What a place!!! Not enough stars, everyone is so helpful, patient and compassionate. I am an in home caregiver for the elderly and discovered this place. As a caregiver, I ask a lot of questions, want many scenarios and am firmly assertive as I am doing my job and protecting those in my care. Lakewood is nothing but kind and helpful. This office is one of the most reasonably priced offices I have dealt with, I have seen clients pay a lot more for less and will be recommending this office to all. I have been a patient since when this clinic location in Lakewood open. Had the pleasure of meeting the husband/wife. The reason why I chose this clinic is that I suffer with anxiety. The wife shared with me that she does too and this is why she have a different theme of pictures in each exam room to help patients to relax. All great staff -back then. Fast forward, I had a visit to the dentist March of 2024 to discuss a crown, whitening and scaling. Scheduled an appointment for June to have procedures done. Upon checking out, the front desk manager informed me I need to pay "xxx" dollars for my visit. I paid, $250 on this day. The next visit, I the Manager stated what I need to pay. I have Federal BCBS. I applied for Sunbit. Sunbit approved me for the entire work I had to have done. After that, the Manager continued to tell me I had a balance to pay. HOW? Sunbit paid you in full, now I have to pay them monthly. I phoned Federal BCBS and they informed me the Dentist cancelled her contract with BCBS end of March. Upon checking in, no one mention this, checking out, the Manager did not mention this Until, I mentioned it to her. At an awe, after asking for payment again. The check out Manager seems to be scamming patients for money. Then, they scheduled an appointment for me a Lakewood Dentistry on Normandy for a filling. As I am trying to have ALL this work done before my Wedding in October last year. Because I suffer with anxiety, the dentist phone in a prescription to my pharmacy. Take one pill one hour before the filling and the other 30 minutes before my procedure. This way I am calm and relaxed. I scheduled an appointment for teeth whitening with Lakewood W Univ. Blvd. After my whitening, the informed me again, that I owe a balance and that my insurance will be mailing me a check. Per the Office Manager, "once I receive the check" bring it to the office. I received an a detail invoice in the mail. Over charging me for the the 2 pills and services rendered! Not to mention, 3 weeks after getting the filling, it is no longer there! I am not HAPPY! I have this hole in my tooth and I cannot eat/drink anything too hot or cold! NO! the insurance company stated, # 1. someone should have informed me they were no longer in network with BCBS. #2 once the insurance company mail me the check, I can do as I please with it.. Since Sunbit paid them in full. I am not sure why the Office Manager insist upon me paying towards a debt that had been satisfied in full. I asked for the owners information and was given an email for the back office Manager. I really use to love this place...NEW Management are money hungry! Don't come here. They are scammers. Every time I come there is a discrepancy with price and what I was quoted. The dental assistants always try and blame the front desk staff which is wild to me. Considering the front desk is the one who usually works with billing and insurance. We went to Lakewood Dentistry in 2017; the facilities were nice, the location was new and convenient, and it was easy to get an appointment. My complaints then were only that I didn't like how they'd try to upsell you, that some of the staff was unprofessional (receptionist announced my balance due to the whole room!), and the dentist didn't check cleanings (like every other dentist I've ever been to). So, I didn't love Lakewood, but they weren't that bad either. We moved to a different part of Jacksonville at the beginning of 2018 and decided to switch dentists. From there, things just went downhill. We received a letter a WHOLE YEAR LATER stating that we had an unpaid balance from a previous appointment. No statement, no detailed explanation... just a letter stating that we still owed them money. This was unbelievable to me, so we called for an explanation... and were told that they couldn't find anything in their system that said as much, so it must be a mistake. They apologized, and we thought it was all cleared up. It's the middle of 2019, people, and we recently received a letter stating that they're sending the balance to collections. You better believe I'm "not happy." They claim they tried to text us multiple times about the balance... really? TEXT? Final verdict: Stay away. Beware. Etc. They might look good on the outside, but when billing practices are shady... that's a huge red flag. read more

