We had a great experience at Stonebrook Post Acute, where my dad stayed to recover from an injury. The care he received was exceptional. The physical and occupational therapists (Shilpa was fantastic) and the nursing staff went above and beyond. A special shoutout to Patrick, the CNA, who was always attentive and responded to his call lights promptly. I highly recommend Stonebrook for short-term stays. My mom just spent 20 days here. In her case it was a fall at home that required some aftercare to heal properly. I visited often and was always impressed with what I witnessed. And my Mom, she just keeps talking about what great care they took of her! Her room had her right next to a window that she opened every day. She had a dresser, a closet, and a shared room with a good size bathroom. The food was pretty darn good. She got to choose her meals and they all looked and smelled great. Now to the really important stuff. Her care. Outstanding!! She had OT and PT almost everyday! She wanted me to mention 2 of them by name. Chris was her OT and John was her PT. She also wanted me to include how kind and gentle the aids were to her. Every doctor, specialist, therapist, nurse, assistant, even housekeeping deserves a shout out. It takes a lot of people to make a place like this run smoothly while keeping patients comfortable and happy. When she was released her mobility appears to me to be even better than it was before the fall! Mom has had a promising beginning to a full recovery thanks to Stonebrook. I am so grateful. Horrible care. Our nextdoor neighbor was placed there for 3 weeks to get physical therapy after breaking her hip. On our daily visits, not once did we see them providing any therapy. Days went by without moving her for more then 10 minutes, which led to bed sores. She was eager to walk again but all the management could offer were excuses that they were under-staffed. How should this be a problem for the patient? Accept less patients if you can't manage your care. Seemed like a desperate money grab of a facility. And with a general lack of hygiene, the placed smelled of death. Our neighbor died 2 weeks after leaving because she was too weak to recuperate after 3 weeks of being neglected. I am sad and angry that we lost a good friend. read more

We were given 3 SNF choices by Kaiser and choose this one based on the reviews. I'm glad we did as it was very clean, organized and well run. The people were nice and nurse station always responded to whatever was needed. They seem adequately staffed and have Kaiser staff and a dedicated Kaiser office. We got weekly updates from a team of people. When my MIL was anxious, they put her in a chair bed by the nurses station so they could watch her. I went kicking and screaming and was very surprised by the quality of care as well as the quality of meals. They give you choices and if you don't like either choice, you can always have eggs another way, a sandwich or a cheeseburger. I was there for rehab and I chose to go back for my second rehab. arrived at Stonebrook Dec 29 6pm aftefr 16 days in the hospital for surgical hip replacement. Arrived via ambulane and gurney. Left with use of walker Jan 9. Received excellent PT and OT as I began to walk.CNA and nursing care was excellent. I lost 20 pounds while in the hospital. I was starving, the meals at Stonebrook were excellent and I began to put on weight. I am glad I selected Stonebrook for post acute care. As a side note, I was familiar with the facility because my grandmother had received end of life care there 15 years ago. She passed away Thanksgiving morning 2008. She was 101 years old. read more

The facility is impeccably clean, though understaffed; consequently, patient needs are not always adequately addressed in a timely manner.

My mother was there for a number of years. The aides did their best for Mom. When Mom passed away after a long illness and dementia, the social services director, Shirley Jackson, immediately orchestrated the emptying of Mom's room, without my permission. Money and irreplaceable mementos were missing when I went back--the day after Mom's death!--to clean her room. Also Ms. Jackson and the administrator, Yvette Ortega, pressured me into paying my mother's bill. Which I now realize I was not legally responsible for. Stonebrook has recently been sold. Ms. Ortega has been replaced by the new owners. I don't know if Ms. Jackson still works there. Maybe the new owners will manage things more ethically. I hope so! read more

