This place is absolutely horrid! My aunt was placed here for 3 years while we fought in court to bring her home. This facility was supposed to treat her dementia, to which they didn't. When we finally won custody of her, she came home with a prolapse bladder, severe UTI, intestinal infection and chronic diarrhea, and her dementia had only gotten worse. We would take her new clothes to wear, only for it to disappear a week later. I gave her a tote bag that cost quite a bit of money that she asked for, and that turned up missing as well. She slept on a gurney the entire time... all the while, every time we would visit, they kept trying to reassure us that "she was receiving the best of care". They charged over $13,000 a month for the lack of care she had received, a total rip off and absolutely disgusted with this facility. Please keep in close touch & take care of your loved ones... Heaven forbid the county find them and put them in this God-forsaken hellhole. read more

While the care was good, Amanda, the social, discharged my cousin to our house without our permission and unable to care for himself. She had permission to discuss his case with us yet wouldn't return phone calls. Just because he had an emergency contact didn't give her the right to send him to our house 90 miles away. He is out of his element and had to go to a local hospital for care. FIND ANOTHER SOCIAL WORKER IF YOU HAVE TO GO HERE. read more

This place is not that great, paperwork not filled out properly making us wait longer and it's no wonder they keep changing the name of the business. I give it a one star!They know nothing about patient care and unable to ensure the patient's get there medicines on time. Would not recommend sending your loved one to this facility! Also trying to call and get ahold of anyone is impossible for they don't answer the phones. Lack of training of employees. read more

My experience was beyond horrendous!!!!!! Only 3-4 days after I was admitted the staff blamed me for getting a staff member fired. I never once saw her or know her name. Next, they put me in a room with a female patient whose husband lived in the room with her and slept right next to me...yuck!!!!! Last and best was the night I had a severe asthma attack and needed to call 911. Not one staff member would help and call 911. I had to call 911 myself with 6 staff members just standing there like morons. The guy that runs the place came to see me and asked me to return to his facility!!!!!!! This facility now goes by the name of Yucaipa Hills Post Acute. BEWARE! Articles and clothes were constantly "misplaced"(stolen). Administrator/support personnel would not take or return calls: voicemails always full. Frequent COVID lockdowns because of infected staff, heavy turnover and refusal to give patients COVID boosters. Family member assaulted by another resident. Inconsistent billing practices. Everything written here is backed up by documentation. read more

Had a family member moved here to their COVID-19 rehab unit (despite our family wishes not to use this facility) Grandma was here for 3 weeks. Lack of communication is terrible. I finally had to advocate for my family and demand to speak to a supervisor or social worker. Not once did our family get a call from Dr Scandar. Social Worker Amanda not once reached out even after repeated calls to go over my grandmas care plan. Finally spoke to Judy (Case Manager and she was very helpful to get the ball rolling) My grandma said the nurses treated her nice but she was lonely. Sad thing is we weren't allowed to see her at all. This place doesn't appear very clean and unkept based on the appearance from outside. You are not allowed to just walk in, it's locked 24/7 and they will page you in. I would not recommend sending your loved ones to this facility, unfortunately this was not a great experience for my grandma or family. read more

Very friendly location SW Amanda is caring towards her families and CM Judy is also along with a very sweet DIrector of Nursing . When you walk in the front door you are greeted with a smiling person. I also like the location it's very calming and serene and there's a patio area to visit your loved one. read more

I am currently a patient at this facility. This is the absolute worst care facility possible. The facility is run down and filthy. I wouldn't feed the food to a dog. Today I received a box of wilted iceberg lettuce with nothing else on it and a bowl of canned fruit salad for dinner...nothing else. The average response time to pushing your call button, especially at night, is 30+ minutes. Tonight it took 2 full hours for someone to respond to the call light. I have been here for four days and no doctor has ever come to see me, no one has ever taken my vital signs, no one has even looked at my wound (broken leg, due to being hit by a drunk driver while bicycling). Fortunately, I am mobile enough to be able to get to the restroom myself. The woman in the bed next to me was repeatedly left to lie in her own feces for hours on end...for DAYS. This is not an acceptable place to send even your worst enemy. Someone needs to close this place down for good. I figure I will continue adding to this as my horrific stay continues. This morning, they tell me no nurses showed up, so I can't have anything for pain, even though I am only asking for Tylenol. Breakfast arrived ice cold (eggs and potatoes) and no beverages are being served. So since this review is still open, I guess I will add more. For lunch today, they brought me a cup of raw spinach and a cup of raw carrots...nothing else, and about a quarter of the spinach was rotten. Total calories for lunch...59 calories. Going to try to add a photo of it. Well, here we are one week later. Thankfully, I have been discharged to home. I won't bother you with all the other ridiculous things that continued to happen for the duration of my stay ( but on the weekends, I guarantee your loved ones are being completely neglected and possibly even abused). I will however, tell you about their gross oversites. My surgeon faxed over orders for staple removal and discharge to home. I'm home, but I still have all those staples because no one felt the need to actually read the patient care orders. They had also transported me to a facility about 30 miles away from where I live with the promise of transport home, which they couldn't come up with for FIVE days. The high points of this place are the Physical Therapy staff...every one of them exceptional, especially Eddy, the young woman who brings beverages is exceptional, and a select few of the nursing staff...Lorraine, Emelda, the woman with the super long eyelashes, and one other lovely day nurse who I never got a name for. Choose wisely when sending your love one somewhere. If I could give this a negative stars review I would. read more