I really wanted to give this place a five star review. Our first two visits here were very good. They have great facilities, very patient centric staff, the doctor really seems to be professional and know what he's talking about. We came in a week or so ago and the doctor showed us x-rays etc. showing that she needed a root canal. I told them that at that moment and I did not have dental insurance in place but that my insurance was going to be effective November 1... they said no problem and they scheduled the root canal procedure for November 2. At that time They gave me a quote of $1,100 of which I left them a 10% deposit. I figured with insurance that my portion would be about $600 - $700 or so. I received our insurance information and drove by their office to drop by the information a day ahead of the planned procedure so they could have everything cleared. When we arrived early this am as scheduled they said they were still trying to get hold of the insurance company. After insurance they said we would owe $1,100 today. They said the price when insurance is involved is over $2,000 for the same procedure. Seems they have one price without insurance versus with. I protested. My wife paid their higher amount. I told them that I was not happy, and that I felt they had mis represented their pricing. I hate bait and switch! Not coming back here again! read more

I had a tooth extracted a few weeks ago. the experience was not good and communication with staff including Chelsea wee non productive. I discovered that there were pieces of tooth in my jaw and was and still is very painful. The Dentist came to the room and put numbing meds in before extraction. He left the room and over an hour later returned to begin the procedure. In the mean time, I began to experience that the Novocain was beginning to wear off and feeling was returning. I advised the Dentist and he put more medicine to deaden the tooth then began the extraction. The next day day I felt that there were pieces of tooth still in my jaw and the pain was increasing. I called the office and several times and finally got in touch with Chelsea the Office Manager who advised me that I would have to pay $265 more dollars to extract another tooth that I declined to have removed upon my first visit. I attempted to relate to her that there was no other tooth on the side of my upper jaw to be removed. She told me that she was looking at the X-Rays and there was another tooth next to the extracted one. I still have shooting pain up to my sinus cavity and the remanence of the extracted tooth affect my chewing food. There was only one tooth that needed extracting and not another that Chelsea insisted was there. No effort was offered to bring me in to look at the area of concern and that I would have to pay for another office visit. I would NOT recommend Lakewood on University Boulevard to anyone. The Dentist (whose name escapes me) did not give me the feeling of confidence and was distant and appeared to be pre occupied. Although the price was acceptable but the results and the customer concern after the procedure left a great deal to be desired. read more

I've noticed the staff is trained well & knows a highly amount of knowledge on the work they do. Which is a plus. I cannot say enough about Dr. Z and Shane. I came in today as a new patient with a tooth bothering me. Found out I needed 2 pulled. They were short handed but he worked me in and was AWESOME!! I have a fear of dentist and have since I was a child. But Dr. Z, Shane and the rest of the staff put my fears aside and were terrific. I was offered a drink and blanket numerous times. They tell you one price you pay it then they say you still have a balance , never had this problem with my old dentist (who retired and moved my records to Lakewood). My first visit to Lakewood Dentistry was pretty good, as the office is nicely decorated and everyone was nice. However, they MADE me fill out a survey and give stars to them as soon as I sat down in the Dentists chair, before anyone but the receptionist saw me. She said that her pay depended on the stars she received, and looked over my shoulder while I completed the Survey. I gave 3 stars, as I didn't want to give a BAD review as I had not received any treatment yet. However, being pressured to give a review while the office you are reviewing is looking over your shoulder is not my idea of a pleasant office experience. I assume this is why the location has so many good 5 star reviews on Google. Also, I requested NO Fluoride and had to tell the Hygienist 4 times that I did not want the treatment. She just kept stating that "it is HIGHLY recommended". We get plenty of fluoride in our Florida water as far as I know, and also, my insurance doesn't pay for the treatment. When I went to pay/settle-up, the billing rep automatically charged me for the fluoride treatment ($25) and had to manually remove it from the bill (as I suppose it is assumed you will get the treatment and is coded that way). All in all, not a bad experience, the Dentist was good and the Hygienist, though rough, was very nice. My 2nd visit to Lakewood Dentistry did not go so well. First off they assigned me a different Dentist. Mind you both of my visits were only for cleanings. Again, the Dentist was nice and the Hygienist was great. Again no Fluoride, this time with no issues. However, a month later I get a bill from my insurance company saying that the cleaning was not