My father stayed at Stonebrook for just about 3 weeks. I was referred here by a couple people, stating that the rehab here was top notch, and that is something my dad needed a lot of, so after doing a walk through and my own research, I decided it was a good option for him and his needs. It doesn't look fancy and doesn't have the newest facilities, but let me tell you, sending my dad here was one of the best decisions I made for him throughout this recovery process. The staff there is so attentive and knowledgeable. They are so caring, and let me tell you, they do their job perfectly. You might be able to dress a place up with the newest, most spacious, most modern facilities, but that doesn't mean they are necessarily going to care for you or your loved ones in the way they need to be cared for. Stonebrook did that for my dad in every way. If I had a concern, someone helped me, and no, I didn't have to wait 30 minutes. It was usually within minutes, and even when they're busy, they'll still take the time to answer my questions or print out documentation for me. I have been an acute care RN for over 10 years, and I was truly impressed with every aspect of this experience. I need to take a moment to point out specific people and there are so many so here goes: Maryiam was often my dad's LVN, and she was so sweet and smart. Grace was a PT that took so much time and care with my dad. She went out of her way to show me how to safely get my dad in and out of bed. Arnel, RN helped me with a late night phone call and did his best to field my questions. Maria helped me with making sure my dad had a menu for the week, which let me pick his meals, which then meant he ate a lot more food and got stronger quicker (they weren't getting his accurate choices before that, and he wasn't eating). This showed that they were mindful about making sure he got the food he wanted. Densley, RN, helped me often with my dad when he had complicated things happen overnight. Joy, PT, was someone who didn't have a lot of words, but you could tell she had a ton of pride in her work and cared for her patients immensely. Joel, CNA, was great with my dad and was always there to help with all of our requests to change my dad's sheets or bring him water. Wellington, RN, was a great charge nurse who always knew what was going on and was never too busy to help, even with the smallest things. Desiree, Tara and Glenna were all super helpful and made sure my dad's transition when he first got transferred there was smooth and as possible. I am a huge fan of this facility and could not rave about it more. The parking kinda sucks, and if you get a shared room, it does feel on the smaller side, but trust me, it's worth it to have competent staff and a small room/outdated facilities than having top of the line facilities and people that you can't trust to even take your vitals!! I have been on both sides of that with my dad (just look at my other reviews) and it is not worth it. read more

For years I sent patients from Kaiser ER for rehab care. Family members came to my Kaiser office sobbing because their loved one were not attended. Lights not answered ....recent example: 97 yr old woman in for IV therapy, no one answered her call light, she called again and told we are looking into it.... desperate she got up and then soiled herself and the floor .... a huge fall risk. She was told: if we don't come just poo in your bed!!! I called the management a week later as I was simmering with anger...and explained to the manager that I was DONE with hearing these stories happening at Stonebrook... So sent him to talk to my 97 yr old friend and I am sure that she BLASTED him. So as an RN for 45 yrs I know things are run by corporations and the staff try but work arounds are not acceptable. Let me get an executive and tell him or her to poop in their bed. read more

First- in a beautiful setting, and immaculately clean Almost all staff very caring and attentive- average call button wait was 10 minutes The physical and occupational therapy were a plus- after 10 days got stronger and had moving and transfer strategies to go home- and they also advised us how to modify home,and helped us select equipment. When home health came, found they were not necessary as already well prepared. As a Kaiser patient, experienced on site coordinator and doctor who seamlessly worked with doctors treating me, and arranged for transfer to medical appointments The food was surprisingly good, with only a few meals that were not quite appetizing. This is heartbreaking to write, but its Factual information to make a Conscience decision if you love your loved ones, do not take them here. After reading a few reviews here it pains me to relive our nightmare. We had a issue, two years ago and they still have not addressed it. They Killed my Mother. I have all the proof needed. Dr. T. C.& J. B. are the worst team of two medical professional I have ever run into. We made numerous calls to the doctor and we never had one, not one returned call to us. We took or concerns to the highest level, we had meetings we did everything possible taking it all the way up to administrative law judge his decision was that they were MEDICALLY INAPPROPRIATE I have all the proof you need..... I can't believe that there's still reviews popping up that this place is in business and still not taking care of people they way they should be after how many times they have let family's down with loved ones. Reviews here say it all. My Mother is dead today because of their lack of caring. I contacted their head office back East, to this day I am waiting for a response back from Mrs. L. C. Their head director of operations. What's up with all these people thinking they aren't accountable for their poor skills in the job they hold? Don't and I repeat if you love your family do not take this chance at this facility. It really should be torn down. The whole story from My Mothers case, is Heartbreaking, from them taking her off of Intravenous anabiotic's for four days when she was there to take them for 28. Was the first mistake they didn't catch till, I spoke up. Then the PT department deeming her permanently bed ridden and she hadn't even got the stitches outta her hand to be able to use her Walker again.. yeah it gets worse. Just don't do it it's not worth your family's life. Mom I'm sorry I couldn't save you, I tried these people just didn't care nor do their Jobs! Very impressive, I was there a week a few months ago and will be returning after my current hospital discharge for rehab. Very clean facility, the rooms look like motel rooms clean and neat. Walking into the facility it is clean, no smells. Very friendly staff. The food for me is a hit or miss. I'm not a big fan; however it is freshly cooked. The portions are very small so you need to ask for extra to get a full meal. I had my daughter bring me heathy snacks to eat in case I skipped a meal. I am still looking forward to returning. This is a top notch facility if you are searching for a place for a loved one. read more

