I've been a patient at Forever Smile since 2023, initially for Invisalign, and have always had great experiences with Dr. Shelyan and the dental hygienists. Unfortunately, my most recent visit with the front office left me looking for a new dentist. I recently switched insurance and accidentally enrolled in a plan that Forever Smile doesn't accept. I apologized and acknowledged that it was my mistake, and said I would come back once I had insurance they accepted. The front office lady then told me it would be a $150 cancellation fee. As I was getting ready to pay the cancellation fee, she told me I could instead pay $180 out of pocket for the cleaning, which I agreed to. Once I got seated for my cleaning, the dental hygienist was getting ready to take my x-rays and I asked if that would cost extra since I'm paying out of pocket and I wanted the cheapest service. The hygienist checked with the front office and came back with a revised total of $145. I confirmed that amount, received my cleaning, paid $145, and left. As I was driving home, I received a text from the front office lady saying I had been charged the wrong amount and now owed an additional $35. I asked for an itemized receipt for both the $145 I paid and the new $180 total so I could understand the difference before paying. Instead, I was told they would only provide an itemized receipt after I paid the remaining balance. This did not sit well with me because, in our text conversation afterward, the front office lady said she had already removed certain services because she knew I wanted the cheapest option. However, when I was in the chair, the hygienist was preparing to take my x-rays until I specifically asked whether they would cost extra since I was paying out of pocket. That left me confused about what services had actually been included, what had been removed, and how the final amount had been determined. What bothered me most wasn't being asked to pay another $35. If there had been a genuine billing mistake, I would have paid it. What bothered me was being asked to pay more money while being refused an itemized receipt explaining why. Once I asked for the itemized receipts, it seemed the front office lady was no longer interested in pursuing the additional $35. I never received a response to my final message or any further follow-up regarding the additional balance. She also told me that no one other than her knew the office's fees. I found that unsettling and it further reduced my confidence in the billing process. As a customer, I don't think it's unreasonable to want to understand what I'm being charged for before making an additional payment. I also mentioned at the office that it would have been helpful if my insurance had been verified before my appointment since the office knew I had changed insurance and was returning because I had obtained new coverage. The front office lady replied that too many patients change insurance for them to verify everyone's coverage. While I understand they may not verify every patient's insurance in advance, it would have been a thoughtful gesture for a long-time customer in this specific circumstance. However, I realize it was my fault for getting the wrong insurance, which is why I was willing to accept the cancellation fee. Dr. Shelyan and the dental hygienists have always provided excellent care, and I appreciate the service they provided. My review is directed at the front office lady and how she handled the billing and customer service throughout this experience. The lack of transparency and communication ultimately caused me to lose confidence in the practice and look elsewhere for future dental care. I genuinely hope they improve this aspect of their business so it reflects the same level of care and professionalism that Dr. Shelyan and the dental hygienists provide. read more

I have been coming to this dentist for a while now and everyone is great. I suffer from TMJ and grinding of the teeth. Recently, I had toothache on teeth that were healthy. The dentist recommended that I try the EMFACE treatment for TMJ and it has made a big difference after 6 sessions. I do not wake up with tension in my jaws in the morning anymore. My jaw feels more relaxed and I haven't had a toothache since I started. I would definitely recommend the treatment for anyone who has TMJ and grinds their teeth. read more

