Calling the State ASAP regarding a friend they refused entrance, threatened to call police while a patient. My friend has been through so much pain and suffering since. January 2019. Attempted to steal money from his bank via using bank card. Yes we know about it. And still attempting to bill insurance company ( Medical ) as if my friend is still there. No Mt. Rubidoux, he is in the hospital not at your location and has not been since you kicked him out with. You are trying to cover your butts, but we have proof. I'm tempted to contact channel 2news on your side. This needs to stop. And why when my friend was at your facility you and/or staff did not seek immediate medical care when your wheelchair that he occupied and pavement at your location was uneven causing the wheelchair to fall backwards causing my friends head to hit pavement. He should have immediately been transferred to a hospital for evaluation, making sure he was ok. This did not happen. What you have put my friend through is unforgivable and I personally can't imagine not reporting this. And get your paperwork in order. My friend is not at your facility and has not been since January 4, 2019, you kicked him out with no safe haven and upon his return from his doctor appointment. You deserve zero stars and people please do not take your love ones there read more

Too bad I can't give this dungeon no stars. My husband was placed there under what we were told as a Physical Rehabilitation facility. It turned out to be the exact opposite. I was wondering why some were calling it "gods waiting room". He was also a dialysis patient and would have to be picked up for his treatments three days a week, but they would not give him his required meds before hand. When I would question it, the nurses acted dumb. So bringing up our concerns to the social worker at the dialysis facility, she said, I know that place, get him out right away. So we did, they were not happy about it and said they would report us. They did, we got a visit from ASHS for a "wellness check". We showed them the bed sores my husband developed there since they barely did any physical therapy on him. Now my husband has recently passed and I still get visits from the ASHS because of these assholes. All they care about is money. Go to hell read more

This place is HELL on earth, I feel for the patients that have no family to visit them. My Mother was admitted here by a hospital that is affiliated with this place. This place is a insurance place scam! And I have seen patients attempting to escape this place, like if they are abused, neglected, not properly cared for. THERE IS 20 PATients to 1 cna...ridiculous ratio. My goal in life is to make sure places like this close!!! read more

If you have a choice I wouldn't bring my family here this place doesn't seem to care about their patients they let them wander off and not know where they go they let strangers in the don't even know anybody there telling inappropriate things to their patients and they let their employees and other patients steal other ppl things... and when I say they don't watch their patience I mean they let them take poops in the hallways on the floors and pee all over everything it's just it's just sad that they let things like this go on in these types of places you would think people of it do this type of work would care more about the work their supposed to be doing... I would never recommend this place for anyone or any other place like this unless you can afford a nice one for your loved one... sometimes that's all you can afford or your insurance will cover and it's sad I wish there was something more that I can do for some of these patients. read more

I read mix reviews online but decided to have my dad stay here since it was close to my house. No television when we arrived but within a day, they installed one for us. This place is The Number One place for u to send a relative (who u sincerely hate) to rot and die. I promise u this place will offer nothing but the worse treatment anyone can ask for! I got my mom out of there and SHE WILL NEVER GO BACK TO THAT DISGUSTING, PIECE OF SHIT, HELL HOLE AGAIN! She was neglected the whole time she was there. Her large suitcase was stolen, other patients would just roll on in the rooms and steal whatever they could. I've seen it! My mom hardly ever ate (mainly becuz she needed someone to help her eat and nobody bothered to see if she needed help), she was constantly dehydrated, she never got to take a bath, shower, or bird bath. Was never involved in any activities (don't think there were activities, except watching a movie in the large room), they gave her anti-anxiety meds to shut her up. She is a happy woman who loves making people laugh. She don't need no damn anti-anxiety meds. The rooms are too small to fit 2 beds, her wheelchair got stolen. She tried to escape 4 times cuz that place is so horrible! And Pacific Hospice is in that same category. If u have a loved one there right now, GET THEM OUT OF THERE! And if you're considering this place for someone u love, DON'T DO IT! This place doesn't even deserve to get 1 star. It doesn't even deserve to be available for the public! My father in law is currently at this facility. The staff is friendly. The facility is clean and they are helpful. The only negative experience was remedied quickly. I came in to visit one day and my father in law coloring books and colors were missing because they gave him a new night stand. By the next week all of his possessions were back in his nightstand. Annette in the business office has helped me so much! It is hard navigating some of these medical documents and Annette was a blessing. Everyone in the business office has always been helpful. My grandmother has been here at week almost and will be here another 2 or 3 for therapy . And my family had rotated shift here to be with her, the staff is so friendly especially the activity director and their team. They paint, play games, even have a fun physical therapy video that my grandmother loved. We were hesitant when looking for nursing homes especially when you think of the treatment most places offer but we chose this place because of all the active staff walking around. Sometimes it might take a minutes for the nurses to come when they have push the button for the call light , but it's because they are sooo busy . People are allowed to volunteer here as well to visit the elderly without family.And the food isn't bad here either , my grandmother actually eats all of it . If you ever have any concerns or request for whomever you might place here , they are on it to meet your needs . read more