It was great with minor hiccups meds didn't get there on time and not given left meds on discharge. Medication confusion. No fresh fruit or salad. Prompt attention but nurse was late with pain meds. Staff is great! Just have to stay on top of things. I will recommend. read more

I was in StoneBrook for 2 weeks due to a broken femur. The food was delicious and plentiful. The staff were friendly and efficient. PT worked with me usually 2 x a day. Although I never thought I'd need that kind of care, it was a great place to rehab. read more

My mom was transferred to this facility after being hospitalized for a few days. Due to Covid protocols I was unable to visit my mother but I called nearly every day. When admitted the staff contacted to let me know of the protocols and procedures. She received a lot of physical therapy, more than she received at Kaiser. They also did a care conference with me which seemed helpful. They lost my mom's clothes. Be sure to label everything and the parking can fill up fast. read more

I received excellent care and treatment while I was there for several months. I wasn't strong enough to go home after being hospitalized; Stonebrook was the perfect place and alternative for me. Everyone working there is compassionate and caring. Around the clock, their staff, nurses, therapists, maintenance, cleaning crews, kitchen crews, etc... strive to do their best every day in every way! I felt like my health & recovery mattered to them, by the time and effort they extended towards me. The food there is delicious (especially their baked chicken!) and varies from day to day. Most of the staff here try REALLY hard to give their best care for patients!!! The facility is consistently very clean. Bedding is changed often, and aides are attentive and responsive to patient call lights. The food is pretty awful institutional stuff. The activities vary, depending on Covid quarantine requirements. The nursing staff are mostly great! Kudos to Cornelia, Sonya, and D'asha, especially, for their care of my mom! The managers have really tried HARD to work with my mom re: managing her care and bill-paying; and Mom has been extremely difficult to deal with for several years. NB: The facility doesn't have an on-staff licensed clinical social worker. The Director of Social Services is probably great at finding placements for patients leaving Stonebrook. But isn't an LCSW. She seems more focused on facilitating the financial end of placements, vs. on patient care. That may be all that Stonebrook expects of her. The young ladies at the front desk who deal with patients and their visitors are phenomenal!!! Major kudos to Erica, especially!!! I do recommend this facility for re-hab stays. It's far better than some of the other local facilities. read more

Do not leave any of your love ones there! Especially if they previously had a stroke. The effort in trying to get the patient to participate in therapy is a joke. The therapist Rebecca either need replacing or they should have more than one therapist to tending to multiple facilities. She lacks patience and competency. read more

I can't say enough about the Stonebrook Facility and Staff. My Mother has been diagnosed with a terminal disease, and suffice it to say, this has been the hardest moment of our family's lives. Without the care, love and support from Stonebrook's staff, I don't know what we would have done. Let me share just one of many instances where a Stonebrook staff member's actions was more than I expected. Background: If you are visiting someone that is at the facility you can share a meal with them for $5. If you are able to tell the staff early enough in the day, they can get you a plate of whatever is on the menu for that day, if not they can make you a sandwich. You place your order with the administrative assistant at the front desk as you come into the facility. It was the second day my mom was at the facility, and I wanted to share a meal with my mom. I went up to the front desk to place my order, however the admin was not at the desk. It wasn't a big deal, I figured she would be back in a few minutes. So I decided to wait until someone came back. As I was standing there, a staff member whom I had never met, saw me waiting and asked me if there was anything I needed. I told her that my mom was here at the facility and that I wanted to know if it would be possible to get a PB&J so I could have lunch with her. The staff member laughed and said "I'm sure we can do better than that." Referring to my PB&J request (I actually love PB&J's but...) and called down to the kitchen staff and ordered me a tray, that would be served together with my mother's tray in her room. As I returned to my mother's room there are pictures hanging on the wall of the main hallway of the heads of the different departments at Stonebrook. I was floored when I saw the picture of the person who had just helped me and her title. It was Yvette, the facility's administrator. And that's when I knew we couldn't be in a better facility. If the head of the facility is setting the example by jumping in and helping take a lunch tray order, you know the rest of the staff will truly work as a team to meet the needs of their residents. I can honestly go on and on, with more stories about the staff at Stonebrook, but I will just end this review with this: if ever there was an extended family I guy could be grateful for, this is mine! My Mom was admitted to this facility and I have to say it's really gone down hill since my Mom was there 4 years ago! They are definitely understaffed!! My Mom is a good patient and was just at Kaiser Antioch. The nurses there were amazing, they took such good care of her. She had a intestinal blockage, she to have a double ostomy so she wouldn't burst which was very hard on her. She was doing great when Kaiser released her, she was bright and cheery and as soon as she got to Stonebrook she digressed I believe because of a lack of care. They definitely need more care nurses!! read more

