Creekside Care Center - Getting ready for Cinco De Mayo Celebration!! - Rehabilitation Center Near Me - Yucaipa, California

I've had enough...This place is a joke. It's been several months of nonstop bull. There are some staff members who are great like the two ladies I've met at the front desk. The PT staff is great but the nurses/cnas leave much to be desired. There is maybe a third of the staff that's great and on it but the others need to be held accountable for their actions. Weekend and night staff will leave patients sitting in their own urine or feces for long periods of time. The social worker onsite is rude and only professional in front of other people. She's very passive aggressive with comments. They come and take stuff out of patients closets and give it to other patients to use twice I've had to retrieve my family members jacket that has their name on it from another patient or the staff will use the patients personal items and steal their personal drinks and drink them. When the upper management leaves on the weekend and at night the staff gets really loud shouting to each other and laughing up and down the halls when patients are sleeping. Counted several times over the last several months where they leave patient lights on and don't respond for close to 30-40 minutes. The response given to me was they are all on break.(everytime?) it's not just been the one night or day. Wonder how many patients have had medical emergencies that they don't address right away. They feed diabetic patients nothing but sugar when their restrictions/preferences are listed at least from what I was told from other patient's family members and from my own observations, are they trying to send them back to the ER. Or the patient whose daughter told me has diverticulitis and they feed her items that can cause flare ups. The cnas come and take pull ups and wipes from other patients because according to them there is not enough supplies. They do this while in the middle of cleaning others up. That's a nice way to cause cross contamination and cause other patients to develop rashes/possibly catch diseases. I'm sure the CDC, OSHA and the state would have a field day. I'll be sure to be filing a concern with the state. read more

I would give zero stars if possible. The staff, specifically social services is horrible at getting back to people. Overall, they are horrible with communication and keeping you informed on the whereabouts of your loved one. At one point I had to call around to multiple places to figure out where my father was. Also, when he was transferred, I wasn't notified by them. They didn't even contact me to pick up his belongings he left behind there. I had to hunt someone down to make that happen and still am waiting on one more thing to be sent to me via email and have been waiting for almost two weeks. When I physically went to the location to pick up my dad's belongings, nobody was up front to greet me or check me in. I had to wander around the building until I finally pulled someone aside because absolutely nobody acknowledged me. This place is super unprofessional. I'm still trying to figure out why they charged my dad's bank account, or how they even got that information, when insurance should have covered his stay. He was there for less than a month and they have caused so much headache at this point. I can only imagine how bad the quality and type of care is considering everything I have experienced. I strongly don't recommend this place for a loved one. read more

Because of the negligence of the nurse's staff is the reason why my grandma passed away.... They didn't bother to do better.Basic assessment on her, she was severely impacted, and by the time she went to the hospital she was on life support in the icu.... if the nurses actually did an abdominal assessment and saw that she was distended, they should have been able to send her to the hospital days ago.... Then they had the audacity to lie on their documentations and say that everything was fine with her, but yet at the hospital she was septic... her urine was the color of prune juice... my grandma would often call me and tell me that her call light was on for hours and she was sitting in her feces for hours... i would have to call the office and ask them to check her... this place is filthy, and the staff is horrible read more

Initially,my Mom was taken care of when she first got there for her seven week stay after a stroke. It was the only facility that had an open bed and no known Covid cases. But gradually it became a nightmare! After my first visit there it broke my heart to see my Mom in that dump. We were only allowed to visit our Mom in the lobby with an " appointment". New clothes ( Christmas gifts that disappeared) glasses, and lotions were stolen. Constant Covid " lockdowns" were issued so my poor Father had to sit in the rain outside my Mom's sliding glass door to actually just look at her. No touching, hugging or speaking to Mom resulted in my Mom withering away and just wanting to die. Patients " escaping" the facilities, locking themselves up in the visitors bathrooms, I called it the " Cookoos Nest". I brought the nurses station a 5 lb. box of See's candies for Christmas and receptionist ate it while we visited with my Mom in the lobby! They changed my Mom's room at least three times in the short time before we got her out of there. It was only after filing complaints to the State Board that family was able to begin visiting our Mom. The last straw was when my Dad was visiting my Mom and she asked to use the bathroom. Staff wheeled her back to her room. My Dad waited for over thirty minutes for her return. He finally asked what was taking so long. The manager came out and said they dropped her while transferring her back to her wheelchair and were waiting for an ambulance to " get her checked." She was ok but it was just " procedure." They didn't even bother to let my Dad know about the incident until he asked! Thank God my Mom was ok and she is finally home now surrounded by love and getting stronger. If you have a choice, don't leave your loved one here. Sadly, if you don't have a choice, stay on their asses and report them to the state if you have any concerns. Oh, did I tell you my Dad once saw the owner or someone in management arrive to work one day in a limo? Bad look don't you think? read more

Do not go here; I would not trust them at all. Run the other way! We took our father here after surgery and they kept him doped up on drugs. He was sleeping 24/7 and they would bring him out in a wheelchair to us while he was out of it! He could have gotten hurt! They never performed wound care to his surgical site and it got so infected so fast he had to go back to the hospital or he could have died! They never helped him with getting showers or brushing his teeth. THEY DIDIN'T EVEN FEED HIM! He lost so much weight during his short stay! We had to fight to take him out of this place too! Places like this need to be shut down! Obviously they don't care about the patients and I'm pretty sure they receive tips for when the state comes to visit so that they pass! Amazing Adminstrator, Clinical team and all the Staff! The facility presents well.. It really shows how everyone love where they work... Residents looks happy and well cared for! I recommend this place to everyone! read more

