This was the second time that my husband had to go to this facility. Each time I am very pleased with the care and attention given to my husband. His case worker Ava is very warm and a pleasure to deal with. I found the nursing staff to be pleasant and very helpful even when I had to call the facility. cannot recommend Riverside Post Acute Care. My blind brother suffered greatly here and passed away on Saturday. Staff often ignored him, walked away while he tried to communicate, and delayed his medications even tho he was on hospice and had routine medication, leaving him in pain even in his final days. He was sometimes left in soiled clothing because staff refused to help him use the commode. A few RNs were sweet and genuinely tried to help, and the front office and cleaning ladies went above and beyond. But most of the other staff--medication aides and caregivers--were indifferent, inexperienced, or lazy. The medication aides often would not answer the phone, and I had to repeatedly call and sometimes speak directly to the RN supervisor to get any information about my brother. It was extremely frustrating and stressful. My brother endured unnecessary suffering, and I was left with trauma, anxiety, depression, and PTS from watching him suffer. Riverside Post Acute Care failed him and failed me as his advocate. read more

My mom was diagnosed with Guillain Barre syndrome at the end of 2024 and has been at Riverside Post Acute since January of 2025. I am so thankful for the care she has been given there. Ava, the Case Manager, as well at the amazing nurses, CNAs, and Physical Therapy staff (shout out to Kate and Tommy!) have been nothing short of amazing. Having a loved one go from being completely independent to almost paralyzed turns your life upside down, and the staff here has walked with us through it all. Ava cares so deeply for her patients and their families and is determined to provide nothing but the best care for them. The staff truly love and care for her as if she was a member of their family. I felt comforted knowing my mom was in such good hands during such a vulnerable time. I won't say that everything has been perfect, but any time there was an issue it was swiftly taken care of. My mom has gone from not being able to walk or move her hands to taking steps! I know she still has a long road ahead, and I am thankful she is at Riverside Post Acute. read more

I was Sent here and was told by the hospital that this place I was going to was a horrible place After being there less then one week I discovered my nursing staff and my dietary needs were met satisfactory but the I started physical therapy and discovered the most amazing PT Staff They had patience and working with me every day After less them 3 weeks I was walking I highly recomend it They are relatively new and have some glitches yo work out but I highly recomend Good care from a caring staff and they really want everyone to suceed when they come and stay to recouperat my overall rating is a 8 out of 10 read more

Probably one of the worst SNF/rehabs we have dealt with over the last few years for our grandma. I can only blame the staff for so much because I'm sure they are doing their best with what they have. The problem is, it's not enough. They don't answer their phone 80% of the time. When we were fortunate enough to reach a human we got mixed and FALSE information from nurses. By biggest concerns and complaints are regarding neglect, however. We began receiving concerning messages from our grandma who was having new onset of confusion and HALLUCINATIONS!!!!! We called to ask for someone to check on her and got no follow up. We tried calling back and got to answer. We requested she be tested for a UTI and there was no follow up for over a week despite our efforts. She had a fall in the facility that we were never made aware of (she is the only who told us and they confirmed) while waiting for the bathroom for a urine sample and there was no attempt after that. She was complaining of pain and messaging us that she would use her call light and nobody would come for long periods of time. Her pain wasn't being treated either. We ultimately felt like they were not able to provide her proper care but she could not return to her care home so we asked them to send her to the hospital to be assessed. She was sent to the hospital and directly back the same night with no follow up about the pain, hallucinations, nada. Fast forward to a week later and everything was getting worse, and mind you...still no results from the UA. She wound back up in the hospital and is now having surgery for a hernia (abdominal pain she was complaining of), a raging uti (that this facility NEVER TESTED ON), and a multitude of other issues. If you cannot physically be there in person to advocate for your loved one, do not send them here. read more

My Grandmother was here and I went to visit her and this place was disgusting! I actually witnessed a nurse yelling at a patient When my Nana went on hospice I went to see her and they denied me because of my non Covid vax. They didn't want her to get Covid they said and 1 week later she died from COVID! This place should be shut down and looked into by the police. do not have your loved ones stay here!!!!! read more