My mother spent about a month at Stonebrook on 2 occasions followed by her last days on Hospice care. Here's my experience as well as hers... On the first two occasions for post surgery care, she had a wonderful experience both times. The rooms were clean, the people were compassionate and the overall feel was very pleasant considering what so many folks were going through. On the last visit, my mother was failing fast and as soon as she arrived everyone jumped into action to make her comfortable...I've never seen so much sympathy and caring. One of my close relatives was there for ~3.5 months. Never got a shower and we asked repeatedly and specifically got a waterproof neck brace for it (why he needed a neck brace outlined below). Was always dressed in a hospital gown (which he didn't like) when we specifically asked to have him dressed in his own clothes. They were tuning him over and he rolled out of bed, breaking a vertebrae and needing an ER visit followed by several X-rays and will need a neck brace for two months at least. He went to a board and care and they didn't let the board and care know he was coming, they didn't send any prescription or medication information with him. They just ordered a transport and dropped him off basically on the doorstep. I am so glad he is out of there. If I could give 0 stars I would. read more

My mother was there for about 25 days and during that time she called me several times so I could call the desk to get someone in her room to help. This is not the quality of care patients need to feel safe. I do not recommend them. read more

This is a HORRIBLE FACILITY! DO NOT send your loved ones here. My husband 66yr old dual transplant recipient was sent here., Because his primary Dr., Dr. Demonterio is the Medical director here.BAD MISTAKE. He just had a stroke and was dizzy and was still able to walk and talk. just needed PT to get stronger. My husband walked into this facility. He was hospitalized twice due to my complaints! He declined soOO fast here, and could no longer walk, eat on his own etc.... WTH. Not even 20 days. The 1st and only call I get was that his Medicare time was up since he was not improving and now I had to start paying them $450/day. ARE YOUR KIDDING ME. 2 days notice. They refused to feed him, every time I would go there, food not cut. I have pictures and videos. Not even on a mechanical diet like he was supposed to be. Nurse said it was a behavioral issue, she would not feed him until it "became medically necessary". Are you kidding me! Is she a shrink! told her to contact the Dr., they said he was totally depressed. I would be depressed to if I was left alone in a dark room all day long. They were still not feeding him, mind you he is also diabetic. I wrote a letter to the Administrator threatening to go to the media and mandated he gets fed, gets medical attention now! She refused my calls. next call I got they were admitting him to the hospital. When he arrived, he had lost weight, severely dehydrated! lost his ability to walk, and now possible organ rejection. He has now been transferred to Stanford Hospital. They still have all of his belongings, cell phone, clothes, electric shaver and all the food I brought him. NO one has called me since he has been discharged from there....DO NOT SEND YOUR LOVED ONES HERE! Family is now looking into legal action. read more

I'm surprised this place is still open. When my mom was alive she was sent to dialysis with a soiled diaper. Also when the ambulance dropped her off they gave her a room away and instead of putting my mom in another room they left her outside for the ambulance to pick her up and send her back to Kaiser. Finally when my mom had her seizure, I called 911 and the nurse refused me to call 911 for my mom. read more

This is my first time ever leaving a bad review. The people that took care of my mom were awesome and I love the care she received but the lady that works the front counter during the week in the day was rude several times. Today I went there because they forgot to give my mom her medication when she left so I had to go back and get it and the lady at the front desk made me go stand outside in 107 weather to wait for my mom's medication. I left and I decided instead of writing a yelp review let me try calling and telling her how I felt at that point a lady name Alicia answered the phone and told me that she had stepped out and that they are trying to keep people from hanging out in the front. Mind you there was nobody there and did I say that it was 107 outside. I have a real problem with this because this is a place where you take your loved ones and you already are having a hard time yourself leaving them there and to be treated like this is not OK. read more
