I have a good friend whose Grandmother lives at Fauquier Health Rehabilitation and Nursing Center. My friend is in constantly to visit her Grandmother - and she does volunteer work at Fauquier Health Rehabilitation and Nursing Center as well. She volunteered to take me on a tour of the facility. I was astonished at the difference in ambiance. The place felt good - it has a nice vibe to it. And it was clean. The staff are on top of everything. She knew many of the residents and we stopped and chatted with some in the hallway, in the dining room - everyone was so happy here! We visited with her Grandmother for a while and she told me several times how much she likes the place. She commented to me several times that the food is very good. I asked my friend what the response times were on average when a resident pressed the "call" button. She told me around 30 seconds or so. Fauquier Health Rehabilitation and Nursing Center is fully committed to the Planetree patient-centered care alliance. The Planetree philosophy is a holistic approach to patient care that encompasses the body, mind and spirit, and encourages the participation of family and friends in care and rehabilitation. http://planetree.org/ The facility is very popular due to their level of care and service. So it's hard to get someone in - hopefully if you have a loved one that needs this level of care - you'll be able to get them into Fauquier Health Rehabilitation and Nursing Center!

Pro tip: if you person in the facility needs something specific it is best to go every day. Nearly every time someone from my family could not see my dad for a day he would get dehydrated. Also keep an eye on a catheter if they have one. If and when they are changed keep an eye that the bag is filling (gross, but important). The other good thing about this facility is how close it is to fauquier hospital, in case something happens. There are different wings depending on need. The rooms are rather small, but do the job. There is a small area for visitng with drinks and coffee and water. there is also free internet.

This place might be okay for someone who is more able to care for themselves and can walk around on their own, etc. However, if you or your loved one require true constant care and supervision, I would not recommend this place one bit. My dad was admitted due to him having terminal cancer, and all the other medical issues that come with that. He suffered many strokes, so he had some confusion at times, and over 6-7 months became less and less mobile. The last few months he could not walk on his own more than a foot or two. He was a fall risk almost the whole time of his admittance. From the very beginning I noticed major issues with this place: 1. They were not coordinating his appointments/transportation adequately and he was missing them at times. 2. he needed help to get to the bathroom as he was a fall risk, and he would tell me all the time that he would ring for the nurse to come help and it would take them way too long to get there, which resulted in him just trying to go on his own and then he would inevitably fall. 3. He was a fall risk, and yet they had him at the end of the hall farthest away from the nurses station, and when he would ring for a nurse they were not punctual. 4. One day I came to visit, and I walked into his room to find him on the floor. This was truly traumatic for me, and he had fallen out of his wheelchair. They left him sitting in the chair after his dialysis appointment, instead of bringing him to his bed, he tried to get up to go to bed and fell. This resulted in a hospital appointment because he had a deep gash on his head from the fall. I mentioned that he fell out of the wheelchair to the staff, but I was brushed off. I should have taken a picture as proof, but I was so flustered in the moment that I didn't think to do so, and then I was put out of the room when the nurses came to help him up after I informed them he was on the floor.... God knows for how long because they do not do regular room checks... even for a FALL RISK person. 5. He was moved to a new room at the end of his stay, and when I came to visit, all of his belongings were still in boxes. No one could be bothered to open up his stuff so he had access. Yes it was mostly clothes, but he also had some snacks and enrichment items, that were just in boxes. 6. He passed a week ago, and no one has bothered to reach out to me, my mother received a phone call to let her know, but me - nothing. I am the main POC. Not even a condolence phone call. Don't get me wrong, some of the nurses were okay, and the last two he had in his new room seemed very nice. But overall, this is not the place you want to be when you need more attentive care. They don't care.

