The facility billed my 101-year-old uncle $1,425, claiming he had remained two days beyond his Medicare-covered stay. However, a Kaiser-affiliated physician had extended his stay for an additional two days of observation after he returned from the Kaiser Walnut Creek Emergency Department. What should have been a simple correction became a lengthy ordeal. Despite numerous phone calls, emails, letters, and other communications with both Kaiser and the facility, neither organization initially acknowledged that the physician had authorized the extension. The matter remained unresolved for more than three years, including an extensive complaint process with the California Department of Public Health. Although the Department initially did not substantiate my complaint, it later reversed course and ultimately confirmed that my complaint was substantiated. Sadly, my uncle passed away in May 2024 at the age of 103 before the matter was fully resolved. More than three years after the disputed charges were incurred, Kaiser finally paid the $1,425 bill. While I received verbal apologies from Kaiser, I never received any acknowledgment or apology from the facility. In my view, no family should have to spend years correcting what should have been a straightforward billing error involving an elderly patient

Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.

I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan--they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I'd also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. We definitely had our moments--lol--but she was truly supportive and helpful every step of the way. I also want to thank Claire, the activity director. She was so lovely and caring, and she truly tried to make every day fun and different for all the patients. She recognized each patient's unique qualities and worked hard to make their time at the facility as enjoyable as possible. I have to mention the name of one nurse in particular...Trina. She really showed me, everyday, how caring she could be. All of the RN's and CNA's were excellent, but Trina was special! Obviously I do not ever want to need post acute care again, but I would only choose Shadelands again if I did.

We were happy with the care she received, that's why we brought her back again. First of all, it's a very clean facility, small and personal, but most of all great staff. The nurses my Mom had were professional & helpful. The CNA's were amazing. The ones I will mention were the very best. Mario, Smile, Rain, and one special CNA on swing shift named Michelle. Jessica on the weekends was also an excellent help with my Mom, Nursing director Teri was extremely helpful, always available to help. Vanessa social worker was also fantastic at assisting with so much. The physical therapists & occupational therapists were also great. I hope my Mom never has to go back, but if she ever did, Legacy would surely be our number 1 pick. Much love & thanks to the staff. I was sent to Legacy for nursing and physical therapy services, following Spinal Surgery. This…read morefacility was clean. The frontline staff was professional, kind, and engaging. However, I experienced a lack of preparation, an absence of communication, and a failure to reorder necessary medications --on multiple occasions. These negative aspects of my time at the Legacy stemmed from the Administration's systemic failure. Administration provided no orientation or introduction to the facility. Thus, I had no knowledge of visiting hours, food menus, or any patient rules. I did not even know there was an onsite Social Worker until the last week of my 5 week stay. I had to track her down to find out if administration had a discharge plan for me. I was initially provided a wheel chair with no foot rests and a bed with no arm rests. This was at a time when I was non ambulatory. Eventually, these medical items were provided. Still., when the facility ran out of my two heart medications (needed 2x a day) I wasn't told til the time I was to receive them that they ran out and had not yet reordered. It took 24 hours to get these medicines. All in all, my review of the Legacy is mixed.. I think if the Administrators strengthened themselves into a team that promoted clarity and transparency to their patients/clients this would be a very good facility. Patricia McGuire Lopas

I was truly impressed by how kind, compassionate, and attentive she was. She treated my godmother with so much patience, dignity, and genuine care. I waited outside as she got her ready for physical therapy, I quietly observed and It was clear that she takes pride in her work and truly cares about the residents she looks after. As a family member, seeing someone provide that level of kindness and respect meant so much to me. I visited about two weeks ago, and her kindness really stayed with me. CNAs have such an important and demanding job, and I wanted to take a moment to recognize someone who is clearly doing it with her heart. She is a wonderful asset to the facility, and my family is very grateful for the care she provides.

