PLEASE READ MY STORY!!!!!!GET YOUR LOVED ONES OUT OF HERE!!!!!!!! i regret having my brother in this facility! Unfortunately i had no one to warn me. This place should be shut down! Im giving a negative star! Nurses DO NOT CARE OF THEIR PATIENTS. I lived it. No one told me about it. My brother was left so many times dirty for hours when he would go #2. ( due to health reasons) to the point he had to call me and tell me he was in pain with skin rashes due to not being cleaned. I went off on nurses many times, their excuse was short staffed...bs! They were on their phone, caught one cleaning her shoes with the cavy wipes! I told them if they are short staffed to get their charge nurse and have her lazy behind on board, not just there to oversea but to be a team player and HELP! He was left unattended many times! To the point that it started getting weird. I would call his phone to see how he was doing and magically his phone would be off when he was asleep towards his end of days. I wanted to put a camera to have proof but he passed away before i could have! I can bet my life he passed away due to neglect!!!!!!!!! When patients pass away and nurses are too busy or just on their phones they wont even know what time they pass...all they gonna say is that he was expired on so and so time and well thats the time they passed by to check if pt is alive... DR'S are so hard to get a hold of. They dont return calls after messages being left to nurses!! Plenty of messages !!! To the point i had to get in contact to their own managers at their clinics they usually come from, because at the hospital they just do rounds...and the manager still tells me, "well i cant control their fingers" after i asked why doesnt the dr. Respond to my messages. All staff is rude, they just help their own race(asian may be Filipino) with other people they are short, brief and try to leave the room right away...i would have to drag my brothers information out from their mouth (ofcourse with my brothers permission to obtain info) AT NIGHT, DOCTORS ARE ON CALL!!!!!! NO DOCTORS IN THE FACILITY!!!!!!! MY BROTHER PASSED AND THERE WAS NO DOCTOR ON ON SITE!!! I HAVE THE RECORDING OF THE NURSE THAT WOULD MAKE THE DECISIONS! I DEMANDED A DR. This is not a hospital it's an asylum! I was a patient for 5 weeks. I saw the primary doctor who was assigned to me only once. Zero lack of communication between doctors who saw me (internal medicine, physical therapy, urology, respiratory). The nursing staff were, for the most part, very kind and hard working, but they too complained about the facility and many of them did not have a good grasp of the English language and were difficult to understand. The food was served cold or, at best, barely warm, and rarely did it match what was listed on the menu slip that was in the tray. Items were in disrepair, the TV never worked correctly. I, as a paraplegic, require some medical items that were not provided by the hospital, yet were requested by the doctor/specialist contracted with the hospital. Therefore, I had to supply these medical items on my own. I was not a patient there, I was revenue stream for Kindred. read more

The one star is because Yelp makes you put something. If you want your family member to stand a chance at survival, DO NOT SEND THEM HERE. They will die due to lack of quality care and a subpar facility. My mother was admitted here on October 10, 2023 after suffering a stroke in September. Initially she received care at Arcadia USC and was on the road to recovery. Because she was doing so well, Arcadia USC transferred my mom to Kindred Baldwin Park for extended care. Once she came here her health declined rapidly and she passed away 7 days later on October 7th, 2023. I failed her! I knew this place was a problem from day one. The place is filthy, smells horrible and the staff is incompetent. I have attached pictures of nasty hair on the floor and a nurse in the ICU busy scrolling social media instead of caring for critical patients. The doctors are impossible to get a hold of and no one calls you to update you on your family member. If are reading this too late and your family member is here, do not leave them alone. Stay with them as much as possible. If not, they will be neglected. My mother wasn't turned regularly as required and was put in a room with the door closed while having active nose bleeds in a reclined position. She could have choked and aspirated had I not arrived when I did. I tried to get my mom moved but once admitted I was told by staff my only choice was to take her in my car back to Arcadia USC. That was impossible since she was on IVs. If you do nothing else please listen to me and from others on Yelp. This place should be closed down. I posted two unfavorable reviews in October 2024. I received the generic response of Matthew that the matters have been discussed with the administration and staff. My reviews disappeared. I was there due to severe blood oxygen desaturation on exertion. The