While she did make some improvements in her mobility she was not able to transfer out of her bed or wheelchair without assistance. She also wasnt able to walk independently with a walker during her last PT session yet they still seemed to think 8 hours of in home care upon returning home would be adequate. They sent us with a wheel chair that she cant even use because due to her arthritis she cant bend her knee enough to use the foot rests!!! They didnt give me a list of equipment I needed to get (aside from wheelchair, walker, and bedside camode) when her in home caregiver arrived this morning (her first morning at home) she asked about a lift belt and she was shocked they didnt send her with one. My mother ended up back in the ER after a sleepless night for both of us with her in so much pain. Encompass refused to take her back and her physician there blatantly LIED to her ER Dr about her abilities prior to discharge. The DR in the ER absolutely didnt belive them as she has eyes on her now and can see how in pain she still is. What they have done is criminal and I am FOURIOUS that they would out right lie to cover their asses. Believe all the one star reviews and try somewhere else, the hospital is now working on getting her transferred somewhere else with, hopefully, competent staff. STAY AWAY is my only advice here.

We had problems with both the staff and the administration. Most of the nurse assistants were very nice and helpful but they are overworked & short staffed and my father would often be neglected by them. I would have to go find someone to assist him in urinating, etc., and that often turned into an extensive search as often not a single staff member was in sight. I am a male and one of the nurse assistants, a black lady named Sarah, made sexual advances towards me on several occasions. She would grab me in a bear hug and rub her body up against mine and attempt to kiss me on the neck. The first time it happened, I just pushed her off me and thought she got the message. The second time it happened I complained to the Social Services Director, a lady named Maureen, and told her what had transpired and that I was offended by it. Maureen was very rude and actually made a joke about it and said that was how Sarah shows affection. Sarah continued to work there even after this. Also my Dad had given Sarah some peanut brittle for Christmas and after it was gone, she started bugging him for more. She was relentless to the point where he really became upset by her persistence. I also complained to Maureen about this but as in the other issue, she just brushed it aside. I had elected to do my father's laundry instead of having it done on site by their laundry service. My father's clothes started disappearing and to the point where we lost about a dozen articles of clothing. Apparently even though I had notified them I would be doing the laundry, his clothes were still being picked up, taken to their laundry area and then mysteriously disappearing. When my father finally left the facility and came home, I wrote Heritage Hall a short letter regarding his missing clothes & that I would like them back but they never responded. Plenty other facilities to choose over this one and I would certainly not recommend Heritage Hall.

    I was attempted to have my father admitted into their skilled nursing care facility and they kept…read moretelling me that a 2014 workman's comp claim was halting his admittance as it was documented as his primary care insurance. I spoke to Medicare who confirmed that this claim was NOT his primary insurance. I also spoke with the workman's comp commission, who faxed them all of the pertinent information for him to be admitted. They still swore this was his primary insurer, which it is not! He was hit while driving a work truck and they awarded him a lifetime award for his knee injury, which is out of his control. The only way to have this removed is to have my father engage in a negotiation settlement. He is in the hospital and has been pending discharge for days! How can he engage in a settlement when he needs to help! 48 hours of efforts for them to tell me NO and for them to be unwilling to help- TERRIBLE CUSTOMER SERVICE! Imagine someone without resources trying to navigate this difficult process, they should be ashamed-- they are supposed to help others, not make it worse!

She has been there for five days and has lost almost 10 pounds. Add that to the fact that she still hasn't been able to bathe after a week. We had mom enter on the weekend (first mistake-no choice) to a staff that was less than welcoming and a dismissive nurse that did nothing more than take vitals and push her way past us. Never introduced herself and only after complaining to administration did she ever manage a grunt. So here's our 92 year old mom, needing help and basically left to sit and figure out what's what on her own. She was never given a tour (different wing than she had before and things were considerably different); no schedule of meals until we asked! no menu and no considerations of her diet (cardio diet needed-no salt!!! and everything there is salty, cold and disgusting--mom really did not eat much while there except what we brought her and picked at enough to curve hunger pains when we could not make it)--and this was expressed to nursing and staff several times. She also spent the weekend with no TV (or distractions)--after my sister and I left. When someone is coming from the hospital and still a bit disoriented, this is NOT the experience anyone should encounter. The only positive: I will say that PT/OT seems to be good there.
