The facility is dirty and very outdated. If there were a proper cleaning crew, the outdated condition might not be as noticeable. During my father's two-day stay at this facility, we observed multiple incidents that raise substantial concerns regarding the facility's compliance with basic standards of care, patient‑safety protocols, and regulatory obligations. My father, who had been clearly identified as a fall‑risk patient, was found on the floor. Despite this, his bed had no safety rails installed, a fundamental fall prevention measure required in any competent care environment. His wife was not notified of the fall for approximately three hours. Staff later claimed they delayed notification because he "said he was fine," a justification that is wholly inconsistent with accepted post‑fall procedures and represents a significant breach of duty. Although we were initially informed that he would be transported to the hospital, he was instead "evaluated" by a provider via an iPad. No physical examination was conducted, and no hands‑on assessment occurred to determine whether he sustained injuries. The facility insisted on performing in‑house X‑rays but proceeded with no urgency, demonstrating a disregard for timely medical evaluation. Communication and responsiveness were grossly inadequate. On Monday, I placed 27 calls to the facility before anyone answered the phone. My brother made repeated requests for staff assistance to help transfer my father into a chair, yet it took 45 minutes for staff to respond. During this time, multiple employees were observed sitting at the nurse station using their phones rather than attending to patient needs. Delays in changing and cleaning him were routine and prolonged, resulting in preventable skin irritation. Additionally, we later discovered that my father had not been receiving his prescribed pain medication because the facility "did not have it," despite two supervisors previously confirming that the medication was on-site. This failure directly caused him to experience opiate withdrawal while under their care, a serious and entirely avoidable medical event. Taken collectively, these incidents reflect a pattern of systemic negligence, inadequate supervision, and disregard for patient welfare. The facility's actions and in many cases, inactions, appear inconsistent with accepted standards of care and may constitute violations of regulatory requirements governing patient safety, medication management, and timely clinical response. Based on these observations, I cannot recommend this facility and strongly advise others to exercise extreme caution before placing a loved one in its care. read more

This facility is a danger to vulnerable patients and a trap for families with no other options. I wouldn't leave a dog here, let alone someone recovering from spinal surgery. But after an emergency procedure and being new to the area, I had two choices, and this place should never have been one of them. The building itself is in disrepair: cracked walls, crumbling plaster, and a bleach-stained ceiling in my mother's assigned room. But the real danger is the care or lack thereof. My mother is not supposed to be out of bed without her back brace. She had spinal surgery. Yet I've personally witnessed two CNAs attempt to move her without it. One even admitted they haven't been using the brace. Without it, her spine could literally snap. At night, she was told to "just go in her Depends" because help would be minimal. That's not care. That's neglect. Some might say the facility is understaffed. But understaffing doesn't excuse unsafe practices, emotional disregard, or the systemic exploitation of people on fixed incomes. This place feels like a money factory profiting off desperation while delivering substandard, dangerous care. I'm working to get my mother out ASAP. No one should have to choose between safety and affordability. And no one should be treated like this. read more

This place has a horrible staff!!!! very unprofessional and can care less if you have falling down or press the red button for assistance, they have no sense of urgency. Patient had to call a relative 3 miles away to assist after she have fallen down and press the assist button for over 30 minutes. When arrived at the facility at least 8 staff member were in the front socializing, some on their phone not caring for needed patients. Very disappointing. :( read more