My spouse is a current employee here and comes home every day disgusted as to the lack of professionalism and ethics and total lack of concern for the patients. Documents are falsified as to medication given that was not given, clogged G-Tubes, over flowing containers of used needles that has not been properly disposed of. Patients have to call the other Wing to answer their call lights. There are many call lights on the display board as the staff are sitting at the nurses station talking and playing on their cell phones ignoring the patients needs. Just a few days ago a patient had his call light on for several hours and wanting to call a family member, but was told that every one was too busy to help. He died without ever having to talk to his family. These are few examples of patient abuse. My spouse was told that the company does not like to pay over time and was told by several other staff members to clock out at their shifts end, but to keep working, I'm not sure how many laws that is breaking. It is very difficult as an employee that cares about the patients and yet sees fellow employees lack of concern for the patient's well being, using "that's not my job" and other reasons not to just help the patient and each other. My take away is do NOT put a loved one here. Too many things need to change at this "Facility", for lack of a better word, to make this "Home" a place you would feel comfortable having a loved one here. My spouse continues to fight the good fight at this place, cause she Cares!! read more

This is the worst facility ever! If you love your loved one do not take them to this facility. As soon as you walk in it smells very very bad, the rooms are outdated, the staff pretends like they care, but they don't. Our loved one had a stroke while at that facility and they had no idea, so they left our loved one in the room with no help for hours. Once my family member showed up to visit and realized something wasn't right, he forced them to call 911. When the fire dept and ambulance got there they asked to speak to the nurse in charge to ask what was going on and their answer was "I don't know, the family member told us to call you." These people have no idea what they are doing. They are not medical professionals. Do not risk your loves ones life by bringing them here. Once we got to the hospital, we noticed our loved one had a huge bruise on one arm and a small very dark bruise on the other and a swollen lip. Til this day they have not called to follow up, within days they had someone else in his bed. These people just care about the money. I hope they get shut down. read more

DO NOT COME HERE! The overall health care here sucks. the staff here is very rude. And today somehow they call me and tell me my grandma is missing they don't know how long she's been gone or who took her and when I arrive the police are already there and then try to accuse me of taking her and according to one of two of her roommates she was last seen rolling out of the room in a wheelchair. I don't know how the hell the stuff did not see Lady who can barely walk rolling out of the building in a wheelchair. Where the hell was the staff are they paid to watch them. How is it that no one knows where she went or what time she disappeared. Staff doesn't even seem to care that she disappeared I was there watch and as for me I am beyond pissed. How can they have no clue to when she possibly left. but for some reason no one knows exactly what time she's disappeared. Don't leave your things unattended here they will steal your stuff and don't know if it's the staff of a strangers because she didn't have a roommate. read more

Since the recently new administrative takeover this place has really cleaned up their act. I came here to visit the residents for Christmas and was pretty impressed with the place. I read the past reviews and things must have made a turn for the better since the new manager took over. I would be happy to have my kids send me here. read more