This facility is extremely disorganized, and I am deeply concerned about the level of care my…read moremother was receiving. She was admitted for rehabilitation following a stroke and has specific medical instructions that are not being properly followed. She is supposed to have thickened liquids without the use of straws to prevent aspiration; however, even after having a meeting with their section managers about my mom's needs, the staff were still giving her straws. Additionally, she requires assistance with eating, yet staff routinely drop off her meal tray and leave without ensuring she is able to eat or drink. When I visited, I can see her tray of food is untouched on the table that is not even in reach of my mom. She is not supposed to get up without assistance. There appears to be little to no concern for whether she is receiving adequate nutrition or hydration. During day 3 of my mom's stay (same day as issue above), I found my mother shivering uncontrollably and unable to speak. After I alerted staff, they checked her temperature and found it to be 104°F. She was given Tylenol and an IV - the nurse did what she could. A couple of hours later, I requested that her temperature be checked again. The assistant used a no-touch thermometer but quickly moved it around her face and neck, then stated her temperature was 97°F. This seemed highly unlikely given how recently she had a high fever. After the assistant left, I checked my mother myself and found that she still felt extremely hot. I asked the assistant to return and recheck her temperature. It appeared he was not allowing the thermometer enough time to register properly, as he was moving it too quickly. When I asked him to hold it in one place where she felt hottest, he then reported a temperature of 100°F (I did not see the exact number, but the device indicated a red light - a fever). Overall, the staff demonstrate a lack of proper training, coordination, and communication. There are cork boards in patient rooms that could be used to clearly display care instructions, yet they are not being utilized (photo attached). This would be a simple but effective way to ensure consistency in patient care. Her inability to eat and drink independently, along with her shivering and high fever, went unnoticed by staff until we brought it to their attention. At the end of this experience, we requested that staff call an ambulance to transport my mother to the hospital, as we no longer felt she was safe in this facility. I will not be bringing her back. At the hospital, it was discovered that her urine was amber in color and she was diagnosed with a UTI. As for the facility itself, the environment is outdated and poorly maintained. The air circulation is inadequate, and the overall atmosphere feels depressing. Improvements in both the physical environment and staff performance are urgently needed.

What I found instead was compassion, patience, and people who cared about my healing. The staff didn't just do their job, they showed up with their hearts. They listened when I needed to talk, supported me when I felt weak, and believed in me even when I struggled to believe in myself. Every counselor, physical therapist, nurse and staff member treated me with dignity and kindness, reminding me that I was worthy of care, hope, and a second chance. This place became a safe space where I was able to heal grow, and rediscover my strength. I learned tools that I will carry with me for the rest of my life. I left with a renewed sense of purpose and self-worth. The structure, guidance, and encouragement I received help and re-build not just my recovery, but my confidence and spirit. I will always be grateful for the love, support ,and dedication I experience here. This Rehabilitation center didn't just help me recover it helped me find myself again. I would wholeheartedly recommend this place to anyone who is struggling and searching for hope, There is real healing here. Every place however has it's cons and for me it was Mr.Curry his follow through was horrible. He could not get shit right. My other issue was with one of the dialysis tech Josh who was extremely rude at times.

The physical and occupational therapists (Shilpa was fantastic) and the nursing staff went above and beyond. A special shoutout to Patrick, the CNA, who was always attentive and responded to his call lights promptly. I highly recommend Stonebrook for short-term stays. Having had very negative experiences with other local facilities in the past, we were honestly terrified to trust another rehab center. Thankfully, several people recommended Tampico, and what a difference this facility was. From the moment we walked in, the building felt clean, welcoming, and well cared for. Every staff member greeted us warmly and treated my mom with dignity and compassion during one of the most vulnerable times of her life. Her care team was wonderful. Her CNAs were Helen, Marie, and Romel. Her PTs were Mimi, Deepika, and Nicole, and her OT was Aarti. Her absolute favorites were Helen, Emiliano, and Aarti. The therapy team pushed her in all the right ways while also taking time to educate our family and encourage my mom throughout recovery. When my mom arrived, she could not even sit up independently. By discharge, she had improved dramatically and was strong enough to transfer to John Muir's intensive rehab program, which prior she didn't qualify for due to her level of weakness. Most importantly, we never once felt like her needs were ignored. The staff consistently responded with kindness, patience, and professionalism. The facility truly felt like a home away from home while still providing excellent rehabilitation care. The only complaint my mom had was the food, especially the fish meals, but she is also a very picky cook-from-scratch person. We are incredibly grateful to the entire Tampico team and would absolutely recommend this facility to families looking for compassionate, quality rehabilitation care.