I read the reviews when my dad was admitted here for rehab and hoped our experience would be different, but unfortunately it wasn't. While some staff members were kind, there were major gaps in communication and family involvement. It was often difficult to reach the nursing staff -- we routinely had to call 4-5 times just to get through to a nurse on to the 2nd floor nursing station. On most occasions I was left on hold for over 5 minutes until I gave up, and sometimes I was even hung up on. Even when a nurse did answer, the background noise was so loud that it was difficult to get clear information about his health or recent tests. My family was also not proactively included in planning or updates about my dad's rehab. The main social worker's voicemail was full, emails went unanswered, and promises of follow-up from therapists never happened. This was not his first time in rehab, and another facility he had stayed in previously had much better communication, proving this is not an impossible standard. My dad suddenly passed away after just two weeks here from cardiac arrest. In the days leading up to this, my mom and I had raised concerns about his increasing delirium, something he had experienced before due to infections, and specifically asked staff to monitor and update us. Those updates rarely came unless we actively chased them. The lack of basic compassion after he died was the cherry on top of this whole experience. We received no outreach or condolences. The only communication we received was a form letter six days after his death stating he had been taken to the hospital (wow, thanks!). We had to initiate all contact about collecting his belongings - of course, after trying multiple times to get the nursing station to answer the phone... Basic communication, compassion, and inclusion should be standard as a part of rehab. They were not here. I was starting to research other facilities to try to transfer him elsewhere because of the poor communication, but he passed before I had the chance. read more

Oh no! RUN RUN RUN!!!! I've been here 6 weeks and it's gone from bad to worse (if that's possible). Reston Hospital discharged me on 8/18/25. Unfortunately, this place and Dulles Rehab were the only places with room. I was transported here by ambulance. At first I thought the care was better than the other 2 rehabs I've been in over the past year but nope, they're not. I've already written a lengthy review on Google from a few weeks ago so consider this an add-on. 1. Call light response is beyond the pale (as in mostly non-existent). A couple aides are quite good and a couple nurses but otherwise that's it. 2. Bring food such as snacks and drinks because the aides will either be too busy or they forget plus there's no snacks for late at night unlike Potomac Falls or Dulles. 3. At least at Dulles and Potomac (which are sister rehabs), you'll at least get a menu so you know what will be served for your meals and if you don't like it you have the option to order soup and/or sandwich. Not here at FFX. I've asked if there's a menu to look at and you would think I was speaking another language so you won't know what the meal is until the tray is in front of you. The aides deliver your meals and leave before you've had a chance to tell them the food is cold and/or the tray is missing items that were on the order forms. I don't know who writes up the order form for each of our orders but in my 6 weeks here there's only been 1 meal that was hot and matched the order forms. What I've also found out is the kitchen is constantly running out of drinks such as ice tea, juice and milk. This is a constant. I've requested water and ice and that may or may not appear. The aides are short-staffed and chances are that as soon as they walk out of your room chances are you'll never get the requested item or it will be delivered so late that you're wondering what happened. 4. At the other 2 rehabs you'll get a directory of who's who and who to call when you need to talk to someone. Common sense right? Not here, I've asked if they have a directory and they look at me like I've wished for the moon. 5. This is a HUGE factor and that is the renovation going on late at night. For My floor that started last week. One night they went to 8:45, the next night 9 pm, the night after that until 10 and tonight until 10:45 pm. I kid you not! They started about pm and either they're either inside my door or right outside my door drilling, sanding, etc and it's he loud abrasive equipment. Now who in the hell allows workers to be this loud and disruptive this late at night in a care facility? I've asked multiple times who I can talk to about this and all I get is the run around. No accountability whatsoever - nada. It seems no one is in charge or they feign ignorance. I wish I had family nearby but they're in CO and AZ as my sister would rip into them and tear them a new a-hole as she worked in the ER for years and knows how places should run. So, I apologize for the long vent and like so many reviewers have posted I also found out today I have a UTI. Imagine that. So, run, run, run. Unless you're independently wealthy and can afford round-the-clock personal care, just be aware of what is happening. Best of luck. read more

Do not go to this rehab center. My mother was sent here after a kidney infection that became septic. On the positive side the occupational and physical therapy staff were superb. That is all the good I have to say. My mother and her roommate were both non-ambulatory and were put in adult diapers. There were times they were made to wait hours before they were cleaned and changed. While there were a handful of people who were extremely helpful during my mother's 3 week stay most were not. They are understaffed and have too many patients, some rooms have up to 4 people in them. My mother was on the 2nd floor which was downstairs and felt like a dungeon. It was dark and always smelled of urine. Needless to say she ended up back in the hospital with e-coli and MRSA after here stay here. There were times my mom would ask me to call the nurses station because no one would respond to the call button and if it was after 6:00 at night the phone would just ring and nobody would answer. This place is terrible. On the weekend, you basically lie in the bed 24/7. I visited my 82 yo husband this past Saturday and he told me he thought he was having a heart attack and he was scared. (He has never told me that in 44 years of marriage.) No one answered the call button so, I went to the nurses desk and the "head" nurse finally came up. She told me to "wait they were monitoring him" -- there was absolutely no monitoring going on, except checking his blood pressure. I finally said are you going to call 911, or should I. When EMT's came, they said he was showing symptoms of a stroke. INOVA Fair Oaks sent him there -- huge mistake. read more