covered. The Lakewood Dentistry office did not "code" the Dentist's Tax ID in correctly and therefore the insurance did not recognize the Dentist, even though they had his name in their system. This was a year ago and I have called at least 7 times, each time they say it will get resolved and each time I call they read the notes from the last call and say they will resolve it. It is still an ongoing issue and they tell me I owe money when I should not. I am now changing Dentists due to this billing issue, and in fact have never been sent an actual bill for the amount they state is owed. Also, I have requested my Bite Wings and X-Rays 3 times, and they have yet to send them to me. I am hoping this last call, made today, will address the billing issue and the records issue. read more

To the owner of Lakewood Dentistry Mr. Adam H or Sadie Shipman, On Tuesday 6,18, 2019, I had an appointment at Lakewood Dentistry for regular cleanup and I was super excited and looking forward to it due to the very good reviews I found on the internet regarding their services. And I chose this location even though it is about forty-five minutes away from my house by car. As I was going to enter my vehicle, I received a call from Lakewood Dentistry informing me that my appointment was cancelled and if I can come in at 10:30 am instead of 9:00 am, the previous scheduled appointment, and I agreed even though I had an important meeting at 10:30 am that day that I had to cancel to be able to make it to my dentist. When I arrived to my appointment at Lakewood Dentistry on time and approached the check-in desk, there was an occasion lady between the age of 30-40 years old that wanted to handle my check in documents. This lady asked me to fill out a new patient application even though I have been a regular customer to this location and this visit was my third visit. When I asked why, she replied with an attitude that I had to without any explanation and then she insisted to see my driver's license. I complied to her and handed her my drive's license and after she looked me up in her system, she stated that I have a due balance of $16 from the previous visit that I already paid at the time. I replied that the due balance wasn't a problem and if we can continue with the cleanup and I will pay after we finish. She said No you have to pay right now and called another African American employee that was working in the same day with her. The new lady stated the same thing that I must pay or they won't proceed with the procedure. I asked if I can speak with the manager in charge regarding the suspicious balance, they were so rude, disrespectful, and unprofessional to me and said no you can't talk to the manager and he can't help you. As they were insisting to not serve me and nor speaking to the manager, I felt mistreated and humiliated in front of another customer that happened to be there with her kids at the time. I felt that there was a plan from them to not serve me and kick me out and the $16 due balance was an excuse. I don't know what was the reason for them to mistreat and humiliate me like this. It could be due to my accent, skin color, or my middle name, which is Mohammed. I was really disappointed by the service I received that day and I feel that I was mistreated and humiliated and I really hope from the management to take initiatives in treating their customers in a better and more professional way regardless of their skin color, background, or accent. read more

Beautiful office and I had a great overall experience. I was able to make a same-day appointment and they are also open on Fridays. My appointment was for a standard annual + cleaning and I was seen from 3:20 p.m. until 5:20 p.m. with little to no gaps/wait time. Be cautious of any practice that does sneaky tactics to bill insurance and get more out of pocket costs from patients. TEXTING...billing statements, what the world is Susan thinking! Businesses will try and take advantage of people if you let them. BBB or Consumer reporting agency will be contacted about this clear and blatant billing fraud. The Billing person Susan is nasty and rude. I was referred here for my family of 4 but quickly realized it's more about how much money they can get out of you and sell services that you don't truly need, shameful. First, my hygienist reeked of cigarette smoke. And to be subjected to that for an entire cleaning period speaks to the standards of this facility. I literally became nauseous from the smell as she had just came from a smoke break before I came in. She was nice but come on Lakewood Dentistry! No different than monitoring hygiene of employees this is Unacceptable. Especially with the closeness of their body to patient. Secondly, we have a high and very good Federal military program insurance carrier and plan. How do I know, the front desk lady was very impressed with it and spelled out what they do versus what others dont cover. So this meant they'd try and "maximize our benefits" and upsell their services by doing unnecessary treatments. Third, my daughter had a tooth that was bothering her. I called and they got her in the next day. But