My friend was in the dementia area. My concern was with lack of communication. It took repeated phone calls from her conservator to get the TB test, Covid test and Dr. paperwork filled out so we could move her. Also received notice from Medicare that she had been seen months ago in the hospital with a head wound and xray of legs, etc. No notification about the incident even though I had been able to speak with a staff member finally a couple of times. This place is not a good place to place your loved one. The social worker Amanda is very rude and does not keep her word on calling you back - she does not really care about the patients or the family concerns. My relative has had a man wonder in her room without supervision and harass her. The staff does not keep up with the demented patients and the less demented are terrorized. They do not change the patients - the patients sit in their diaper all day and are smelly. The place smells and most of the staff are not skilled and don't care. They will also hold your family member hostage when you want to move them by influencing their Dr. not to release the patient. There is something really wrong and just go visit and see especially the Secured dementia facility in the lower level. I will make all the noise I can. read more

I absolutely love the occupational and physical therapy department! They work so hard to get the patients needing rehab strong again and able to go back home. Yumi and her staff are always so happy and patient. They love what they do and it shows. read more

The staff here is passionate and dedicated to the patients. I really appreciated the social service department. Amanda was kind and very informative with all the resources available to us and Judy made sure to update us with our family member's care. We even received a follow up call after discharge. Staff here is nice but not efficient, with in 4 days family became so ill she stopped breathing. This was after repeated requests to have her cough checked by a doctor. They ordered a chest xray and stated it was done and showed nothing. She stopped breathing on her own and had to be intubated within 24 hours of them saying there was nothing wrong. Also, every request had to be made 3 - 4 times before anything happened if it ever happened. Very poor unprofessional treatment here. read more

This is the worst place IVe ever witnessed. This place did not have running water for three days, they had to cut the water off because stool kept coming out of the sink. The facility bought a bunch of bottle waters and told us to use this to flush the toilet.. I wonder how they washed the patients sheets or cooked their food... during this time, they made the patients eat canned tomato soup and grilled cheese. For 3 days...? I've witnessed the staff hitting patients.. the dementia patients have sex with one another, and keep passing std's around but they tell they document it as a uti.. they have some patients who have friends that come in the middle of the night and give them drugs, hard drugs, management is aware of this. Some nights there is not a rn in the building and Iv meds get hung, who gives them? At night everyone is sleep. The background check is so bad and cheap, they hired people posing as nurses and didn't find out until weeks after the nurses were working. The dsd was fired for sending staff info back to her country where they steal people's identity, and ruin their credit. All this isn't the half, if you choose to keep your family member in this place, you are saying u hate your family. Drive a little further and put them in redlands or San Bernardino.. read more

This place is horrible. Had my dad in there for 5 days.... In that time he was not fed right, they gave him wrong medication, they went against Dr. Orders and gave him coffee and caffeine.......Mind you he has had 3 strokes. They did not change him and he smelled of urine every day. They watched as he bled from his arm and made us change his bandages......They did not test him for anything. So we decided he needed to be moved. When we arrived he was incoherent and unable to talk and had a fever... they did nothing for that. They made me take him to the next place and would not transport. When I got him to the next place his fever went to 107. He was transported to Loma Linda. They stated he was not cared for at Calimesa. He ended up with a severe UTI. He would of died had I not stepped in and made them call AMR. Now he is in worse shape. Do not take your loved ones here. They do not give a crap about them. read more

My mother in law was in the facility for one night and that was one night too long. She was brought over after a stay in the hospital because she was weak and could not walk without assistance. The room was dark and dingy. The mattress on the bed looked like it came from a cot and not clean. The furniture consisted of a old cabinet with a broken drawer, old dresser and a bed. She asked for a chair so she could sit. The arms on the chair were sticky and felt dirty. There were tiles missing on the floor and looked like a cheap motel. The bathroom floor looked filthy and the shower was not much better. My mother in law is a diabetic and for breakfast they brought her cottage cheese, a quarter of banana that was turning black and a peanut butter and jelly sandwich. She can't have jelly. No concern for her diabetic diet. My husband and I arrived at 8:25 in the morning for a visit and she was standing, dressed, packed and ready to check herself out of the facility. She was not staying one more day and wanted to get home to her clean apartment. The staff told her if she waits to speak with the doctor they can send her home with her meds and oxygen. Because the doctor would not release her and she left with a AMA (Against Medical Advice) he would not give her the meds and oxygen. He told her there was a couple of reasons he didn't want to release her, the first reason is he would not be paid and the second reason was a good chance she would not make it. Hopefully the first reason was his attempt at a bad joke. When I finally got the staff to bring the paperwork so we could take her home I had to ask for a wheel chair which never showed up. She actually walked to our car which was quite a walk for someone that had not been able to walk much in the last few days. I guess motivation is all you need sometimes to get you on your feet. The administrator stopped her in the hall as he questioned her about her stay. She was standing the entire time during the conversation and the last thing he asked her was if the place was clean and the food was good would she be willing to stay. Her answer was NO. read more
