The intake staff, "Dianne" was adamant they do not take Medicare for residential rehab, our insurance department assured her he can be enrolled as Medi-Cal into residential. "Dianne" was not open to discussion, referring to a colleague or supervisor, nothing! The indifference and callousness was appalling, not to mention the incompetence (because she clearly does not understand how insurance works). I hope and pray "Diane" from Admissions will never be in a position where she is literally dependent upon certain people to do a little extra digging for her well being, God forbid she is ever placed in a situation similar to the one she placed our patient in tonight, God forbid!!!

I came to Pathways Venice Beach with very high hopes just a week ago but have been deeply…read moredisappointed and dissatisfied. I very much like the owner, Lea, however, my experience is that this is a much bigger project than one person can handle. She is around part-time and addressing the issues when possible, however she is often in another state and seems to be very hard to get a hold of when I have pressed for a refund, or at least a serious conversation regarding the problems I've encountered. The "house staff" are not appropriate for their positions and actively add to the dysfunction. When I came to tour the house before moving in, two weeks ago, the place had been cleaned up and tidied. From what the other residents tell me, this is the common practice. I did not realize, could not have realized, and, of course, was not told anything about the rat, cockroach, and flea infestation that have been on-going problems for months, according to some of the other residents. There is often dog urine and/or feces that is left, unattended in the common areas. I have needed to clean it up in my room a total of 4 times since I moved in last week. I watched and waited to see how long it would take for one of the "house managers", who was aware of the mess, to clean up a puddle in the one bathroom that 9 women share, and it was well over an hour. The house stays in a state of filth and disrepair. The house itself is very old and is basically a tear-down but being used to house as many women as can be packed in here. There are health/hygiene issues of which the health department should be made aware. The icing on the cake happened a few days ago, however. I came back to the house around 10:30pm to discover a huge rat feasting on leftovers that had been left, uncovered, on top of the stove. I immediately called the owner and told one of the "house managers" in person. I then watched the same "house manager" simply move the food from the stove to a different counter, where he left it to be packed up later. I woke up in the morning to find the rat-eaten food stored in tupperware and placed in the fridge ready to make a nice snack for one of the unknowing residents. I have asked repeatedly for a refund, been vocal about my deep concerns, asked to have a conversation with the owner and been either avoided or told that a refund is not possible. I get the feeling the money that I paid has already been spent. Oh, and you want to know the cost of this place? They ask $3500/month and I've been told by other residents it has gone as high as $5500/month. The rats are living well

My daughter was referred to Pathways and this is the opposite of everything a sober living should…read morebe and should be shut down. A nice tour of the home was created for her and she met with the owner who gave her an overview of the program and support she would receive as a resident of the home. However, the reality of Pathways Sober Living in Venice Beach upon moving in became apparent and was quite opposite of what was presented and promised by this 'sober living' home. Lea, the owner is a likable person, however it is difficult to say the least to create, operate and maintain an effective program when you are not on site 85% of the time and your house staff does not have the necessary skill set, maturity, training, education or experience to work with addiction or eating disorder issues or run a safe and effective sober living program. Lea travels out of state and often times she was extremely difficult to reach and would not return calls or texts for days. In her absence, the house is run by literally a 20 year old and her friend who act as the house manager and staff. Pathways is not a true sober living by any means, it is basically a 'flop house' runs on dysfunction and by no means promoting of a safe or sober environment. The home itself is in poor condition and serious issues were often not addressed properly. There was black mold in the home, plumbing was often an issue, bed bugs and rodents were reported while my daughter was there and the food handling is often an unsafe issue as well. The cost of the monthly rent is exorbitant, $3,500/month and many of the so called program components that were to be included as part of this rent were non-existent. This program takes complete advantage of parents that are doing their best and often sacrificing due to the monthly cost to try to provide a safe, supportive, structured and effective sober living program and environment for their young adult kids in recovery. Unfortunately, it's the clients who often may be vulnerable that are placed at great risk in multiple areas in this environment.

The nurses were very sweet, attentive, take their job serious. I felt as if it was their own loved one. The PT was top notch my mom was already standing up after almost not getting up in 3 months at the hospital. Everyone from admissions, case management, PT, OT, CNA, medicine nurses, nurses supervisors. I would recommend anyone who has a loved one needing a care Nursing Facility to Brentwood Healthcare. Oh, most importantly this Facility was always spotless. Thank you Marsha, Tara, Tamara, Jessica, Leslie, Victoria, Ian, Lupe, Ashley and all thr staff! Oh her roommate Irma was the sweetest! They tried to blame the transport company but that was a lie and proven as such. At discharge my husband, tagged gurney only, unable to move on his own, was lifted into a chair using a Hoyer lift, they said to take him to the medical transport van to bring him home. I was there when this happened and left so I'd be home when they arrived. I've learned since the gurney was supposed to have been moved. And they didn't take him to the transport van. He was left sitting in the chair for hours. They moved him into the rec room and simply left him there. In excruciating pain. He had multiple cancers and was coming home for Hospice. I called repeatedly asking where he was. Every time I was told he was on his way, over and over for hours. I was afraid to drive over there and not be home if he arrived. They kept saying he was in transit! I spoke to many different people. Later I learned he'd been calling out for me ... I was told that the following day by a social worker when the blame was still being put on the transport co. I had every intention of filing a law suit over this. I don't care about the money, they can't get away with this ever again. I haven't yet because he passed and I'm grieving. To be revisited. DO NOT BRING SOMEONE YOU LOVE HERE.