what I didnt expect was even though she was due for her 6 month visit, they billed and treated differently, it was something she could have waited 3 days for. She had her wisdom teeth surgically removed by a surgeon 1 month before her 14th Birthday 4 years ago and apparently cause damage to her root of one molar. So here we were, seen twice in a day because of her schoolday. Then they said she needed so much extensive work. I requested a predetermination and it took 3 calls to them just to have it submitted to insurance. When they submitted, the treatment plan they had me sign was completely different. Teeth numbers were not the same as what was submitted to insurance. Not to mention the thousands of dollars they wanted to charge. This, too, became a red flag. I cancelled my sons' appts. The billing issue came in was that instead of doing a complete exam, they billed the limited exam, on a Thursday, Memorial day weekend, and brought her and I in 3 business days later for the exam and cleaning the following Wednesday. A few weeks later, received a bill, mailed to me, for the uncovered exam. Practices fully understand what they are doing when doing this. They fully know what will and will not be covered because they check benefits before a patient comes in. Not at anytime on second appointment did they say her exam would not be covered. We pay a high premium for what we consider to be excellent coverage for a reason, to avoid high out of pocket expenses. I talked with susan when bill received June 2019. MetLife also advised me to file a grievance because the supervisor in customer service questioned if Lakewood had disclosed that the 2nd exam would not be covered. I said no. She said that typically, that close of appointments, 3 business days later, providers should know how to bill without patient occurring any unnecessary out of pocket expenses. I filed my grievance with MetLife and found a new dentist for my family. Today, October 7th, I was forwarded in the mail a letter threatening to send 40 dollar balance to collections. 1. I assumed since I had heard nothing from them, it was resolved. 2. Susan and receptionist stated that they text and email Billing statements. I found it off to have received 1 bill in june and today Susan claimed they texted the statements. Well, my number is a landline, but receive texts through Xfinity app. But not with attachments and a size limit. I have received NO texts as Susan stated, sounds like this is common of them. Then she tried to get me to tell her my cellphone number as if I am stupid. I have always given my home phone as primary contact. Its business. And she tried to say my cell number was on the account. Too bad she wouldn't say what it was. Tisk, tisk, tisk! That is because she sat there, lying, saying that I was texted. Then she said she will send out a bill. She refused to allow me to record call. Even with a recent move, my mail is forwarded to me. Not 1 statement or notice was mailed since June. And to mention, I was alarmed when they sent the bill I june. I stated to her that I will get Xfinity to print all texts received to my landline as well as my Verizon cellphone texts. She eluded to the fact that I was the one lying saying that I never received text messages from them. I ALWAYS PUT HOME PHONE AS CONTACT AND I ALWAYS OPT OUT OF RECEIVING TEXT MESSAGES. At first glance and visit, they seemed like they were going to be my family's permanent dentist. But their fraudulent business practices over cloud any positive opinions about Lakewood Dentistry. read more

I don't understand how if they're participating with your insurance. Why would they charge you excessively?? I had to stand my ground and dispute what they were telling me and ultimately be proven correct. I didn't even get an insincere apology for the hassle they put me through. On Jan 08, 2020 I went to Lakewood Dentistry on University to get 4 fillings completed on my teeth. I have nerve damage and it takes an extreme amount of time and meds to get me numb. During this visit the Dr. was only able to complete two fillings due to pain and not adequately being able to numb my mouth. However, I had paid for all 4 fillings in full using my FSA card a total of $273.00. I now have learned that the total amount that I was told to pay included 4 fillings and a balance of $30.00 from the previous year that I was not advised of at the time of payment. I was due to return once they ordered and received a special tool and stronger numbing agent to complete the remaining 2 fillings on my teeth. I called into the office to follow up several times with no prevail. By Feb of 2020 I was contacted by my FSA administrator advising that Lakewood needed to refund me $182.80 back to my card because I was over charged for services once the claim was submitted to my insurance carrier and paid. I placed a call to the office and spoke with Tameka to advise of the issue. I was then directed that I needed to send in a copy of my EOB to show that I was overcharged in which I did. Tameka then called me back to tell me that they were going to issue a refund to my card. And this STILL has not happened. March 