I received outstanding service from Case Manager Ava Tiangco. She went out of her way to ensure that my friend (patient) was protected and cared for. She responded immediately to all of my phone calls and text messages. Even though her workload was enormous, she call me after her normal working hours to make sure that I received a timely response. She also was on top of all the necessary details to help transfer the patient and even made sure that that a package lunch was provided during transport in case my friend got hungry. Additionally, when it was discovered that the patient was discharged but his medication was delivered after the discharge, Ava personally drove over 3 hours to deliver the medication to the patient. This kind of loving care and compassion is hard to find. I can't say enough good things about Ava's customer service. Honestly, she was the best part of my experience with Riverside Post Acute. read more

My mom was here for 3 weeks in September of 2024. My mother has dementia, and I know it is difficult, but they really dropped the ball. My mother was at fall risk, she ended up having two falls while staying with them. I don't know that they took her case very seriously, they did not even bill Medicare for her stays with them. Is as if she was never even registered to begin with. Very unprofessional, I was never able to get hold of registration or social services department. Nursing and nursing supervisor try their best with all of the patients under their belt, but it seems like there is no support/guidance from Administration. 0 stars. This place is an absolute joke of a "skilled" nursing facility. Every time 911 has to get activated because these clowns can't do their jobs, they're constantly scrambling to find someone to give report, can't figure out how to work the computer in order to print paperwork on a patient. If I had a nickel for every time someone said, "this isn't my patient" or "I just got here", most paramedics would be millionaires. Remember, if this staff works here, it's because they can't hack it in a real hospital. Incompetence is one thing, but having a bad attitude is quite another. If this place wants to improve patient care, they can start by firing their rude night shift charge, I'm sure there are plenty of real people out there who just got there LVN who could do a better job than her. If you have a relative in this place, pull them out of here and find a real facility. read more

This place is very disorganized. Sometimes they check you at the front sometimes they don't. On our first visit they told us to go in to visit my mom and we came in a group of 5. I called beforehand and asked if she could have children visitors and if the amount of visitors mattered. They said this was fine, we were there with her when a nurse told us she couldn't have visitors and especially not children because we were in a Covid19 restricted area. No one told us about this or else we wouldn't expose our whole family to Covid19. The last week my mom was there she told me that one of the nurses took all her clothes off for no reason. I noticed that when I would visit my mother all the patients had clothes on except my mom. There are so many things wrong with this place I am overwhelmed regarding which story to share. Bottom line is that if you want care for your loved one do not bring them here. read more

Riverside post acute is the worst place ever! I would not put my WORST enemy there. One day I called to address a transportation issues 16 phone calls no answer, the next day 8 calls and did not get a answer until 545 in the evening when of course the person I needed to speak to was gone for the day. I had serval concerns for my loved one I sent an email and as of this day approximately six weeks later still no response, not even to say I am sorry we received your email let me investigate Nothing. I sent a second email 1 1/2 weeks ago and ask that she follow up with me still no response. One day my loved one asked for hot water to wash themself up and the CNA brought a coffee cup with hot water. SMH. Residents will ring call light for assistance and it can take an hour or more to get assistance The communication is zero. They do not administer all meds, they are not in tune to needs of residents my loved one was sick unable to breathe told CNA to get Nurse, Nurse was not proactive and had to call 911 themself and nurse had nerve to get mad Come to find out my loved one had a blood clot in lung. Upon return took 2 days to get medication to prevent blood clots. I have reached out to DON, ADON and Case mmgt NO response Ombudsman has not been helpful I really do not understand how a place is allowed to stay open. Loved ones work their whole lives spending their life savings for care only to be given such crappy care. The facility even looks horrible and un kept I don't understand how the licensing agencies State , JACHO allow this place to remain in existence. AWFUL!!! Every time I call someone new answers the phone and every time they have no clue what they're doing, they must have an everlasting turnover of employees with next to no training. I've been trying to get paperwork to help my mother but either the email addresses are no good and when I do (answer machine continually answers) reach someone they transfer me into oblivion. I'm just glad my mother's dementia is so advanced she doesn't see their incompetence. I ask for 'certified' copies and receive faxes that aren't even legible. Everyone at postacute care is so wonderful and caring. They really made my mom feel comfortable and cared about.