I'm updating my review because my original one was motivated by a $5 Starbucks gift card that the office offers if you leave a review. My review didn't reflect my full experience, and I can't help but wonder how many of the other glowing reviews were motivated by the same gift card... Now that I've dealt with the billing side of this practice, I can confidently say my experience has not been five stars. Because of their billing practices, I no longer return to or recommend this office. Here's what happened: I came in for treatment and was quoted a price I thought I would pay. I agreed to the procedure based on that quote. Weeks later, I received a bill for $642.50 because my insurance benefits were apparently maxed out at the time of treatment. I was blindsided and felt trapped. I understand insurance can be complicated, but if I'd known the real out-of-pocket cost, I would have scheduled the work later when I could afford it. Instead, I had to pay the unexpected balance or risk it going to collections. Beware: the price they quote you may not be what you actually pay. This office also requires you to leave a credit card on file to book appointments -- you can't get around it (see attached text from the office). I rarely write reviews, but I feel obligated to share my experience so others can make informed decisions. After receiving treatment at this office, I was later contacted and asked to pay additional money beyond what I believed I owed. When I tried to understand the charges, I reviewed my insurance explanations of benefits and billing statements. During that process, my insurer, Delta Dental, required the office to issue refunds due to overbilling concerns. Even after those refunds were issued, the office later attempted to bill a procedure nearly a year after the original visit. This raised serious questions for me about the accuracy and timing of the billing. I also asked to review my dental records so I could better understand what occurred during my visits. In writing, I was told I would need to pay more than $2,500 just to review the records, otherwise I was told there were no records to review. That response was extremely surprising and made it very difficult to resolve the situation directly with the office. Because I could not get clear answers, I ultimately filed a complaint with the Dental Board of California so the matter could be reviewed independently. If you are a patient here, I strongly encourage you to carefully review your billing statements and insurance explanations of benefits. If something does not seem right, you may want to contact the Dental Board of California to report your experience so it can be looked into. I had a discrepancy on insurance coverage on a bill for services received. While we were researching to figure out the discrepancy they doubled my charge and sent me to collections even though I had open communication with them about the matter. I called to speak to them about it to rectify the original balance and they were extremely rude and aggressive about the matter. This is disheartening because I had been going to this dental office all the years I've been in LA. They not once prior to this issue had any problems with my account carrying a balance. There was one hiccup and this is where we're at. The call was held so unprofessionally that I highly doubt I'd ever wanna go back. UPDATE: while I appreciate your very long response, it is not true. The account manager got on the phone with a chip on her shoulder to begin with. She handled the call unprofessionally from beginning to end. She's probably upset about the balance and maybe that's why. However, I wouldn't expect you to get on here and admit that. Also, the late fees DID immediately double my balance which is what I've been asking to have adjusted and why the balance has sat the additional time after coming to the conclusion that insurance was not covering it. Maybe this is my perception because the fees were accumulating while we were researching the insurance situation. However, no one has explained that AND I don't think that's very ethical way to handle the balance. No one is weaponizing anything, the fact of the matter is that you were unwilling to help me from the moment you picked up the phone. read more

The dentist and staff are very nice, but make sure to triple-verify that they actually take your insurance plan. Their ZocDoc page said they did, and they confirmed that in multiple text messages before my appointment. When I actually took time off work to travel to and attend the appointment, though, they said they were having trouble verifying the insurance and I might owe a bit of money, but they weren't sure how much. I should have walked away then but really needed this appointment. Two months after the appointment, I got a bill from them for $267. For a routine cleaning, exam, and x-rays that were supposed to be $0 through my insurance!! I tried to reason with them, since their ZocDoc page had been incorrect, and I was given no indication I'd owe anything until the day I arrived, but they wouldn't budge. After the dentist meets with your their office manager will come in and give you a rundown of everything cost wise, treatment, whatever it may be which is really great and helps avoid any surprise payments or treatments. She literally lays it all out the best I've ever had a dentist office do so. I did a deep cleaning and the hygienist made me feel really comfortable considering I do not like getting shots and they have to numb your mouth with a couple before proceeding. These people had the temerity to report me to collections, when they did work that was NOT approved by Delta Dental and that I did NOT agree to pay if Delta Dental wouldn't cover it. Delta Dental says they should've gotten PRE-APPROVAL before doing work on me. If that had been done and I had been told it wasn't approved and I'd have to pay $1,000, I'd have said NO to the procedure. But that isn't what happened. They said I owed $400 over the $1,000, which I paid. But now they are pretending I owe this $1,000, that I *never* agreed to pay. How do I know this? I've asked for a copy of the document signed that day SEVERAL TIMES and they've ignored me and NEVER sent it to me. Suspicious much? Of course they don't want me to have proof, as they are trying to cheat me out of $1,000, which if they'd done their job, I wouldn't owe! Dr Shelyan is a very experienced dentist and oral surgeon! i didn't realize the poor quality of my old dentist until i came into this office. incredibly clean, attentive and knowledgeable team. I had 2 wisdom teeth extractions and i used oral sedation and felt very comfortable during the whole experience. I had anesthesia last year with another ortho but i have to say i prefer oral sedation with the Dr because the after effects were very minimal!
