Debbie M. was very kind and helpful in directing me to the Outpatient Testing Center. As I arrived at the Patient Registration/Waiting Area I was greeted with a smile from a very kind and helpful Alex Arrioga. Alex registered me for the Electromyography Test. At this time Sue an EMG Tech took me to the room where she explained the procedure to me in a calm, compassionate, and clear manner. Sue responded to me in such a sweet and truly caring manner as to make me feel very much relaxed as she performed the 1st part of the test. Dr. Bassel Kazkaz was kind and professional and performed the 2nd part of the test. My most sincere thanks to you all. Steven S.

    I went to Silver Cross urgent care with chest discomfort and shortness of breath and they advised…read moreme to go to the emergency room at Silver Cross Hospital. After blood testing and imaging showed nothing heart related , the doctor thought I should stay overnight for a stress test. They put me in an observation room on the first floor near the ER. The first thing I noticed was a toilet right out in the open. Not even a curtain around the toilet. No bathroom, no sink with mirror, no shower. Basically just the exposed toilet, a bed, a trash can and a utility sink with a used cup and straw sitting there. There also was a large window, about 4' x 4' with no blinds or covering. If anyone walked past outside that window they could see everything in the room. When I went for a CT I was left in a cold hallway with several other people for about 30 minutes before I got the test. When I got back to the room I told the nurse there was no way I could stay in a room that offered no privacy and stripped me of my dignity. He told me that I didn't have a choice. I told him that I wasn't staying and to remove the IV port from my arm and get me the paperwork to sign myself out. Horrible experience. I've been to their emergency room twice and both times have left me feeling traumatized. You couldn't pay me to go back there.

We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Thank you LifeChoice for all of the help you provided my father and our family.

Their whole demeanor and attitude is that of angels, compassion, empathy, love..When he was in the hospital, he was not receiving the proper pain management, they kept delaying the hospitals hospice program while my dad was dying in pain. I was desperate to get him home. Alena, the administrator, moved mountains to make that happen. They got my dad home in less than 18 hours, they were by my father's bedside within two hours of me calling desperate for help, which felt like a miracle during such a difficult time. Daniel, the nurse who came to the hospital, was so attentive and caring such a such a genuine person. And once we were home, nurse Julie and nurse Grace took over and truly went above and beyond. They made sure the bed oxygen tanks supplies and meds was delivered before my dad even got home.They made sure my dad was comfortable, out of pain, and surrounded by dignity and respect--something he was not receiving in the hospital. there was also a Sweet CNA that came to help as well, Because of them, my dad was able to pass peacefully at home, just as he deserved. That means everything to me my father and our family. All of my father's friends and loyal family were able to come visit with him before he passed the next morning.. I highly recommend this hospice agency to anyone going through end-of-life care for a loved one--or even for themselves. Don't wait until the last minute. Have this hospice team set up ahead of time--you will not regret it. They are truly angels doing God's work. Shannon Johnson-parson

Facility design could be more senior friendly: common area seating is too low for people with mobility issues; hallways are long and dark. Maintenance guys are fantastic--prompt, patient, helpful. Administration is scattershot. Got multiple different packages of papers to sign prior to move in despite also having three face to face meetings with managers. Would have been helpful to see a checklist and timeline leading up to move in. Also would have been helpful if someone greeted resident on day one and went over his meal times, housekeeping, laundry, and bathing schedule And provided a print out of this information. Difficult to get accurate information related to health care staffing. No one seemed to know for sure the days staff nurse or geriatric doctor are onsite. No one seemed to know how the facility lending library operated in conjunction with Shorewood library.

Maintenance, activities ,CNA's, and the receptionists at the front desk were all very good. However, I am only giving the Timbers a one star rating because when she moved out they kept $1,000 of her security deposit. The only explanation given-Damages: replace carpet. After numerous attempts to speak to someone in charge with no one calling back, I gave up. I don't think it is right that someone on a fixed income should have to replace carpeting that just needed shampooing. Isn't there normal wear and tear after 2 1/2 years? The residents should not be responsible for decorating after they move out.