Creekside post acute is by far one of the best facilities my grandmother was admitted to. First off I want to say how clean and well organized the place is. On top of that their new staff and management are great. The new administrator Marie was so friendly and showed she really cared for my family, she was very attentive to our needs. All of the staff was on top of their game. We have no complaints about this place, highly recommend! read more

In need of Updating this Review... Everything I posted before still stands except on Steroids (Meaning it's gotten Worse) Now they are beginning to Steal from me! First a CNA named Mark who is actually not a CNA until he takes The Test on the 16th tells me that when I asked for them my Blankets have been taken to Laundry to be cleanedFa. I told him that I didn't think so as they were not dirty and they were folded neatly. Later on that night a different CNA named Andrew who by the way is and always has been an incredible Nurse found my Blankets in Two l different Patients Closets as he recognized them as being mine. On another occasion I had ordered Two Bottles of Catsup and offered One to CNA Sandra and she said oh no Daniel you keep that, you will probably need it later and she was right so I asked her to put it up on my closet shelf then when I asked for it it was gone as someone had stolen it. My brother passed while in their care. He was not doing well and I knew he was slowly going. My concern was the staff waited over 2 hours after he died to call me and let me know. This is totally unacceptable. I also believe the staff might have been able to call sooner to let the family know to come because the end was near for him. Unfortunately, my brother passed without a family member with him...and maybe not even a staff person around! read more

My grandfather was admitted to this care center, and we couldn't be more happier. This facility is under new owners and new managers and they are now called Creekside Post Acute. I am so glad we found this place. My grandfather is now back home with us and all he can talk about is how caring all the staff was especially Marie, the Administrator. The managers are all new but they sure know what they are doing. Every time we visited the place was always clean and smells good. Just want to say "Thank You" to Team Creekside Post Acute for a job well done! My grandfather is not an easy person to please but you guys won his heart over. Would highly recommend this care center. read more

My husband Juan Ojeda has been here since end of May. I was doubtful at the first time he came here and had him transferred back to the hospital at Redlands he stayed in the emergency room for several days until they find a facility to accept him. There was no bed available anywhere so he had to go back to where he came from which was Creekside I decided to try them out and my husband agreed to it. During his stay we've been noticing the prompt care he's getting each day so I said I'm beginning to like it here. I am pleased with every staff that attend to my husband there are a few things that needed to get corrected and I brought the issue to the upper management and it's amazing how they took care of the issues that occur. It has gotten better as days go by and it's unbelievable that we've found the right facility to stay. My husband is happy with everyone of the staff. He wants to commence Sandra who is the best CNA here and she tops any facility for an excellent care she gives. Sandra is an experience CNA that she deserves to be a supervisor. RT is excellent Rocky knows how to make a patient comfortable he's great at everything not just as a RT he gets my husband comfortable and is quick to come when my husband needs anything. All the RT here knows their job and we're very thankful for them. RN's and LVN's are amazing they're great at assisting any needs to my husband I have to say Brenda gives my husband the best care, she knows how to get him calm and prompt when giving meds always on time. All the other RN and LVN are great too. Everyone here are very nice and respectful I never thought I will find a facility to love so much with staff that goes beyond expectations. I will recommend Creekside' Post Acute to anyone it is the best facility to be at I will not move my husband anywhere else we love it here. read more

As a patient here, I felt the care was unsatisfactory. The response to call lights was slow and often as late as an hour and a half later. A heart condition patient would be in danger. The food was generally bad and inedible. The positive aspect was the physical therapy. However, I will not return here if it becomes necessary for rehabilitation. - Yvonne. read more

They are understaffed to give every person the care they need. put them in a hall and just let them sit in the wheelchairs. The staff ignores cries for help. My mother was there till she passed last year and we always payed in advance for her monthly stay, I'm still getting bills and already gave them her death certificate. read more

Don't let the the appearance of the inside of the building fool you, it is clean, and this skilled nursing facility offers very good care. My husband was there for physical therapy for 10 days and I very impressed with the staff. They are caring, professional and attentive, and that of course is the most important thing. I would especially like to show appreciation for Kristie LVN and Andrea, physical therapy, whose warmth and good humor helped us through. Good job, everyone! read more

I do not recommend this facility AT ALL!! If you care for your loved one DO NOT SEND THEM HERE! They do not take care of the patients as they are PAID to do. I believe a facility should not accept a patient if they do not have the ability to tend to their specific needs. My FIL went to this facility and sustained injuries caused BY STAFF!! If they don't listen or need help when a staff wants to be "lazy" or they feel "bothered" by patient needs they manhandle them. When he first went to the facility, he only needed PT for his left arm after a mild stroke and was able to walk/talk/groom himself with little assistance. He went to this facility only needing physical therapy, by the time we had him sent to a different facility he was bed ridden!! My FIL was thrown into his bed and he was sent out to the hospital for a "fall," he sustained a fractured shoulder as well as fractured ribs. From his mouth to my ears - "staff threw him in his bed." He has restless leg syndrome and cannot stay still he had mobility of his right leg so he would slide down in the bed and need to be adjusted back up. By the time he left the facility (only there for approx. 6 weeks - with hospital stays in between) he was 100x worse than when he arrived and it is ALL DUE TO LACK OF CARE, PLEASE DO NOT SEND ANYONE YOU LOVE HERE! They also keep patients' personal belongings. This place needs to be shut down. read more
