My biggest concern was not simply that they wouldn't…read moreprovide the treatment I was hoping for, but the apparent lack of flexibility and understanding when treating a patient with opioid use disorder who was also dealing with significant acute pain. I understand that pain management involving opioids and OUD is complicated, and I completely respect the need for appropriate safeguards. What concerned me was feeling that the practice's policies were being applied in a way that didn't adequately account for the actual clinical circumstances of the patient. In my experience, the approach felt extremely limited rather than individualized. For a practice that specializes in pain management, I expected a much stronger understanding of the modern approaches to treating acute severe pain in patients receiving medication for opioid use disorder. This is where I believe the situation becomes potentially dangerous. A patient with OUD can have two legitimate medical needs at the same time: treatment for their OUD and adequate treatment for severe acute pain. Those issues shouldn't automatically be treated as mutually exclusive. I left feeling that the policies were more important than developing an appropriate, individualized pain-management plan. That's particularly concerning when dealing with patients who may already be at increased risk of returning to illicit opioids if legitimate pain isn't adequately addressed. I hope this practice seriously considers updating its approach to patients with OUD and ensuring that its clinicians are familiar with current evidence-based approaches to acute pain management in this population. One star based on my personal experience. I would encourage anyone with OUD who needs pain management to ask very specifically about how the practice handles acute severe pain while a patient is receiving MOUD before choosing this practice.
