 45 minutes while I sat in my own SH*T. I rode myself over to the door to the hallway and tried to get attention. Another nurse came into the room to attend to my roommate for something routine. I asked her for help and was told she wasn't assigned to me that my nurse must be on break or at lunch so I continued to wait because I wasn't physically able to take care of myself. I ended up. I decided to roll myself out in the wheelchair into the lobby and I parked right next to the receptionist at the main entrance to the building and believe me she must've smelled me then after only a few minutes, someone came to wheel me back to my room and clean me up. Similar incidents happened to both my roommates while I was there that was disgusting. The patient is helpless in that situation and has to completely depend on Assistance. I love the staff and the activities director. They made my stay worth. They washed my clothing etc.The physical therapy works with your mind & body. they took excellent care of me there, and they truly truly care about you as an individual and the people as a group. Love this place I was blessed to be able to sing from day one and they have me singing there even after I've been discharged from the rehab physical therapy. This is an amazing place. My mom came to this facility after having her foot amputated due to her diabetes. First of all, mom came from Romania when all this happened and she speaks no English. I was concern about the language barrie. From the get go everyone was super nice and welcoming. I don't even want to mention names because I am afraid I will miss some But from the administration, social worker, director of nurses,RN, LVN, all nurses, CNAs, front desk, appointment scheduler, activities directors, physical therapist, wound care, food administration, housekeeping , everyone treated mom with the utmost care, kindness and made her feel like part of a big family. The place is very clean, there are tons of activities, food is designed for every patient's need and doctor recommendations. Mom was there for over three month and she came home today with her foot healed thanks to them, and also with lots of tears from all her good buys. Mom was treated as if she was their mother. She did not missed anything and she was treated with love, respect and compassion regardless of the language barrier. She will be missing this place dearly. I would give everything 10 stars if it would be possible, that's how happy we are we had found this place. Thank you very much everyone, for making mom's pain and suffering so much easier with your kindness and compassion, love and support every single day for her entire stay. They made sure to guide us in the right direction once discharged and Stephanie coordinated everything to the last detail. I am so much on ease cause she thought of everything before hand. My Aunt was admitted to Palm Terrace C.C. For Rehabilitation Services and the rehab department were very compassionate and understanding and had patience with my aunt . She received excellent care starting from the rehab department and the CNA and LVN and housekeeping staff. When it came to discharging back home her Social Worker Stephanie helped us find a caregiver agency to help her out with doing some errands and provide companionship while I was at work. The Administrator Ryan Leet was very supportive from the beginning to the end of her stay there and made us feel welcomed and we are grateful for his staff at palm terrace. This place sounded like a great place for my 92 year old father to recover from heart surgery, but it was NOT! No one seemed to talk to anyone else, resulting in poor coordination of care for my father and inhibiting his recovery. We made the decision to bring him home under hospice care and Palm Terrace made it as difficult as possible! They blocked the Hospice nurse from entry, so she couldn't do her evaluation and put every roadblock possible in our path! They obviously cared more about keeping the insurance money than the well being of my father. It felt like he was in jail... After three days of foot dragging, they finally allowed him to leave. (After having him sign a mountain of paperwork, in spite of him having dementia.) The last day went on so long because of them, he didn't get home until 10:00pm. I would NOT recommend this place to anyone! read more

The evening care leaves a lot to be desired. I had to get water myself for the client after she tried ringing three times. The phone isn't answered regularly and "quiet time" is non-existent. I was extremely happy with the care my father received at Palm Terrace Care Center. I have had a few relatives require skilled nursing, and the other facilities do not compare to Palm Terrace, they were awful. Palm Terrace was great. The facility is clean, the staff is very intelligent and helpful, everyone is kind, caring and I always had piece-of-mind while my father was there. If anything ever happened outside of normal procedure, I would be called and informed. The nurses, CNAs, and entire staff were kind to my father as well as all of us family members. So helpful, and always smiling and friendly. That stated that they didn't want us to worry about my dad while we were not there. They were right, I did not have to. If I ever require skilled nursing, I hope I am lucky enough to be admitted to Palm Terrace. It is by far the best in the area. Thank you Palm Terrace! read more