purpose of pulmonary rehabilitation was to strengthen my core so that desaturation would be less extreme. This was a horrendous experience. I was sent here for acute pulmonary rehabilitation. I did not feel safe here. One CNA slithered into my room after midnight. I sensed the presence of someone in my room and bolted up in bed and looked his way. He said, "I was trying to sneak in on you." I thought that odd. What was his purpose for sneaking in? My gown was too small so another male CNA would take my temperature in my armpit so he could get a view of my breast. All other CNAs would put the thermometer in my mouth. I emailed my experience to Matthew and again it was conveyed to the staff. A large enough gown was finally brought in. I had no wounds but wound care came in to take photos on my admission. They took photos all over my body surface and in between my butt cheeks and other skin folds. Then they came back a few hours later and say the camera malfunctioned and they had to do it all again. Wound care came in multiple times each week to take pictures. This was traumatizing as I had no wounds to begin with and was not bedridden. The bathroom had no hot water when I first arrived. This room was obviously designed for one person but they have managed to fit two beds in there, with one side of the room (my side) being smaller than the other side. The staff kept pushing up equipment and pieces of furniture onto my side of the room, literally trapping me in the bed. One time, they did this and it had unplugged my oxygen line. Because I was sitting in a chair, I did not notice the oxygen was out until I stood to get up and had to immediately sit down. I called the nurses station at least 10 times to come reconnect the line, but no one answered. I had to text my family to call the hospital to reconnect the oxygen. The nurse that came in to reconnect me subsequently blamed me for the oxygen line separating from its connection even though it was from the abrasive manner in which the other staff had pushed up all the equipment up onto my side. Another time, the CNA ran over my foot with my tray table. One day, the staff pushed the tray table so forcefully onto my side that they knocked off my water pitcher and my personal incentive spirometer and broke it. The food was disgusting and looked like slop. The food was fatty, greasy, covered in sauces or gravies. Even soup was heavy. I often gagged when I lifted the cover and got hit with a foul stench. After two weeks (Oct 23), the medical staff decided that Kindred was not the right place for me. They said they are 70% medical/hospital and 30% rehabilitation. On October 28, I was asked if I felt safe there and I responded no and gave them a host of reasons. The main safety factors were the constant pushing over of equipment onto my bed side, the constant flow of staff in and out without a reason, the lack of monitoring on my blood oxygen saturation, the frigid temperatures produced by the air conditioning. and the lack of accountability of the staff members for their inattention to detail. I asked to take a shower and was told I could not because of my poor blood oxygen saturation levels. They said the physical therapist would have to take me but there was no time for physical therapy to do so. I was there for three weeks with no bath or shower. The wipe down with their cheap wipes (Stryker) left me feeling sticky with residue. I was stink. There were ants crawling on the floor, walls, and in the bedding. After some housecleaning, they were less but not gone. I completed documentation to have all the medical records, doctors notations, and treatments sent to me on my discharge October 31, 2024. It is now December 12, 2024. Nothing has been sent to me. I believe it is part of the coverup of saying the treated me medically in a way that they maximize their compensation from my health plan while mostly they would walk in, ask a question and leave. The only worthwhile part of my experience here was the physical and occupational therapy. read more

Some people are very conscientious with their job. When you're in different facility, you can tell when they're really the caring one. This was a small place and workers know each other well. Very friendly people from front desk, admitting, Esmeralda to the administrative , Mr. Lennin Lim and everyone I met were very helpful and professional . But then , there's always a rotten apple that ruined everything and she was the case manager in charge of my brother. My dad stayed there for 5 weeks, nurses kept changing/rotating and they were not warm to patient and family; my dad got several blisters due to incorrect IV; he also had bedsores when he transferred to a nursing home. Before moving to Kindred hospital Baldwin Park, my dad stayed in Alhambra hospital where nurses were a lot nicer, willing to spend time with you explaining whatever questions you might have. read more