2020 Covid-19 hit and the office was closed and I still tried to contact the billing office and no response. My FSA card was then blocked by my employer for future use UNTIL my refund was administered back to the card. Lakewood was closed until mid-May 2020. Once I received an email stating that they were back open for business I began calling again to attempt to resolve my issue STILL no prevail. June 2020 I spoke with Tameka again and was told this would be escalated to Chelsie who is an Office Manager. I was told by Chelsie that she would issue a refund once I came in to pay $30.00 of a balance that was carried over from 2019 that I was unaware of in 2019 and was included in the initial $273.00 paid. I went into the office in June for a cleaning and paid the $30.00 and Chelsie advised she would be doing the refund that day. I came back to the office the next week for 2 more fillings only to find out that she STILL didn't complete the refund and I had a credit on my account. Once I reached the front desk I spoke with another person who then went and got Jenny who is Chelsie manager, she looked over the file and told me she would complete the refund that day. Now here is it 08/05/2020 STILL NO REFUND. I have made countless phone calls to Lakewood I have spoken with Tameka, Chelsie, Jenny, Ronald among others about this issue. I was advised it would take 4-6 weeks to receive a check since they can no longer refund my card because 120 days has pasted and I told Ronald this is not acceptable. I was denied to speak with the owner or someone above Jenny who I was told is over the entire University location since Jenny will not call me back or answer my call. Ronald advised he told the billing team to escalate and move me to the top of the list for issuing a check but still could take up to a week for them to receive and this was on July 28th 2020. This is beyond ridiculous. I don't know why I have to continue to wait on a refund that was due back to me in Feb 2020 and here it is now Aug 5, 2020 still no refund and no one has called me. read more

I would have given them a 5 but there is lots of improvement needed in their billing dept. I had a lot of work done so I had them pre authorize the work with my insurance company. It went thru and they did the work. Although their crown machine broke, it took a little longer, but I understand and that was out of their control. A few weeks later I am bombed every few days with texts saying that I owed them a balance. They gave me a number to call and did not answer the phone. After 5 attempts over a weeks time I left a message. I explained to the very rude person that there was no way that the claim was denied as it was pre authorized. She denied the fact and insisted that I pay it and was downright rude saying that it was not her responsibility. Long story short I called the insurance company and the said that the charges were pre authorized and a mistake had been made. She gave me a reference # and the pending claim is more than the balance they are trying to collect. The lady that I talked to the 2nd time was very nice when I explained what happened and she noted my acct. I wonder if I will get a refund?.. I wouldn't recommend them to anyone. They are terrible. I was not given a treatment plan at the visit because it was a Friday evening. When I called the following week, they tried to sell me some kind of discount plan that would cost me a monthly price. When I said, no thanks I am not interested. They refused to talk to me. I have been trying to get records and to no avail. I felt like the dental hygienist and dental assistant were sales personnel, and they were trying to push me into something that I didn't want. When I said no, the entire attitude and demeanor changed. It was so uncomfortable that I wanted to leave before my appointment was completed. Please do not use this dentist. They are rude and highly unprofessional. In addition to violating my rights as a patient. I have not been able to get records even with multiple dental release forms and despite me going in to their office. read more

I really like the service, but the billing which I received over 3 months after the service was the real rub. I was promised in-network pricing, however, I took the price subtract amount already paid and the invoice was about 250.00 higher than anticipated. I had to in addition put an appeal with my insurance because based on this provider being in-network the claim processed wrong. So, not only is their bill 250.00 high, they should have received better reimbursement from insurance. I try to talk with them, but always told an AR person will call. I only received one VM which included a statement that agreed with what I believed was the total bill amount, but the AR person left on vacation. I like to be timely on payment, so this is a real ordeal being 250.00 apart. I cannot pay immediately what I know I owe, because their invoicing system dictates what you are allowed to pay. On top of this I keep getting these invoices by text and email with no hope of any resolution. I do not understand the disparity, this is simple math! read more