My mother has been in here for 7 years, it has gone down hill so bad in the last year. None of the nurses take accountability and they always pass the blame on others. If you ask a question about your loved ones care the answer is always ... I don't know... Or I didn't work that shift?! Where is the charting and follow up?? The doctors that work here are a joke and only come see their patients once a week. They over medicated my mother to the point of her passing out into her dinner twice. She went without pain medication for 4 days once due to miss management and with through serious withdrawal. My mother was sent to the ER and was never notified and when confronting the nurse... She gave me attitude and told me she was too busy to call me!! My Moms insurance sucks so this is one of the only places it pays for... The doctors are a big problem here... Lack of patient care... They do the bare minimum of state requirements to call this place a nursing home. read more

1 star is being kind. Choose anywhere else first. Yes, I spent 6 weeks in this place. I was Non-Responsive at first, and finally able to get around in a wheelchair on my own. When you depend on "Nurses" for basic needs this is NOT the place to end up. If you are stuck in this facility, make sure you have family visit regularly, or you won't even have a chance of being treated like a human. I don't speak Spanish, which gets to be a problem when the CNAs speak Spanish first (and limited English). The patients without regular visitors, suffered the most. The staff didn't even have to try to treat them decently, and they didn't. After a devastating Brain Bleed Stroke, I was sent here to recover. The PHYSICAL THERAPY team was very good, but if you needed an actual Nurse, or Doctor, good luck. 90% of the staff were young female students classified as CNAs (Certified Nurse Assistants) getting trained to become actual Nurses (at some later point) but patients are not respected or even treated like humans. If you needed help, you had a buzzer. Ha! Ha! Ha! One of the CNAs would simply walk by, stick their arm 18" into the room to hit the buzzer reset button, and keep walking. You only saw part of their arm. It was a game for them since CNAs had no recourse (it wasn't like anybody was watching who cared) It would always take 60-90 minutes to get a CNA to help you in an emergency. Otherwise, you wouldn't see your CNA until the next meal period (4 hours). If you went to the Nurses station to complain to a supervisor, your CNA would treat you worse than before. It wasn't like a supervisor was going to do anything about it. At night, the employees who really didn't care, showed up for work. NOTHING happened at night, and good luck if you had a problem. One night, I had to wait 11 hours to get a dirty diaper changed. I hit the buzzer 15 times (they reset it each time) Since I'd had a stroke, they assumed I wouldn't remember, and it didn't matter. I later found out I had a nasty Urinary Tract Infection during my entire stay. But since nobody was checking for it, nobody even noticed. There were usually 1 or 2 RNs or LVNs during each shift, but you would have a better chance of winning the lotto than seeing an LVN or RN in a patient's room. All of the Room duties were delegated to CNA's, exclusively. Sure, twice a day your medication was distributed by an LVN, but they would enter your room, leave a cup with pills, and scamper back out as fast as they could. One night the medication LVN (a temp filling in) gave me somebody else's meds. I tried to complain, but the RN just laughed about it (at the Nurses station). The staff weren't there to actually care. CNAs would drop off your food platter 3 times a day, as quickly as possible, swinging by 2-3 hours later to pick up an empty tray. I had an actual RN in my room twice, to help with a failed catheter, over 6 weeks. I never once had a Doctor stop by! I have to admit the food was pretty good, but the cook was from Spain. Unless you liked REALLY spicy food, you were out of luck. read more

I was extremely hesitant about my mom staying here to receive help because of the reviews I had read everywhere. I wasn't even going to consider them because of that but I was asked to call Rachael, who is the Director of Business Development. I told her my concerns and expressed what my expectations were for the care for my mom. She reassured me that a new management had taken over and put in money to have the place revamped and got rid of all old staff and hired new people. She told me whatever I needed for my mom to be ultimately comfortable there, she would do her best to make it happen. I went ahead and took her word for it and the facility was clean and staff was very nice. There was one staff member that was not so nice and when I heard of that, I immediately let Rachael know and she took immediate action to make sure that staff member was made aware that her actions were not appropriate for the field she was working in and got assigned to something else. I'm very pleased with the care that my mom received while getting rehabilitated and for the staff that was extra helpful to her. I know no place is perfect and they'll be some bad seeds no matter where you go but I truly do appreciate what was done for her. read more