I recently had to be admitted into Fairfax nursing center, because I need physical therapy to help me walk again. I had been in the hospital off and on from September 2024 until now. I gave the center a one star only because I could not go any lower. If I had the power I would have this place condemned. I do not understand how they get so many bad reviews and they're still allowed to be open. This is a brief explanation of some of the things that happened to me in the one week that I was there. On the first day I was left in the room by myself with no call bell and no handrails on a bed. When I complained about how I was being treated on the first day the nurse told me that "it's always get worse before he gets better". The CNAs take their time coming to help you and they come with an attitude like I'm bothering them. I asked for bedside commode I was told I had to bring my own from home. Once my insurance sent me my own equipment they then wanted to use my equipment on other residents. The food was disgusting they did not go by the diet that I am placed on. I told them that they were giving me the wrong amount of medicine that I needed. I asked the nurse to call the doctor to ask for a prescription to help me breathe. She told me that she could not contact the doctor that day because it was snowing outside. Things got so worse for me that I had to check myself out of the rehab. There's so much more that I could tell you about what happened to me in that one week. I can tell you all of the symptoms that I fought for months to get rid of, that's rehab put them all back and now I am currently in the hospital and have to have a procedure done.. If it wasn't for the few people that had a heart and was really trying to help, I think that I would not have made it out of that place if I stayed one day longer. I feel so sorry for the people that do not have the voice to speak up for themselves, if this is the way they treated me and I can speak only God knows what they do to those that can't. read more

My mom spent two weeks here for physical therapy after a hospital stay, and she was not a fan. It seems like the facility has fallen on hard times since Covid, but the nurses and techs were nice and tried as best they could with limited staffing and resources. I was happy with the progress my mom made with daily therapy visits, but she was very unhappy with the food (very basic, more like school lunches than quality nutrition) and the constant noise from the other long-term patients. read more

Believe what you read here. I would not put my dog in this facility, let alone a beloved parent. My mom was transferred here from hospital after a fall with a broken kneecap. I did not release the level of neglect she would encounter, which led to her developing a UTI, which went untreated, which ultimately led to her death. Worse, when I protested and ultimately took her out after 3 weeks, they tried to STOP ME, blocked our egress, and chased us down the hall. Horrible!! If I could give a negative number, I would do so. My grandfather was transferred here after surgery, the level of care quality was borderline illegal and definitely at least negligence. They did not clean a wound that he had, even after it had a contaminant on it, this lead to him not only getting a UTI due to poor handling of the catheter, but contract SEPSIS from the lack of care he was provided. He went into toxic shock and displayed Alzheimer's-like symptoms, and only then were the EMTs called, I nearly lost my grandfather as he was in toxic shock for around three to four days. I had to advocate for the people around my grandfather to make sure that they got their food, which is a basic human right. Though the food wasn't much better, I wouldn't be surprised if they cooked a human body back there with how low quality the food was. Overall, this place is hell on Earth, the only nice thing about it is the backyard, and trust me, I wouldn't send my loved one to a place because the backyard is pretty. read more