My Mom stayed here for assisted care then hospice. The staff treated my Mom very well. If I had any questions or concerns the head nurse always made time even when their busy. Their were 2 night nurses that my Mom complained about but everyone else on staff made up for it. My mom smelled so good and so clean after they showered her, those women really did Great job. Linen was also clean. Housekeeping did very good job. The women had change mom diaper cleaned her very well I couldn't smell anything. My mother-in-law stayed at Palm Terrace for about 2 weeks after being in and out of the hospital and other skilled nursing facilities. What a breath of fresh air the Palm Terrace staff were! They were so caring with my MIL and super communicative and responsive to our family during a very difficult time. The care she received was loving and respectful and good for her (more PT than any other facility). If you have a loved one that needs to go into a SNF, I recommend this one over others in Riverside. My 92 year old father was a patient there after he broke his femur. The Rehab and Nursing staff were patient and kind to him. This was a rehab that openly communicated with us and responded to all our questions and requests. The staff are top notch and we couldn't have been happier with the care he received. I researched many facilities before placing my mom here. This one was the best fit. My mom has been here almost a month and I have only good words for this review. Kim and Laura in admissions are the best. Kim was wonderful to talk to to get things in order and if I have questions. Laura handles all the appointments and transport. The real deal is the nurses and physical and occupational therapist though! Those are the ones my mom raves about. The ones that spend the time with the family member are the most important! Noah and Nancy are the kindest and sweetest CNA's. I just adore them. They treat my mom so kindly. My moms nurse Paulina is great, and her wound care nurse Pricilla is great also. Everyone has been wonderful from the doctors, speech pathologists, nutritionists, social worker (Stephanie), etc. The fact that the facility has allowed me and my dad to visit so often and be a part of my moms recovery is the best part. That's huge when someone is rehabilitating. Having the support of one's family can make or break someone and the fact that they understand that makes this lace the best. As far as location and cleanliness this place beats the rest. The outdoor area is beautiful and has a view for taking walks. The inside is always kept clean and I know because I come all the time. Vaccines are required to enter the facility for protection to the patients. Otherwise you must test and do dinning room or outdoor visiting. Those are the rules at the moment at least. I thank them for even allowing me (fully vaccinated) to be with my mom because she was bed ridden for most of her stay. I have seen several nursing homes. This one has a beautiful location and clean facility. And does not have that pervasive odor of urine that so many do. The facility always appears clean. The nursing staff here seem to be very caring, professional and helpful. But make no mistake, this is not a place you want your loved one to be. And none of the patients want to be there. The patient to staff ratio is quite high and our parents didn't get the attention they needed. They both needed to be fed and given drinks through out the day and both left underweight and dehydrated. Their records were always incorrect and when we were first visiting Dad there (who had a broken hip) they were trying to get him up for OT while he was screaming in pain. Oops, wrong patient, its the guy in the other bed who is up and walking around! But the real icing on the cake was when we came to take our mother home. The Doctor ordered Mom to be released, we arranged round the clock care at home. The nursing staff knew she was leaving but the Administrator decided he had "concerns" that insurance would not pay for as much OT (occupational therapy) at home. We were there Thursday when the Doctor's order was received and the administrator said he was thinking maybe next Tuesday! They drug their heals, the Administrator, David Gunnell, even questioned the doctors order to the physician! (Good, the Dr. told us he is re-thinking sending any patients there!) We finally left there with Mom 4 1/2 hours later. I gave them 2 stars because there were some very nice staff there. But seriously I think the administration is more concerned with revenue stream than patient care and quality of life. read more