They won't tell us how my Grandma passed away all they said was her heart stopped. And now that I'm reading the reviews now I'm even more broken because her blood was spilled all over the Floor from her IV the first day she was there and that was already a red flag. I think it's just odd how she passed away and it happened out of nowhere. She was only there for a breathing issue and had to be monitored often by nurses on her breathing. All I have to say is you people are messing with peoples lives and from what I see on the reviews it's happening way too much. Kindred hospital in Baldwin Park is terrible, I was flat out lied to about my fathers care. Although the nurses are helpful at times, The doctors are not responsive. My father was moved without the consent of the family to another facility, that my family had not even had a chance to visit. He had surgery one day and they shipped him out the next. The level of care is unsatisfactory and I am planning to file a formal complaint for the negligence and the lack of transparency regarding his care. Very sad, families are not supported her. I also would hate to think it had something to do with our race. Just sickening. read more

Three stars only because this place does have a contingency plan to feed patients. I wasn't feed one evening. They had no sandwiches, soup, or nothing else. Only jello and that's not food. Management sucks. Nurses are awesome. Tracey, Geraldine, Ashley, Rosita and all the others too. Cleaning staff. Food services are great too. Restaurant quality. Just don't deal with Agnus, Ramos and the director. Only three staff because of the nursing staff. If it wasn't for them I give them maybe one star. Even writing this is hard but needs to be said as others deserve to know before placing there loved ones in this facility Our family member recently passed in this LTAC we were called as he was de compensating to come in immediately which we of course did. By the time we got there to say our goodbyes (less than an hour) they had already bagged the body, eliminating the closure we can never get back. No one checked in on us in the room, even though they called us in to see him. So there we stood with a warm plastic bag, while the nurses joked around sitting in the nursing station. I've worked in healthcare for many years and of course seen so much. My heart broke for my partner who asked me if this is common practice. I down played everything, as the situation was enough and anger will not bring his father back. But shame on anyone one who calls themselves a healthcare worker who can show zero sympathy/humanity as this. I spoke with the nurse manager and could hope others are treated better. read more

If I could give zero stars, I would. I want to start by saying that I know often hospital's Yelp reviews aren't great because more often than not they are written by emotionally driven patients or family members and people are much more apt to write a bad review than a good one. I'm writing this, five months after my mom's stay, as someone who is not a "Karen" or one to normally leave a bad review. I have nightmares about visiting my mother here. She ended up at Kindred after coming out of a medically induced coma with pancreatitis, Covid, and kidney failure. Their Covid restrictions were never consistent and I felt like I was going to battle for my mom every time I walked through those doors. I spoke to Administration THREE separate times, in just the first two weeks she was there. From the negligent nurses, to the lack of charting and documentation, to the downright disrespectful staff, I would not wish anyone to stay here. It is just as bad as it seems online. Their Admin team seemed to care, but there was simply no follow up. I was only listened to once I asked them "what would you do if this was your mother?" For the beginning of her stay, she was unable to speak as she had been intubated. She was barely strong enough to communicate via a notepad and I can only imagine the horrors that happened then. Once she could communicate through writing, she expressed that many nurses were far too rough, called her names, and negligent in taking care of her. Many of the nurses are family members and this creates an AWFUL environment. There is no accountability for their poor actions. I witnessed nurses working on her and carrying on their own conversations, not explaining what they were doing to her or myself, and completely disregarding the fact that they were dealing with a human being and not a test dummy. If I ever said anything, they were disrespectful to me, as if it was too much to ask that they treat any patient with dignity. I saw this not just with my mother, but with other patients as well. Some days you would get a unicorn nurse, but it always ended up that they were the specialist, not the usual RNs. Because I spoke up on behalf of my mother and other patients, I was disrespected at the nurses station and it was well known when I was coming to visit. I felt like I was walking into a high school pep rally where everyone