After two weeks in rehab my 89 year old father came out weaker than when he was admitted for PT/OT There was no release program planning and only 48 hours notice No communication, the placed smelled of urine and I question the validity of their reports to medicare, especially when I challenged/appealed his release The issue with the Medicare Appeal Process is the people who mistreat your loved ones are the same people writing the "report" to medicare for review.. hardly a valid 3rd party oversight process I would not send my dog here.. they talk a good game but at the end of the day Addie, Mary, and Rhonda who are nursing director, director and discharge coordinator seemed nice and talked a good game but virtually impossible to reach. I emailed Addie twice and left voice mails and NEVER HEARD BACK!!! There is no one with any real authority there on weekends so when I came by Saturday to discuss his release on Monday was told I would have to come back Monday which in effect gave me hours to figure out how the support my father after release. They finally tried having medical equipment delivered to my house 4 days after release but by that time my father was back in hospital and now in hospice with sepsis, pneumonia and MRSA infection..at 89, 5 days after release from their center. He had sores on his backside and bleeding around his penis and since he was also in isolation for COVID their claims of his ability and exercise were, and I use this word with no regret.. LIES If they were tending to him how did the sores go unnoticed? His nails were log, skin was dry and ths was discovered by the aides I hired to help him . I visited almost every day and there were at least 2 times when he was not fed his first meal until after 10 AM and the food was cold. If you are helpless they treat you like a burden and bother versus providing compassionate care. 5 days after release where they claimed he could walk with walker etc, he is in Hospice PLEASE DO NOT SEND A LOVED ONE HERE... you are warned , again research beyond these reviews..ask Hospice providers, ambulance drivers and others., their reputation by those in the know speaks volumes, something that I unfortunately found out too late. I am not sure how they got a positive rating from Newsweek but if they are number one in Virginia, I would hate to see those rated 2 and below read more

Indeed there is a banner in front of the facility which brags about its Newsweek high rating. I looked up that rating and discovered it is rated "average" in the most important care areas. The food was poor, the lack of professional and assistive staff was obvious; people calling "nurse, nurse" from their rooms, incontinence because there wasn't enough staff to respond in a timely fashion to requests for toileting. 4 men sharing one bathroom. Inadequate shower facilities. My friend reported very little rehabilitation evaluation and treatment. In some small rooms - 4 residents crammed in - are these medicaid patients? I presume this facilty - owned by Innovative Healthcare of Richmond, VA - is mainly interested in billing Medicare and Medicaid for marginal services. STAY WAY FROM THIS FACILITY. read more

Sub-standard care, understaffed, poor communication between staff members and with families. A supervisor is not present at all times and should your loved one suffer a fall the staff can't call emergency services because there is no supervisor present (yes, crazy). I wish that I could leave NO STARS! My mother has been at this facility for over a year. This facility is horrible! The staff is unprofessional. No communication or call backs, so that you can follow-up on your loved one's health. The up-keep and cleanliness is a joke! The turn-over for a Director at this nursing home has been on going. The CNA's are not trained, none the less do not care for the residents. When I asked a CNA for assistance;- during a recent visit, my mom had a bowel movement, I asked for her to be changed and the response I received -" I am not her nurse"! I then asked, "Who's my mother's nurse?" I was told that her nurse was not here, because it's a shift change. I said; It's 3:45 pm, what time will her nurse arrive? No response. I was LIVID! When the person in charged asked another CNA to assist, she became angry and made sound gestures toward the person that gave directions. My mother has been left soiled, has fallen, and does no activity what-so-ever at this facility. As soon as I can find another nursing facility, my mother is out of there! If I could give less than one star I would! I was told that this place used to be great and then it was sold a few years ago. But the rehab section was still good by a local hospital representative for finding rehab facilities. Upon walking into the place I wanted to turn around and just take my mom right back out. I knew already it was a mistake. The first night my mom was there they never gave her the pain pills because they said the pharmacy had not filled it. 22 hours later we're still waiting for her pain pills to be filled! Unacceptable. Only to find out that if she needed it that bad enough that the nurse could've gotten it from her request sent upstairs somewhere else in the facility. They don't give anybody a name band and are supposed to ask who you are before you give medicine. Which my mom was never asked who she was before she did get medicine. They also never brought her medicine at night time saying she wasn't prescribed anything which she was when she was released from the hospital. They brought the wrong food to her. The food was sub par. Unfortunately, she was stuck in a room with a lady that had dementia who had her nights and days mixed up and would scream all night for her daughter who was not there so my mom got no sleep. We requested to move my mom which was not done. When my mom would call the bell to ring for a nurse, nobody would show up. After a few days in there I requested her to be discharged I knew I could take better care of her at her own house. Since I didn't feel comfortable, leaving her there, it defeated the purpose of me trying to go back to work. Because I would stay there for hours to make sure that she was OK. They're only certain days where you can shower and nobody knew what day they were. The facility said she needed to stay in and would not discharge her. So I took her out against medical leave. I feel terrible for the people that are stuck in there and don't have an advocate to help and get the best care possible. Don't take your loved ones there! read more