Please do not send a loved one here! We sent my 91 year old Grandma here in hopes that she would become stronger after a stroke and get the proper care she needed to come home. Sadly, this did not happen! 1. My Grandma's roommate was a younger, kind women. When we'd visit we'd find her talking to gma and taking care of her. She told us that the nurses would leave my Grandma for hours and hours in the room. So she would take it upon herself to give my grandma water with the sponge sticks. Sadly at the end of her stay, my Grandma ended up being extremely dehydrated and neglected!! We had to insist they give her IV hydration. ***Neglect is an understatement!!*** ******When we finally brought her home, our in home nurse found black and blue bruises all over her bottom. The nurse told us it's the pressure points and caused from being left for numerous hours sitting in a wheel chair. She was so dehydrated that the skin on her hands was almost cracking open in spots. ****** 2. About a week into her being there, we got a call explaining that Gma had been transported to a hospital and that she was "declining quickly". We called family members and started preparing for the worst Only to find out an hour later, that they made a HUGE mistake and mixed her up with another patient! 3. Grandma had gone into a deep sleep state after about a week of being there. One day we called to check in on her and after being left on hold several times, we were finally given an update that she was "resting in her room waiting to eat dinner" what!? We couldn't believe it...she went from practically being in a coma to waiting for dinner and resting. We questioned the nurse again explaining how she was in a coma state and the nurse replied, " oh, sorry about that, I confused her with another patient" she's actually still in the sleeping state and not eating. WOW!! Get it together people!! She said this with zero empathy and acted as though our phone call was a burden. 4. When my Grandma arrived there, she was perfectly stable and perfectly coherent! She was sitting up in the wheel chair and communicating with us at the door when we visited. Well they lost her eye glasses and She was suffering and could barely see. Sadly it would take serval weeks to have a new pair made due to covid. These people managed to "loose" several things that were so important! Acted as if they could care less but this was a colossal issue as they lost them!! My Grandma had her glasses upon arrival. She was partially blind and desperately needed them. This was a big deal! 5. We would visit her, only to find her wearing "loaner" clothes. We sent her favorite comfy sleeping gowns etc and she was never wearing them. When we asked , they would say they didn't know why: however her bag of clothes was always on the floor in the room. We called the director who apologized and said she'd be sure, that her clothes would be hung up in the closet where they should have been in the first place. Well, it still didn't happen and she continued to be put into old loaner clothing with her nice clean clothes left in a bag on the floor. *Several times, we found other patients clothing in her laundry bag. *several of her items went missing! We never got them back. *Over the course of time she was there, they gave us the wrong bag of laundry, (someone else's clothes), many times. 3 of her important belongings were just "lost" and no one knew what happened. 6. You have to jump through hoops to get an update! We called to check in on her daily only to get left on hold over and over. rudely they told us to call back around 7 or 8 pm! Who wants to anxiously wait until 7 or 8 pm to get an update on a loved one!? They treat patients like another number. They had no time to take a few minutes to update us over the phone ever! , HOWEVER, prior to us choosing Palm terrace as our facility, they called us every 30 minutes, so kindly persuading us to chose their facility. Giving all the time we needed on the phone with them. Then once we chose them, it was a completely different story and we got the run around. My mom was in tears(we all were) multiple times a day from the lack of empathy, and tremendous added stress of worrying, and the downright awful treatment from staff. Bring your loved one home if you can! If you must send them to rehabilitation, please DO NOT pick Palm Terrace!! read more

My aunt is currently seen at Palm Terrace and I don't have enough good things to say about the place. The staff for one is super friendly, whenever they see our family coming to my aunt's room they stop by to say hello offer us chairs, drinks, amd always ask if we need anything. They are very attentive to their patients,they speak to each patient with dignity and respect which I find very soothing to know that my aunt is in a place where the staff really cares. The facility is extrodanaraly clean in comparison to other convalescent homes that I've visited in the past. I think it's great that they offer crafts, movie night, game nights, karaoke and a beauty shop day where the patrons can go and get their nails painted, manicures and their hair done. They want their patrons to have a good time. I highly recommend this facility to anyone who is looking for a place they can feel confident in knowing their loved one is getting top of the line care, the staff genuinely seems to care about their patients in a lovely environment. read more

There is nothing a family member dreads most then putting your loved one somewhere when they are sick to be taken care of. My husband had a stroke and we chose this place because of its rating and that it was on top of the hill and there was a beautiful view of Riverside that is over 180°. You can sit out there all day under an Olivetree and rehabilitate. It could not have been a better experience. I never saw one staff member without a smile and they were so helpful. My husband's occupational therapist and physical therapist Sean and Shelley were so amazing! I loved that he had the same therapist every day. We were here for a month and a half so that's a long time. Anyone who has their left when you come here is indeed in a good place! read more