The experience…read morewas difficult but ultimately helpful--I needed stabilization after a month-long marijuana relapse and acute depression. Therapist-led groups were supportive, though tech-led groups were less effective. I learned a lot about advocating for myself, which I had to do often. My first month was marked by severe marijuana withdrawal: intense emotional swings, dysphoria, anger, sleep problems, nausea, and digestive issues. Staff and the nurse practitioner never acknowledged that I was in withdrawal, which made the experience much more distressing. During one crisis week, despite expressing suicidal ideation, I received minimal support outside a brief therapist session. I requested non-12 Step recovery meetings due to trauma, and while initially approved, chronic understaffing meant I was required to attend 12 Step meetings with the group. The required 1:6 staffing ratio was rarely met. This left high-needs clients without proper supervision, and at times clients were informally monitoring someone who clearly needed a higher level of care. Case management was also strained. Only one case manager served all five houses, leading to repeated paperwork delays and mistakes. Limited phone access made managing essential tasks difficult without more support. Accessibility was another issue. I disclosed mobility limitations before admission, but the facility had far more stairs than described, including at off-site meetings. Only after persistent advocacy were accessible options provided near the end of my stay. Food was challenging as a diabetic. Meals were often too carb-heavy, breakfast options were minimal, and the only accommodation offered was frozen frittatas. I ultimately purchased my own food. Overall, Clearview provided some meaningful therapeutic support, but significant issues with staffing, accessibility, crisis response, withdrawal education, and individualized needs remain. I stabilized largely because of my prior long-term sobriety, but I worry about clients newer to recovery who received far less support.

Unfortunately, the reality was very different. The facility appeared outdated and poorly maintained, and the level of care was unacceptable. Throughout his stay, communication from the staff was poor. We repeatedly had to call and ask for basic information. During the last five days of his stay, my husband became increasingly confused and incoherent. I contacted the facility multiple times, expressed my concerns, and specifically requested that a doctor evaluate him. My concerns were ignored. The most alarming incident occurred when my husband fell on a Sunday and was taken to the ER. The facility never contacted me to inform me of what had happened. Instead, I learned that he was in the emergency room through a message from my husband himself. At the hospital, we discovered that his infection had significantly worsened. Given the symptoms he had been experiencing and the concerns I had repeatedly raised, this was deeply troubling. The problems did not end there. The facility failed to send his belongings to the hospital, and when I later returned to retrieve his medications, I was given the wrong medication. When you place a loved one in a care facility, you trust that they will be treated with dignity, compassion, and proper medical attention. We did not receive that level of care. I cannot recommend this facility to anyone. Our experience was stressful, frightening, and completely unacceptable.

The building itself was clean and well-maintained, and it didn't have the typical odor that many skilled nursing facilities are known for. I know places like this can sometimes get a bad reputation, but that wasn't our experience here. While we appreciate the efforts of the staff and recognize the challenges involved in providing care and support, we encountered ongoing difficulties with communication throughout our family member's stay. Communication with the assigned case manager was often limited, and requests for updates regarding our loved one's progress and well-being were not always acknowledged or addressed in a timely manner. As family members, we relied on these updates to remain informed and reassured. The lack of consistent communication frequently left us uncertain about his status and concerned for his overall well-being. In addition, maintaining direct contact with our family member proved challenging at times. Multiple attempts were made to obtain information and establish communication, but responses were often delayed or unavailable. This created unnecessary stress and uncertainty for those of us who were actively involved in supporting his care and recovery. We also received feedback from our family member, as well as observations shared by other residents, indicating that some interactions with case management staff could have benefited from greater patience, attentiveness, and responsiveness. While we understand that staff members manage significant responsibilities, respectful and compassionate communication plays a critical role in fostering trust and supporting residents effectively. Equally important is the partnership between a facility and a resident's family. Families are often the primary source of emotional support, advocacy, and continuity of care for their loved ones. Maintaining open communication and working collaboratively with family members helps ensure the best possible outcomes for residents and strengthens the support system that contributes to their success. Family involvement should be viewed as an essential component of the care process rather than an afterthought. These comments are offered respectfully and constructively, with the hope that they may assist in enhancing communication practices, strengthening family engagement, and improving the overall experience for both residents and their loved ones.