A truly bad place. Do not bring anyone you care about here. During a brief 1 month stay, my wife ended up with 6 bed sores due to neglect. The CNA's would sit at the nursing stations and play with their phones instead of working. The staff appeared to try to steal my wife's phone, after she was moved from her room. Staff misplaced my power of attorney so I could not direct her care or request medical records. Staff blocked legal attempts to visit my wife and threatened to call police on me, for trying to see my wife. The nursing home doctor refused to treat my wife for a smashed knee and lied about getting a sonogram for the knee, instead said, she has osteoarthritis. Which is now disproven by the facts. When Donna was finally released after I saved her life from this Doctor sent home care nurses who where imbeciles, and who had to be fired. The Doctor is a evil greedy man, do not let your relatives be treated by him or stay at his dungeon. This "Dungeon", is still blocking attempts to legally obtain my wife's medical records, which is unlawful. I would put the photos of my wife's leg, and heel bed sores, but they are so ghastly that it is to revolting to see dead skin hanging off my wife's leg and the massive scab on my wife's heel, just despicable! Or the large crater like sore on my wife's anke. Just too Disgusting! The social workers were also despicable, even though Ramona Reh. expressed an interest in transferring my wife out of the "Dungeon", they would not take any action to help my wife. Very unprincipled, social workers who are supposed to protect patient rights, did nothing to help my wife. read more

The nurses and nurses aids seem to have developed compassion fatigue or they never had compassion for the sick and disabled in the first place. If your heart isn't into this line of work you should find another career. I've been here for 13 days and they have run out of my pain meds twice. they have a contract with a pharmacy in Los Angeles which totally doesn't make sense and unacceptable. They're a hundred miles away. The CNA's are very lazy and rude. You ask them for something and you never get what you need. They only make $12.00 an hour, maybe if they were paid more they would feel more appreciated and treat their patients better. It's annoying when you have two CNA's in your room at 6:30 in the morning talking real load. How would they like it if I went into their bedroom and woke them up and talked on my cell phone? I don't think they would like it do you? The DON is real nice, but I think the rest of the staff should have a meeting on sensitivity, and compassion training. I hear the workers talking about other patients which is wrong. They are breaking the HIPAA act. I see so many HIPAA violations that maybe the state would be interested in. I would call ombudsman but they always take the side of the facility. I only here for 3 weeks for physical therapy then I'm out of here. I'd give this 1 and a half stars. I wouldn't recommend this nursing home to a friend or family member. Thank goodness I'll be out of here next week. read more

I stayed here for 8 days. Sure they arent perfect, but their attendants are extremely nice, most everyone i met was professional, and i guess my only complaint was the food wasnt gourmet. Don't bring your loved ones here. The place reeks of urine the minute you walk through the front door. The patients are crammed in rooms 3 beds to one bedroom like sardines. Some have TV's and some don't. I went to visit my son and could not even visit him in his room because the urine smell was so so absolutely strong that we had to go outside. Horrible absolutely horrible. I told the staff about the man in his room that needed to be changed,but the hole time I visited my son outside on the patio,they NEVER did anything about it!!!. Riverside Post Acute helped our mom to recover. She came out in much better condition than she went in. The Social Workers were very helpful and attentive as well. They are very overwhelmed, but they do a good job taking care of their patients and they treat them well. They even helped us to set up a home health nurse. We are very happy with the services we received. read more

My mom was here a couple months ago for rehab and even though it wasn't a brand new facility, the staff were very nice. She got the care she needed and they were very attentive to our needs. The nurses were nice and answered any questions we had (this was my moms first time at a rehab facility). Although we're glad she's home now, she made some friends she misses. read more

I'm so impressed with this facility, especially after my family member was moved from another Rehab and Skilled Nursing facility that had been such a horrifying experience! I went to Post Acute Care in Corona prior to her move from her hospital stay as I wanted to make sure it was not like the other place. I was immediately impressed by the outside beauty...as you walk up there are flowers, grass and a waterfall. There were also residents in there wheelchairs outside many seemed to be enjoying the waterfall. Once inside the staff was incredibly friendly and helpful taking me into an empty room so I could see the type of room she would be in...letting me know that every room has a window! She hadn't seen the outside at the other place for over 7 mths! She has bed bound and can not even get into a wheelchair - so I felt it was imperative that she have a bed by the window. Now that she has resided in her new 'home' she has been so much more content and happier. She has so many challenges and I'm so impressed with this facility, as they treat her (and from I've witnessed other residents are treated) with so much care and kindness. I talked to some of the people there and have heard them say how much they like it there. They have Bingo games, communal dining and many other activities that make life more fun for the residents. I hope my family member can soon be a part of those activities after her Rehab. However, what impresses me the most is the dignity that is shown in this very challenging field. They are professional yet caring and after seeing what the other place was like Post Acute Care in Corona is such a relief for my family as we know our family member is getting the best possible care for her conditions.