had just finished spreading a rumor about me (you couldn't miss me walking in, I was 8 months pregnant). THIS IS EXTREMELY UNPROFESSIONAL. It is simply not that hard to do your job well, remember that you are in the business of caring for human beings, and be respectful or empathetic to family members. You get paid to do this. You signed up to do this. We expect you to do your job. I am NOT one to make waves and certainly not one to leave bad reviews. But there were far too many negligent incidents for me to stay silent. There was an incident with my mom's roommate that I will not speak on publicly just yet as there is an ongoing case, but that should let you know how simply terrible this place is. My mother is still alive and fighting for her life everyday but since we got her out of this place, we are never going back. If you can help it, place your loved ones somewhere else. read more

This place is an absolute joke. This is where you take your family member to die. The level of incompetence and unprofessionalism is baffling. I wish I could have my father moved out of this place. None of the nurses or doctors communicate with each other. They aren't charting consistently, no one is coming in and shifting the patient. It's impossible to get the doctor on the phone. The Case Manager doesn't do her job! They basically want the money, so God forbid they attempt to help your family member move to a BETTER facility. For the most part everything that is expected of the employees is done well. There is always one in the bunch that is new, or slow, or that disappears. so expect that. Just be a grown up and take care of yourself, don't pity yourself because they forgot you - so call and get it taken care of. The best experience- RN's hard working, smart and helpful. Ray a patient advocate, the best!!!! Venessa the physical therapist, I did all my exercises because she had so much spirit. And sincerely believe that you were going to get better. Thank you! The wound care team was very bad. One gal that was not even a nurse lectured me on how much she knew and that she was educated by Drs from USC. I later found out that the wound care specialist that made the rounds was from USC. So that was the girls so called USC experience She actually told me that she was friends with my doctor for many years and she was helping me as a favor to my doctor. Thank you- I guess? On her first visit she was yelling at me harshly, and vey loud I had to ask her to lower her voice or leave my room. She was the worst experience I had. They forgot to change my wound dressing for many, many days, I finally complained. There was a lot of finger pointing on day three it finally got done. You have to be able to ask the nurse to assist with reviewing Dr orders. The RN's, LVN, and CNA's are all excellent people. Thank you guys!!! Background: 8 day stay, 200 unit. Overall Thoughts: This place is the absolute worst and should lose any type of governmental funding/license given their negligence and inability to comply with basic guidelines. Mask Wearing - I've noticed that several staff do not fully wear their masks. I'd say about 35% of staff don't wear their mask properly while in indoor spaces (ie nurses station, seating areas, walking around the hallways). Folks are either wearing them as chin guards, not wearing them at all, or just letting their nose hang out. Reminder: It's currently mandatory to wear masks indoors. Also, you're a "hospital." Positives Staff (Nurses & CNAs) - There are a handful of Nurses (RNs and LVNs) and CNAs that seem to actually care. While this doesn't mean the facility or their training have enabled/set them up to succeed, they seem to genuinely want to help. The following are staff members we've interacted with that either acknowledged our concerns, really tried to help find answers, and/or in generally had really positive attitudes: Alice , Elena , Chris, Avani , Charles and Nuha. Avani and Nuha were particularly awesome in trying to help. Doctor - Lung and Radiology Doctors were both wonderful. Lung doctor listened to our concerns, and it felt like he pushed to get the ball rolling a bit. Radiology doctor pulled up scans and talked me through the procedure while addressing all my concerns. Case Manger - Mai Tran was kind and helpful. Facility(Cleanliness) - While the facility itself is old and worn, it's okay. Negatives Nursing (General) 1)The outright negligence is comical. - Something as simple as charting/documenting output of drains wasn't done, even when I reminded/asked of this repeatedly. Even when the "good" nurses endorsed the need to. Consistently charting accurately was not done. - More seriously, the lack of charting/documenting concerns and changes with the patient led to an infection that could have ben prevented or resolved sooner. IE, I noticed something and brought it up 3 days in a row, it wasn't until I was outright crying and demanding to speak to doctors and supervisors that it was "all hands on deck." And by that time, it was too late. - One more example of not charting/documenting. I was discussing my issues with the supervisor and indicated lack of charting/documenting as an issue. We moved on to discussing how xx had happened and no one noticed. That it was a bit ridiculous that I'm the one who noticed. The supervisor said "what, I didn't read about that; it's not indicated here" I then showed him, physically and with photographs I had previously taken, proving that the charting did not occur and he was shocked. The nurse was charting the next day and asking me for help to recall - TELL ME this is not ridiculous. 