Within a year they had her in a wheelchair, unable to stand and in diapers. The residents are all in wheelchairs and typically sitting outside in flannel pants in 100° weather. Most of the staff is rude and if you have to deal with the Executive Director, Keith, be very aware that he is retaliatory. If you leave a message that Keith doesn't like he will retaliate. My mother was brought to the hospital yesterday, and when Maryland Gardens failed to call us and let us know I called and left a message letting them know that I plan on having that facility shut down before they killed my mother. Keith took it upon himself to call the hospital and to let them know that there was suspected abuse and that we shouldn't be allowed to see our mother unless we went through the Fiduciary. Fortunately, Keiths attempt at sabotaging our visit (after a 4 drive) failed and we were permitted to see her. The Executive Director at Maryland Gardens is immature and should not hold the position he has. I DO NOT recommend placing your family in this facility.

The staff is all ages which I find a plus. The rooms could definitely benefit from more ventilation, ac and privacy options. While I agree about being outside in 100% heat is not good, it is Phoenix folks and they need nature/fresh air. My mom has been there for 3 months now and she says everyone is nice and she likes the food.

Here we go: I had a…read moredeeply disturbing experience with Bayada Home Health, and I strongly warn others to avoid them. After breaking my ankle, the VA assigned Bayada for in-home care. Joanne conducted the initial visit, ensuring my vitals and the home were in order--my home was spotless, as I take pride in that. Afterward, Anika, the supervisor, called to assign care. I explicitly requested a young Black woman--no children, single, and looking--because I wanted someone who could relate to me; as opposed to a married black mother of three as this is the person who was supposed to help with bathing and so forth. Anika ignored this, repeatedly calling me Vernon instead of Kabad, despite my corrections. In fact, Imani, the caregiver they assigned, never did much work at all--maybe two dishes one day, and once she took out the trash. Mostly, she just sat with my well-behaved dog, who needed no special care. When Imani took a day off, her sister Tiara came. I didn't even know they were sisters until Tiara volunteered that information. Instantly, I felt extremely uncomfortable--what had they shared about me? Tiara did nothing but talk on the phone to Imani for the entire three and a half hours she was supposed to be working--outside, smoking pot, and doing nothing else. She wore sandals, ripped sweatpants, and was obviously eight months pregnant--completely unfit for the job. Then, she left early, having shown up late, no excuse. And all this, while on the phone with her sister, the entire time. Bayada dropped me after I gave them an accurate review. This wasn't a joke or a grudge; it was a complete failure of care, professionalism, and respect. As a veteran, I expected dignity--and Bayada failed me. Stay away from them.

I WOULD RECOMMEND PHX Mountain to anyone! The care is TOP NOTCH! I'm on the younger side (For once) to be in a Skilled nursing home! I took care of my father and mother before they passed so I know nursing homes ALL OVER PHOENIX AREA including Tempe, Chandler, Mesa, P.V., I know how scary and terrifying some of these places are! That is not the case for Phoenix Mountain on Tatum. The nurses like nurse Barry and nurse. Tristan are amazing. I can't say enough great things. The rooms were clean. The food was fantastic. OK there was a couple days a couple meals are weird, however, 90% of the time the food was phenomenal! When I was there, they had a fantastic chef! The physical therapy team outstanding, the nurses aids or the CNA's down to the housekeeping crew we're incredible. They kept that place immaculate. Although the nurses are overworked, they need a raise and they need to have a one nurse per hallway!!!!! then it will run so much smoother for the nurses. The nurses worked their buns off! and I am so appreciative of that and I am so grateful! I cannot tell you how pleasantly surprised I was of how great this place was!, the people that lived there and that were in my same situation were awesome. My room was lovely. I overlooked a fountain and had lots of sunlight and I had my own bathroom. My roommate was incredible Patricia. She's a 93-year-old blind woman who is the most interesting person. She was awesome, again the place was fantastically clean! It looks a little old older, but it makes up for everything of how great the nurses are and the care is, it was great. The food was great. They even have an area outside where they had a big barbecue and we had hamburgers and hotdog for everyone and they have tons and tons of activities. If that's what your people are into, they had games they had workouts. It was incredible. I can't say enough good things and I'm only in my 50s and I was so surprised but so grateful! I also did have a loved one who stayed there and she had a good experience there too!, so take it from me, I was just there the last day of February 2026 for an entire month until April and I cannot say enough good things about this place! Hit me up if you need more info. Hats off to Phoenix Mountain post acute on Tatum and Thunderbird fantastic! Kind Regards Rosy Slater 714-910-1943

I worked at Phoenix Mountain Post Acute as a CNA, and unfortunately my experience there was very…read morediscouraging. The facility is constantly short staffed, which makes it extremely difficult to provide residents with the quality of care they deserve. With so few CNAs on the floor, call lights would often go unanswered, residents were left waiting for long periods, and basic care like hygiene and toileting was sometimes delayed. Many of us CNAs voiced our concerns, filed complaints, and even reported the building to the state in hopes of bringing positive change. Instead of addressing the issues, several staff members who spoke up were terminated. It felt like management, corporate, and even some of the nursing staff were more focused on protecting themselves than supporting the caregivers or prioritizing resident safety. There are good caregivers who truly care about the residents, but the system in place does not support them. The turnover rate is high, new hires often don't receive proper training, and the overall culture makes it hard to provide the compassionate care that families expect and residents need. Until major improvements are made in staffing, management, and overall accountability, I cannot recommend this facility as a safe workplace or as a place for loved ones to receive care.

These were the services agreed to prior to moving in. After moving in, these prices changed. * Services not asked for were added without my parents' knowledge, leading the facility to charge more than $18,000 extra over the space of 4 months. This was only resolved through months of communication with the Brookdale management. To this day, the billing is not standardized, itemized, or consistent. * The food that is served is substandard. Think about high school cafeteria food. Some meals were not delivered, causing my father to have to order out. A large percentage of the breakfast meals were delivered without toast. Missing items per the order submitted is a regular occurrence. * The cleaning of the apartment is supposed to take place weekly. Sometimes weeks have gone by without a cleaner coming to the apartment. Cockroaches have been seen inside the apartment. * The care of my mother who is bedridden is supposed to be completed by 2 caregivers as per the contract. This has rarely been the case. This is my personal observation by my weekly visits on Sunday mornings. 90% of the time, there is only one caregiver, forcing my father who can barely stand to help. This puts both of my parents in jeopardy. As a side note, the caregivers that come to my parents' apartment are great with them, giving them the best care they can though in many instances are untrained and have had to be shown how to care for my mother by my sisters. By the words of the caregivers themselves, they are constantly shorthanded. Look elsewhere!