The care, knowledge, generosity and kindness that the staff gave to my MIL was incredible. They kicked my 80 years old mom out the facility and said after her doctor appointment don't come back. Wow, that very unprofessional!. So, they transferred her to a different facility in this old small van and throw her in the back. It was very hard getting her out. They paid this young guy under the table to drive her to the appointment ( which is illegal). This guy had no experience or medical license to transport patients. The first incident started November 15, when I take my mom for her follow up. The doctor notices she had an knee infection and wasn't look well so, she was rush to the hospital. Also, the hospital stated she was very dehydrated. She was hospitalized for seven days. So, I called the facility and spoke to Jane the director of nursing to inform what was going on. Jane got very defensive. She been a problem every since the first incident. After the hospital stay, my mom was transported back to Palm Terrace facility. Jane and facility Doctor Kamalpour didn't want my mom there. They stated, Why is she here?. They were like two big bully's with there high class appearance and fancy vocabulary(the doctor). Because Medicare pay for any facility of her choice and Madison of admission said, "they can accommodate my mom needs". Case worker at the hospital talked to Madison several times before transferred her back to Palm Facility. Jane failed to look at my mom medical records and stated, How did my mom fall? I stated, she had totally knee replacement due osteoarthritis. Maybe, you need to read the medical records before assuming things. Last Sunday, my mom had breathing problem. I notify the nurse but the Doctor came in. "Do I need to called 911?" She was very pale and gasping for air. They said no, wait for the chest results. As soon as, the doctor entered, she stated, did you find a different facility. Really!! I'm worrying about my mom health. She ordered chest x-ray which I had to go-to the nursing station to get the results. The tests show that she had fluids on her lungs. Instead of, the doctor treating her for this medical condition or suggest to take her to the hospital. They transferred her to different facility and within hours my mom was back in the hospital for fluids on her lungs. Which the hospital drain it out. But, the moral of this bad experience at this facility please watch over your love ones. When complications arise at the facility that don't want to take the responsibility. Jane and the doctor was very stiff and stern. They had no compassion for others. I felt harass and started to cry. Jane gave me some Kleenex and wave her hands telling me to leave her area.( Like I was fetching dog). For Jane, to be a director of nursing, it appears she didn't know too much of the medical field. The facility was clean and cna, nusring, and the activities was on top of there duties. This facility might seen upscale but the food was horrible. The tuna sandwich was the main course everyday. 'Only had five tv channels, no family room, too many directors(nursing, transportation, admission, activities) and a lot of miscommunication. It not about the top rating and the cleanness. My brother was there about 2 weeks, the staff was wonderful very helpful and friendly. Answered any questions I had, the facility is clean and bright. The physical and occupation therapy, help him get his strength back. It never had that urine smell, that some other places have. I recently was referred to this facility to receive daily IV infusions of antibiotics. Other than the requirement and pain from the infection I did not have any significant illness. The facility is kept clean and neat. The staff were outstanding. I never wanted for anything and they all seemed genuinely interested in my well being. I could name all those who were impressive but it would take up too much space. Discharge coordinator, Trina, CNAs and Nurses were all responsive and professional. Physical Therapy was very helpful and a very nice diversion from normal activities. Christie, Tom and others were great. The dietary department tried to serve tasty and healthy meals. However, the food, for me was too plain and bland. However, it was, I am sure, healthy. I general, it is crowded and seems to be understaffed at times. They do the best they can given the type of patients and facility. I would definitely recommend this facility for those In need of this kind of care. read more

I cannot say enough good things about this facility. My mom had a massive stroke and was left with paralysis and many other complications. She was here for 2.5 months and everyday she got better and better. The staff is amazing. They are loving and caring and they treat the patients like they are family. The facility is clean and the grounds are beautiful. The rehab department including speech therapy is amazing. They all had such patience with my mom, even on her worst days. They all cheered her on and encouraged her so much. They made her laugh and when she cried they made her happy. If you have to go to a facility in Riverside, then this is the place! I was very pleased with the safety measures they have in place to keep him from falling and, under Palm's excellent care, he recovered from his UTI. Palm Terrace Care Center is amazing!!! It is the nicest and cleanest facility I have been to in Riverside, with a beautiful outside patio area over looking all of Riverside. Having recently been a patient there from July 2014 to September 2014 after being in a horrible car accident, Palm Terrace put me back on my feet and made it possible for me to get back to my normal life, something I thought would never happen again. The nursing staff is amazing, they took care of me like I was their own family member. They would check on me every hour to make sure I had everything I needed and would assist me with anything I needed. They are true angels who do everything they can to make sure their residents get the best care they can provide them. They also encouraged me and helped me get through the rough days I had while I was there. From the CNA's to the LVN and RN, the nursing staff is outstanding and some of the sweetest and caring people I have met. Just the same the physical therapist and occupational therapist are some of the best therapist I have met. They put me back on my feet and helped me walk again. When I first entered Palm Terrace I thought I would not be able to walk again having been in the hospital prior for 2 months and being de-conditioned. After my time at Palm Terrace, I am walking now walking on my own, getting out of bed on my own and needing little assistance from my family. All the therapist there are wonderful and supportive; when it was tough and I wanted to give up, they would encourage and push me to do a little more each day, which paid off because I am now walking and using my arm (which was also hurt in the accident) without any problems. If it had not been for the therapist at Palm Terrace I would not be walking today, but because of their support and care I am, and for that I am forever thankful. And lastly the administration team at Palm Terrace is composed of some of the nicest, sweetest and caring people I have met. They each made me feel welcomed and like family. There wasn't one day that the Administrator David didn't come to say hello and see how I was doing. They were also very welcoming to my family who would spend the night with me and stayed with me during the day for my entire stay there. They not only treated me like I was a family member but they also treated my family as if they were family. I am and will be forever grateful to Palm Terrace Care Center for the care they provided me. From the Administrator and his team, to the nursing staff to the therapist, they all in their own way helped me get back on my feet, something I though I would never be able to do.