2) Pressure sore. While the pressure sore was not caused by this facility, it got worse here. Which can only mean there was pressure on it. On several occasions I had to remind staff that being on the back/butt was not an option and the patient should always be on the side. Particularly on days when the patient was a little delirious as a result of the infection that could have been prevented if concerns were properly addressed and documented. 3) Repositioning, they don't do it every 2 hrs. I sat there for 6 hrs and they did it twice. One of the CNAs told me "we turned the patient 3 times last night", which is not every 2 hours. 4) Basic Care and Decency. A staff member, let's call him/her Jamie, told me that the patient had asked for a sip of water in the middle of the night and a woman had said no. Jamie tried to find this woman with no luck. Jamie also informed me that it wasn't just this patient, a patient in another room pressed the call light for assistance and this woman turned off the light without assisting, leaving the patient to sit in his/her soiled bedding for hours. Lady, just find another job that will fulfill you professionally. Wound Care - Their wound care team (while mighty in terms of staff size and years of experience) lacks appropriate supplies and certifications for a hospital/facility that uses wound care as a selling point. Their team cannot debris wounds, a basic care needed for patients with open wounds including pressure sores. When patients need this they call in plastics. It wasn't until day 5 of the patient stay that plastics was called (vs being debride every other day). In terms of supplies, I had provided a handwritten list (ostomy nurse from previous hospital was kind enough to write the supply list + detailed instructions) on day 1. Efforts were made to purchase items but mistakes were made and items were back ordered due to purchasing power. It's on Kindred to figure it out without impacting patient care. read more

Dont send your love ones here!! Please dont send them here If they aren't alert or terribly sick . They will only get worse . They place my dad in a room with 2 other people. With one TV. Nurses dont come by often to see the patient , only to pass meds . They dont reposition the patient . I was here form 11am till 9pm . My dad is in same position. He has stage one bed sores and his buttocks skin is completely ripped off due to diaper rash and sitting in his poop for hours . Terrible place , they say they are like hospital scene and bill Medicare hospital rate so I expect good care here . I wish my dad can stay at an actual hospital than this piece of crap . Nurses here don't know what they are doing . They dont have bed side manners . Not even the doctors. No doctors ever called me to talk about my dad situation. They only call you if they need consent. I asked a nurse where did they place the new dialysis catheter. He didnt even know. He had to go look at my dad and find out . His femoral catheter was infected due to poor care here . Nurses here dont know their patient even though they had them for 10 hours ! Maybe a couple is good but majority dont know . Alwaya say. "I'm not sure . Let me check the chart . " simple question like , has my dad been this tachycardia all day ? They dont even know !! I said roughly was it over 100 ? They said they have ti check the chart. My dad bassline is about 75 to 80 . I really wish were back at the hospital, not here. These nurses aren't focus on their patient . We been here for a month and this is my judgement of THEM. **Of course there's good reviews for patients who can talk and walk. But terrible if you cant talk or move because you are too ill. I'm speaking for my dad . He has tears coming out of his eyes . He cant moved because he had a stroke, lung problems, kidneys are failing etc . He needs alot of attention . Nobody will pay attention to the ones who can't talk !!! If I can take care of my dad I would !! Gosh , I feel really helpless . He has too many tubes all over his body. He still wants to fight through but not at this place !! My dad was neglected many times , he pooped and dried up onto his scrotum and leg. Charge nurse and cna saw it, she said that's sad . So went to speak with cherry , the nurse supervisor. She seems busy, and just told me she will talk to them . I question her , who's them? She apologize but it seems like it was to get me out of her face . She only cares about what she was busy doing then my poor dad laying there, lethargic because hes critically Ill. I believe in karma and everyone here who neglected my dad will end up worse. I cant wait for my dad to get well soon . It's been 1 month here and we want to go back to the nursing home. It's way better care there than here . Please if you want your family to die faster or get worse. Come here then and be evil like them . They'll contract more bacteria and neglected and cause more problems. read more