I found Fairfax Nursing Center professional, kind and provided excellent care for my 94 year old mother in law who was a resident for a month for rehab. They have been named #1 nursing home in Virginia by Newsweek magazine. They have excellent physical therapists who take the time to explain patients progress and give advice about next steps. The only negative employee I dealt with was case worker Addie. She seemed to enjoy upsetting me. I would have to walk away and tell her I needed some time to calm down before I could talk with her. She never asked what she had done to upset me, or what she could have done differently so I wouldn't be upset, so it was obvious she didn't care. She was unsupportive and unhelpful and should either change her attitude or occupation. All in all I highly recommend Fairfax Nursing Center. read more

i have respect for people that are working in the era of covid. however my mom spent 5 weeks here and it was a living hell. Disrespected by the staff, I WAS told 5 different things by 5 different people? Who do you believe? Operationally this place is the worst. Told Wednesday that she could not leave until she had 2 negative covid tests, then less the 24 hours later told "if shes not showing symptoms even if positive, she could go home? really? Everytime you contacted someone (after multiple try's and hang ups) through persistance you m a y get someone. i get it covid is horrible, i've had it myself but my opinion would be to do more to cross the t's and dot the i's because of it. The major thing that was evident was disrespect. mom hasn't walked since march and after 3 therapy sessions they try to have her transfer from bed to wheelchair with limited help and she was injured. Case managers are argumentative and nasty. Complaints go nowhere. Mom came home 1/13 and i waited to post this for fear of retaliation. "worst of the worst" and by the way they sent her home with clothes, n o t her clothes. I implore you to stay away from THIS facility at all costs. I wouldn't put a dog here. You would be lucky if your loved one gets out alive. if this is their "new management" they have a big problem. VERY DISTURBING, SHOULD BE INVESTIGATED. read more

I would give Fairfax Rehab and Nursing Center ZERO stars! I am perplexed as to how this place won a 'Best' designation from U.S. News and World Report. Operationally, this place is the worst - the absolute worse!!! The admissions team has no idea what they are doing. It was a very painful experience dealing with them and it continues to be to this day. Once you get past dealing with admissions your loved one will encounter the following negatives: 1.) lack of medical staff on the weekends 2.) medical staff with very heavy accents - it is very difficult to understand what they are saying 3.) very poor internal communication among the staff - i.e. the left hand does not know what the right hand is doing 4.) no maintenance staff on the weekends to address issues with televisions not working, phones not working etc. 5.) laundry service which is infrequent and constantly looses patient clothing 6.) the inability of the staff to get a newspaper to a patient I can't wait to get my loved one out of here! I do understand this place has been sold. The new owners take over November 1, 2020. One can only hope they make a MAJOR overhaul of this place. My grandma is living here now, as she's no longer able to live on her own without help. That kind of thing is never easy, and frankly it's rather depressing, but from what I can tell she's in very good hands here. The facility is nice and clean, and they have a lovely little patio and garden out in the back. All of the staffers I met and spoke with were polite and calm. It's clear that they have structure and schedules to adhere to, but they were never intrusive during our visit and were incredibly kind when dealing with Grandma. There are plenty of places to sit and visit including the garden outside, and from what I can tell, they encourage socialization amongst the residents as well. Living in a nursing home is probably never going to be Club Med, but it was heartening to finally see where my grandmother is living and to see that she's being well taken care of. read more

