I got to know several of the caregivers personally as we cared for my father together: I especially treasured my father's time with Yvonne, Rihyana, Sebastien, and Kaye. All four of them were outstanding: deeply empathetic, and highly skilled. I also felt very comfortable leaving my father alone with Home Instead, as they were trustworthy, and even kept my father's apartment clean. I had a few concerns along the way, and the Home Instead administrators were responsive, and made all the changes I requested. Thank you for making a difficult time in our family's life a little less difficult and a little more joyful. Home Instead was everything I was hoping for in hospice caregiving.

For someone reason my first review did not post and so I am resubmitting because I want to share…read morewith all of you our experience with Sonata Hospice. In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. I hope this review makes the post and can provide another family with reassurance.

I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Thank you from the bottom of my heart and the hearts of my family.

Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

He worked so hard, never retired, super healthy, until we were introduced to dementia, you hear about it, reality is brutal when happens to someone you love... You think you can deal with it, but as you go, it is devastating, sad, scary. This disease starts slow, and suddenly every week, every month there is a big change, never gets better, only worse, a new dad every week... It was like seeing someone you love becoming a new person all the time you meet. Only thing you can do is to provide comfort and excellent care. Dementia hurts everyone and everything, I was new to this, so I had to take online courses, and read books to understand the disease and what was going on with my dad. I have a full-time job, so I would stop before or after work when I placed him at the memory care, and on my days off. My dad would scream in happiness when he would see me coming, it was a party for sure, and that was my absolute pleasure in life. To see him running after me to give me hugs and kisses as if he was seeing me for the first time ever, all the time he saw me. Unfortunately, he was very difficult to deal with for the folks at the facility, he would get very angry, sometimes violent with the staff there. I would have to rush there, leave work to calm him down. He would also yell at other residents, and when hallucinations started, he was gone and trapped on a sort of hell loop, seeing things, screaming, suffering as if he was in physical pain. One day, I decided to go to a support group with others like me that were losing their loved ones to this disease, we shared stories, some were disastrous, yet very funny. That day, I had the immense pleasure to meet the founder of G&G, Mrs. Jane Porter. It was an impact on me to meet someone so kind, and knowledgeable. At that point seeing my father suffering was hurting me more than it was hurting him... Only great thing about dementia is that they forget what they did, unfortunately you don't. Jane spoke with me about comfort for him, deep inside I was sort of desperate to give him peace while he was still with me. I was concerned at one point the facility was going to quit us. He was a genius man when he was well, an engineer, a scientist, a builder, so that with dementia would make him put everything apart. He would dismantle his closet and other residents closets, beds, ac units, tables, chairs... Jane spoke with me, and mention that we just need to find a proper medication for him, and assure he will have not just a better care, but a better quality of life. As a nurse herself, she monitored him with her doctor and team for 2 weeks until they found the proper dosage and medication that transformed him from the man I described above to the sweetest, calm, happy man. I was shocked, many times I would drive away from him crying in my car alone, that day I left with happy tears. I was able to take him out for dinner at his favorite restaurants and bring him home to spend the day with the dogs that he loved so much. The greatest NEWS is that the hospice was covered by his medicare, so I didn't have to spend anything out of pocket. Jane and her team made sure he was groomed and showered, clean and happy. I think that made me happier than anyone, some light and life got back into me. He had some incidents like bad falls, flu, respiratory issues, and they even provided oxygen tanks, and a hospital bed. Once the big facility was not a good fit for him anymore, I moved him to a this wonderful place called Brentwood Care Home with Rose Malabato, still, I brought Grace & Glory with us. He was doing so well for so long besides not being able to speak anymore and walk. Suddenly he could eat anything comfortably, and one of main wishes to me is that he would not pass in a hospital, he hated hospitals, with the disease he was really afraid to go in, he would get worked up and fearful. Jane made his room at his new facility my father's hospital bed, and he didn't know any difference. He passed peacefully in his room, very comfortable with no pain. There are people in this earth that you remember forever, some are angels on earth, and that is how I describe Jane and her team. When he was gone, I didn't know what to do with myself, part of me was gone, but I knew it was the best for him to rest. Now, I considered Jane and her family part of mine, and there are nothing I can do to express my gratitude for their love and kindness to my father, and our whole family. 1 year has passed without him... I don't write a lot of reviews, but I feel that by sharing my personal life story here with you it can be helpful. Typing a big journey in a such a short essay brought me many memories and tears, you don't realize what you and your family is facing when you are going through with it. I couldn't do without Grace & Glory. Thank you so much, from the bottom of my heart.