Horrible!!!! Non-attentive. All phone voicemails are full so you can't leave messages for anyone to call you back. My mother was there for 5 weeks and we didn't find out her doctors name until the nurse Tony told us. Would give zero stars but Tony was the only helpful person on staff. He deserves Patient relations staff was was useless. Nurses (besides Tony) were useless (told us to send a tablet down to FaceTime mom... nurses claimed they didn't know how to use it so mom never got to call us) ... needless to say my 74 year old mother. With no other underlying diseases and no covid died unexpectedly in their awful facility! If your loved one is there move them now before it is too late like it was for us. I changed my mind about this hospital. Doing the pandemic, when my grandmother was hospitalized here, there was no visitation. The staffs were very friendly and their contacts were very prompt. Now my aunt has long term issues and she is hospitalized here, the nurse is very unfriendly. There one nurse seems like she does not want to do anything. I have not spoken to a doctor. I had asked many times to have the doctor call me. No one call. They are also not following orders to get the patients better. They are supposed to do a lot more physical therapy (it can be done when she was in ICU), also need to taper her off the ventilator, but they are not doing it. We know who to call to get information regarding my mothers care and and guidance. When our elderly mother is ill in the hospital, we feel helpless especially during the pandemic. It was a rough start at this facility in the beginning of my father's care. I was not pleased that during my frequent visits to see him that he had fresh and/or dried poop that he's been sitting in for who knows how long. There's definitely room for improvement for that. However, when speaking to designated individuals to help listen to my concerns, prompt actions were taken and I'm forever grateful. The amount of kindness, care and the duty of going "far and beyond" that my father and I have experienced were impeccable. From the front desk ladies greeting you with a smile, nurse practitioners and nurses being professional yet bubbly, representatives being adamant with being proactive with concerns, physical/occupational/speech therapists just being great at what they do, and overall staff being friendly, it's a welcoming place. I would like to give specific individuals a shout-out but there's so much of them to name! It only speaks to the culture that they are constantly building and adapting. My sister DID die from the incompetent "care" she received at Kindred Baldwin Park... less than six weeks after she was admitted. She was sent there for lung treatments, but then she died of new, unrelated conditions: sepsis, kidney failure, and low blood pressure leading to heart failure... all directly due to their ignorant actions and inactions. I do appreciate that there were a handful of doctors and nurses who were heroic exceptions, but for the most part, this place was horrible beyond your worst nightmare. We likely don't have the resources for a lawsuit, which is what these monstrous people count on. BEWARE. read more

My brother had been transferred from one long-term care facility to another, and when he had been at the Kindred Baldwin Park facility, he needed to go into the ICU fairly quickly. The doctors and staff communicated with us carefully and respectfully over the phone, and suggested we come visit. My sisters and I flew in from outstate. We are all so very glad we did. Thanks to the ICU staff, doctors and full-time staff, the regular staff, front desk people, and everyone for caring for my often difficult brother. I saw nothing but love and respect in dealing with him and with the three of us, his sisters. The suggestion of music was wonderful. Although we didn't have a way to play anything, we hummed and sang to him a little. We appreciate the integrity you showed in caring for my brother. A big THANK YOU to all. read more

I don't understand the negative reviews. No one likes being in a hospital but this place isnt bad. My wife was there for a month for wound care and rehab and received good care. It's an older facility but clean and well maintained. Doctor and nursing staff was friendly and professional. Our case manager Agnes went above and beyond dealing with insurance and preparing for her return home. read more