All of the workers there are treated my father like he was their own I was so pleased. Horrible treatment. I've been here since April 2021. I am anemic and need blood work. FairfaxRNC has denied me necessary blood work against my primary doctor's recommendation. I could walk with walker in April, May and part of June 2021; now I can't walk at all and just lay in bed all day with no therapy. I've had 10 different roommates since April 2021 and I'll just die here because of inadequate care and treatment. Cost of room and board is around $12000/month. Food service is not good. Limited menus and food choices breakfast, lunch and dinner. Staff are solicting good reviews from patients to post on-line. Management severely incompetent with health care practices and awful communication between staff shifts. I've been here too long and my condition has deteriorated. I'm still a dissatisfied resident at Fairfax Rehabilitation and Skilled Nursing Center. Zero rating truly! Advice to all: Avoid this facility at all cost! read more

Over the past 10 years or so, I've needed to spend time in four different rehab facilities to regain strength after hospitalizations. Fairfax Nursing Center was definitely the worst. I was assigned to a double "room" that had no coverings on the window and no privacy screening, although my roommate's section had a curtain. The therapy was almost non-existent. Although I use a walker at home, and had been using a walker in the hospital, the therapists refused to give me one so I spent most of my time in bed instead of being up and getting stronger. I found the frontline staff to be very caring and generally attentive, but the management is poor. This obviously has a significant impact on patient care and family relations. The food is horrible. Like other facilities of this type, they are short staffed and are challenged to retain staff - especially the good ones. My loved one passed away while in their care and it was very poorly managed. read more

This needs to be said: Because if you need to actually have a loved one go to a rehabilitative center and not worry, this is the NOT that place... Besides the facility needing to be completely hosed in disinfectant of some sort and the decor not being update since 1989 (They did up date a common area but don't be mesmerized-that's it!) the level of general competence is far below that of rationale. My father was at this "rehab" center after suffering a stroke and was mistreated, neglected and he even fell in the "care" of a assistant or nurse- and get this he didn't have any rehab sessions (that's why I used the quotation marks back there). Instead of calling 911 or offering any assistance we had to call 911 ourselves because they only wanted to call the doctor on staff (who was not at the facility, as no doctor remains on grounds round the clock). I would have given 0 stars if that was the option, but alas it wasn't; but I will say that I have actually been to (state run) animal shelters where dogs are treated better than Fairfax Rehabilitation Center treats their tenants. Save yourself the time and instead dropping your loved one off here, take them to the local Fairfax County Animal Shelter- they might have a chance! read more

I was sent to Fairfax Rehabilitation for PT/recovery following release from INOVA Fairfax. I would not recommend FRC for patients that are cognizant and able to get along on their own. I was placed on a 'memory' floor - not allowed to leave the floor unless accompanied by '..a responsible adult.' During the 2 1/2 weeks I was there, I did not find anyone that could carry on a reasonable conversation (or, in many cases, remember th one you had 5 minutes earlier. Most of the patients on the 4th floor were either mentally or physically unable to fend for themselves. I found the lack of social interface to be detrimental to my recovery. Imaging, 2 1/2 unable to hold a reasonable conversation with anyone other than someone who might be visiting you. I was told by the PT/OT trainers to get out and walk, but could never do it as the center did not have anyone available to follow me off the floor. The rooms on the 4th floor are approx 15' by 20'. I was put into e bed in a room with a long term patient. He had about 13' while I had 7'...enough room for a bed, small chest, and a plastic folding chair. There was no place on the floor where I could sit and read other than the plastic folding chair...and, an uncomfortable chair it was. I questioned why I was on the memory floor and was told it was the only bed available. It seems unreasonable to me that that room was the only one available for 2 1/2 weeks...especially when I could often see people being discharged. Oh, on the 4th floor, you get showers twice a week...Monday and Thursday. I was admitted on a Wednesday afternoon, and the first I heard about this shower restraint was when hey announced Shower Day was over. Whlle I believe the PT and OT contributed to my getting better, in the long run, being a prisoner fn the 4th floor was detrimental to my recovery. If anyone NOT NEEDING to be on a memory floor is told by FRC that their room is on the 4th floor - recommend looking for another center. If I could, I would give FRC zero stars.... read more
